The general objective of this anthropological research is to verify the impact of diseases on the everyday life of institutionalized elderly people and their present and future consequences. The investigation took place at Asilo São Vicente de Paulo de Londrina, a nursing home with 37 residents. Data were collected through observations and interviews and analyzed through the description of the observations and the hermeneutic-dialectical method. Results showed loss of autonomy and personal control. Based on the participants comments, the study detected a perception of the sickening body, mobility loss, lack of freedom, physical dependence to carry out routine activities, occupation reduction and isolation, difficulty to sleep and hopelessness in regards to the future. In its final considerations, the study highlights the importance of the anthropological investigation in a Long-term Care institution for the elderly / Apesar de haver um grande número de pessoas idosas saudáveis, a velhice também é acompanhada pelo declínio geral das condições de saúde, especialmente entre idosos institucionalizados. Verificar o impacto das doenças na vida cotidiana em pessoas idosas institucionalizadas e seus desdobramentos no presente e no futuro foi o objetivo geral desta pesquisa antropológica. O campo de investigação desenvolveu-se no Asilo São Vicente de Paulo de Londrina, com 37 residentes. Na metodologia utilizou-se para a coleta de dados a observação e a entrevista, que foram analisados através da descrição da observação e pelo método hermenêutico-dialético. Nos resultados observou-se a perda da autonomia e do controle pessoal. Nas falas dos participantes, constatou-se a percepção do corpo que adoece, o acometimento da mobilidade, a perda da liberdade, a dependência física nas atividades do cotidiano, a diminuição da ocupação, o isolamento, a dificuldade para dormir e a esperança no futuro. Nas considerações finais, foi destacada a importância da investigação antropológica em uma Instituição de Longa Permanência para Idosos

Elaborating the Actimetric Profile of Fall Sensitive Patients for Early Detection of Fall Incidents / Élaboration du profil actimétrique de patients sensibles aux chutes pour détecter de manière précoce une possible chute

La croissance et le vieillissement sont inévitables pour la race humaine. Chez les personnes âgées, le vieillissement est souvent accompagné par de nombreuses formes de maladies et de dangers dont les chutes qui affectent la qualité de vie et qui posent un enjeu socio-économique. Mais les chutes sont évitables. Les acteurs de santé, les scientifiques et les chercheurs combinent actuellement des efforts pour développer des systèmes de détection et de prédiction des chutes. Dans le contexte de la prédiction des chutes, l'objectif de cette thèse est d'élaborer le profile actimétrique des patients sensibles aux chutes afin de les alerter d'une possible chute. Ceci consiste principalement à développer un système capable de surveiller les paramètres de la marche des personnes durant leurs activités quotidiennes avec un minimum d'intrusivité. Dans une première contribution, nous avons proposé une classification générique des systèmes liés à la chute en fonction du déploiement de leurs capteurs. Nous avons distingué les systèmes portables, les systèmes non-portables et les systèmes qui combinent les deux. En se basant sur cette classification, nous avons proposé notre plateforme WMFL v1.0 dans une deuxième contribution. WMFL combine une chaussure équipée par des capteurs de force avec des dalles où nous avons intégrés des capteurs optiques infrarouges. La fusion de ces deux systèmes assure une prévention à l'intérieure et à l'extérieure des locaux. WMFL peut être aussi déployées dans une clinique. Dans une troisième contribution, nous avons proposé une méthode de prédiction des chutes en se basant sur l'analyse du déplacement du centre de pression (projeté du centre de masse) sur la surface plantaire du pied durant la marche. La méthode utilise la fenêtre glissante spatio-temporelle pour alerter le patient d'une chute potentielle et pour déterminer le risque de chute à la fin de la marche. / Growth is the normal change of the human body and getting old is inevitable to human race. As a result, elderly people are subject to many forms of diseases and dangers among which falls are considered very serious in terms of quality of life and socio-economic costs. But falls can be manageable. Health practitioners, scientists and researchers currently combine efforts to develop systems capable of detecting and predicting falls. In the context of fall prediction, the goal of this thesis is to elaborate the actimetric profile of fall sensitive patients to alert them from a potential fall. It mainly consists of developing a system capable of monitoring gait and balance parameters during their daily activities with minimum intrusiveness. These are usually assessed in clinical settings using high-cost tools. In our first contribution, we proposed a generic classification of fall-related systems based on their sensors deployment. These are classified as Wearable, Non-Wearable and Fusion Systems. Based on the generic classification, we proposed the WMFL v1.0 platform in our second contribution. WMFL fuses a Foot Wear Force Sensing device with an Ambient system using IR-sensing floor tiles. The platform can be deployed at homes or in clinics. It ensures an indoor-outdoor protection. In a third contribution, we proposed an early fall detection approach to determine the risk of falling by analyzing the displacement of the Center of Pressure projecting the amount of sway of the Center of Mass on the foot plantar surface. The method uses the spatio-temporal sliding window to alert the patient of a potential fall.

The aim of this thesis was to study elderly people’s experiences of and approach to their usage of home care services with tax deduction. The aim was also to describe how elderly people reason about their underlying motives of using home care service with tax deduction. The study is based on six qualitative interviews with people between the ages of 65 and 80, living in Nacka, Sweden. This thesis is a collaboration with Stiftelsen Stockholms läns Äldrecentrum/ Stockholm Gerontology Center and part of their follow up study concerning simplified administration for elderly people applying for home care services in Nacka. The theory of rational choice was used in the analysis of the results. The primary results showed that elderly people’s usage of home care services with tax deduction was limited though regular and consist mainly of basic domestic work. The overall experiences of using home care services with tax deduction were positive and described in terms of satisfaction and as a possibility of independence and influence regarding their needs of services and how they were executed. The results indicate that the usage of home care services with tax deduction is partly based on elderly people’s lack of confidence in publicly-financed elderly care. Due to the distrust in elderly care and the self-determing factors home care services with tax deduction generated, their choice of using home care services with tax deduction seemed more appealing than publicly-financed home care services.

This study is based on interviews with four managers in public elderly care and four managers in private elderly care. The aim of this paper is to examine how these managers are experiencing their leadership in relation to conflict management, and if the managers leadership differs depending on private or public elderly care. Leadership and conflict management are theories that have been used in order to analyze the empirical findings. Previous research that has been used in the study concerns “leadership/leadership styles”, “conflict management”, “to be active in the private/public elderly care”. Social workers often choose to work as a manager in private and public elderly care. The work as a manager is demanding with pressure from several/different parties and the manager faces new challenges. Conflicts are one challenge that mangers faces, and conflicts can be a sensitive topic. The study deals with the complexity that managers in the private and public elderly care faces, mainly based on the leadership role in conflict management. The results show that the interviewees leadership in this study, for the most part can be linked to the situational leadership style, which means that managers believe that leadership adapts to any situation that arises. They also experienced that the private elderly care has higher demands and that there is a bigger responsibility on the individual manger than it is in the public elderly care. There are many different support functions in the public elderly care that managers have the opportunity to take help from daily, that opportunity does not exist in private elderly care.

'Unregarded age' : texts and contexts for elderly characters in English Renaissance drama, c.1480-1625

This study seeks to provide historical and literary contexts for elderly characters from English play-texts c.1580 to 1625. Its primary aim, from a literary perspective, is to draw attention to the ways that a better understanding of elderly characterisation can enrich the appreciation of much-studied play-texts, and to indicate some interesting features of more obscure ones. Its secondary aim is to suggest the value, for social historians of old age in early modern England, of play-texts as social evidence. I have examined most of the published extant play-texts of the period, and have found approximately 150 of these to be relevant (the most important of these are listed in the Appendix). Because of the problems of handling all aspects of such a large amount of material, I have chosen to consider the plays chiefly as texts to be read, with little reference to their performative aspects. However, I analyse the dramas as literary as well as social documents. Specific plays provide illustrations for observations and support for various hypotheses about dramatic representations of the elderly. In some instances, I address plays which have received little critical attention. The thesis falls into two parts. In the first three chapters, I discuss the socio-historical, cultural and non-dramatic literary contexts for representations of elderly men and women in play-texts. In chapters four through seven, I examine elderly characters in specific role or relationship categories: as sovereigns and magistrates, in sexual and marital relationships, and as parents. In the final chapter, I offer a detailed analysis of The Old Law by Thomas Middleton and William Rowley.

Bakgrund: Förmågan att utföra oral egenvård kan påverkas av skörhet hos äldre, därmed krävs fler för gruppen anpassade hjälpmedel. Syfte: Att hos sköra äldre i ordinärt boende analysera upplevelsen av att använda ett tekniskt munhälsohjälpmedel, en så kallad trigger, för påminnelse av tandborstning. Metod: Rekrytering av informanter gjordes i samverkan med folktandvården i Region Jönköpings län och institutionen för Gerontologi på HHJ. Triggern testades i egna bostaden av sju informanter i en vecka vardera. Två intervjuer utfördes om livssituation/munhygien och kring upplevelsen av användandet. En kvalitativ, induktiv innehållsanalys av materialet utfördes. Resultat: Analysen gav huvudkategorin, ”användning”, vilken kopplades till subkategorin ”förhållningssätt och vanor”. Där ansågs munhygienvanorna goda och värdet av användningen sattes istället i relation till om kapaciteten att upprätthålla munhygienen hade blivit försämrad. Behovet anpassa triggern till föränderliga vardagstider upplevdes i relation till subkategorin ”fungerande vardagstruktur”. Huvudkategorin ”Funktion”, upplevdes i stort som god, subkategorin ”otillförlitlig” syftade dock till teknik som upplevts brista. Subkategorin ”utvecklingsmöjligheter” berörde signalsystemet som upplevdes behöva kompletterande ljud/ljusintryck.              Slutsats: Informanterna upplevde sig för friska i relation till huvudkategorin ”användning”. Gentemot huvudkategorin ”funktion” kom dock detta att resultera i användbara aspekter till vidareutvecklingen av triggern, såsom uppmärksamhetsförhöjande ljudintryck. Detta bör fokus läggas på innan fortsatta studier utförs. / Background: The ability to perform oral self-care can be affected by frailty in the elderly. Aim: To analyze the experience of using assistive technology, a so called trigger, as a reminder of tooth brushing, for the home dwelling elderly. Method: The recruitment was done in collaboration with the public Dental service and the Institute of Gerontology at HHJ. Seven informants tested the trigger for a week. Two interviews regarding their life situation/oral hygiene and the experience around the trigger was performed.A qualitative inductive content analysis was performed. Result: The main category "use", was linked to the subcategory "approaches and habits", here the value of the use was linked to if the capacity to maintain oral hygiene was impaired. In "everyday structure" a need to adapt the trigger to a changing schedule was experienced.  The main category ”function” and the subcategory "unreliable" referred to the parts that malfunctioned. “Improvement opportunities” regarded the need to develop the signal system. Conclusion: The informants perceived themselves as too healthy in relation to "use". In “function” this can be perceived as a strength. It resulted in improvement suggestions, such as sound-developments for attention, that should be taken into account before further studies are performed.
