Methods: The study was developed from confirmed notifications of ATL in the region of São José do Rio Preto. The studied variables included: date of notification, age, gender, rural or urban zone, occupation and probable location of infection. Reinvestigations, reclassifications and visits to the probable infection sites were made for all suspected autochthony cases which were then georeferenced and photographed. Results and conclusions: The number of indecisive cases was reduced from 27.2% to 7.8%. The number of imported cases increased from 50.0% to 70.8%. Autochthones dropped from 22.8% to 21.4%, however with alterations in their probable infection sites. There was a change in the classifications in 31.1% of cases. The most affected age range was between 31 and 40 years old and men (68.2%) were infected more than women. Occupation was not associated with the disease in 89.9% of the cases. The foci presented with similar landscape, always with woods, even in the periurban areas (9.1%). / Estudar a ocorrência de Leishmaniose Tegumentar Americana (LTA) nos municípios da região de São José do Rio Preto, entre 1998 e 2005. Descrever as variáveis das fichas de investigação. Reclassificar os casos de LTA notificados no período do estudo, segundo local provável de infecção. Analisar e caracterizar espacialmente as áreas de transmissão. Métodos: O estudo foi desenvolvido a partir das notificações de casos confirmados de LTA nos municípios da região de São José do Rio Preto. Variáveis estudadas: data da notificação, faixa etária, sexo, zona urbana ou rural, ocupação e local provável da infecção. Realizaram-se reinvestigação, reclassificação e visita aos locais prováveis de infecção dos casos com suspeita de autoctonia, os quais foram georreferenciados e fotografados. Resultados e conclusões: os casos indeterminados foram reduzidos de 27,2% para 7,8%. Os importados aumentaram de 50,0% para 70,8%. Os autóctones passaram de 22,8% para 21,4%, porém com alteração dos locais prováveis de infecção. Ocorreu alteração na classificação em 31,1% dos casos. A faixa etária mais acometida foi entre 31 e 40 anos (31,8%) e o sexo mais atingido foi o masculino (68,2%). A ocupação não teve relação com a doença em 89,9% dos casos. Os focos apresentaram paisagens semelhantes, sempre relacionadas à presença de matas, mesmo nos casos periurbanos (9,1%).

Prédisposition génétique au mélanome : de la génétique à la recherche clinique / Genetic predisposition to melanoma : from genetics to clinical research

Ce travail avait deux objectifs: 1) définir des groupes de patients (pts) susceptibles de bénéficier d’un conseil génétique par l’identification de facteurs prédictifs de l’existence d’une mutation du gène CDKN2A, un des gènes majeurs de prédisposition au mélanome, dans les familles ne comportant que deux cas (Fam_2 cas). 2) la caractérisation épidémiologique et clinique d’entités particulières du mélanome dans l’objectif secondaire de contribuer à l’identification de gènes de prédisposition à ces entités. Les 2 entités étudiées étaient le mélanome cutané (MC) associé au cancer du rein (CR) et les mélanomes muqueux de la sphère ano-génitale (MMAG).Les populations d’étude sont une collection de 293 pts atteints de MC recrutés de façon consécutive sans connaissance à priori de l'histoire familiale et la collection française MELARISK qui comprend ≥ 3000 sujets prélevés appartenant à des familles à cas multiples de mélanomes ou ayant un MC survenant dans un contexte particulier (association à un autre cancer, topographie rare, survenue avant l’âge de 20 ans, MC multiples sporadiques). Nous avons étudié l'effet de 3 facteurs prédictifs potentiels sur la présence d’une mutation de CDKN2A dans une famille en fonction du nombre de pts atteints dans une famille (2 pts versus ≥3 pts). L’étude a été menée dans 483 familles françaises comprenant 387 Fam_2 cas, et 96 familles avec ≥3 pts atteints de mélanome (Fam_3+ cas). Les facteurs étudiés dans la famille un à un puis conjointement étaient : l’âge médian <50 ans au diagnostic de MC, la survenue de ≥1 cas de MC primitifs multiples (MPM) et la survenue de ≥1 cas de cancer du pancréas (CPCP). La fréquence des mutations était plus élevée dans les Fam_3+ cas (32%) que dans les Fam_2 cas (13%). Alors qu’un âge jeune au diagnostic et la survenue de ≥ 1 MPM étaient associés à la présence de mutations de CDKN2A dans les Fam_2 cas, un âge jeune au diagnostic ainsi que la présence de ≥1 cas de CP était associé significativement aux mutations de CDKN2A dans les Fam_3+ cas. L’étude a montré que les caractéristiques cliniques associées aux mutations de CDKN2A varient, en France, pays de faible incidence de mélanome, en fonction du degré d’agrégation familiale. L’identification de facteurs prédictifs de mutations de CDKN2A dans les Fam_2 cas a contribué à définir des sous-groupes de familles (âge jeune au diagnostic, survenue de MPM) dans lesquels la fréquence des mutations de CDKN2A est supérieure à 20% et auxquels il est légitime de proposer un test génétique. L’analyse des deux séries de pts MM+CR et MMAG a permis d’identifier, en les comparant à la série de MC recrutés de manière consécutive, leurs caractéristiques cliniques et histologiques. Dans ces deux séries, nos résultats ont mis en évidence un contexte de prédisposition héréditaire en partie indépendant de CDKN2A. L’étude de l’association MC et CR chez un même patient a eu deux conséquences pratiques: pour les cliniciens ces résultats suggèrent l’intérêt d’un examen dermatologique en cas de CR et l’intérêt de l’échographie abdominale dans le bilan initial d’un MC pour le dépistage du CR; pour la recherche en génétique, cette série a contribué à l’identification d’une mutation germinale dans le gène MITF qui augmente le risque de développer un MC, un CR ou l’association des deux cancers et qui a des propriétés biologiques intéressantes. L’étude des MMAG a montré que ces mélanomes pouvaient être associés à des MC chez un même malade et/ou survenir dans un contexte familial de mélanome. Le corollaire clinique de ces résultats est que l’examen dermatologique de dépistage ou de surveillance doit être à la fois cutané et muqueux dans un contexte familial de mélanome et qu’en cas de MMAG un examen dermatologique des apparentés doit être proposé comme c’est la règle dans les MC. L’absence de mutation de CDKN2A dans ces localisations muqueuses incite à entreprendre des études génétiques pour identifier les gènes impliqués. / This thesis had two main objectives: 1) To define groups of patients which may benefit from genetic counseling by identifying predictors of mutations of the CDKN2A gene, a major gene predisposing to cutaneous melanoma (CM) in families with only two cases. 2) Epidemiological and clinical characterization of specific entities of melanoma with the secondary objective of contributing to the identification of susceptibility genes for these entities. Coexistence of CM with renal cell carcinoma and mucosal anogenital melanomas were studied.The study populations are a collection of 293 melanoma patients that were ascertained systematically and the French collection MELARISK which is a collection including over 3000 subjects drawn from families with multiple cases of melanoma or melanoma occurring in a particular context (association with another cancer, rare locations, occurrence before the age of 20, multiple sporadic melanomas).We investigated association of three clinical features with the presence of a CDKN2A mutation in a family by extent of CM family clustering (2 versus ≥3 CM patients among first-degree relatives in a family).The study was conducted in 483 French families including 387 families with two melanoma patients, and 96 families with three or more patients with melanoma. The factors examined individually and in a joint analysis in a family were: median age at diagnosis <50 years, ≥1 patient in a family with multiple primary melanomas (MPM) or with pancreatic cancer. The frequency of CDKN2A mutations was higher in F3+ families (32%) than in F2 families (13%). While early age at melanoma diagnosis and occurrence of MPM in ≥1 patient were significantly associated with the risk of a CDKN2A mutation in F2 families, early age at melanoma diagnosis and occurrence of pancreatic cancer in a family were significantly associated with CDKN2A mutations in F3+ families. Thus this study showed that clinical features associated with CDKN2A mutations vary, in France, a country of low incidence of melanoma, according to the degree of familial clustering. Identifying predictors of CDKN2A mutations in families with two melanoma cases has helped to define subgroups of families (early age at CM diagnosis, and/or ≥1 MPM patient) in which the frequency of CDKN2A mutations is above 20% such that these subgroups of F2 families should be offered genetic testing.The analysis of two series of patients, either patients with melanoma coexisting with renal cell carcinoma or patients with anogenital mucosal melanoma identified their clinical and histological features by comparing them to a series of melanomas that were ascertained systematically. In both series, our results suggested a genetic predisposition at least partly independent of CDKN2A. The study of the c renal cell carcinoma; coexistence of CM and renal cancer in the same patient had two practical consequences for clinicians: it suggests the interest of a dermatologic screening visit in patients with renal cell carcinoma and that abdominal ultrasonography or computed tomography scanning performed at the initial workup and during the follow-up of patients with CM may be of value for the early detection of renal cancer. Regarding genetic research, this series has contributed to the identification of a germline mutation in the MITF gene that increases the risk of developing melanoma, renal cancer or both cancers and has interesting biological properties. The study of anogenital melanoma has shown that these melanomas could be associated with cutaneous melanoma in the same patient and it has also shown a high frequency of family history of melanoma associating mucosal and CM suggesting a shared genetic predisposition. Consequently dermatological screening or monitoring must include examination of both skin and mucosa in families with multiple cases of CM; and in case of a mucosal melanoma, a dermatological examination should be offered to relatives. Epidemiologia da leishmaniose tegumentar americana (LTA) no município de Xapuri, Estado do Acre, Brasil: estudo em população humana, cães domésticos e vetores / Epidemiology of American cutaneous leishmaniasis (ACL) in Xapuri municipality, Acre state, Brazil: study in human population, domestic dogs and vectors

Introdução. A leishmaniose tegumentar americana (LTA) é uma endemia de importância em saúde pública na Amazônia Brasileira, onde possui diferentes perfis de transmissão com a participação de diversas espécies de vetores e de protozoários do gênero Leishmania. O Estado do Acre apresenta altos índices de LTA e, no ano de 2015, apresentou o mais alto coeficiente de detecção de casos (137,7/100.000 hab.) da doença no Brasil. Objetivo. Analisar aspectos epidemiológicos da LTA no município de Xapuri, Estado do Acre, Brasil, envolvendo população humana, cães domésticos, vetores e identificação de Leishmania. Materiais e Métodos. Para a avaliação epidemiológica, foram analisadas fichas de notificação dos casos humanos no período de 2008 a 2014 obtidas do sistema de vigilância epidemiológica do município. Foram analisadas as variáveis: sexo, grupo etário, escolaridade, forma clínica, diagnóstico, tratamento e evolução clínica. Os dados foram submetidos à análise descritiva e teste estatístico do qui-quadrado de Pearson utilizando o pacote estatístico STATA. Também foram obtidas amostras de material de pacientes (escarificação das lesões fixadas em lâminas) atendidos no centro médico do município, durante o período de novembro de 2014 a janeiro de 2016. As amostras foram submetidas à análise molecular para diagnóstico de Leishmania spp. O inquérito canino foi realizado em áreas urbanas e rurais; nestas, predominantemente, em seringais, onde a doença foi reportada em humanos. Após avaliação clínica dos animais para busca de lesões e sinais característicos de leishmanioses, foram coletadas amostras de sangue venoso por punção jugular ou cefálica, e quando havia lesões sugestivas da doença, foram anestesiados e submetidos à biopsia para colheita do fragmento de lesão. Amostras destes fragmentos foram submetidas a técnicas parasitológicas, inoculação em meio de cultura Neal, Novy e Nicolle, exame direto e técnicas moleculares para detecção do parasita. A identificação de Leishmania spp. foi realizada por técnicas moleculares. Os flebotomíneos foram coletados em ambiente domiciliar e florestal de dois seringais (Floresta e Cachoeira) e na área urbana, com armadilhas luminosas tipo CDC, instaladas mensalmente, no período de agosto de 2013 a julho de 2015. Neste período, apenas no seringal Cachoeira, também foram feitas coletas utilizando armadilhas de Shannon nas cores branca e preta, e de janeiro a maio de 2016, coletas em trocos de árvores com aspiradores manuais. Uma amostra de fêmeas coletadas pelas diferentes técnicas foi dissecada viii para investigação da presença de flagelados. Para a análise do comportamento da fauna flebotomínea foram utilizados índices ecológicos como de Shannon, Pielou e Abundância das Espécies Padronizado, média geométrica de Williams e análise dos componentes principais. Para estimar a atratividade dos flebotomíneos pelas cores branca e preta foi utilizado o teste de Mann-Whitney (p<0,05). Análises morfométricas utilizando o teste de Gabriel (teste F, p <0,05) foram feitas para distinguir alguns táxons. Para as análises moleculares dos parasitas foi empregada a técnica de Nested-PCR SSU rRNA utilizando os iniciadores S4/S12 e S17/S18 e sequenciamento. Resultados. No estudo de casos humanos, constatou-se que a doença ocorre predominantemente em populações rurais e isoladas do município, em indivíduos de ambos os sexos, com as incidências mais elevadas em crianças e adolescentes. Em 33 dos 45 pacientes com clínica positiva para LTA foram detectadas a presença de DNA de Leishmania spp. Nos cães domésticos, verificou-se alta taxa de infecção (20,0 por cento ) por Leishmania (Viannia) sp. Nos estudos de flebotomíneos, foram coletados 21.197 espécimes (14.210 fêmeas e 7.107 machos) com armadilhas CDC, e 6.309 (864 machos e 5.445 fêmeas) com armadilhas de Shannon. As frequências, abundâncias e densidades mais elevadas foram dos gêneros Nyssomyia, Psychodopygus e Trichophoromyia, coletados em ambientes silvestres, peri e intradomiciliares. Em ambiente rural foram coletados 99,9 por cento dos espécimes e no urbano apenas 0,1 por cento . Nyssomyia shawi predominou no Seringal Cachoeira, e Trichophoromyia spp. (Th. auraensis/Th. ruifreitasi) no Seringal Floresta. Espécies do gênero Psychodopygus predominaram no período chuvoso, enquanto as de Nyssomyia, no período seco. Infecções por Leishmania spp. foram detectadas em Brumptomyia sp., Nyssomyia antunesi, Ny. shawi, Lutzomyia sherlocki, Psathyromyia aragaoi, Psychodopygus carrerai carrerai, Ps. davisi, Ps. hirsutus hisutus, Ps. llanosmartinsi, Ps. lainsoni, Thrichophoromyia ubiquitalis e Trichophoromyia spp. Por meio de análises morfológicas e morfométricas das fêmeas de Trichophoromyia sugeriu-se a distinção de Th. octavioi de Trichophoromyia spp. (Th. auraensis/Th. ruifreitasi) e descreveu-se, Psathyromyia elizabethdorvalae sp. n. Conclusões. A LTA em Xapuri apresenta um perfil de transmissão silvestre e outro domiciliar. As populações humanas e caninas que frequentam ambientes florestais estão expostas a uma alta diversidade de vetores e de agentes etiológicos, o que aumenta o risco de infecção de LTA. As informações aqui apresentadas podem nortear as medidas de controle, planejamento das ações e definição de prioridades dos órgãos de vigilância epidemiológica do Estado do Acre, visando o diagnóstico precoce e tratamento adequado dos casos de leishmanioses da população humana de Xapuri / Introduction. American cutaneous leishmaniasis (ACL) is an endemic disease that deserves the attention of public health in the Brazilian Amazon, where it has various transmission profiles with the participation of several species of vectors and protozoa of the genus Leishmania. The state of Acre registers high rates of ACL, having in 2015 the highest coefficient of detection of cases (137.7 / 100,000 inhab.) in Brazil. Objective. To analyze the epidemiological aspects of LTA in the municipality of Xapuri, State of Acre, Brazil, involving the human population, domestic dogs, vectors and the identification of Leishmania ssp. Materials and Methods. For the epidemiological evaluation, records of the human cases notified between 2008 and 2014 obtained from the epidemiological surveillance system of the municipality, were analyzed. The following variables were selected for analysis: sex, age group, schooling, clinical form, diagnosis, treatment and clinical evolution. The data were submitted to descriptive analysis and the Pearson chi-squared statistical test using the statistical package STATA. Samples of patient material (scarification of lesions fixed on slides) were also obtained from the medical center of the city during the period from November 2014 to January 2016. These samples were submitted to molecular analysis for the diagnosis of Leishmania spp. The canine survey was carried out in both urban and rural areas, in the latter rubber plantations where the disease had been reported in humans predominated. After a clinical evaluation to search for lesions and characteristic signs of leishmaniasis, samples of venous blood were collected by jugular or cephalic puncture, and when the animals presented lesions suggestive of cutaneous leishmaniasis, they were anesthetized and submitted to biopsy to harvest the lesion fragment. Samples of these fragments were submitted to parasitological techniques, inoculation in Neal, Novy and Nicolle culture medium, direct examination and molecular techniques to detect Leishmania spp. The samples were submitted to molecular analysis for the diagnosis of Leishmania spp. The phlebotomine survey was carried out in forest and the domiciliary environment of the city and in two rubber plantation areas (Seringal Cachoeira and Seringal Floresta), monthly using CDC light traps, from August 2013 to July 2015. In this period, only in the Seringal Cachoeira were collections also made using black and white Shannon traps. Collections in tree trunks and among tree roots with manual aspirators were also undertaken from January to May 2016. xi Samples of females collected by different techniques were dissected to investigate the presence of flagellates. For the analysis of the behavior of the phlebotomine fauna, ecological indexes such as Shannon, Pielou, and Standardized Species Abundance, Williams geometric mean and main component analysis were used. The Mann-Whitney test (p <0.05) was used to estimate the attractiveness of the black and white colors to sandflies. Morphometric analyses using the Gabriel test (test F, p <0.05) were made to distinguish between some taxa. For the molecular analyses of the parasites the Nested-PCR SSU rRNA technique was used using primers S4 / S12 and S17 / S18 and sequencing. Results. In the study of human cases, it was found that the disease occurs in rural and isolated populations and in individuals of both sexes, especially in children and adolescents. In 33 of the 45 patients with positive ACL, the presence of Leishmania spp DNA was detected. The domestic dogs have shown a high infection rate (20.0 per cent ) attributed to Leishmania (Viannia) sp. In the sandfly studies, 21,197 specimens (14,210 females and 7,107 males) were collected in CDC traps, and 6,309 (864 males and 5,445 females) were collected in Shannon traps. The highest frequencies, abundances and densities were of the genera Nyssomyia, Psychodopygus and Trichophoromyia collected in wild, peri and intradomiciliary environments. Nyssomyia shawi predominated in the Seringal Cachoeira, and the Trichophoromyia spp. (Th. auraensis/Th. ruifreitasi) in the Seringal Floresta. Species of the genus Psychodopygus predominated in the rainy season while those of Nyssomyia in the dry period. Natural infections by Leishmania spp were detected in Brumptomyia sp., Nyssomyia antunesi, Ny. shawi, Lutzomyia sherlocki, Psathyromyia aragaoi, Psychodopygus carrerai carrerai, Ps. davisi, Ps. hirsutus hirsutus, Ps. llanosmartinsi, Ps. lainsoni, Trichophoromyia ubiquitalis and Trichophoromyia sp. Morphological and morphometric analyses of Trichophoromyia females were suggested to distinguish Th. octavioi from Trichophoromyia spp (Th. auraensis/Th. ruifreitasi), Pa. elizabethdorvalae being described for the first time. Conclusions. ACL in Xapuri presents one profile of wild transmission and another of domiciliar transmission. Both populations, human and canine, because they live in forest environments are exposed to a high diversity of vectors and etiological agents, which increases the risk of ACL infection. The information presented here may guide the measures of control, planning of actions and definition of priorities taken by the organs of surveillance and epidemiology of the State of Acre, aiming at the early diagnosis and appropriate treatment of the human cases in Xapuri

Impulsionadas pela resistência neoplásica aos fármacos, as pesquisas com produtos naturais tem recebido atenção especial nos últimos anos, pois esses são promissores agentes para prevenção e tratamento do câncer. Assim, o objetivo do trabalho foi avaliar a atividade citotóxica dos óleos essenciais de Origanum vulgare, Origanum majorana e Rosmarinus officinalis e dos extratos hidroalcóolicos de Origanum vulgare, Schinus terenbithifolius, Eugenia uniflora e Bauhinia forficata frente à linhagem celular de melanoma murino B16F10 e de rim bovino - MBDK (Madin Darby Bovine Kidney), além de fazer uma retrospectiva de melanomas em cutâneos em cães atendidos no HCV. Os óleos essenciais foram obtidos através de arraste de vapor em Clevenger, sendo testados nas concentrações de 6,25 a 0,024 mg.mL-1 e os extratos hidroalcóolicos através de rotaevaporador, sendo testados nas concentrações de 6,25 a 0,02 mg mL-1. As diluições dos óleos e dos extratos foram distribuídas em placas de 96 poços, sob as monocamadas de células das linhagens B16F10 e MDBK, que foram incubadas por 24 e 48 horas, em 37°C com atmosfera úmida e 5% de CO2. A viabilidade celular foi avaliada pelo ensaio MTT e a leitura feita em espectrofotômetro (540nm) em dois tempos (24 e 48 horas). Todos os óleos avaliados foram efetivos em inibir as células de melanoma, porém não houve diferença significativa entre eles e entre suas concentrações, assim como em relação a citotoxicidade na linhagem MDBK. As concentrações efetivas e não tóxicas dos óleos essenciais de O. majorana e O. vulgare foram 490 e 245 μg.mL-1. O óleo de R. officinalis, apresentou elevada toxicidade nas concentrações efetivas nas células B16F10. Os compostos majoritários identificados foram 1-8 cineol, canfora e α-pinene, no óleo essencial de R. officinalis; 4-terpineol, 4-carene e 5-isopropyl-2-methylbicyclo no óleo essencial de O. majorana e γ-terpineno, 4-terpineol e hidroxy-p-cymene no óleo essencial de O. vulgare. Os extratos avaliados apresentaram atividade antiproliferativa em células tumorais e normais dependente do tempo e da dose, tendo maiores porcentagens de inibição celular em 48 horas do que em 24. Os extratos hidroalcóolicos de B. forficata, E. uniflora e O. vulgare em 48 horas, apresentaram os melhores resultados antiproliferativos em células de melanoma, uma vez que atingiram moderada atividade inibitória nas células B16F10 em concentrações com baixa toxicidade para as células MDBK. O extrato de S. terebinthifolius não exibiu atividade satisfatória nas células tumorais, quando cruzadas as concentrações efetivas e tóxicas. Assim, conclui-se que os óleos essenciais de O. majorana e de O. vulgare e os extratos hidroalcóolicos de Origanum vulgare, Eugenia uniflora e Bauhinia forficata apresentam melhor atividade antiproliferativa in vitro em células de melanoma, quando comparados ao óleo essencial R. Officinalis e o extrato hidroalcóolico de S. terebinthifolius,e representam promissoras fontes de estudo para o desenvolvimento de novos fármacos antineoplásicos. Os melanomas cutâneos são frequentes na rotina do HCV, estando o seu comportamento biológico relacionado principalmente com o tamanho do tumor e a presença de metástases. / Melanoma has great importance in veterinary oncology due to their capacity of invasion, highly metastatic and poorly responsive to conventional chemotherapy treatment. Powered by neoplastic drug resistance, researches of natural products have received special attention in recent years, probably because they are promising agents for the prevention and treatment of cancer. The objective of the study was to evaluate the cytotoxic activity of essential oils of Origanum vulgare, Origanum majorana and Rosmarinus officinalis and the hydroalcoholic extracts of Origanum vulgare, Schinus terenbithifolius, Eugenia uniflora and Bauhinia forficata against the cell line of murine melanoma B16F10 and kidney veal - MBDK (Madin Darby bovine Kidney), and make a retrospective of skin melanomas in dogs treated at the HCV. ). The essential oils were obtained by dragging a water steam by using a Clevenger apparatus, and they were tested at concentrations from 6.25 to 0.024 mg.mL-1 and hydroalcoholic extracts through rotaevaporator being tested at concentrations of from 6.25 to 0.02 mg mL -1. Dilutions of oils and extracts were distributed in 96-well plates in cell monolayers of MDBK and B16F10 lines which were incubated for 24 and 48 hours at 37°C in humid atmosphere, 5% CO2. Cell viability was assessed by MTT assay and the result was analyses using a spectrophotometer (540nm) in two stages (24 and 48 hours). All oils tested were effective in inhibiting melanoma cells, but there was no significant difference between them and between their concentrations, as well as in relation to cytotoxicity in MDBK line. Effective and non-toxic concentrations of the essential oil of O. majorana and O. vulgare were 490 and 245 μg.mL-1. The essential oil of R. officinalis showed high toxicity at effective concentrations in B16F10 cells. The majors compounds identified were 1-8 cineol, camphor and α-pinene in essential oil of R. officinalis; 4-terpineol, 4-Carene and 5-isopropyl-2-methylbicyclo in essential oil of O. majorana and γ-terpinene, 4-terpineol and Hydroxy-p-cymene in essential oil of O. vulgare. The extracts tested showed antiproliferative activity on tumor and normal cells dependent on time and dose, with higher percentages of cellular inhibition at 48 hours than at 24 hours. The hydroalcoholic extracts of B. forficata E. uniflora and O. vulgare showed, in 48 hours, the best anti-proliferative outcome in melanoma cells, once they reached moderate inhibitory activity on B16F10 cells at concentrations with lower toxicity to MDBK cells. The extract of S. terebinthifolius did not exhibited satisfactory activity in tumor cells, when crossed the effective and toxic concentrations. Thus, it is concluded that the essential oil of O. majorana and O. vulgare and the hydroalcoholic extracts of Origanum vulgare, Eugenia uniflora and Bauhinia forficata have better antiproliferative activity in vitro in melanoma cells, when compared to the essential oil R. officinalis and the hydroalcoholic extract of S. terebinthifolius and they represent a promising study sources for the development of new anticancer drugs. Cutaneous melanomas are common in routine HCV, being your biological behavior mainly related with tumor size and metastasis.

Compression nerveuse chronique : évaluation des complications fonctionnelles et moléculaires sur la microcirculation cutanée et la régénération nerveuse / Chronic nerve compression : study of functional and molecular impairments on cutaneous microcirculation and nervous regeneration

La compression nerveuse chronique (CNC) est un modèle d’étude expérimental définit pour l’examen du syndrome du canal carpien. Notre travail consistait à étudier chez le rat sain, l’effet d’une CNC de courte ou de longue durée sur la microcirculation cutanée, ainsi que la capacité de récupération fonctionnelle du nerf suite à une décompression, et les mécanismes moléculaires impliqués dans la régénération nerveuse. L’originalité de l’équipe de recherche est d’avoir montré un mécanisme physiologique original permettant d’étudier l’interaction neurovasculaire cutanée qui peut être observée sur le nerf. La PIV (Pressure‐Induced Vasodilation) est un outil original de commande de la vasomotricité par les fibres C. Les dysfonctions neurovasculaires cutanées et nerveuses induites par une CNC de courte durée sont associés à l’altération des fibres C et, à des mécanismes angiogéniques et inflammatoires du nerf lésé. Ces atteintes sont réversibles après une décompression nerveuse, malgré une modification de la balance des facteurs angiogéniques, une augmentation de l’expression de facteurs inflammatoires et l’activité de protéines à profil apoptotique. La CNC de longue durée provoque des traumatismes plus importants auxquels s’ajoutent des atteintes vasculaires. Par conséquent la décompression du nerf sciatique n’a pas permis de rétablir les fonctions nerveuses périphériques. La PIV fournit un diagnostic précoce de l’atteinte nerveuse suivant sa réduction, synonyme d’une CNC de courte durée, ou de son abolition, synonyme d’une CNC longue durée. Ces niveaux d’atteintes nerveuses et inflammatoires pourraient être responsables ou non de la récupération fonctionnelle nerveuse / Chronic nerve compression (CNC) is an experimental study model for the examination of the carpal tunnel syndrome. Our work aimed to study the effect of short or long‐term CNC on the cutaneous blood flow, on the nerve ability to recover following to nerve release, and the molecular mechanisms involved in the nervous regeneration. For that purpose, our laboratory established an original physiological mechanism in the study of the relationship between vessel and nerve in the sciatic nerve but also at the periphery, in the cutaneous tissue. Therefore, PIV (Pressure‐Induced Vasodilation) is an original tool of vasomotion linked to C‐fibers. Cutaneous and nervous neurovascular dysfunctions induced by a shortterm CNC were associated to cutaneous C‐fibers impairment and, angiogenic and inflammatory mechanisms of the injured nerve. These achievements are reversible after nerve release, even though the nerve showed a modification of the balance of angiogenic factors, an increase of the expression of inflammatory factors and the activity of proteins with apoptotic type. The long‐term CNC induced more important trauma that further affected the vascular function. Consequently the decompression of the sciatic nerve did not allow restoring the peripheral nervous function. PIV allows to early diagnosing nerve damage with a reduction referring to a short‐term CNC or an abolition referring to a long‐term CNC. Activation of the inflammatory pathway during CNC could compromise nerve recovery following nerve decompression

L'objet de cette thèse est l'étude de la pénétration des actifs cosmétiques dans la peau. Les axes d'investigation principaux ont concerné l'influence des propriétés physicochimiques des actifs et des ingrédients de la formule sur les mécanismes de pénétration. Les actifs cosmétiques choisis sont le rétinol, actif lipophile, et la caféine, actif hydrophile. Les formulations investiguées sont des émulsions de type huile dans eau, comparées aux solutions de tensioactifs correspondantes. Trois huiles cosmétiques ont été utilisées: Butylène glycol de cocoate, Octyldodecyl myristate et la Paraffine liquide, stabilisées en émulsion avec des tensioactifs ester de polyéthylène glycol (PEG20 et PEG6) possédant des longueurs de chaîne carbonées variables (C8, C12, C18 et C18:1). La pénétration percutanée a été mesurée quantitativement en utilisant la méthode des cellules de diffusion de Franz en fonctionnement statique et dynamique et qualitativement par la microscopie confocale Raman. Avec cette combinaison de techniques analytiques, il est possible, de mesurer la pénétration et d’évaluer l'impact de chaque composant de la formulation sur la pénétration cutanée d'un actif. Une corrélation a pu être établie entre l’effet fluidifiant d’une huile et l’augmentation de la pénétration du rétinol. Par ailleurs les tensioactifs, même s’ils ont montré un effet moindre en terme de fluidification conduisent également à une augmentation de la pénétration en raison d’une variation du coefficient de partage de l’actif entre la formule et la peau. Concernant la caféine, l’influence de la structure des tensioactifs et en particulier de la longueur de chaîne carbonée a été mise en évidence / The purpose of this thesis is to study the penetration of cosmetics actives into the skin. The main lines of investigation concerned the influence of actives and formulation components physicochemical properties on the penetration mechanisms. The selected cosmetic actives are retinol, lipophilic, and caffeine, hydrophilic. The investigated formulations are oil in water emulsions, compared to their corresponding surfactant solutions. Three cosmetic oils were used: Butylene glycol cocoate, Octyldodecyl myristate and liquid paraffin. Emulsions are stabilized with polyethylene glycol ester surfactants (PEG20 and PEG6) having variable carbon chain lengths (C8, C12, C18 and C18: 1). Percutaneous penetration was measured quantitatively using Franz diffusion cells in a static and dynamic way and qualitatively by confocal Raman microscopy. With this combination of analytical techniques, it is possible to measure the penetration and evaluate the impact of each formulation component on skin penetration of an active. A correlation could be established between the fluidizing effect of an oil and the increase in retinol penetration. Moreover, the surfactants, although they showed less effect in terms of fluidizing also lead to an increase in penetration due to a variation of the active partition coefficient between the formula and the skin. Os antimaláricos, cloroquina e hidroxicloroquina, têm sido usados há décadas com bons resultados terapêuticos para o tratamento do lupus eritematoso e são considerados medicações seguras, muito embora, haja preocupação em relação à retinotoxicidade, notadamente com a cloroquina. O objetivo deste trabalho foi avaliar a ocorrência dos efeitos adversos desencadeados pelo tratamento com 250mg/d de difosfato de cloroquina em doentes com lupus eritematoso, dando ênfase à retinotoxicidade. Foram estudados 350 doentes e reavaliados seus respectivos prontuários, que datavam de 1980 a 2003. Os doentes foram acompanhados no ambulatório de colagenoses da Divisão de Dermatologia do Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo. A ocorrência dos efeitos adversos foi de 35,7%, sendo que 17,4% decorreram de alterações oculares. Em 12% dos doentes ocorreu pigmentação retiniana sugestiva de retinopatia antimalárica, em 3,1% depósitos corneanos e, em 2,3%, sintomas visuais agudos. Em 10% dos doentes houve alterações gastrointestinais: epigastralgia (6%), náuseas e vômitos (3,7%) e diarréia (0,3%). Alterações dermatológicas ocorreram em 3,4% dos doentes: rash cutâneo no início do tratamento (2%), exacerbação de quadro de psoríase pré-existente (0,3%) e pigmentação cutânea (1,1%). Ocorreram ainda cefaléia (2,9%), alterações neuromusculares (1,7%) com quadro gripal símile no início do tratamento (1,1%), neuropatia sensitiva (0,3%) e miopatia compatível com miastenia (0,3%) e, sintomas neuropsiquiátricos (0,3%). A droga foi suspensa devido aos efeitos adversos em 22,9% dos doentes, principalmente, em decorrência de alterações oculares, gastrointestinais e dermatológicas. A reavaliação oftalmológica de 12% dos doentes com pigmentação retiniana, confirmou a retinopatia antimalárica em apenas 2,6%, o que demonstrou uma tendência à valorização de alterações retinianas inespecíficas, discretas e unilaterais, com indicação desnecessária da suspensão da droga em 9,4% dos doentes. Não ocorreram casos de retinopatia antimalárica avançada com lesão do tipo bull-eye. Não houve associação estatisticamente significativa entre a ocorrência de efeitos adversos e alterações retinianas com dose diária de difosfato de cloroquina por quilo de peso e com o tipo clínico do lupus eritematoso. As alterações retinianas foram estatisticamente significativas nos doentes acima de cinqüenta anos quando comparado ao grupo abaixo dos cinqüenta anos, possivelmente pela dificuldade em diferenciar as alterações iniciais da retinopatia antimlárica daquelas decorrentes da degeneração macular senil. O controle oftalmológico foi realizado em intervalo médio de 10,5 meses, demonstrando que o controle anual foi eficaz para o acompanhamento dos doentes. Nove doentes foram expostas durante o primeiro trimestre gestacional, não ocorrendo casos de mal formação fetal / Antimalarial agents, chloroquine and hydroxichloroquine, have been used for decades leading to good therapeutic outcomes at treatment approach for lupus erythematosus and are considered safe medication; however, the main concern is retina toxicity, especially with chloroquine. The purpose of the present study was to conduct analysis of the occurrence of adverse effects, triggered by use of 250 mg/d of chloroquine diphosphate at treatment for lupus erythematosus, especially retina toxicity. We analyzed 350 patients and reviewed their medical charts, from 1980 to 2003. The patients were followed up by the outpatient unit of collagenosis, Division of Dermatology, Hospital das Clinicas, Medical School, University of São Paulo. The occurrence of adverse effects was 35.7%, and eye affections were detected in 17.4% of patients. Impairment of retina pigmentation suggestive of antimalarial retinopathy occurred in 12%, cornea deposits in 3,1%, and acute visual symptoms in 2.3%. Gastrointestinal affections were detected in 10% of patients: epigastralgia (6%), nausea and vomiting (3.7%) and diarrhea (0.3%). Dermatological affections occurred in 3.4% of patients: skin rash in the beginning of treatment (2%), exacerbation of preexisting psoriasis (0.3%) and skin pigmentation (1.1%). We also detected headache (2.9%), neuromuscular disorders (1.7%) with flu-like episode at the beginning of treatment (1,1%), sensitive neuropathy (0,3%) and myopathy compatible with myasthenia (0.3%) and neuropsychiatric symptoms (0.3%). Discontinuation of drugs owing to side effects occurred in 22.9% of the patients, being that the main affections were eye, gastrointestinal and dermatological occurrences. Ophthalmologic reevaluation of retina pigmentation affections occurred in 12% of the patients, but we confirmed antimalarial retinopathy only in 2.6%, detecting a tendency to value nonspecific, discreet and unilateral affections, which generated unnecessary recommendations for discontinuation of drug in 9.4% of the patients. There were no cases of advanced retinopathy with bull-eye type lesion. There was no statistically significant association between occurrence of adverse effects and retina affections with daily dose per kg of chloroquine diphosphate and the differents types of lupus erythematosus. In patients over the age of 50, there was statistically significant increase in number of retina affections when compared to the group aged below 50 years, possibly owing to difficulty to differentiate between initial affections in antimalarial retinopathy from those resultant from senile macular degeneration. Ophthalmologic control was conducted on average after 10.5 months, showing that annual follow-up was effective to keep track of patients. Nine of the patients were exposed during the first gestational trimester and there were no cases of fetal malformations

Co-infecção HIV/Leishmania: manifestações clínicas em uma série de casos / HIV/Leishmania co-infection: clinical manifestations in a serie of cases

Em áreas endêmicas para leishmaniose visceral, a AIDS aumenta o risco de aparecimento desta doença de cem a mil vezes. Conhecem-se muitos estudos sobre leishmaniose visceral (LV) em HIV/AIDS no Mediterrâneo, mas, pouco sobre leishmaniose tegumentar (LT) em HIV/AIDS. No Brasil não se constatou, até o momento, um aumento de casos de co-infecção HIV-Leishmania, mas, isto pode ser conseqüente ao subdiagnóstico por várias causas incluindo o não conhecimento da apresentação clínica nesses casos. Pela prevalência maior da LT em relação à LV no Brasil, é de se supor que ocorram com freqüência casos de co-infecção com a forma tegumentar de leishmaniose no nosso meio. Como no Brasil não dispomos de estudos de descrição sistemática da apresentação clínica nos casos de co-infecção, um parâmetro importante visando o diagnóstico, realizou-se um estudo transversal e descritivo, durante um período de dois anos a partir de fevereiro de 2002, em dois centros de tratamento de doenças infecciosas em São Paulo. Partimos de uma população alvo de 113 pacientes com sorologia positiva para HIV, procedentes de área endêmica para leishmanioses, 69 com algum sintoma sugestivo de leishmaniose visceral ou tegumentar e 44 assintomáticos. Em dez pacientes foi diagnosticada leishmaniose por encontro de parasitos ou antígeno de Leishmania em tecidos, maioria (8/10) por busca ativa, sendo cinco casos de LV e cinco de LT. Baseados nesses casos, observamos que, o contato com área endêmica de leishmaniose mesmo por um período curto como duas semanas deva ser valorizado, principalmente se foi recente. Houve dificuldade no encontro de casos porque a leishmaniose não é considerado como diagnóstico diferencial de infecções intercorrentes nos casos de HIV/AIDS no nosso meio, houve presença concomitante de outras doenças oportunistas mais prevalentes e manifestações clínicas atípicas ou semelhantes a infecções oportunistas. xvi Os casos de LV apresentaram manifestações clássicas, enquanto que os casos de LT apresentaram lesões diversas, desde úlcera única até lesões múltiplas e polimórficas (placas infiltrativas, pápulas, nódulos, máculas), com tendência à disseminação atingindo mucosa e pele, incluindo área genital (em quatro dos cinco pacientes com LT). Em dois pacientes observaram-se aparecimento de lesões (num caso) e piora (noutro caso), após início da terapia antirretroviral altamente efetiva (HAART), com características sugestivas de síndrome inflamatória de recuperação imune. Quanto à classificação de HIV segundo CDC, oito pacientes eram C3 e dois B3 e todos apresentavam contagem de linfócitos T CD4+ menor que 200 células/?l. A sorologia para leishmaniose foi positiva em todos os casos de LV mas, positiva em três dos cinco casos de LT. O teste de Montenegro foi negativo nos dois casos de LV onde o teste foi realizado e positivo em todos os três pacientes de LT submetidos ao teste. No tratamento da leishmaniose, freqüentemente os pacientes apresentaram efeitos adversos aos medicamentos e obervou-se mortalidade alta (40%) e freqüência alta de recidiva nos pacientes sobreviventes (100% dos LT e 66% dos LV) / In endemic areas of visceral leishmaniasis (VL), AIDS increases the risk for VL in 100 to 1000 times. There are many studies on VL in HIV/AIDS patients from the Mediterranean basin but data on tegumentary leishmaniasis (TL) are scarce. In Brazil, no increase in the number of cases of HIV/Leishmania co-infection was reported so far, but it may be due to subdiagnosis owing to various reasons including ignorance on clinical features of the co-infected patients.. Since the prevalence of TL is greater than VL in Brazil, we presume that cutaneous or mucosal leishmaniasis would be frequent in cases of coinfection in our country. Since there are no studies on systematic description of clinical manifestations of HIV/Leishmania co-infection in Brazil, an important basis for diagnosis, study was done for a period of two years from February 2002 aiming description of clinical manifestations in a transversal study in two infectious disease reference centers in Sao Paulo city, Brazil. The target population was constituted by 113 HIV positive patients that reported some kind of contact with endemic area for leishmaniasis, 69 being with some symptoms suggestive of visceral or tegumentary leishmaniasis, and 44 asymptomatic. In ten patients diagnosis of leishmaniasis was established by the detection of amatigotes or Leishmania antigens in tissue samples, five presented VL and five TL. Eight of 10 were found by active search. Based in these cases, we noted that contact with endemic area, even for short time like two weeks has to be considered in the diagnosis, mainly if it occurred recently. The search for these cases were difficult because leishmaniasis is not considered within differential diagnosis ofopportunistic infections in HIV positive patients among physicians, and in addition more prevalent opportunistic infections were concomitantly present and the clinical features were atypical for leishmaniasis or similar to other opportunistic infections. Clinical manifestation of VL was classic, while that of TL was quite variable, ranging from single to multiple ulcers, with polimorphic xviii presentations (papules, nodules, infiltrative plaques, macules) that tended to disseminate to the other areas of skin and mucosa, including genital area (in four from five TL patients). In two patients appearance of lesions (in one case) and worsening of tegumentary manifestations (in another case) after introduction of highly active anti-retroviral therapy (HAART) were observed that were suggestive of the so called immune reconstitution inflammatory syndrome. Concerning CDC classification of HIV, eight patients were C3 and two B3, and all presented CD4+ T cell count lower than 200/?l. All the VL patients had anti-Leishmania antibody test positive, however in TL three of five cases of was positive. Leishmanin skin (Montenegro) test was negative in two cases of VL submitted to this test, and positive in all three TL patients evalued. Frequent toxicity and adverse events with treatments were observed, as well as high mortality (40%), and high frequency of relapses among survivors (100% of TL and 66% of VL cases)

Vigilância da leishmaniose visceral americana no município de São Paulo / Surveillance of American visceral leishmaniasis in São Paulo municipality

Introdução: As leishmanioses são doenças que se apresentam como um sério obstáculo ao desenvolvimento socioeconômico. A leishmaniose visceral americana (LVA) tem como agente etiológico a Leishmania (Leishmania) infantum chagasi, como reservatório doméstico principal o cão e os vetores são flebotomíneos (Diptera:Psychodidae). Objetivo: investigar a vulnerabilidade do município de São Paulo para a ocorrência de LVA, utilizando os parâmetros de casos humanos notificados, presença do vetor e infecção em reservatórios domésticos. Método: A área de estudo compreende a região dos distritos administrativos do Anhanguera, Jaraguá e Perus (município de São Paulo) que incluem o Parque do Anhanguera e o Parque estadual do Jaraguá. A justificativa para o estudo nessa área é o eixo viário oeste-leste de expansão da LVA no estado de São Paulo. Foram levantados notificações humanas de LVA entre os anos de 2007 a 2014, para análise espacial pelos programas SaTScan e Terra View, num total de 140 e nenhuma dessas é considerada autóctone. Realizamos levantamento entomológico nos parques Anhanguera e Jaraguá. A análise estatística foi realizada pelos cálculos da Média geométrica de Williams, pelo índice de diversidade de Shannon-Weaver (H) e a dominância pelo índice de Pielou (J) também foi calculado o índice de Abundância de espécies padronizado (IAEP). Realizamos inquérito canino amostral e obtivemos 126 amostras de sangue canino que foram submetidas aos testes EIE. Foram colhidas amostras de Swab bucal do cão sororreagente e contactantes, as quais foram analisadas por PCR (RFLP), juntamente ás amostras de coágulos sanguíneos dos cães dos parques Anhanguera e Jaraguá num total de 31 amostras. Resultado: A análise espacial mostrou Clusters nos distritos do Brás e Guaianases (p<0,05) e, a espaço temporal em Raposo Tavares e Parelheiros (p<0,05). Os distritos do Carrão, Cachoeirinha e José Bonifácio formaram clusters secundários (p>0,05). Foram capturados 3660 espécimes de flebotomíneos identificadas 16 espécies de flebotomíneos e as mais frequentes na amostra foram: Pintomyia fischeri (82,46 por cento ), Psathyromyia pascalei (5,57 por cento ), Migonemyia migonei (5.25 por cento ) e Expapillata firmatoi (3.22 por cento ). Dentre as espécies capturadas, está uma fêmea de espécie similar à Lutzomyia longipalpis. O inquérito canino sorológico apontou um sororreagente no distrito de Perus. Do total de 31 amostras analisadas por PCR, 10 foram positivas para Leishmania sp. Foi possível chegar á espécie em seis delas, três positivas para a espécie L. (V.) shawi ou L (V.) lainsoni e três para L. (L.) infantum chagasi. Conclusões: A análise espacial das notificações de LVA humana dá direcionamento para investigações de uma possível transmissão autóctone no município. Há elos epidemiológicos que sugerem que as leishmanioses tegumentar e visceral caninas se tratam de doenças emergentes no município. Possivelmente, os casos caninos encontrados na região de estudo sejam parte da disseminação do eixo oeste-leste da leishmaniose visceral no Estado de São Paulo, pelo eixo viário. Palavras-chave: Leishmaniose visceral canina; Leishmaniose tegumentar. / Introduction: The leishmaniases are diseases which are a serious obstacle to social economic development. American visceral leishmaniasis (AVL) has Leishmania (Leishmania) infantum chagasi as its etiological agent, the domestic dog is its main reservoir and sand flies (Diptera, Psychodidae) its vectors. Objective: to investigate the vulnerability of São Paulo municipality to the occurrence of AVL, using the parameters of notified human cases, the presence of the vector and infection in domestic reservoirs. Method: The study area covers the region of the distinct administrative districts of Anhanguera, Jaraguá and Perus (São Paulo municipality) which include the Parque do Anhanguera and the Parque Estadual do Jaraguá. The reason for undertaking the study in this area is the existence of the roads linking the west and east of São Paulo state along which AVL has expanded eastwards. A survey was made of the 140 human cases (none of which was considered autochthonous) of AVL notified from 2007 to 2014, for the spatial analysis of the SaTScan and Terra View programs. An entomological survey was conducted in the two parks mentioned. The statistical analyses of the insects were undertaken using Williams geometrical means, Shannon-Weavers diversity index (H), Pielous evenness index (J) and the standardized species abundance index (SSAI). A canine sampling inquiry was conducted and 126 blood samples were submitted to the Enzyme-Linked Immunosorbent Assay (ELISA) test. A total of five samples of mouth swabs of the serum positive dog and four of its contacts and also blood clot samples of a total of 26 dogs from the two parks were analyzed by PCR (RFLP). Results: The spatial analysis showed clusters in the Brás and Guaianases (p<0.05) districts and the temporal/spatial analysis in Raposo Tavares and Parelheiros (p<0.05) districts. The Carrão, Cachoeirinha and José Bonifácio districts formed secondary clusters (p>0.05). A total of 3,660 specimens of sand flies were collected, representing 16 species, Pintomyia fischeri (82.46 per cent ), Psathyromyia pascalei (5.57 per cent ), Migonemyia migonei (5.25 per cent ) and Expapillata firmatoi (3.22 per cent ) being the most frequent. One female of a species similar to Lutzomyia longipalpis was captured. The canine serological enquiry showed one reagent in the Perus district. Of the total of 31 samples analyzed by PCR, 10 were positive for Leishmania sp., among which six were identified, three as species L. (V.) shawi/L (V.) lainsoni and three as L. (L.) infantum chagasi. Conclusions: The spatial analysis of the AVL human cases notified suggests the direction to be followed for future investigation of a possible autochthonous transmission in the municipality. There are epidemiological links which suggest that the canine cutaneous and visceral leishmaniases are emergent diseases in the municipality. Possibly the canine cases of visceral leishmaniasis found in this study region are part of its dissemination along the west-east axis of the diseases expansion in the state of São Paulo along the highways.

Introdução. As leishmanioses são doenças infecciosas, causadas por protozoários do gênero Leishmania, transmitidos por insetos da subfamília Phlebotominae (Diptera, Psychodidae). No Estado de São Paulo a incidência da leishmaniose tegumentar americana (LTA) e da leishmaniose visceral americana (LVA) está aumentando tanto em regiões rurais como urbana e periurbanas. O Parque Estadual da Cantareira (PEC) é uma Unidade de Conservação e abrange parte da Serra da Cantareira considerada a maior floresta urbana nativa do mundo e abrange parte dos municípios de São Paulo, Caieiras, Mairiporã e Guarulhos. Objetivos. Investigar a incidência de leishmaniose nos municípios que incluem áreas do Parque Estadual da Cantareira e identificar a fauna de flebotomíneos (Diptera, Psychodidae, Phlebotominae) e seu comportamento no Parque Estadual da Cantareira (PEC) e Parque Estadual Alberto Löfgren (PEAL) e área do entorno, inferir sobre as espécies importunas ao homem e que atuam na transmissão de leishmaniose na região. Metodologia. Estimou-se a incidência das leishmanioses na população humana por meio de dados obtidos no Centro de Vigilância Epidemiológica do Estado de São Paulo (CVE/SP) e Instituto Brasileiro de Geografia e Estatística (IBGE). Para o estudo dos insetos as 6 capturas foram feitas com armadilhas automáticas luminosas (CDC) e de Shannon branca e preta entre janeiro a dezembro de 2009 em ambiente de mata e peridomicilio nos dois parques e também no seu entorno, município de Mairiporã, para se obter informações sobre abundância, riqueza, diversidade, equitabilidade ,similaridade, sazonalidade das espécies e infecção natural por flagelados, também foi investigado ritmo horário noturno, comparação de atratividade da armadilha Shannon branca e preta e antropofilia das espécies. Resultados. Não houve casos de LVA notificados para a região dentro do período estudado. O coeficiente de incidência médio acumulado/100.000 hab/ano (CI) para LTA no período de 2000 a 2009 para os municípios da região estudada foi Mairiporã (2,08), Caieiras (0,03), Guarulhos (0,008) e São Paulo (0,03). A fauna flebotomínea compreendeu 12 espécies: Brumptomyia cardosoi, Brumptomyia carvalheiroi, Expapillata firmatoi, Migonemyia migonei, Pintomyia bianchigalatiae, Pintomyia fischeri, Pintomyia monticola, Psathyromyia lanei, Psathyromyia pascalei, Psathyromyia pestanai, Psychodopygus ayrozai e Psychodopygus lloydi. Foram obtidos os seguintes valores para índice de abundância de espécie padronizado: Pi. fischeri (0,813), Mg. migonei (0,571), Br. cardosoi (0,396), Pa. pascalei (0,341) e Ps. ayrozai (0,308). A similaridade das espécies de mata e peridomicilio foi de 0,59. Os ecótopos de mata e peridomicílio com os maiores índices de riqueza e diversidade foram respectivamente, Trilha das Figueiras e em um canil de um morador do PEC. Nas armadilhas Shannon branca e preta capturou-se uma média de 217,46 insetos/armadilha e 33,45 insetos/hora; a razão de fêmea/macho de Pi. 7 fischeri foi (preta=15,08:1,0; branca = 11,43:1,0), Mg. migonei (preta = 0,57:1,0; branca = 1,65:1,0). Fêmeas de Pi. fischeri foram significativamente mais atraídas para Shannon preta (2 = 8,3; p < 0,005) e Mg. migonei para branca (2 = 59,8; p < 0,001). Picos horários de atividade para fêmeas de Pi. fischeri e Mg. migonei foram diferentes em relação às estações do ano: Pi. fischeri foi mais atraída pelas armadilhas Shannon preta e branca na primeira metade da noite durante o verão, outono e inverno e na primavera ocorreu o oposto. As análises de correlação de Pearson mostraram uma correlação positiva e estatisticamente significante em relação as média das temperaturas mensais de 7 e 15 dias anteriores ao dia de captura. Nenhuma das 60 fêmeas dissecadas apresentou flagelados. Conclusões. O município de Mairiporã em comparação com os municípios abrangidos pelo PEC tem um CI muito mais elevado. Para Brumptomyia carvalheiroi é o primeiro registro para a região da Serra da Cantareira. As altas freqüências de Pi. fischeri e Mg.migonei nas armadilhas de Shannon em peridomicílio do PEC permitem inferir sobre a antropofilia destas espécies, causando incômodo aos moradores e que podem estar implicadas na transmissão do agente da leishmaniose tegumentar na região / Introduction. Leishmaniases are infectious diseases caused by protozoa of the genus Leishmania, transmitted by insects of the subfamily Phlebotominae (Diptera, Psychodidae). In the São Paulo State the incidence of American visceral (AVL) and cutaneous leishmaniases (ACL) is increasing in rural, urban and periurban areas. The Parque Estadual da Cantareira (PEC) is a conservation area and covers part of the Serra da Cantareira, considered the largest native urban forest in the world and covers parts of the municipalities of São Paulo, Caieiras, Mairiporã and Guarulhos. Objectives. To investigate the incidence of leishmaniasis in the municipalities that include areas of the Serra da Cantareira and to identify the phlebotomine fauna and its behavior in PEC and the Parque Estadual Alberto Löfgren (PEAL) and the surrounding area, identifying species annoying to humans and which can transmit Leishmania spp. in the region. Methodology. We estimated the incidence of leishmaniasis in the human population using data obtained from the Epidemiological Surveillance Center of São Paulo (CVE/SP) and the Brazilian Institute of Geography and Statistics (IBGE). The insects were captured with automatic light traps from January to December 2009 in the 9 forest and peridomiciliary environments of PEC and its surroundings (in the municipality of Mairiporã) and also in forest of the PEAL, to obtain information on abundance, richness, diversity, evenness, and seasonal variations. In a peridomicile of the PEC captures with black and white Shannon traps were undertaken to: investigate the nocturnal activity per hourly period; the attractiveness of these colors; the natural infection of the sandflies by flagellates, and infer the anthropophily of the species. Results. No case of AVL had been notified in the area. The accumulated average incidence rate /100,000 in hab. (AAIR) for ACL during the period 2000 to 2009 for the municipalities studied was Mairiporã (2.08), Caieiras (0.03), Guarulhos (0.008) and São Paulo (0.03). The phlebotomine fauna comprised 12 species: Brumptomyia cardosoi, Brumptomyia carvalheiroi, Expapillata firmatoi, Migonemyia migonei, Pintomyia bianchigalatiae, Pintomyia fischeri Pintomyia monticola, Psathyromyia lanei, Psathyromyia pascalei, Psathyromyia pestanai, Psychodopygus ayrozai and Psychodopygus lloydi. Pi. fischeri was the most abundant species, the standardized index of abundance of species (SIAS) = 0.813, followed by Mg. migonei (SIAS = 0.571), Br. cardosoi (0.396), Pa. pascalei (0.341) and Ps. ayrozai (0.308). The similarity of species in the forested and peridomiciliary environments was 0.59. The ecotypes of forest and peridomicile with the highest levels of richness and diversity were respectively, the Figueiras trail and a kennel near a residents house. In the black and white Shannon traps averages of 217.46 insects/trap and 33.45 insects/hour were captured, the female/male rate of Pi. fischeri being, in black, 15.08:1.0 and, in white, 11.4:1.0, of Mg. migonei , 10 in black ,0.6:1.0 and, in white, 1.7:1.0. Comparing the attractiveness of the black and white Shannon traps for females, those of Pi. fischeri were significantly more attracted to the black (2 = 8.3, p < 0.005) and Mg. migonei to the white (2 = 59.8, p <0.001). Females of Pi. fischeri were more attracted to black and white Shannon traps during the first half of the night in summer, autumn and winter and in the spring it was the opposite. The Pearsons correlation test showed a statistically significant positive correlation of the average monthly temperatures during the 7 and 15 days immediately preceding the day of capture. None of the 60 females dissected presented flagellates. Conclusions. The municipality of Mairiporã presented a much higher AAIR of ACL than the municipalities covered by PEC. This is the first record of Br. carvalheiroi for the region of Serra da Cantareira. The high frequencies of Pi. fischeri and Mg.migonei in Shannon traps in the peridomicile are suggestive of their anthropophily, and account for the bother they cause residents and the danger of their possible implication in the transmission of ACL in the region
