Att leva med icke-malign prostataförändring : en litteraturöversikt / Living with a non-malignant prostatic condition : a literature review

Bakgrund   Icke-maligna prostataförändringar är ett utbrett samhällsproblem och en vanlig anledning till att människor söker sjukvård. Prostatit och benign prostatahyperplasi är två icke-maligna förändringar som kan te sig på olika sätt och kan orsaka lidande hos de drabbade, ofta genom ”lower urinary tract symptoms” och smärta som är vanliga symtom. Att lindra lidande är vårdens uppgift och en sjuksköterskas ansvar.     Syfte Syftet med denna litteraturöversikt var att belysa hur det är att leva med en icke-malign prostataförändring.    Metod  Metoden som användes var icke-systematisk litteraturöversikt där 18 artiklar analyserades.    Resultat Resultatet efter att ha analyserat de inkluderad studierna visade att icke-maligna prostataförändringar kan ha inverkan på samliv och relationer, ge upphov till känslor av skam, rädsla och oro, att det finns faktorer som kan vara mer eller mindre gynnsamma vid dessa tillstånd samt att det finns rum för förbättring vid mötet med sjukvården.   Slutsats Resultatet i denna litteraturöversikt visar på att leva med icke-maligna prostataförändringar och symtomen de ger upphov till kan ha en tydlig inverkan på drabbade personers liv och livskvalitet i ett flertal aspekter / Background Non-malignant prostatic conditions are a prevalent problem in society and a common reason as to why people seek medical care. Prostatitis and benign prostatic hyperplasia are two examples of non-malignant conditions that can appear in different ways and that could cause the afflicted person suffering, oftentimes through symptoms like pain and lower urinary tract symptoms. Alleviating suffering is an undertaking of health-care professionals’ and a nurse’s responsibility.     Aim The aim of this literature review was to illuminate what it is like to live with a non-malignant prostatic condition.    Method Non-systematic literature review where 18 articles were analyzed.     Results The results of the included and analyzed studies show that non-malignant prostatic conditions can influence intimate relationships, cause feelings of shame, fear and worry, that there are factors that can be more or less favourable in relation to these conditions and that there is room for improvement in the meeting with healthcare professionals.    Conclusions The results in this non-systematic literature review show that living with non-malignant prostatic conditions, and the symptoms they cause, can have a significant influence on the affected persons’ lives and quality of life in several aspects.

A Cross-sectional Exploration of Lower Urinary Tract Storage Symptoms Among a Sample of Female Undergraduate College Students

Thesis advisor: Katherine Hutchinson / Lower urinary tract (LUTS) storage symptoms, including overactive bladder (OAB) and urinary incontinence (UI), are common conditions among women with significant health and economic consequences. Much of the existing literature on LUTS focuses on older, often postmenopausal women, and there is limited research available about prevalence, incidence and severity of LUTS in young women. For many young women in the United States, the period from the late teens through early twenties coincides with the period of emerging adulthood and college enrollment. The unique factors influencing women at this stage of development may be influential in understanding prevalence and correlates of OAB and UI later in the life-course. The purpose of this cross-sectional descriptive survey-based study was to explore and describe the experience of urinary storage symptoms, specifically OAB and UI, among female undergraduate college students, and to identify associated factors. Qualtrics online platform was used to create and distribute the survey to a sample of 1,800 female college undergraduate students at a private Catholic university in the northeast. Two instruments previously used to assess LUTS, the ICIQ-FLUTS and LUTS Tool, were combined into the Urinary Symptoms Scale with a one-week recall. Twelve items assessed LUTS storage symptoms of OAB and UI. The final sample consisted of 456 female undergraduate college students with a mean age of 20.3-years-old. The sample was predominantly White non-Hispanic. Most commonly reported symptoms included urgency (47.6%), frequency (52.6%), urinary incontinence (21.3%), stress urinary incontinence (28.8%), and urge urinary incontinence (16.4%). Total severity scores were low and highly skewed towards the lower range (M = 3.31; SD = 3.91). Participants with symptoms, most commonly reported experiencing symptoms rarely or sometimes during the past week. Perceived bother from urinary symptoms mean scores were low (M = 1.77) but extended the full range on a 0 to 10 scale. In this study, perceived bother from urinary symptoms as well as perceived impact of urinary symptoms on activities of daily life (ADLs) were significantly associated with care-seeking and use of self-management strategies. Interestingly, LUTS storage symptom severity was not significantly associated with care-seeking, but it was related to use of self-management strategies in this population. Perception of overall health, history of constipation/IBS, sexual activity, delayed toileting behaviors, and premature toileting behaviors were significant in multivariate analyses when controlling for other factors. Further research on the relationship of these factors and LUTS storage symptoms is needed. This study represented a first step in understanding college women’s experiences with LUTS storage symptoms and identifying the unique personal, behavioral and environmental factors associated with LUTS. The study found that OAB and UI symptoms are common among female college undergraduates. In addition, a number of personal and behavioral factors were found to be associated with LUTS storage symptoms. Given that many health-related behaviors established during college years may persist later in adulthood, identifying experiences and influences of young women’s LUTS storage symptoms is important in informing future research and practice recommendations. / Thesis (PhD) — Boston College, 2018. / Submitted to: Boston College. Caracterização das disfunções miccionais em pacientes portadores do espectro da neuromielite óptica e suas associações com o comprometimento neurológico e a qualidade de vida / Voiding dysfunction in patients with neuromyelitis optica spectrum disorder and its association with neurological impairment and quality of life

INTRODUÇÃO: Neuromielite óptica (NMO) e suas formas limitadas são doenças desmielinizantes autoimunes do sistema nervoso central que acometem preferencialmente a medula espinhal e o nervo óptico. Várias formas clínicas do espectro da NMO (NMO-SD) tem sido descritas e incluem desde um evento único de mielite transversa longitudinalmente extensa (MTLE) à NMO recorrente. O comprometimento neurológico destes pacientes pode levar a diversas disfunções autonômicas, incluindo disfunção miccional. OBJETIVOS: Determinar a prevalência e as características dos sintomas do trato urinário inferior (STUI) e dos achados urodinâmicos em pacientes portadores de NMO-SD, e analisar suas associações com o grau de comprometimento neurológico e qualidade de vida (QV). MÉTODOS: Avaliamos 30 pacientes (23 mulheres e 7 homens) com diagnóstico estabelecido de NMO-SD, que foram convidados a participar do estudo a despeito de apresentarem ou não STUI. A avaliação neurológica foi realizada por meio da Escala Expandida do Estado de Incapacidade (EDSS), ressonância magnética de crânio e coluna vertebral, e dosagem de NMOIgG. Os STUI foram avaliados pelo Questionário de Avaliação da Bexiga Hiperativa (OAB-V8) e pelo Escore Internacional de Sintomas Prostáticos (IPSS). A qualidade de vida de forma geral foi avaliada pelo questionário de Satisfação com a Vida (LiSat-9). Todos os pacientes foram submetidos ao estudo videourodinâmico. RESULTADOS: A idade média dos pacientes foi de 41,1 ± 13,5 anos (intervalo de 13 a 70) e o tempo médio de duração da doença neurológica foi de 33,8 ± 30,8 meses (intervalo de 3 a 135). A avaliação neurológica mostrou pacientes com EDSS médio de 5,3 ± 1,8 (intervalo de 1 a 8,5). O escore médio do OAB-V8 foi de 17,5 ± 14,0 (intervalo de 0 a 40) e de 14,3 ± 10,6 (intervalo de 0 a 35) para o I-PSS. A média da QV geral medida pelo Lisat-9 foi de 38,9 ± 6,8 (intervalo de 26 a 49). Os STUI mais comuns foram urgência em 15 (50%) pacientes, noctúria em 15 (50%), jato urinário fraco em 15 (50%), intermitência em 14 (46,6%), esvaziamento incompleto em 13 (43,3%), hesitação em 13 (43,3%), aumento da frequência urinária em 13 (43,3%) e urge-incontinência em 5 (16%). Os achados urodinâmicos mais comuns foram hiperatividade detrusora (HD) com dissinergia detrusor-esfincteriana (DDE) em 11 (36,6%) pacientes, DDE sem HD em 7 (23,3%), HD sem DDE em 6 (20%), e incontinência urinária de esforço em 1 (3,3%). Cinco (16,6%) pacientes apresentaram estudo videourodinâmico sem anormalidades. Sintomas urinários medidos pelo IPSS e pelo OAB-V8 correlacionaram-se com maior comprometimento neurológico (r=0,42; p=0,018 e r=0,48; p=0,006 respectivamente). Pacientes portadores de DDE foram aqueles que mostraram maior comprometimento neurológico (p=0,027). Da mesma forma, pacientes dissinérgicos apresentaram maiores escores ao I-PSS (p=0,029) e ao OAB-V8 (p=0,008). Pacientes com maior comprometimento neurológico foram aqueles que apresentaram pior QV (r=-0,410; p=0,022). CONCLUSÃO: Encontramos alta prevalência de STUI e disfunção miccional em portadores de NMO, sendo DDE e a HD as anormalidades urodinâmicas mais frequentes. Dissinergia detrusor-esfincteriana e STUI se correlacionam com a gravidade da doença neurológica. Several clinical forms of NMO spectrum disorders (NMO-SD) have been described and range from a limited event of longitudinally extensive transverse myelitis (LETM) to relapsing NMO. The neurological damage in these patients may lead to a range of autonomic dysfunctions, including voiding dysfunction. OBJECTIVES: To determine the prevalence and characteristics of the lower urinary tract symptoms (LUTS) and the urodynamic findings in patients with NMO-SD and analyze their correlations with the level of neurological damage and quality of life (QoL). METHODS: We evaluated 30 patients (23 women and 7 men) with an established diagnosis of NMO-SD based on stringent criteria. All patients were invited to participate irrespective of the presence of LUTS. Neurological impairment was assessed with the Expanded Disability Status Scale (EDSS), magnetic resonance imaging of the brain and spinal cord and NMO-IgG status. LUTS were evaluated with the Overactive Bladder V8 (OAB-V8) questionnaire and by the International Prostate Symptom Score (I-PSS). Quality of Life was evaluated using the Life Satisfaction questionnaire (LiSat-9). All patients underwent videourodynamics, transabdominal urinary tract sonography, urine culture and serum creatinine levels. RESULTS: The mean age of the patients was 41.1 ± 13.5 years (range 13 to 70) and the mean time of neurological disease duration was 33.8 ± 30.8 months (range 3 to 135). Neurological evaluation showed a mean EDSS score of 5.3 ± 1.8 (range 1 to 8.5). The mean OAB-V8 score was 17.5 ± 14.0 (range 0 to 40) and the mean I-PSS score was 14.3 ± 10.6 (range of 0 to 35). Mean general QoL measured by the Lisat-9 was 38.9 ± 6.8 (range 26 to 49). The most common urinary symptoms were urgency in 15 (50%) patients, nocturia in 15 (50%), weak urinary stream in 15 (50%), intermittence in 14 (46.6%), incomplete emptying in 13 (43.3%), hesitation in 13 (43.3%), increased urinary frequency in 13 (43.3%) and urge-incontinence in 5 (16,6%). The most frequent urodynamic findings were detrusor overactivity (DO) with sphincter dyssinergia (DSD) in 11 (36.6%) patients, DSD alone in 7 (23.3%), DO without DSD in 6 (20%) and stress urinary incontinence (SUI) in 1 (3.3%). Five (16.6%) patients had normal findings. Voiding dysfunction assessed by I-PSS and OAB-V8 increased with the degree of neurological impairment (r=0.42; p=0.018 and r=0.48; p=0.006 respectively). Patients with DSD had significantly higher symptoms based in the I-PSS (p=0.029) as well as the OAB- V8 scores (p=0.008) and greater neurological impairment (p=0.027). Patients with more severe neurological impairment were associated with worse Qol (r=-0.410; p=0.022). CONCLUSION: We have shown a high prevalence of LUTS and voiding dysfunction, with DSD and DO as the main urodynamic findings. Detrusor-sphincter dyssinergia and LUTS correlates with more severe neurological impairment. The severity of neurological impairment correlates with QoL

Perfil da sexualidade e dos sintomas do trato urinário inferior em idosos não institucionalizados / Profile of sexuality and symptoms of lower urinary tract in non-institutionalized elderly

As disfunções urinárias ou sexuais do idoso são subnotificadas. Porém, são de alta prevalência e associadas a diversos prejuízos psicossociais. O presente estudo visa à identificação da prevalência dessas condições por meio de questionários. Utilizou-se o Androgen Deficiency in Aging Male (ADAM) para avaliação de função sexual do homem, questões sobre função sexual feminina e masculina e, o International Prostate Symptom Score (IPSS) para avaliação de função urinária masculina e feminina. Temos o objetivo de realizar uma investigação nos indivíduos não institucionalizados, acima de 60 anos de idade, para obtermos dados referentes à sua saúde sexual e urinária nas cidades: São Paulo, Campinas, Santo André, São Bernardo do Campo, São Caetano e Londrina. Foram distribuídos cerca de 6000 questionários, sendo utilizados para estudo 3425, com 1575 homens e 1850 mulheres. Em relação ao ADAM, obtivemos 92,49% dos 1385 avaliados com suspeita de apresentarem distúrbio androgênico do envelhecimento masculino (DAEM). Quanto à função sexual masculina, evidenciamos que 383 (32,19%) apresentavam queixas eretivas, 458 (37,54%) de ejaculação precoce e 790 (59,13%) admitiram a necessidade de tratamento para melhorar o desempenho sexual. Quanto à função sexual feminina, 1300 (74,07%) não praticam relação sexual e os principais motivos foram: falta de parceiro, falta de desejo sexual e problema de saúde do parceiro. Além disso, 988 (78,41%) das mulheres que não têm relação sexual admitem estar bem assim e não querem sexo e mais importante: aproximadamente 21% dessas gostariam de ter relação sexual. Quanto à qualidade da relação sexual feminina, 272 (64,92%) acham a relação boa para ambos, 105 (25,06%) boa só para o parceiro e 33 (7,88%) ruim para ambos. Em relação ao IPSS masculino, observamos piora gradual no padrão miccional com o aumento da idade, dos sintomas moderados e graves, principalmente após os 75 anos, sendo os mais prevalentes: nictúria, urgência miccional e aumento da frequência urinária. Quanto ao IPSS feminino, notamos que, mesmo após os 80 anos, a maioria das mulheres (53,37%) apresenta sintomas leves relacionados à disfunção miccional; com o aumento da idade, ocorre um aumento gradual do resultado do IPSS, relacionado com os sintomas moderados e graves, sendo que o pico ocorre após os oitenta anos. Sendo assim, devido ao grande número de distúrbios sexuais e urinários encontrados, deixamos um alerta para que um maior número de medidas de Saúde Pública sejam implantadas, melhorando promover uma melhor qualidade de vida nessa população idosa / Urinary or sexual dysfunction in the elderly are underreported. However, are highly prevalent and associated with various psychosocial damage. This study aims to identify the prevalence of these conditions. We used the Androgen Deficiency in Aging Male (ADAM) to assess sexual function of men, questions of male and female sexual function, and the International Prostate Symptom Score (IPSS) for evaluation the urinary function. The aim is to carry out an investigation in non-institutionalized individuals over 60 years of age, to obtain data on its sexual and urinary health in: São Paulo, Campinas, Santo André, São Bernardo do Campo, São Caetano and Londrina. They were distributed about 6,000 questionnaires being used to study 3425, with 1575 men and 1850 women. Compared to ADAM, obtained 92.49% of the 1385 evaluated suspected of presenting androgen disorder of aging male (ADAM). As for the male sexual function, we showed that 383 (32.19%) had erective complaints, 458 (37.54%) of premature ejaculation and 790 (59.13%) admitted the need for treatment to improve sexual performance. As for the female sexual function, 1300 (74.07%) did not practice sexual intercourse and the main reasons were: lack of partner, lack of sexual desire and partner health problem. In addition, 988 (78.41%) of women who haven\'t sexual intercourse admit to being like that and not want sex and, more importantly, about 21% of them would like to have sexual intercourse. As for the quality of the female sex, 272 (64.92%) think the relation is good for both, 105 (25.06%) only good for the partner and 33 (7.88%) bad for both. IPSS when compared to males, we see gradual worsening of urinary pattern with increasing age, related with the moderate and severe symptoms, especially after 75 years, being the most prevalents: nocturia, urinary urgency and urinary frequency. As for the female IPSS, we note that even after 80 years, the majority of women (53.37%) have mild symptoms related to voiding dysfunction; with increasing age there is a gradual increase in the result of the IPSS, associated with moderate and severe symptoms, where the peak occurs after the age of eighty. Thus, due to the large number of sexual and urinary disorders found, left a warning to a greater number of public health measures are implemented, improving promote a better quality of life in this elderly population

The most frequent urodynamic findings were detrusor overactivity (DO) with sphincter dyssinergia (DSD) in 11 (36.6%) patients, DSD alone in 7 (23.3%), DO without DSD in 6 (20%) and stress urinary incontinence (SUI) in 1 (3.3%). Five (16.6%) patients had normal findings. Voiding dysfunction assessed by I-PSS and OAB-V8 increased with the degree of neurological impairment (r=0.42; p=0.018 and r=0.48; p=0.006 respectively). Patients with DSD had significantly higher symptoms based in the I-PSS (p=0.029) as well as the OAB- V8 scores (p=0.008) and greater neurological impairment (p=0.027). Patients with more severe neurological impairment were associated with worse Qol (r=-0.410; p=0.022). CONCLUSION: We have shown a high prevalence of LUTS and voiding dysfunction, with DSD and DO as the main urodynamic findings. Detrusor-sphincter dyssinergia and LUTS correlates with more severe neurological impairment. The severity of neurological impairment correlates with QoL

Perfil da sexualidade e dos sintomas do trato urinário inferior em idosos não institucionalizados / Profile of sexuality and symptoms of lower urinary tract in non-institutionalized elderly

As disfunções urinárias ou sexuais do idoso são subnotificadas. Porém, são de alta prevalência e associadas a diversos prejuízos psicossociais. O presente estudo visa à identificação da prevalência dessas condições por meio de questionários. Utilizou-se o Androgen Deficiency in Aging Male (ADAM) para avaliação de função sexual do homem, questões sobre função sexual feminina e masculina e, o International Prostate Symptom Score (IPSS) para avaliação de função urinária masculina e feminina. Temos o objetivo de realizar uma investigação nos indivíduos não institucionalizados, acima de 60 anos de idade, para obtermos dados referentes à sua saúde sexual e urinária nas cidades: São Paulo, Campinas, Santo André, São Bernardo do Campo, São Caetano e Londrina. Foram distribuídos cerca de 6000 questionários, sendo utilizados para estudo 3425, com 1575 homens e 1850 mulheres. Em relação ao ADAM, obtivemos 92,49% dos 1385 avaliados com suspeita de apresentarem distúrbio androgênico do envelhecimento masculino (DAEM). Quanto à função sexual masculina, evidenciamos que 383 (32,19%) apresentavam queixas eretivas, 458 (37,54%) de ejaculação precoce e 790 (59,13%) admitiram a necessidade de tratamento para melhorar o desempenho sexual. Quanto à função sexual feminina, 1300 (74,07%) não praticam relação sexual e os principais motivos foram: falta de parceiro, falta de desejo sexual e problema de saúde do parceiro. Além disso, 988 (78,41%) das mulheres que não têm relação sexual admitem estar bem assim e não querem sexo e mais importante: aproximadamente 21% dessas gostariam de ter relação sexual. Quanto à qualidade da relação sexual feminina, 272 (64,92%) acham a relação boa para ambos, 105 (25,06%) boa só para o parceiro e 33 (7,88%) ruim para ambos. Em relação ao IPSS masculino, observamos piora gradual no padrão miccional com o aumento da idade, dos sintomas moderados e graves, principalmente após os 75 anos, sendo os mais prevalentes: nictúria, urgência miccional e aumento da frequência urinária. Quanto ao IPSS feminino, notamos que, mesmo após os 80 anos, a maioria das mulheres (53,37%) apresenta sintomas leves relacionados à disfunção miccional; com o aumento da idade, ocorre um aumento gradual do resultado do IPSS, relacionado com os sintomas moderados e graves, sendo que o pico ocorre após os oitenta anos. Sendo assim, devido ao grande número de distúrbios sexuais e urinários encontrados, deixamos um alerta para que um maior número de medidas de Saúde Pública sejam implantadas, melhorando promover uma melhor qualidade de vida nessa população idosa / Urinary or sexual dysfunction in the elderly are underreported. However, are highly prevalent and associated with various psychosocial damage. This study aims to identify the prevalence of these conditions. We used the Androgen Deficiency in Aging Male (ADAM) to assess sexual function of men, questions of male and female sexual function, and the International Prostate Symptom Score (IPSS) for evaluation the urinary function. The aim is to carry out an investigation in non-institutionalized individuals over 60 years of age, to obtain data on its sexual and urinary health in: São Paulo, Campinas, Santo André, São Bernardo do Campo, São Caetano and Londrina. They were distributed about 6,000 questionnaires being used to study 3425, with 1575 men and 1850 women. Compared to ADAM, obtained 92.49% of the 1385 evaluated suspected of presenting androgen disorder of aging male (ADAM). As for the male sexual function, we showed that 383 (32.19%) had erective complaints, 458 (37.54%) of premature ejaculation and 790 (59.13%) admitted the need for treatment to improve sexual performance. In addition, 988 (78.41%) of women who haven\'t sexual intercourse admit to being like that and not want sex and, more importantly, about 21% of them would like to have sexual intercourse. As for the quality of the female sex, 272 (64.92%) think the relation is good for both, 105 (25.06%) only good for the partner and 33 (7.88%) bad for both. IPSS when compared to males, we see gradual worsening of urinary pattern with increasing age, related with the moderate and severe symptoms, especially after 75 years, being the most prevalents: nocturia, urinary urgency and urinary frequency. As for the female IPSS, we note that even after 80 years, the majority of women (53.37%) have mild symptoms related to voiding dysfunction; with increasing age there is a gradual increase in the result of the IPSS, associated with moderate and severe symptoms, where the peak occurs after the age of eighty. Thus, due to the large number of sexual and urinary disorders found, left a warning to a greater number of public health measures are implemented, improving promote a better quality of life in this elderly population

Os sintomas do trato urinário inferior (LUTS) têm alta prevalência, ocorrendo em 50 % dos homens com mais de 50 anos e em 90% após 80 anos e está associado à hiperplasia prostática benigna (HPB) em 50% das vezes. LUTS e HPB têm sido associados mais recentemente à síndrome metabólica, a fatores de risco cardiovascular e ao baixo nível de atividade física, porém estas associações são complexas e nãe estão bem esclarecidas. A atividade física para ser adequada deve gerar melhora da aptidão cardiorrespiratória, sendo esta avaliada pelo volume máximo de oxigênio (Vo2max). O objetivo deste trabalho é avaliar a associação do LUTS com Vo2max, flexibilidade, o nível de atividade física e variáveis antropométricas em homens com 50 a 59 anos. PACIENTES E MÉTODOS. Foi realizado um estudo caso-controle no qual o LUTS foi considerado como o desfecho e o Vo2max, a flexibilidade, o nível de atividade física e variáveis antropométricas como a exposição. Foram avaliados 100 homens com idade entre 50 e 59 anos provenientes do ambulatório de especialidades clínicas do Hospital Universitário (HU-CAS) da Universidade Federal de Juiz de Fora, sendo alocados 49 pacientes no grupo caso (com LUTS) e 51 no grupo controle (sem LUTS). Os pacientes passaram pelo seguinte protocolo experimental: anamnese, avaliação do nível de atividade física pelo Questionário Internacional de Atividade Física (IPAQ) em sua versão curta, avaliação do LUTS pelo Questionário Internacional de Sintomas Prostáticos (IPSS), avaliação antropométrica com a medida do índice de massa corpórea (IMC), relação cintura quadril (RCQ) e circunferência abdominal. Para a avaliação do Vo2max foi realizado o Polar Fitness Test e para avaliar a flexibilidade foi utilizado o teste de Sentar e Alcançar. RESULTADOS: na análise bi-variada foi identificada associação estatisticamente significativa do LUTS com o nível de atividade física (OR = 0,37, IC95% = 0,16 a 0,84, p= 0,02); com o Vo2max (OR = 0,206, IC95% = 0,086 a 0,495, p<0,001) e com a flexibilidade (OR = 0,309, IC95% = 0,130 a 0,337, p= 0,01). Porém, o modelo multivariado adotado indicou que controlando o efeito da flexibilidade e do nível de atividade física, o Vo2max foi a única variável associada de forma estatisticamente significativa com o LUTS, com Odds ratio ajustada de 0,30 ( IC95% = 0,105 a 0,875, p= 0,027). CONCLUSÃO: Homens com idade entre 50 a 59 anos com Vo2max adequado, têm aproximadamente 70% a menos de chance de desenvolver LUTS. / INTRODUCTION. Lower urinary tract symptoms (LUTS) are highly prevalent, occurring in 50% of men over 50 years old and in 90% of men over 80, and is associated with benign prostatic hyperplasia (BPH) in 50% of the cases. LUTS and BPH have more recently been associated with metabolic syndrome, cardiovascular risk factors, and low physical activity level, but these associations are complex and not well understood. Physical activity, to be considered adequate, should generate improved cardiorespiratory fitness, which is assessed by maximal oxygen uptake (VO2max). The objective of this study is to evaluate the association of LUTS with VO2max, flexibility, physical activity level, and anthropometric variables in men aged 50 to 59 years. PATIENTS AND METHODS. We conducted a case-control study in which LUTS was considered as the outcome and VO2max, flexibility, physical activity level, and anthropometric variables as the exposure variables. We evaluated 100 men aged between 50 and 59 years from the outpatient specialty clinics of the University Hospital (HU-CAS), Federal University of Juiz de Fora, assigning 49 patients to the case group (with LUTS) and 51 to the control group (without LUTS). The patients underwent the following experimental protocol: anamnesis, physical activity level assessment using the International Physical Activity Questionnaire (IPAQ) in its short version, LUTS assessment using the International Prostate Symptom Score (IPSS) Questionnaire, anthropometric assessment measuring body mass index (BMI), waist-to-hip ratio (WHR), and waist circumference. For the assessment of VO2max, the Polar Fitness Test was performed, and to assess flexibility, the Sit and Reach test was used. RESULTS: In bivariate analysis a statistically significant association was identified for LUTS with physical activity level (OR = 0.37, 95% CI = 0.16 to 0.84, p = 0.02); with VO2max (OR = 0.206, 95% CI = 0.086 to 0.495, p <0.001); and with flexibility (OR = 0.309, 95% CI = 0.130 to 0.337, p = 0.01). However, the multivariate model adopted indicated that, controlling for the effect of flexibility and of physical activity level, VO2max was the only statistically significant variable associated with LUTS, with an adjusted odds ratio of 0.30 (95% CI = 0.105 to 0.875 , p = 0.027). CONCLUSION: Men aged 50 to 59 years, with adequate VO2max, have approximately a 70% less chance of developing LUTS.

O presente estudo tem como objetivo estimar a idade de início e conclusão do treinamento esfincteriano nas crianças com síndrome de Down, comparando-as com crianças neurotípicas, assim como avaliar o método de treinamento esfincteriano usado e a associação com sintomas do trato urinário inferior e constipação intestinal funcional. Pacientes e métodos: Foi realizado um estudo caso-controle de 2010 a 2015. Todos os pais ou responsáveis responderam a um questionário elaborado para avaliar qual o processo de treinamento esfincteriano foi utilizado.Os sintomas do trato urinário inferior foram avaliados por meio da aplicação da versão validada e adaptada do Dysfunctional Voiding Symptom Score para a população brasileira. A presença de constipação intestinal funcional foi avaliada de acordo com os critérios de Roma III. Resultados: O estudo incluiu 93 crianças com síndrome de Down e 204 crianças neurotípicas (Grupo Controle). A idade média para iniciar o treinamento esfincteriano foi 22,75 meses nas crianças com síndrome de Down e 17,49 meses no grupo controle (p= 0,001). Em crianças com síndrome de Down, a idade média para concluir o treinamento esfincteriano foi de 56,25 meses e 27,06 meses no grupo controle (p= 0,001). As meninas com síndrome de Down completaram o treinamento esfincteriano mais precocemente (p= 0,02). O método de treinamento esfincteriano mais usado foi abordagem orientada para a criança. Não houve associação com a presença de sintomas do trato urinário inferior ou constipação intestinal funcional e a idade de início e de conclusão do treinamento esfincteriano em ambos os grupos. Conclusão: As crianças com síndrome de Down apresentaram um tempo prolongado de treinamento esfincteriano, sendo que as meninas concluíram o processo mais cedo. Estudos de coorte são essenciais para obter uma melhor avaliação sobre o processo de treinamento esfincteriano em crianças com síndrome de Down. / Introduction: Children with Down syndrome have delayed psychomotor development, which determines the level of difficulty in toilet training. The current study aims to estimate at what age they start and complete toilet training compared to children with typical psychomotor development, as well as to evaluate the toilet training method used and any association with lower urinary tract symptoms and functional constipation. Patients and methods: A case-control study was carried out from 2010 to 2015. All parents completed a questionnaire designed to assess the toilet training process. Lower urinary tract symptoms was assessed through the application of the Dysfunctional Voiding Symptom Score. The presence of functional constipation was evaluated according to the Rome III criteria. Results: The study included 93 children with Down Syndrome and 204 children with normal psychomotor development (Control Group). The average age children started toilet training was 22.75 months in those with Down Syndrome and 17.49 months in the Control Group (p= 0.001). In children with Down Syndrome, the average age when completing toilet training was 56.25 months and 27.06 months in the Control Group (p= 0.001). Among children with Down Syndrome, females completed toilet training earlier (p= 0.02). The toilet training method used most often was child-oriented approach in both groups. There was no association with the presence of lower urinary tract symptoms or functional constipation and the age of begin and complete toilet training in both groups. Conclusion: Children with Down Syndrome experiencedprolonged toilet training time. Females with Down Syndrome complete toilet training earlier. Cohort studies are essential to gain insight into thetoilet training process in children with Down Syndrome.

Urogenital function in morbidly obese men following bariatric surgery

Introduction: Obesity has been suggested to be one of the risk factors for erectile dysfunction (ED) and lower urinary tract symptoms (LUTS). Bariatric surgery has been used for the treatment of obesity and has been suggested to have a significant impact on obesity-related conditions such as diabetes mellitus, ED and LUTS. Previous studies have investigated the effect of bariatric surgery on erectile and urological function in obese men; however those studies used long-term time points post-operatively (more than 1 month). Since it is now known that bariatric surgery can potentially induce glycaemic improvement within one week independent of weight loss, this study aimed to investigate the short-term effect in order to test the hypothesis as to whether improvement in urogenital function after bariatric surgery is due to weight loss or whether it is due to glycaemic improvement. Aim: To evaluate the baseline characteristics of patients with erectile dysfunction and to determine the early effects of bariatric surgery on erectile and urological function in morbidly obese men.
