Early Age of Cannabis Initiation and Its Association With Suicidal Behaviors

Background: Suicide rates in the U.S. have continued to rise over the last 2 decades. The increased availability and broader legalization of cannabis is a public health concern, particularly among adolescents. The objective of this study was to examine the association between the age of cannabis initiation and lifetime suicidal ideations and attempts in a sample of adults aged 18 or older. Methods: Data are from the Collaborative Psychiatric Epidemiology Surveys (CPES), 2001 to 2003 (N = 15 238). The primary objective of the CPES was to collect data about the prevalence of mental disorders, impairments associated with these disorders, and their treatment patterns from representative samples of majority and minority adult populations in the U.S. Logistic regression analyses were conducted to test the association between cannabis initiation age (early ⩽14 years old; later >14 years old) and outcomes of lifetime suicide ideation and attempts. Cigarette use, cannabis use, gender, income, race, education, and age were controlled for the analyses. Results: Overall, 12.5% of participants reported suicide ideation, while 4.2% reported attempt. Early cannabis use was associated with a higher risk of suicide ideation (AOR = 3.32, 95% CI [2.75, 3.80]) than later cannabis use (AOR = 2.15, 95% CI [1.92, 2.39]). Early cannabis use was associated with a higher risk of suicide attempt (AOR = 4.38, 95% CI [3.48, 5.52]) than later cannabis use (AOR = 2.56, 95% CI [2.14, 3.06]). Wald chi-squared tests revealed significant differences between the early and late initiation for both ideation (χ2 = 26.99; P < .001) and attempts (χ2 = 26.02; P < .001). Conclusions: Significant associations were found between early initiation of cannabis and suicide behaviors. As suicide rates continue to rise, it is recommended that clinicians, treatment providers, and other professionals consider the use of cannabis at an early age as a risk for subsequent suicide behaviors.

Adverse Childhood Experiences and Suicidality and Self-Harm in Persons in Secure Forensic Care

Prior research suggests a greater degree of suicidality and self-harm behavior in those involved with criminal justice and forensic mental health systems. Such individuals also evidence increased exposure to early childhood adversity, which is often associated with suicide risk. Other significant predictors of suicidality have been noted within forensic populations, however, including indicators of specific psychopathology and situational and demographic factors. These populations present with overlapping risk factors that remain underexamined. In the current study, 182 persons residing in secure forensic psychiatric care following incidents of illegal and aggressive behavior were evaluated. Adverse childhood experiences and other empirically derived potential predictors of suicide attempts and self-harm were examined via binomial logistic regression. Findings indicate frequent experiences of early adversity across participants, and that a combination of race, individual adverse childhood experiences, number of biological children, and diagnoses of either posttraumatic stress disorder or borderline personality disorder were significant predictors of suicide attempts, self-harm behavior, and first hospitalization resulting from a suicide attempt. Clinical and research implications are discussed.

La dépression, l’épuisement et la détresse psychologique chez les dentistes au Québec : une étude transversale dans le contexte de la pandémie de la COVID-19

Résumé Contexte : À partir de la mi-mars 2020, la plupart des dentistes en pratique libérale se sont retrouvés sans emploi étant donné le confinement imposé par le département de la santé publique du Québec en raison de la COVID-19. La pandémie a induit de multiples changements ayant pu affecter la santé psychologique des dentistes québécois. Objectif: Évaluer l’état de la santé psychologique des dentistes québécois dans le contexte de la pandémie de la COVID-19 lors du déconfinement à l’été 2020. Méthodes: Le 10 juin 2020, suite à la réouverture des cliniques dentaires après l’état d’urgence sanitaire, un sondage en ligne a été envoyé par courriel aux 5001 dentistes possédant un permis de pratique au Québec. L’Association des chirurgiens-dentistes du Québec (ACDQ), l’Ordre des dentistes du Québec (ODQ) et la Fédération des dentistes spécialistes du Québec (FDSQ) ont participé au recrutement des participants. Le statut du travail et les données sociodémographiques ont été recueillis et la santé psychologique évaluée. L’évaluation de la santé psychologique comprenait trois outils de mesures validés : l’échelle de symptômes dépressifs Patient Health Questionnaire (PHQ-9), l’échelle d’épuisement personnel, professionnel et relié au patient Copenhagen Burnout Inventory (CBI) et l’échelle de détresse psychologique Kessler-10 (K-10). La prévalence d’idéations suicidaires et de tentative de suicide a également été documentée. Les données ont été analysées en utilisant des tests statistiques bivariés et des analyses multivariées de régression logistique. Résultats: Le taux de réponse était de 18% (n=902) avec 835 répondants qui ont fourni complètement les données psychologiques inclues dans les analyses. Une prévalence élevée a été observée pour l’épuisement d’intensité modérée à sévère sur le plan personnel (49.7%, 95%CI=46.3-53.1), sur le plan professionnel (48.4%, 95%CI=44.9-51.8) et lié au patient (37.5%, 95%CI=34.2-40.9). Des symptômes dépressifs d’un niveau moyen à sévère ont été répertoriés chez 23.4% (95%CI=20.5-26.4) des participants et 39.5% (95%CI=36.2-42.9) des participants présentaient un niveau modéré à sévère de détresse psychologique. Les participants percevaient leur santé psychologique comme étant détériorée après le début de la pandémie comparativement à avant la pandémique (p<0.001). Des idéations suicidaires passées ont été relevées par 12.9% (95%CI=10.7-15.4) des participants, dont 1.7% (95%CI=0.9-2.8) qui rapportaient au moins une tentative de suicide. Être une femme était significativement associé au burnout personnel (aOR=1.90, p≤0.001), au burnout professionnel (aOR=1.56, p≤0.01) et à la détresse psychologique (aOR=1.47, p≤0.05). Les dentistes plus âgés (≥ 45 ans) étaient plus susceptibles d’avoir des idéations suicidaires dans les deux dernières semaines (OR= 1.57, p=0.055) comparativement aux dentistes moins âgés. Les dentistes vivant seuls ou étant propriétaires de clinique étaient significativement plus à risque de développer des idéations suicidaires dans les deux dernières semaines (aOR=2.52, p≤0.01, et aOR=2.09, p≤0.01, respectivement) et démontraient plus de troubles de santé psychologique. Faire de l’exercice régulièrement, minimiser la consommation d’alcool, un nombre augmenté d’heures de sommeil et un nombre augmenté de semaines de vacances étaient des marqueurs protecteurs contre un niveau modéré à sévère de troubles psychologiques pendant la pandémie de la COVID-19. L’anxiété d’attraper ou transmettre la COVID-19 aux patients ou les membres de la famille était positivement associée avec le burnout. Les trois stratégies de coping les plus populaires étaient les interactions sociales, le sport et l’écoute de la musique. Conclusion: La santé psychologique des dentistes québécois présente plusieurs défis dans le contexte de la pandémie de la COVID-19. Il y a un besoin urgent d’apporter un soutien psychologique aux dentistes québécois. / Abstract Context: Due to the COVID-19 pandemic, as of mid-March 2020, most Quebec dentists in private practice found themselves unemployed due to the confinement imposed by the Quebec public health department. The psychological health of Quebec dentists was challenged because of several sudden changes in their daily lives. Objectives: To assess the state of the psychological health of Quebec dentists in the context of the COVID-19 pandemic during the summer of 2020. Methods: An online questionnaire was sent to the 5001 licensed dentists in the province of Quebec, Canada on June 10, 2020. Working status, psychological health assessment, and sociodemographic data were evaluated. The validated psychological health measurement tools included the Patient Health Questionnaire (PHQ-9) to evaluate depression, Copenhagen Burnout Inventory (CBI) to measure burnout, and Kessler-10 scale (K-10) to evaluate psychological distress. Suicidal ideation and past suicidal attempts were also assessed. Data were analyzed using bivariate statistical tests and multivariate logistic regression analysis. Results: The response rate was 18% (n=902) with 835 participants providing complete data sets for psychological outcomes included in the analysis. Prevalence of moderate to severe level of personal (49.7%, 95%CI=46.3-53.1), professional (48.4%, 95%CI=44.9-51.8), and patient-related (37.5%, 95%CI=34.2-40.9) burnout was high. Moderate to severe depressive symptoms were found in 23.4% (95%CI=20.5-26.4) of participants and 39.5% (95%CI=36.2-42.9) had moderate to severe psychological distress. Participants perceived their psychological health as worse after the beginning of the pandemic compared to before the pandemic (p<0.001). Past suicidal ideation was reported by 12.9% (95%CI=10.7-15.4) of participants with 1.7% (95%CI=0.9-2.8) reporting at least one suicide attempt. Being a woman was significantly associated with personal burnout (aOR=1.90, p≤0.001), professional burnout (aOR=1.56, p≤0.01) and psychological distress (aOR=1.47, p≤0.05). Older dentists (≥ 45 years old) were more prone to have suicidal ideation over the past two weeks (OR= 1.57, p=0.055) compared to their younger counterparts. Dentists living alone and dental clinic owners were significantly more at risk to develop suicidal ideation over the last two weeks (aOR=2.52, p≤0.01, and aOR=2.09, p≤0.01, respectively) and had worst psychological outcomes. Regular physical exercise, minimizing alcohol consumption, an increased number of sleeping hours and an increased number of vacation weeks per year were found to be significant protective markers against moderate to severe psychological outcomes during the COVID-19 pandemic. Anxiety of getting or transmitting the COVID-19 virus to patients or family members was positively associated with burnout. The three most popular coping strategies during the COVID-19 pandemic were social interactions, sports, and listening to music. Conclusion: Psychological health among Quebec dentists presents several challenges in the context of the COVID-19 pandemic. There is an urgent need to provide psychological support to dentists.

Egenerfarnas perspektiv på vad som främjar återhämtning vid självskadebeteende : en integrerad litteraturöversikt / The self-experienced perspective on what promotes recovery in self-harming behavior : an integrative literature review

Självskadebeteende är relativt vanligt förekommande bland personer med psykisk ohälsa. Återhämtning är ett begrepp som beskriver hur en person kan återerövra och finna kvalitet i sitt liv trots svår sjukdom. Självskadebeteende innebär ett stort lidande för individen och påverkar närstående i personens omgivning negativt. Syftet med denna litteraturstudie var att utifrån egenerfarnas perspektiv sammanställa kunskap om vad som främjar återhämtning vid självskadebeteende. Metoden för studien var en integrerad litteraturöversikt, vilken utgick från 15 vetenskapliga artiklar och resulterade i 6 kategorier. I resultatet framkom att återhämtning hos personer med självskadebeteende kunde främjas av att komma till en vändpunkt, att vara i gemenskap med andra med liknande erfarenheter, att få stöd av närstående, att få professionellt stöd, att förstå och kunna hantera svåra känslor och handlingar, och av att finna inre styrka. Forskning visade även att det bland vårdpersonal florerade negativa attityder och brister i förhållningssättet mot personer med självskadebeteende, vilket motverkade deras återhämtning. Vår slutsats är att etiska reflektioner i arbetsgruppen, handledning, stöd, samt utbildningsinsatser till vårdpersonal är angeläget för att öka förståelsen och höja kompetensen angående självskadebeteende; samt för att minska negativa attityder och förbättra omvårdnaden av personer med självskadebeteende på ett sätt som främjar deras återhämtning.

Bakgrund: Psykisk ohälsa präglas ofta av samsjuklighet och/eller komplexa behov som försvårar bedömningar. Suicid är ett globalt folkhälsoproblem och hälso- och sjukvården skall arbeta med suicidprevention för att undvika vårdskador av såväl given som utebliven vård och omvårdnad. Tusentals personer utför självmordsförsök varje år och det är vanligare att dö i fullbordat suicid än i trafikolycka i Sverige. Sjuksköterskor träffar prehospitalt frekvent suicidnära patienter. Suicidriskbedömning är komplext och dess kvalité beror på kompetens och erfarenhet. Bristfällig suicidriskbedömning kan leda till undvikbara vårdskador för patienter. Syfte: Att undersöka och beskriva upplevelser och erfarenheter av suicidriskbedömning prehospitalt. Metod: Mixad metod av kvantitativ och kvalitativ design bestående av enkät och semistrukturerade intervjuer med sjuksköterskor i ambulanssjukvården. Resultat: Sjuksköterskor upplevde stress, oro och fysiska symtom som ont i magen och tungt i bröstet på väg fram till suicidnära patienter liksom känsla av osäkerhet och otillräcklighet gällande bedömning och bemötande. Sjuksköterskor upplevde skyldighet att förebygga suicid och de flesta hade både identifierat suicidnära patienter liksom utfört suicidriskbedömning prehospitalt utan att ha fått utbildning i det. Diskussion: Resultatet överensstämde med tidigare forskning gällande negativa upplevelser av att möta suicidnära patienter. Slutsats: Sjuksköterskor mötte frekvent suicidnära patienter och utförde suicidriskbedömningar samtidigt som de hade negativa känslor inför mötet med suicidnära patienter och upplevde sig ha varierande såväl reell som formell kompetens för suicidriskbedömning. / Background: Mental illness often characterizes by comorbidity and / or complex needs that complicate assessments. Suicide is a global public health problem and health services must work with suicide prevention to avoid injuries from poor assessment. Thousands of people commit suicide attempts every year and it is more common to die in suicide than in a traffic accident in Sweden. Nurses frequently see suicidal patients outside of hospital. Suicide risk assessment is complex, and its quality depends on competence and experience. Poor suicide assessment could lead to avoidable injuries in patients. Aim: To investigate and describe experiences of suicide risk assessment in emergency setting outside of hospital. Method: Mixed method consisting of quantitative and qualitative design with questionnaire and semi structured interviews with nurses. Results: Nurses experience stress, anxiety, and physical symptoms such as pain in the stomach and heavy feeling in the chest on the way to suicidal patients as well as feelings of insecurity and inadequacy regarding assessment and treatment. Nurses feel an obligation to prevent suicide, and most have both identified suicidal patients as well as performed a suicide risk assessment prehospital without having received training in it. Discussion: The results was consisted with previous research about negative emotions when caring for and assessing the suicidal patient. Conclusion: Nurses frequently meet suicidal patients and perform suicide risk assessments. Adolescence is a critical stage of human development, one that can be especially challenging for teens that are navigating certain vulnerabilities in their identity formation. With the World Health Organization stating that 14% of 10–19 year-olds experience mental health conditions, educational environments are increasingly needed to address mental concerns in their curriculums. This qualitative research project used a combination of art based inquiry and narrative analysis to answer the following question, “How can educational environments promote resilience and positive youth development during adolescent identity exploration?” Findings illuminated several emergent themes that culminated in the following four recommendations for educational environments; 1) Integrate opportunities for creative self-expression in general education spaces, 2) Professional development for educators to integrate Social Emotional Learning (SEL), 3) Create opportunities for social interactions between students and educators, 4) Create and support opportunities for social interactions between students, and 5) Ensure educational spaces include mental health support for students.

SAMMANFATTNING  Bakgrund: Patienter med suicidalt beteende uttrycker att avsaknad av en god vårdrelation kan öka risken för suicidalt beteende. Sjuksköterskor beskriver det som komplext och utmanande att vårda och bemöta patientgruppen och upplever en avsaknad av kunskap. Inom intensivvården riskerar psykosociala behov att nedprioriteras då medicinska behov ofta har ett större fokus.  Syfte: Syftet var att beskriva hur specialistsjuksköterskor inom intensivvård bemöter patienter med ett suicidalt beteende samt hur de tillgodoser deras psykosociala behov.  Metod: Åtta semistrukturerade intervjuer hölls med specialistsjuksköterskor inom intensivvård. Intervjustudien hade en induktiv ansats och analyserades med kvalitativ innehållsanalys.  Resultat: Resultatet visar att specialistsjuksköterskan eftersträvade ett respektfullt, ickedömande och lugnt bemötande till patienter med suicidalt beteende. Kommunikation, delaktighet och förtroende beskrevs som viktiga aspekter för att skapa en god relation med patienten. Psykosociala behov tillgodosågs bland annat genom kommunikation, närvaro och tillgodoseende av fysiska behov. I resultatet framkommer även faktorer, som tidsbrist och synen på rollen, som påverkade bemötandet och hur psykosociala behov tillgodosågs.  Slutsats: Trots en strävan om ett bemötande som skapade en god vårdrelation och en positiv inställning till att tillgodose psykosociala behov förekommer faktorer som påverkar omvårdnaden av patienter med suicidalt beteende negativt. Att uppnå personcentrerad vård av patienter med suicidalt beteende inom intensivvården är idag en utmaning och att uppmärksamma detta är avgörande för att förbättra vården av patientgruppen. Studiens resultat kan ge ledare och personal inom intensivvård insikt i hur omvårdnaden bedrivs för denna sårbara patientgrupp med omfattande psykosociala behov. / ABSTRACT  Background: Patients with suicidal behavior express that the absence of a good caring relationship can increase the risk of suicidal behavior. Nurses describe it as complex to care and interact with the patient group and experience a lack of knowledge. In intensive care, psychosocial needs risk being deprioritized as medical needs often have a greater focus.  Aim: The aim was to describe how nurse specialists in intensive care interact with patients with suicidal behavior and how they meet their psychosocial needs.  Method: Eight semi-structured interviews with nurse specialists in intensive care were conducted. The interview study had an inductive approach and was analyzed with qualitative content analysis.  Results: The results show that nurse specialists strived for a respectful, non-judgmental and calm interaction with patients with suicidal behavior. Communication, participation and trust were important aspects to create a good relationship with the patient. Psychosocial needs were met by communication, being present and meeting physical needs. In the results, factors appeared such as time constraints and the perception of the role, that affected the interaction and how psychosocial needs were met.  Conclusion: Despite ambitions of interactions that create a good caring relationship and a positive attitude to meeting psychosocial needs, factors appeared that affect the care of patients with suicidal behavior negatively. Achieving person-centered care of patients with suicidal behavior in intensive care is challenging and this observation is crucial to improve care. The study can offer leaders and personnel in intensive care insights in how care is conducted for this vulnerable patient group with vast psychosocial needs.

Bakgrund: Stödjande samtal är ett verktyg inom den psykiatriska heldygnsvården som används i behandlande och lindrande syfte med suicidnära patienter. Det konstateras att samtalet var specialistsjuksköterskans viktigaste verktyg inom psykiatrisk omvårdnad. Vid granskning inom ämnet kunde det fastställas att det fanns sparsamt med forskning gällande stödjande samtal med suicidnära patienter i psykiatrisk heldygnsvård i Sverige utifrån specialistsjuksköterskans perspektiv. Syfte: Studiens syfte var att belysa specialistsjuksköterskans erfarenheter av att genomföra stödjande samtal med suicidnära patienter som vårdas i psykiatrisk heldygnsvård. Metod: Studien tillämpar en kvalitativ metod med induktiv ansats och baseras på åtta semistrukturerade intervjuer enligt Graneheims och Lundmans (2004) kvalitativa innehållsanalys. För att inkluderas i studien skulle specialistsjuksköterskan med inriktning psykiatrisk vård ha erfarenhet av stödjande samtal med suicidnära patienter inom psykiatrisk heldygnsvård. Verksamheter bestående av rättspsykiatri samt barn -och ungdomspsykiatri exkluderades ur studien. Resultat: Analysen av datainsamlingen presenterades med temat vikten av ett ömsesidigt möte i samtalet och kategorierna; att vara inkännande, att vara tillgänglig samt samspelets betydelse. Slutsats: För att uppnå möjlighet till ett ömsesidigt möte och ett vårdande stödjande samtal krävs att specialistsjuksköterskor har tid för att kunna bedriva en personcentrerad och patientsäker vård samt för att skapa allianser med patienter inom psykiatrisk heldygnsvård. Det stödjande samtalet styrs av specialistsjuksköterskors erfarenhet och intuition, vilket bidrar till att det stödjande samtalet blir olika för varje individ. Det stödjande samtalet inom psykiatrisk heldygnsvård drabbas om specialistsjuksköterskor inte har någon erfarenhet samt om personalbrist eller tidsbrist förekommer. / Background: Supportive conversations are a tool within the inpatient psychiatric care that is used for the treatment and relieving purpose of suicidal patients. It is established that dialogues were the specialist nurse's most important tool in psychiatric nursing. When examining the subject, it was determined that there was sparingly research regarding supportive conversations with suicidal patients in psychiatric inpatient care in Sweden from the specialist nurse's perspective.Aim: The aim of the study was to shed light on the psychiatric nurse's experiences of carrying out supportive conversations with suicidal patients who are cared for in psychiatric inpatient care. Method: The study applies a qualitative method with an inductive approach and is based on eight semi-structured interviews according to Graneheim and Lundman's (2004) qualitative content analysis. To be included in the study, the specialist nurse specializing in psychiatric care had to have experience of supportive conversations with suicidal patients in psychiatric inpatient care. Departments consisting of forensic psychiatry and child- and adolescent psychiatry were excluded from the study.Result: The analysis resulted in the theme of the importance of a mutual meeting in the conversation with the categories; to be empathetic, to be available and the importance of interaction.Conclusion: In order to achieve the opportunity for a mutual meeting and a caring supportive conversation, the psychiatry nurses must have time to be able to conduct person-centered and patient-safe care and to create alliances with patients in psychiatric inpatient care. The supportive conversation is guided by the psychiatry nurses experience and intuition, which contributes to the supportive conversation being individual. The supportive conversation within psychiatric inpatient care is affected if the psychiatry nurse has no experience and if there is a shortage of staff or a lack of time.

Результатом представляется проект «Развитие осознанного отношения к смерти у молодёжи». / The article analyzes the necessity of forming an informed attitude to death among young people. It was found out that today young people live in a prosperous environment and are mostly satisfied with life, but there are factors provoking the growth of risk of suicidal behavior.
