O enfoque teórico subsidiado por elaborações acadêmicas e publicações governamentais possibilita o entendimento dos aspectos principais desta Política e do Estatuto do Idoso, permeado pela preocupação com a contextualização sócio-política do Brasil, especialmente a partir dos anos de 1990. A configuração de instâncias governativas de políticas sociais públicas de administrações municipais de Juiz de Fora evidencia diretrizes ídeo-políticas, norteadoras de ações centradas no segmento de idosos, usuários destas políticas. A retratação analítica da mencionada gestão está subsidiada por posicionamentos coletados em 14 entrevistas realizadas com informantes qualificados e gestores de políticas sociais desta cidade, particularizados nas áreas da assistência social, saúde e educação, sendo antecedida pela apresentação do perfil dos mesmos. A sistematização destes posicionamentos corresponde aos eixos que caracterizam a PNI, contribuindo para evidenciar contribuições, questionamentos concernentes a possibilidades e dificuldades de gestão da referida Política. A identificação de potencialidades sinaliza que existem caminhos a percorrer, visto que a responsabilidade e o compromisso de instâncias municipais com a gestão desta Política ensejam indicações de mudanças que merecem ser reconhecidas como necessárias e indispensáveis para que a fração populacional de idosos, especialmente usuários de políticas sociais públicas, possam usufruir de recursos e serviços públicos inseridos, como correspondentes às demandas do envelhecimento. / The Management of the National Policy for the Elderly in Juiz de Fora which represents the focus of this study seeks to investigate and unveil local management courses of action in order to highlight its significance and importance. The theoretical framework especially academic works and government volumes enables the understanding of the main aspects of this Policy as well as the Elderly Statute along with the concern for Brazil socio-political context since 1990s. The arrangement of governing sectors of public social policies from municipal administration shows ideological and political guidelines which oriented actions centred to the elderly, the users of these policies. The analytical description of this management is supported by the exploratory qualitative research that enabled us to show positions collected in 14 interviews with qualified informants and managers of social policies in Juiz de Fora, in areas such as social assistance, health, traffic, transportation, and education. Previously their whole profiles were presented. The systematization of these positions corresponds to the axes that characterize the PNI, enabling the evidence of contributions, questioning concerning the possibilities and difficulties of management of that Policy. The identification of potentialities indicates that there are ways to be explored, since the responsibility and commitment of local sectors with the management of this Policy can lead to some changes that deserve to be recognized as necessary and essential to the elderly. Thus this part of the population seen as frequent users of public social policies will be able to benefit from public services and resources, corresponding to the demands of aging.

São condições frequentes, passíveis de prevenção, responsáveis pela redução da capacidade funcional e da qualidade de vida e preditoras de institucionalização, morbidade e mortalidade. O objetivo foi verificar a prevalência de fragilidade e analisar os fatores associados. São ainda objeto de análise o estudo e compreensão do evento queda, queda recorrente e medo de cair. Inquérito domiciliar realizado entre outubro de 2014 e março de 2015, com 400 idosos (60 anos ou mais), ambos os sexos, não institucionalizados, na Zona Norte de Juiz de Fora. Foram analisadas características demográficas e socioeconômicas, perfil de saúde, uso de serviços de saúde, fragilidade (Escala de Fragilidade de Edmonton), quedas, medo de cair (Falls Efficacy Scale – Internacional – Brasil) e capacidade funcional (Escala de Lawton e Brody). Os dados foram processados em banco de dados no software Statistical Package for Social Sciences, módulo de análise complexa, versão 15.0. O uso do software Stata 9.2 permitiu o incremento das análises. Foram empregados diferentes métodos de análise estatística para o alcance dos objetivos. Nível de significância foi de 5%. Para a análise dos fatores associados à fragilidade, foi construído um modelo teórico de determinação com três blocos hierarquizados, ajustados por regressão de Poisson. A prevalência de fragilidade foi 34,5% e associou-se com dificuldade para andar, necessidade de dispositivo auxiliar para locomoção, presença de cuidador e dependência funcional para realização das atividades instrumentais. A prevalência de quedas foi 35,3%. A maior parte ocorreu no domicílio e no período da manhã. Foi realizada abordagem teórica hierárquica em 3 níveis para estimativas e ajustes de variáveis. Após todos ajustes, idade e relato de dificuldade para andar permaneceram associados à queda. Para a investigação de quedas recorrentes foi realizado um estudo observacional de seguimento por meio de duas ondas de corte transversal. Para isso utilizou-se também dados do primeiro Inquérito em Saúde realizado em 2010. A regressão logística multinomial foi empregada para estimar a associação das variáveis independentes com os desfechos queda no seguimento e queda recorrente. 44,5% dos idosos não relataram quedas, 39% sofreram queda em pelo menos um dos inquéritos, e 16,5% manifestaram ter sofrido queda nos dois inquéritos. Não foram encontradas associações para o desfecho queda recorrente. Queda no seguimento associou-se à sexo feminino e ao segmento etário de 71 a 80 anos. A prevalência do medo de cair foi 95,2%. A correlação de Spearman indicou correlação do medo de cair com idade, autopercepção de saúde, dificuldade para andar, uso de dispositivo auxiliar para marcha, quedas e capacidade funcional. Os resultados revelam alta prevalência de fragilidade, quedas e medo de cair e evidenciam a multifatoriedade e complexidade das interações de diversos fatores na condição de saúde dos idosos. É eminente a necessidade de desenvolvimento de políticas públicas e ações de saúde voltadas para essa população, com ênfase em ações educativas, preventivas, e de promoção à saúde, de cunho interdisciplinar, a partir da identificação dos grupos de riscos e fatores associados. / Frailty, falls and fear of falling are geriatric syndromes that characterize significant issues of public health. Those conditions are frequent, preventable, responsible for the reduction of functional capacity and life quality, and predictors of institutionalization, morbidity and mortality. The purpose was to verify the frailty prevalence and analyze the associated aspects. Moreover, the comprehension of the events fall, recurrent fall and fear of falling are also issues to be analyzed in this study. Household survey realized between October 2014 and March 2015, with 400 elderly (60 years old and over), both sex, not institutionalized, north zone of Juiz de Fora City. It was considered demographic and socioeconomic characteristics, health profile, use of health services, frailty (Edmonton Frail Scale), falls, fear of falling (Falls Efficacy Scale – International – Brazil) and functional capability (Lawton and Brody Scale). The data was processed in a database on the software Statistical Package for Social Sciences, complex analyze module, 15.0 version. The software Strata 9.2 was used to increment the results. Several methods of statistical analysis were adopted to achieve the goals. The significance level was 5%. For frail associated aspects, a theoretical model of determination with three hierarchical blocks, adjusted by Poisson regression, was built. The fragility prevalence was 34.5% and was associated to walking difficulties, need of auxiliary device for mobility, a caregiver assistance and functional dependency for instrumental activities accomplishment. The falling prevalence was 35.3%. The majority of the events happened at home, during the mornings. For estimation and variable adjustments, a hierarchic theoretical approach, ranked in 3 levels, was adopted. After all the adjustments, age and reports of walking difficulties kept associated to falling. For the investigation of recurrent falls, an observational follow-up study was carried out using two cross-sectional waves. For this, data from the first Health Survey conducted in 2010 was also applied. Multinomial logistic regression was used to estimate the association of independent variables with the outcomes follow-up fall and recurrent fall. 44.5% of the elderly do not report falls, 39% suffered falling events in, at least, one of the surveys, and 16.5% reported falling events in both surveys. None association was found for the outcome recurrent fall. Follow-up fall was related to female and age segment from 71 to 80 years old. The prevalence of fear of falling was 95.2%. The Spearman correlation revealed correspondence of fear of falling with age, self-perception of health, walking difficulties, use of auxiliary device for mobility, falls and functional capacity. The results reveal high prevalence of frailty, falls and fear of falling, and evidenced the multifactorial and complexity of the interactions of several factors in the health condition of the elderly. The need for development of public policies and health actions aimed to this population, emphasizing educational, preventive and health promotion actions of an interdisciplinary nature, is eminent, based on the identification of groups in risk and associated factors.

Syftet med denna studie är att belysa hur äldres sexuella hälsa beaktas på äldreboenden. Samt att titta närmare på omsorspesonalens attityder kring sex och hur de påverkar den äldre brukarens välmående. Det är framför allt viktigt för de äldre men även för personalens framtida arbetssätt. Som enhetschef är det viktigt att känna till hur personalens värderingar påverkar arbetet med den äldre. Genom att använda teorierna ålderism och skam så är syftet att se om åldersdiskriminering förekommer kring frågor om sex inom äldreomsorgen. Teorier om skam används som teoretiska glasögon för att förstå de inre processer som frågor om sex kan väcka hos den äldre och personalen. Studien är en litteraturstudie och ett försök att göra en kunskapsöversikt över frågor kring den äldres sexulaitet på äldreboenden kopplat till personalen och samhällets attityder. Den huvudsakliga litteratur som ligger till grund för arbetet är vetenskapliga artiklar innehållande kvalitativ- och kvantitativ forskning. Artiklarna har gallrats enligt en systematisk litteraturstudie modell (Friberg 2017).Resultatet visar att forskning visar på att sexualitet kan spela en viktig roll för att äldre ska uppnå en god hälsa. Äldres sexualitet är ett tabubelagt ämne och äldre tenderas att diskrimineras pga. sin ålder. Mer kunskap behövs inom ämnet för att bidra till mer positiva attityder gentemot äldres sexuella hälsa och för att de äldre ska våga uttrycka sin sexualitet utan skam och obehag. Resultatet har delats upp i olika teman som framkommit ur de tio artiklar vi har läst. De teman som vi kommer att belysa i den här uppsatsen är äldres inställning till sex, attityder hos personal, attityder hos anhöriga samt samhällets attityder till äldres sexualitet. / The purpose of this study is to shed light on how the sexual health of the elderly is taken into account in nursing homes. And to take a closer look at the care staff's attitudes towards sex and how they affect the elderly user's well-being. It is above all important for the older and also for the staff's future working methods. As a unit manager for a nursing home, is it important to know how the care staff's values colours the work with the eldery person. By using the theories of Ageism and Shame, can we have a closer look and see if age discrimination occurs around issues of of the elderis sexual health in the nursung homes,.. Shame Theories as theoretical glasses can help us understand the internal process and feelings of the eldery. The study is a literature study and an attempt to make a knowledge overview of questions about the elderly's sexuality in nursing homes linked to the staff and society's attitudes. The main literature on which the work is based is scientific articles containing qualitative and quantitative research. The articles have been chosen according to a systematic literature study model (Friberg 2017). The results show that research shows that sexuality can play an important role for the elderly to achieve good health. Elderly sexuality is a taboo subject in both elderly care and in society. More knowledge is needed in the subject to help more positive attitudes towards the sexual health of the elderly and for the elderly to dare to express their sexuality without shame and discomfort. The results have been divided into four different themes: attitudes of staff, attitudes of relatives and society's attitudes to the sexuality of the elderly.

Designing Virtual Reality Experiences for Elderly : A qualitative study focusing on VR suppliers operating within the elder care sector.

Abstract                                                    This thesis provides an in-depth research of five VR suppliers working with creating VR experiences targeted to elderly within the elder care sector in Sweden. Furthermore, a technological knowledge gap between elderly and VR technology are researched. The focus of this thesis is therefore how VR suppliers, as early adopters, design VR experiences to elderly, as digital immigrants. To investigate the chosen topics, two research questions was formulated; RQ1. How do VR suppliers design a VR experience targeted to digital immigrants within the elderly care sector? RQ2. How do VR suppliers, as early adopters, bridge the digital gap between VR-technology and digital immigrants?  This thesis constitutes of a qualitative research where the empirical findings are based on interviews with the VR suppliers. The research took an inductive approach in order to get close to the respondents and interpret their realities. Theories about Customer experience, Value, Value Co-creation, Digital immigrants and Early and Late adopters, have thereafter helped to connect the empirical findings with aspects within these theories, to further understand important aspects for the VR suppliers when they design their VR experiences.  The results of the study present the aspects that are important to take under consideration when VR suppliers design their VR experiences towards elderly within the elder care sector. A model showing the steps when designing the VR experience has been provided and constitutes of identifying needs, customize the service, create trust and security, set the environment, inform and educate personnel, handling impairments and bridging the gap. Furthermore, the participants confirmed a knowledge gap within technology amongst elderly and that there is a need to bridge the gap in order for the VR service to be successfully implemented.

The aim of this study was to analyze how high-tech assistive technology in the elderly care are represented in selected Swedish newspapers during the time between August 2018 and March 2020. The initial perspective of this study was to conduct a discourse analysis based on social constructivism. I have investigated what daily newspapers describe to be central needs for the elderly care and for elderly people. I have also looked at what the daily newspapers describe to be important for elderly people during implementation of high-tech assistive technology in the Swedish elderly care. In the study’s result, I concluded the need for autonomy was central for what the daily newspapers described as important for elderly people. The result also showed that the daily newspapers described high-tech assistive technology to be important for the organization, due to shortage of staff and demographic challenges.

Background: The aim with this study is to explore how elderly care homes in Sweden plan activities in order to counteract the feeling of perceived involuntary loneliness for its elderly service users.  Method: The study is based on collected qualitative data that centers around eight semi-structured interviews with elderly care homes unit managers (SÄBO).  The result: The interviewees perceived involuntary loneliness differently depending on the elderly individual's perspective and the acceptance of their new life process. The range of activities are aimed to activate and socialize the elderly and implicitly counteract involuntary loneliness. The interviewees hint that it is often a matter of profound loneliness rooted in life’s final phase, identified as existential loneliness – a feeling of being separated from other people and the universe itself. Conclusion: The study indicates that there are multiple hues of loneliness such as feelings of existential loneliness, and the effects of the elderly’s changed life situation. The interviewees express a want for a counsellor for the elderly, with whom they can talk about life and death.  Keywords: elderly, elderly homes, involuntary loneliness, activities, gerotranscendence, ageism, person-centered care. / Bakgrund: Syftet med denna studie var att undersöka hur äldreboenden i Sverige samordnar aktiviteter för att motverka den upplevda ofrivilliga ensamheten bland de äldre brukarna.  Metod: Studiens insamlade kvalitativa data baseras på åtta semi-strukturerade intervjuer med enhetschefer på särskilt boende (SÄBO).  Resultatet: Intervjupersonerna uppfattade ofrivillig ensamhet olika beroende på den äldre individens perspektiv och acceptans av deras nya livsprocess. Aktivitetsutbudet syftar till att aktivera och socialisera de äldre och implicit motverka ofrivillig ensamhet. Intervjupersonerna antyder att det ofta handlar om djupgående ensamhet rotad i livets slutfas som identifieras som existentiell ensamhet – en känsla av att vara separerad från andra människor och själva universum.  Slutsats: Undersökningen indikerar att det finns flera nyanser av ensamhet, så som känslor av existentiell ensamhet och effekterna av de äldres förändrade livssituation. Intervjupersonerna uttrycker en önskan om en äldre-kurator, med vilken de äldre kan prata med om livet och döden. Bakgrund I åldrandet kan det förekomma komplexa omvårdnadsbehov och nedsatt förmåga till eget beslutstagande hos den äldre. Sjuksköterskor ska arbeta utifrån personcentrerad vård för den äldre. Personcentrerad vård grundas i en humanistisk vård med en helhetssyn på individen vilket handlar om relationskapande och delaktighet. Äldreomsorgens nationella värdegrund i socialtjänstlagen ska arbetas utefter för äldres rätt till ett värdigt liv. Syfte Syftet med litteraturöversikten var att belysa sjuksköterskans upplevelser och erfarenheter av att arbeta personcentrerat i vård av äldre. Metod Litteraturöversikten grundar sig på 13 vetenskapliga artiklar med kvalitativ samt kvantitativ design. Databaserna som användes för att erhålla artiklar var CINAHL och PubMed med publikation mellan åren 2012 – 2022. Analysen genomfördes som en innehållsanalys där likheter och skillnader jämfördes. Resultat I resultatet av litteraturöversikten identifierades fyra huvudkategorier vilka var delaktighet, att värna om den äldre, kommunikation och upplevelse av personcentrerad vård. Resultatet visade på sjuksköterskors upplevelser samt erfarenheter av personcentrerad vård till äldre. Slutsats Det framkom att sjuksköterskor arbetade personcentrerat dock fanns brister och utmaningar i tillämpandet. Sjuksköterskorna beskrev positiva känslor som uppstod i tillämpandet av personcentrerad vård för den äldre, där hjärtat skulle vara med i arbetet. / Background During aging, there may be complex nursing needs and older people may experience reduced ability to make their own decisions. Nurses must work on the basis of person-centered care for the elderly. Person centered care is based on a humanistic care with a holistic view of the individual which is about relationship and participation. The national values of the elderly in the Social Services Act must be respected and applied in caring for the elderly in order to secure their right to a dignified life. Aim The aim of this literature review was to explore nurse’s experiences of working person-centered in the care of the elderly. Method The literature review was based on 13 scientific articles with qualitative and quantitative designs. The databases used to obtain articles were CINAHL and PubMed with publication between the years 2012 – 2022. The analysis was performed as a contentanalysis where similarities and differences were compared. Results In the result of the literature review, four main categories were identified which were participation, caring for the elderly, communication and experiences of person centered care. The results showed nurses’ experiences of person centered care for the elderly. Conclusions It emerged that nurses worked person centered, however there were shortcomings and challenges in the application. The nurses described positive emotions that arose in the application of person-centered care for the elderly, where the heart would be involved in the work.

Hemtjänstpersonalens upplevelser kring äldre och deras alkoholkonsumtion

Intentionen med denna studie har varit att genom en kvalitativ metod undersöka huruvida personer som arbetar inom hemtjänsten i Malmö stad upplever och uppfattar mötet med äldre där det uppkommer problem för personalen i deras arbete på grund av att den äldre har konsumerat alkohol. Detta genom att intervjua åtta hemtjänstpersonal angående deras förutsättningar att leva upp till lagen och den nationella och den lokala värdegrunden i Malmö stad. Vi vill även utröna om de anser att de har tillräckligt med kunskap inom området för att leva upp till dessa intentioner. Den insamlade empirin har bearbetats utifrån Aristoteles kunskapsteori med hjälp av de tre begrepp episteme, techne och fronesis samt utifrån teorin om handlingsutrymme. Det framkom i studien att intervjupersonerna inte främst uppger att det är deras kunskap som begränsar dem i att uppnå de intentioner som finns i lagar och värdegrunder, det som uppges vara de begränsande faktorerna är den äldres självbestämmande och organisationens resursbrist. Intervjupersonerna uppger att kunskapen de har och vilken de använder sig av i sitt arbete kommer främst från deras arbetslivserfarenhet vilket också styrks av tidigare forskning. När det i studien beskrivs hur det kan vara i mötet med de äldre som åsyftas i studien beskrivs detta att det kan se väldigt olika ut, där en del av intervjupersonerna beskriver att det i kan vara en tuff miljö att arbeta i. Då där det har förekommit rädsla för att utsättas för övergrepp. / The intention with this essay has been to study through a qualitative method whether or not people who is working in the homecare in Malmö city experience and perceive the meeting with elderly where there are problems for the staff in their work due to the fact that the elderly have consumed alcohol. By interviewing eight homecare staff regarding their prospects of living up to the law and the national and local value base in Malmö city. We also wanted to find out if they think they have enough knowledge in the area to live up to these intentions. The collected empiric has been processed from Aristotle's theory of knowledge using the three concepts episteme, techne and fronesis, and from the theory of “handlingsutrymme”. It was found in the essay that the interviewees do not primarily state that it is their knowledge that limits them in achieving the intentions contained in laws and the values, which is said to be the limiting factors, is the self-determination of the elderly and the organization's lack of resources. Interviewers state that the knowledge they have and which they use in their work comes mainly from their work experience, which is also supported by previous research. Implementation of a Standardized Multifactorial Fall Prevention Program in a Rehabilitation Facility

One and a half million people are currently living in residential care facilities; as the baby boomer generation ages, this number will increase to 3 million. Approximately 3 out of 4 residents of these facilities fall each year, and 10% to 20% of those falls result in serious injuries such as fractures, disability, and a decreased quality of living. The BOUNCE Back fall initiative is a multifactorial program that uses a systematic approach starting on admission and to re-evaluate a resident following a fall. Nursing and therapy uses the Morse Fall Scale and the Elderly Mobility Scale to assess and categorize the resident's risk for falls. Guided by Lewin's theory of change, this project was designed to assess the effectiveness of the fall initiative as a quality improvement 60-day (August 2016- September 2016) pilot study in a skilled nursing and rehabilitation facility as a potential means to reduce the number of resident falls. Sixty residents (aged 64 to 98, mean age 81) were assessed at a minimum 2 time points to determine their level of fall risk and needed intervention, within 60 minutes of admission to the facility and 7 days postadmission. De-identified pre- and post-implementation data were provided from the corporate quality measure database, entered into a spreadsheet, and numbers were compared. As a result of the fall prevention pilot, for August 2016, 5 falls occurred with no repeat fallers; September 2016, 3 falls with 1 repeat faller which is a significant decrease from 14-22 falls occurring per month for 2 consecutive years. Following implementation, the facility scored 3%-5% for the number of falls, which is below the 7% threshold set forth by the pilot facility's corporate office. Prior to the implementation of the initiative, the facility had not met the 7% fall threshold in 2 years
