Este trabalho objetivou repensar a prática da Terapia Ocupacional com esta criança à luz de considerações teóricas e conceitos da psicanálise. Duas questões direcionaram o percurso da reflexão e da escrita: em que momento fazer valer a voz da especialidade Terapia Ocupacional na clínica interdisciplinar, institucional e pública à criança autista? A Terapia Ocupacional, atravessada por conceitos e noções da psicanálise como sujeito, pulsão, transferência e estatuto do Outro propõe intervenções possíveis a criança autista sem que seja entendida como uma aplicação da psicanálise? Acredita-se que a escolha do referencial teórico da psicanálise para sustentar a prática e a escrita deste trabalho resultou em novo lugar que sustenta a suposição de sujeito à clínica da Terapia Ocupacional com a criança autista. Este caminho de reflexão foi conduzido por revisões bibliográficas de teorias da prática de Terapia Ocupacional e por leituras de autores da psicanálise como Freud, Lacan, Dolto, Winnicott e colaboradores contemporâneos que abordam especificamente a clínica com crianças autistas. O objetivo não foi aprofundar nas teorias e conceitos propostos por estes autores, mas sim, compor um caminho possível de escrita pelas supostas superfícies de contato entre os dois campos, baseado em minha prática clínica cotidiana como terapeuta ocupacional da equipe do Centro de Atenção Psicossocial da infância e da Adolescência de Brasília. _______________________________________________________________________________________ ABSTRACT / What does the Occupational Therapy clinic have to say (or write) about an autistic child? The objective of this work is to promote the practice of occupational therapy with this child based on theoretical considerations and psychoanalysis concepts. The two main questions that directed this research are: At which point should the voice of the occupational therapy specialty start to be factored into the interdisciplinary and institutional clinic of the autistic children ? Occupational Therapy, crossed by concepts and psychoanalysis notions, like subject, pulsion, transfers and statute of Other makes a proposition of possible interventions to the autistic child without being understood how the “application” of psychoanalysis? It was believed that the choice of theorical reference of psychocanalysis to support the practice and the writings of this work resulted in a new place that supports a supposition of the subject suposto saber , important notion for the clinic with the autistic child. This way of reflection was conducted by bibliographic revisions of theories of the practice or occupational theerapy and by readings of psychoanalysis authors such as Freud, Lacan, Dolto, Winnicott and other contemporary collaborators that specifically deal with clinics autistic children. The main objective was no to get deeper in theories and concepts proposed by these authors, but instead, to build a way of writing by the supposedly “surfaces of contact” between the two fields, based in my daily clinical practice as an occupational therapist on the team from the Center of Psychosocial Attention of children and adolescents of Brasília.

A partir do referencial teórico de autores deste eixo e também, de autores da psicanálise, aqui se apresenta a intenção deste estudo. “O Meu Chapéu Tem Três Pontas” é uma pesquisa qualitativa com estudo de caso de uma pré-escola, e tem como objeto, o sujeito do inconsciente que enlaça a educação infantil, o cantar e a psicanálise. Através da pergunta “por que se canta na pré-escola?”, pretende-se alcançar o objetivo de clarear esse sujeito que atravessa a educação, em especial a infantil, curvando-se para escutar o sujeito em constituição que canta e fala no aluno pré-escolar de uma instituição de educação infantil numa cidade do Distrito Federal, sem desconsiderar o adulto, comprometido com a educação destas crianças. A escuta se faz a partir da observação do contexto escolar como o grande Outro que acolhe e de uma turma pré-escolar desta instituição, também, através de entrevista semi-estruturada com diferentes grupos que compõem a comunidade escolar – alunos, equipe pedagógica, equipe de assistência à educação e responsáveis –, além da análise do projeto da escola. A hipótese que se formula é de que se canta porque, apesar do pedagógico e de toda a força consciente que ele exerce na educação, o sujeito do inconsciente está se constituindo e insiste em falar. Assim, o cantar e as canções assumem o lugar de linguagem, e não de um acessório que fala de um tempo lógico, de uma ciência que define etapas a seguir no desenvolvimento infantil. Os resultados do estudo mostram que o cantar e as canções remetem os personagens da pré-escola para uma fantasia de sedução necessária neste momento, mas que, também existe algo que vai mais além. O cantar e as canções alcançam o mundo do trauma e possibilitam efeitos curativos. Mais do que uma atividade, o cantar é o fio condutor que remete os personagens ao jogo simbólico, assume o lugar de linguagem e que, ainda assim, encontra resistências que dificultam sua presença e permanência na pré-escola. A partir desses resultados o presente estudo considera que é preciso repensar esta etapa da educação e a formação de seus profissionais, apontando para as contribuições que a psicanálise pode oferecer. Ouvir e refletir sobre um fazer da pré-escola foi um momento de descoberta de um sujeito que se pensa esconder, mas que não se pode controlar e que se mostra ao cantar. ________________________________________________________________________________________ ABSTRACT / Scholars axle education and psychoanalysis consider that it is the latter, the function of providing a small light in the field of education. The listening is done from the observation of the school context as the big Other which hosts and a preschool class of this institution, too, through semi-structured interviews with different groups that make up the school community - students, team teaching, team assistance to education and responsible - in addition to the analysis of the school project. The hypothesis formulated is that it sings because, despite the educational and awareness throughout the force he exerts on education, the subject of the unconscious is forming and keeps on talking. Thus, the singing and the songs take the place of language, not an accessory that speaks of a logical time, a science that defines the following steps in child development. The study results show that the singing and the songs refer to characters from pre-school to a fantasy of seduction needed right now, but there is also something that goes further. The singing and the songs reach the world of trauma and possible curative effects. More than one activity, the singing is the thread that refers to the characters symbolic play, takes the place of language and still finds resistance that hinder their presence and stay in preschool. From these results the present study finds it necessary to rethink this stage of education and training of its professionals, highlighting the contributions that psychoanalysis can offer. Uma leitura da produção brasileira sobre psicanálise de crianças, por meio da Revista brasileira de psicanálise /

No Brasil, essa modalidade de atendimento psicanalítico começou a ser difundida a partir da década de 1930 e acabou se disseminando em práticas institucionais bem estruturadas, culminando em produção teórica expressiva. Mas é sob os auspícios das Sociedades de Psicanálise que esse ramo da ciência psicanalítica encontrou maior ressonância para sua aplicação, divulgação, formação de analistas infantis e produção teórica, que apesar de não vultosa, tem se mostrado altamente expressiva. Importante veículo de divulgação do pensamento psicanalítico brasileiro, que nos permite acompanhar a evolução da produção sobre Psicanálise de Crianças no Brasil, abrangendo as últimas décadas, é a Revista Brasileira de Psicanálise. O periódico tem como principal objetivo abranger a pluralidade dos diferentes segmentos científicos existentes nos quadros das Sociedades de Psicanálise, garantindo, assim, sua expressão no âmbito científico. A presente pesquisa consiste em um estudo qualitativo de natureza histórica que busca circunscrever um segmento da psicanálise brasileira, cujo objetivo é caracterizar a produção sobre Psicanálise de Crianças no Brasil no período de 1967 a 2007, por intermédio dos artigos de autores nacionais publicados na Revista Brasileira de Psicanálise. In Brazil, that sort of psychoanalytic practice was introduced from 1930 on as well-structured institutional practices, achieving a significant theoretical production. But it was under the auspices of the Psychoanalytic Societies that such a branch of the psychoanalytic science had a greater diffusion for its application, propagation, education background of child analysts and theoretical production which, although it is not significant, it has proven to be highly expressive. An important means of diffusion of the Brazilian psychoanalytic thought, which allows us to follow the evolution of works written on Child Psychoanalysis in Brazil, comprising the last decades, is Revista Brasileira de Psicanálise. That journal was carried out to comprise the plurality of different scientific branches found within the Societies of Psychoanalysis, thus ensuring their expression in the scientific field. The research at issue is a qualitative-based study of historical nature which tries to bound a segment of the Brazilian psychoanalysis, whose aim is to characterize the works written on Child Psychoanalysis in Brazil from 1967 to 2007, by means of papers written by Brazilian writers published in Revista Brasileira de Psicanálise. Algumas reflexões sobre as vozes do sofrimento psíquico na infância e na família contemporânea /

Partimos da hipótese de que as mudanças no cenário externo reverberam na formação subjetiva dos indivíduos, pois, sendo seres biopsicossociais, têm o atravessamento da cultura como marca intrínseca. Sendo assim, o objetivo principal desta tese é lançar um olhar para as manifestações do sofrimento psíquico infantil na atualidade, a partir da escuta do discurso das famílias que buscam atendimento no contexto da saúde pública, num ambulatório de saúde mental infantil do interior do estado de São Paulo. Para tanto, foi utilizado como método de coleta de dados, a entrevista conjunta e para a escrita das histórias, as narrativas psicanaliticamente orientadas. Compreendemos assim que as mudanças nas estruturas familiares que, atualmente vivenciam a realidade da formação monoparental feminina assim como a fragilização da figura paterna modificam o ambiente da criança em duplo sentido, pois a relação entre mundo interno e externo encontra falhas primordiais e repetidas para a construção do si mesmo e da continuidade do ser. Assim, refletimos sobre as condições destas famílias em ancorarem o desenvolvimento psíquico destas crianças. (Resumo completo, clicar acesso eletrônico abaixo) / Abstract: Since the earliest studies of childhood psychotic suffering, many questions have been raised, especially in the economic and social order, which also leads to changes in the formation of the symbolic universe of children. We start from the hypothesis that the changes in the external scenario reverberate in the subjective formation of the individuals, because, being biopsychosocial beings, they have the crossing of the culture like intrinsic mark. Thus, the main objective of this thesis is to take a look at the manifestations of childhood psychic suffering in the present time, from the psychoanalytically oriented listening in the context of public health, in a mental health outpatient clinic in the interior of the state of. In order to do so, it was used as method of data collection, the joint interview and for the writing of the stories, the psychoanalytically oriented narratives. We thus understand that the changes in family structures that currently experience the reality of female one-parent formation as well as the weakening of the paternal figure modify the child's environment in two directions, since the relationship between internal and external world encounters primordial and repeated failures for the construction of the self and of the continuity of being. Concomitant to this, there is a scenario in which medication and traditional psychiatry are strongly established, making psychoanalytic listening appear as an important working tool to understand the symptom beyond the nosological description, but as a path that points to a type of suffering existence. This discussion has become an important thermometer for reflection on contemporaneity as an environment in which children perform their symbolic exchanges. Freud was able to produce a science that could rethink the formation and the constitution of a subjective knowledge, focused on the essence of the subject and the affective relations that surround it. The school is a great example of this paradigm. It is in school that many children spend most of their time learning to deal with external conflicts in an attempt to mature their own inner struggles. This interiority was so well explored by Melanie Klein. Its vast theoretical framework, based on the exploration of the infantile inner world and its respective constitutive fantasies, is a great watershed for the understanding of the human being within psychoanalytic epistemology. This research will start from the experiences of the author as an educator and clinical psychoanalyst, in an attempt to articulate the kleinian concepts to the emotional situations witnessed in this professional and personal journey. It is not an instruction manual on how to put Melanie Klein s theory into practice, but rather a broad understanding of its general concepts in order to facilitate an understanding of the complex affective situations that permeate the field of school education. Given the character of this dissertation, there is no way to present closed conclusions, but rather to open the way to educational practices that follow an ethics of the subject, considering children and educators as subjective beings, consisting of emotional experiences, irremediably incomplete and with a character incredibly broad and comprehensive that can not (and should not) be simplified by a theory that sees it simplistically / Muito se discute a respeito das contribuigoes da psicanalise para as mais diversas areas do conhecimento. Freud foi capaz de produzir uma ciencia que pudesse repensar a formagao e a constituigao de um saber subjetivo, voltado a essencia do sujeito e as relagoes afetivas que o circundam. E na escola que muitas criangas passam a maior parte de seu tempo, aprendendo a lidar com conflitos externos, na tentativa de amadurecer seus pr6prios embates internos. lnterioridade essa que foi tao bem explorada por Melanie Klein. Seu vasto arcabougo te6rico que teve como embasamento a exploragao do mundo interno infantil e suas respectivas fantasias constituintes, e um grande divisor de aguas para a compreensao do ser humano dentro da epistemologia psicanalftica. Essa pesquisa partira das vivencias do autor como educador e psicanalista clfnico, na tentativa de articular os conceitos kleinianos as situagoes emocionais presenciadas nesse percurso profissional e pessoal. Nao se trata de um manual de instrugoes sobre como colocar em pratica a teoria de Melanie Klein, mas sim de uma compreensao ampla de seus conceitos gerais com o objetivo de facilitar o entendimento das complexas situagoes afetivas que permeiam o campo da educagao escolar. Dado o carater dessa dissertagao, nao ha como apresentar conclusoes fechadas, mas sim de abrir caminhos para praticas educacionais que sigam em diregao a uma etica do sujeito, considerando criangas e educadores como seres subjetivos, constitufdos de experiencias emocionais, irremediavelmente incompletos e com um carater incrivelmente amplo e abrangente que nao pode (nem deve) ser simplificado por uma teoria que o enxergue de maneira simplista

As formas da alma : psicanálise, estética e criação

Tese (doutorado)—Universidade de Brasília, Instituto de Psicologia, Programa de Pós-graduação em Psicologia Clínica e Cultura, 2018. / Coordenação de Aperfeiçoamento de Pessoal de Nível Superior (CAPES). / Este trabalho inscreve-se na tradição psicanalítica e tem como tema a dimensão estética da vida psíquica humana, compreendendo tal dimensão como parte primordial da atividade da psique. Chamamos de dimensão estética todos os aspectos da vida psíquica que se apresentam por meio de formas e/ou de padrões formais. Inclui ritmos corpóreos; aspectos da relação do sujeito com objetos e fenômenos sensoriais, como sons; criação de imagens nos sonhos e brincadeiras infantis, no caso das crianças; aspectos sonoros e poéticos da linguagem; relações do sujeito com os objetos culturais, etc. A partir de observações clínicas da autora desta tese, em interlocução com teorias psicanalíticas que enfatizam tal vértice estético, notou-se que a dimensão estética da vida emocional comparece na clínica psicanalítica, podendo ser um fio condutor central da sustentação do acontecimento clínico. Assim, aspectos não verbais podem ser observados nas comunicações dos pacientes, que comunicam por meio de formas e não de sentidos discursivos. Considerando estes pontos, a partir de três construções de casos clínicos, em contextos diferenciados e variações do setting, este trabalho se propõe a pensar uma mesma questão principal, defendendo que a capacidade do analista de sustentar a comunicação estética presentificada no setting é o que possibilita a transformação das formas da vida emocional do paciente. Antes do desenvolvimento dos casos, contudo, primeiramente é feita uma apresentação inicial do que está sendo chamado de a face estética da psicanálise: os desenvolvimentos presentes nas construções clínicas e teóricas psicanalíticas que envolvem discussões sobre as produções estéticas humanas e as relações da psique com elas, bem como os próprios aspectos estéticos das experiências psíquicas. Esta apresentação é feita com o objetivo principal de reconhecer as construções da tradição psicanalítica sobre o tema, entendendo também que por conta da dimensao estética da vida emocional, o vértice estético da psicanálise é inevitável e inerente a tal dimensão. Na abordagem dos autores, priorizam-se as construções de Freud, dos analistas independentes da Escola Britânica de Psicanálise (especialmente D. W. Winnicott) e seus herdeiros contemporâneos, sendo feita uma breve apresentação das ideias destes autores dentro do campo estético. Há um diálogo também com a filósofa da arte Susanne Langer e com o analista Isaías Mehlson, que desenvolve importantes pensamentos psicanalíticos sobre as formas emocionais e a presentificação delas na clínica. São então desenvolvidas as três construções de casos clínicos, nos capítulos 1 a 3 da tese. Nestes casos, as comunicações estéticas foram observadas como ponto central do trabalho psicanalítico. Os casos analisados acontecem não somente na clínica psicanalítica tradicional, com atendimentos individuais no consultório. É apresentado um caso individual com uma criança, um caso de atendimento em grupo de três crianças que apresentavam sintomas autísticos e uma experiência com oficinas de teatro e cinema em um serviço de saúde mental – um Centro de Atenção Psicossocial II. A partir das elaborações acerca da dimensão estética notada nestes três casos, é feita uma síntese teórica final, no capítulo 4. Nesta síntese, é observado que o sentido estético construído por meio de comunicações não verbais possibilita a constituição do espaço potencial presente na clínica. É percebida também a existência de experiências sensoriais nos casos clínicos, que favoreceram a ligação das angústias que precisavam ser elaboradas a imagens de continência, bem como a criação de experiências integradoras para o self. A partir disto, houve expansão da capacidade simbólica e foram observadas experiências mutativas. Reverberando tais elaborações clínicas, são propostas, em forma de esboço, algumas construções metapsicológicas, que buscam construir pontes entre as noções de sensorialidade, pulsionalidade, sonho, experiência estética e expansão do brincar. Também é apresentada a compreensão de Mehlson sobre as formas expressivas da vida emocional, o que é relacionado às teorias psicanalíticas winnicottianas, bem como à noção de formas da alma. Tal noção é proposta nesta tese para reunir uma série de fenômenos que demonstram o movimento contínuo da psique de busca, criação e encontro de formas que veiculem a vida emocional. As formas da alma se apresentam, assim, como um continuum de experiências psíquicas, das mais simples às mais complexas, em que a vida subjetiva busca criar e encontrar formas no mundo, que dão sentido para o self e suas experiências, bem como para a realidade externa. Tais formas também se presentificam na clínica psicanalítica. Uma vez que é encontrando formas no mundo que a experiência psíquica pode se integrar, se conhecer e se desenvolver, este encontrar de formas é essencial também na clínica, motivo pelo qual a sustentação da comunicação estética é o que permite as experiências de transformações, em psicanálise. Por fim, nas considerações finais, são apresentados os pontos que precisam ser melhor elaborados em trabalhos posteriores e também discute-se um pouco as questões da tese relacionadas com a inscrição do sujeito na cultura contemporânea, defendendo-se a importância da apresentação de experiências estéticas significativas na cultura, como favorecimento para o enriquecimento simbólico. / This work enters in the psychoanalytical tradition and its subject is the aesthetical dimension of the psychic human life, understanding that this dimension is a primordial part of psyche’s activity. We call aesthetical dimension all the aspects of psychic life that present themselves through forms or form patterns. They include corporal rhythms; aspects of the relation of the individual with objects and sensorial phenomena, as sounds; creation of images in dreams and children playing; voiced and poetic aspects of the language; relations of the individual with cultural objects, etc. From clinical observations of the author of this thesis, dialoguing with psychoanalytical theories that emphasize this aesthetical vertex, it was noticed that the aesthetical dimension of emotional life is present in the psychoanalytical clinic and it can be the central thread to hold the clinical happening. We observe non-verbal aspects in the patient’s communication that communicate through forms and not discursive meanings. Considering these points, from the construction of three clinical cases, in different contexts and variations of settings, this work proposes to think one main question, defending that the analyst capacity to hold the aesthetical communication present in the clinical setting is what enables the transformation of the forms of the patient’s emotional life. Before the developing of the cases, however, first it is made an initial presentation of what is being called the psychoanalysis aesthetical face: developments present in the theoretical and clinical psychoanalytical constructions, that involves discussions about the human aesthetical productions and the relationship of the psyque with them, as well the aesthetical aspects of the psychic experiences. This presentation is made with the main objective of recognizing the constructions of the psychoanalytic tradition about the subject, understanding that because of the aesthetical dimension of the emotional life, the aesthetical vertex of psychoanalysis is inevitable and inherent of this dimension. About the authors used in this work, we priorize the constructions made by Freud, the independent analysts of the Britanic School of Psychoanalysis (specially D. W. Winnicott) and his contemporary heirs, and it is made a brief presentation of these authors ideas in the aesthetical field. There is also a dialogue with Susanne Langer, an art philosopher, and the analyst Isaias Mehlson, that develops important psychoanalytical thoughts about the emotional shapes and their presence in the clinic. It is developed three clinical cases constructions, in the chapters 1 to 3 of the thesis. In this cases, the aesthetical communications were observed as a central point of the psychoanalytical work. They happen not only in the traditional psychoanalytical setting, with individual sessions in the clinic. It is presented one individual case with a child, one grupal case with three children that presented autistic symptoms and one experience with theater and cinema workshops in a Brazilian public mental health service, Centro de Atenção Psicossocial II. From the elaborations about the aesthetical dimension presented in the three cases, it is made a final theoretical synthesis, in chapter 4. In this synthesis, it is observed that the aesthetical sense developed in nonverbal communications enables the constitution of the potential space present in the clinic. It is also noticed the existence of sensorial experiences in the clinical cases, that favored the linkage of the anguish that needed to be elaborated to continence images, as well as the creation of integrative experiences to the self. From these, there was an expansion of the symbolic capacity and it was observed mutative experiences. Reverberating these clinical elaborations, it is proposed, as an sketch, some metapsychological constructions, that search for building bridges between the notions of sensitivity, impulses, dream, aesthetical experience and expansion of playing. It is also presented the Mehlson’s comprehension of the expressive forms of emotional life, what is related to the Winnicott’s psychoanalytical theories, as well as the notion of forms of the soul. This notion is proposed in this thesis as a continuum of psychic experiences, from the most simple to the most complex, in which the subjective life searches for creating and finding forms in the world, that give sense to the self and its experiences, as well as to the external reality. These forms are also present in the psychoanalytical clinic. Once it is by finding forms in the world that the psychic experience can integrate, know and develop itself, this finding of forms in essential to the clinical experience as well. This is the reason why this holding of the aesthetical experience is what enables the transformation experiences in psychoanalysis. By the end, in the final considerations, it is presented the points that need to be better developed and also is and also it is discussed a few about the relation of the questions presented in the thesis with the relationship of the subject with the contemporary culture.

A partir das experiências clínicas conduzidas nos CAPS, a psicanálise traz para a discussão a tensão existente nesse campo entre a clínica e a política, destacando a importância da articulação e do enlace entre os saberes para o exercício da clínica nesse espaço, porém poucas evidências desse diálogo são encontradas, principalmente no que diz respeito ao atendimento de crianças. Assim, a presente pesquisa pretende analisar como o discurso analítico na clínica com crianças pode se articular com os outros discursos presentes em um CAPS i e contribuir na direção do tratamento, à luz de recortes clínicos do caso de um menino de nove anos atendido por mim. Para tanto, examinou-se a especificidade da clínica psicanalítica com crianças; identificou-se o lugar da criança nas estruturas discursivas presentes nos laços sociais, tomando como a referência a teoria dos quatro discursos de J. Lacan, a fim de: analisar os discursos e práticas presentes no CAPS i; observar o tratamento dado ao sintoma e o manejo da transferência realizado pelo discurso do analista em meio aos demais discursos em um CAPS i; e, por fim, examinar a articulação entre os discursos na direção do tratamento de uma criança em um CAPS i. Esse estudo teve como referencial teórico a psicanálise e utilizou como método a construção de caso clínico. A realização dessa pesquisa torna-se relevante por possibilitar ampliar a discussão sobre crianças em sofrimento psíquico na saúde pública e seu tratamento em um CAPS i, deslocando o eixo discursivo do transtorno ou deficiência para o campo da estruturação subjetiva na interface dos campos da psicanálise, saúde mental e psiquiatria, fornecendo subsídios para a sustentação da teoria e da prática psicanalítica com crianças na saúde pública. / In the psychoanalytic work with children placed into Psychosocial Care Center for children and youth, CAPS i, the challenges of the psychoanalytic clinic itself, with the real of the psychosis and neurosis taking places in the severe cases of psychoanalytical, social and economic helplessness, associated to the difficulties of the job in a collective institution, convokes the psychoanalyst to question the functionnalty of clinical tools in this multidisciplinary field. On the bases of clinical experiences conducted in the CAPSs, the psychoanalysis brings to the discussion the conflict that exists between the clinic and the politic, pointing out the importance of the articulation between the knowledges that makes possible the exercise of the clinic in this space. However, there are a few evidences of this dialogue, especially in regards to the child’s assistance. Therefore, the present research intends to analyse how the analytic discourse about the child clinic can be articulated with others discourses presents in a CAPS i and contributes on the direction of the treatment, based on the clinical case of a nine years old boy which was my patient. To reach this goal, the specifies of the child analytic clinic was examined and the place of the children in the discourse’s structures in the social bond was identified, based on the Theory of four discourses of Jacques Lacan, intending to analyse the discourses and practice in CAPS i according to the Athos case; to analyse the treatment given to the symptom and to the management transfer in the analyst's discourse in between the others discourses in a CAPS i; and. finally, to exam the articulation between the discourses in the way to the treatment of a child in a CAPS i. This study is based on the psychoanalysis’s theory and will have as a method the construction of a clinical case. This research is relevant because it provides an ample discussion about children in public care who suffer psychology and their treatment in a CAPS i, shifting the focus of the discussion from the deficiency to the field of the subjective structuring in the different interfaces of psychoanalysis, mental health and psychiatry, hence, providing evidence to support the theory and psychiatric practice with children in public health.

Psicoterapia psicanalítica com crianças: avaliação e resultados

The first is a conceptual review about the concept of conflict, highlighting two basic conflicts for the organization and psychic development of a child: Separation-Individuation and Oedipus Complex. This had been done by articulating the thought of classical Psychoanalytic authors, important theoretical exponents regarding child’s development comprehension, such as Freud and Margareth Mahler, with contemporary authors and their contributions. The focus is the importance of Clinical Psychologists’ knowledge about child’s development in order to evaluate and treat situations beyond vital crises, in order to be preventive towards psychic suffering and provide greater welfare and mental health to the children and their relatives. The aim of the second sections was to characterize a clinic group composed by 52 children, evaluated with psychological tests (Bender and WISCIII), CBCL scale (filled in by the children’s parents or responsible) and data collected from the patients’ documents and files in order to better understand the difficulties, which had made their parents look for specialized attendance. It is important to identify variables related to socio-demographic data, clinic and psychology’s evaluation results; examine the existence of relations between socio-demographic variables themselves, complaints and diagnosis hypothesis, diagnosis hypothesis and Total IQ (WISC), diagnosis hypothesis and the results from Bender’s test evaluation, diagnosis hypothesis and CBCL results (filled in by the children’s parents, according to behavioral symptoms observed). Finally, based on the evaluation, the study shows that children are suffering from intense anxiety that causes damages in all aspects of their development. The study also considers the relevance of evaluation and psychological treatment during childhood as a way to prevent future suffering.The third section presents a study of Psychoanalytical Psychotherapy results with three case studies- children who were evaluated before the beginning of the treatment and a year later, by psychological tests (Bender, WISC-III, Fables Test and Rorschach), CBCL scale and data collected from the patients’ documents and files The studied cases had presented improvement in the quality of their affective relations and anxieties’ control. It was possible to observe that Psychotherapy helped to retake children’s development course, providing a better emotional stability for them. Results allowed the measure of the therapy evolution, offering psychologists and parents stronger evidence about the children’s development. / Foram elaboradas três seções para a composição da dissertação de mestrado. Na primeira, foi realizada uma revisão conceitual sobre conflito, destacando as duas conflitivas fundamentais para a organização e o desenvolvimento do psiquismo infantil: separação-individuação e complexo de Édipo. Busca-se, dessa forma, articular autores psicanalíticos clássicos, que são importantes expoentes teóricos para compreensão do desenvolvimento infantil como Freud e Margareth Mahler, com autores contemporâneos e suas contribuições. Destaca-se a importância dos psicólogos clínicos conhecerem o desenvolvimento infantil para avaliar e tratar situações que perpassam as crises vitais, podendo assim agir de forma preventiva no tratamento dos sofrimentos psíquicos e proporcionar maior bem estar e saúde às crianças e aos seus familiares. Na segunda sessão, a proposta é categorizar um grupo clínico formado por 52 crianças que foram avaliadas por meio de testes psicológicos (Bender e WISC-III), da escala CBCL (preenchida pelos pais/responsáveis) e dos dados das fichas e prontuários dos pacientes. Isso com o objetivo de melhor compreender as dificuldades que mobilizaram seus familiares a procurar atendimento especializado. O trabalho visou identificar variáveis sócio-demográficas, clínicas e de resultado da avaliação psicológica; examinar a existência de relações entre: variáveis sócio-demográficas entre si, queixas e hipótese diagnóstica, hipótese diagnóstica e QI Total (WISC ), hipótese diagnóstica e resultados da avaliação com o teste Bender, hipótese diagnóstica e resultados do CBCL ( preenchido pelos pais em relação aos sintomas comportamentais).Por fim, através da avaliação, o estudo demonstra que as crianças estão sofrendo de intensa ansiedade que acarreta prejuízos em todos os aspectos de seu desenvolvimento. O estudo considera ainda, a relevância da avaliação e do tratamento psicológico na infância como prevenção de futuros padecimentos. A terceira sessão apresenta a viabilidade de estudar resultados da Psicoterapia Psicanalítica através de três estudos de casos, que foram avaliados antes de iniciar o tratamento e um ano após a Psicoterapia Psicanalítica, por meio de testes psicológicos (Bender, WISC-III, Teste das Fábulas e Rorschach), da escala CBCL e dos dados das fichas e dos prontuários dos pacientes. Os casos estudados apresentaram melhora na qualidade de suas relações afetivas e no controle de suas ansiedades. Foi possível observar que a Psicoterapia auxiliou na retomada do curso desenvolvimental normal e proporcionou às crianças maior estabilidade emocional. Os resultados obtidos pelos instrumentos de avaliação, possibilitaram a mensuração da evolução do atendimento, oferecendo aos psicólogos e aos familiares indicativos mais objetivos do desenvolvimento da criança.

Esta proposição adquire maior densidade ao considerar a existência de condições significativamente incapacitantes inerentes ao adoecer autístico. No presente trabalho busca-se investigar a viabilização de dispositivos técnicos que permitam a execução do processo de identificação e de diagnóstico precoces de sinais de risco de autismo infantil, em crianças abaixo de três anos de idade. Empregou-se o método psicanalítico para a realização da pesquisa bem como para a análise dos resultados encontrados. Durante a etapa de identificação inicial dos sujeitos, fez-se necessário a criação de um instrumento específico que viabilizasse a detecção de crianças com sinais de risco de evolução autística, mediante as dificuldades em encontrá-las. A construção do instrumento denominado de "Ficha de Acompanhamento do Desenvolvimento Infantil" teve como parâmetro fundamental os sinais precoces de autismo infantil, conforme descritos na literatura psicanalítica, e a idade em que estes costumam ocorrer. O instrumento compõe-se de quatro escalas, divididas em faixas etárias que vão de zero a trinta meses, tendo as mesmas sido submetidas ao tratamento estatístico de análise fatorial do Statistical Package for Social Science (SPSS), após sua aplicação em larga escala. A pesquisa foi subdividida em duas etapas. A primeira etapa consistiu na aplicação do Instrumento tendo em vista a identificação inicial. Na segunda etapa procedeu-se a realização de, pelo menos, quatro sessões de observação diagnóstica da criança e de seus familiares, com duração média de uma hora, utilizando-se a técnica de Intervenções Conjuntas Pais- Filhos formulada por M. Mélega, baseada no Método de Observação da Relação Mãe - Bebê de Esther Bick. Este processo de observações diagnósticas foi realizado com quatro sujeitos que apresentaram resultados significativos na aplicação do Instrumento. Procedeu-se a análise dos resultados dos quatro sujeitos através da articulação qualitativa entre as respostas ao Instrumento e o material transcrito das observações diagnósticas. Esta articulação resultou na configuração de três eixos norteadores da análise e que são, ao mesmo tempo, fundamentais para a constituição do sujeito psíquico: o olhar, o corpo e a palavra. Para a análise, também considerou-se a história e o lugar que a criança ocupa na rede afetivo - familiar. A partir de todos estes elementos pôde-se mapear os diferentes matizes psicopatológicos em que se encontravam os sujeitos. Concluiu-se que, mediante as estratégias empregadas nesta pesquisa, foi possível executar a identificação e o diagnóstico precoces em crianças pequenas, possibilitando que os sujeitos se inserissem, com um grau significativo de adesão, em um trabalho de intervenção terapêutica precoce. Os resultados também apontam para a viabilidade de inserção desta estratégia de detecção precoce de sofrimento psíquico no serviço público de saúde, conjugando-se com o atendimento clínico preventivo em puericultura / Abstract: The research is a re-enforcement for one of the actual formulations in the field of Mental Health, which states that if a child in its first years of life could be relieved from its initial symptoms before their fixation or displacement, it would be offered better conditions to face a life. This proposition acquires larger density when it is considered the existence of some conditions significantly disabled inherent of the autistic sickness. In the present work it is rneant to investigate the viability of technical devices that allow the execution of the identification process and of the precocious signs of infantile autisrn diagnosis' risk in children below three years of age. The psychoanalytical rnethod was used for the accomplishment of the research as well as for the analysis of the achieved results. During the initial stage of subjects' identification it was necessary to create a specific instrument in order to allow the detection of the signs of autistic evolution in children by means of the difficulties in finding them. The construction of the instrument denorninated "The Record of Attendance of the Infantile Developrnent" had the precocious signs of infantile autism described in the psychoanalyticalliterature and the age of its manifestation as parameters. This instrument is composed by four scales divided in age groups that go from zero to thirty months. The scales were submitted to the statistical factor analysis of Statistical Package for Social Science (SPSS) after its application in a significant sample. The research was subdivided in two stages. The first stage consisted in the application of the instrument to obtain the initial identification. In the second stage the accomplishment was proceeded for at least four sessions of the diagnostic observation of the child and its relatives. These observations had a medium duration of one hour and it was used the technique of "Parents-Children Joint Therapeutic Interventions" formulated by M. Mélega, inspired on the Esther Bick's Observation Method. This process of diagnostic observations was accomplished with four subjects that presented significant results in the application of the instrument. The analysis of the results of the four subjects was proceeded through the qualitative articulation among the answers to the instrument and the transcribed material of the diagnostic observations. This articulation resulted in the configuration of three axes to orientate the analysis that are fundamental for the psychic subject's constitution: the glance, the body and the word. The analysis had also considered the history and the place occupied by the ehild in the family affeetionate net. Gathering all these elements it was possible to deseribe the different psyehopathologieal states in the subjeets of the researeh. In conclusion, the strategies used in this researeh turned possible the execution of the identification and the precocious diagnosis in young children facilitating the subjeet's insertion in a precocious therapeutie intervention's work. The subjects inclusion in the treatment was with a significant degree of adherense.
