Obesidade centralizada e stress psicossocial em mulheres de um município da grande São Paulo / Abdominal obesity and psychosocial stress on women from one cty of the great São Paulo

Objetivo. Ao mesmo tempo em que a obesidade aumenta no mundo todo e se torna cada vez mais um problema de Saúde Pública, o stress aumenta no cotidiano das pessoas e na busca pela sobrevivência. Verificar a possível associação entre prevalências de obesidade centralizada e indicadores de stress é o objetivo desse trabalho. Métodos. Utilizam-se dados de um estudo transversal, com informações de 298 mulheres de 20 a 59 anos, moradoras de um município da Grande São Paulo, as quais responderam questionários especialmente elaborados para avaliar o stress psicológico. O diagnóstico de obesidade centralizada foi feito através da medida da circunferência da cintura (CC) e da razão cinturaquadril (RCQ). O stress psicológico foi medido em escores atribuídos às respostas dos questionários e classificado em 3 categorias: isento, resistência e exaustão. A análise estatística foi realizada mediante dois modelos de regressão linear generalizada múltipla entre a variável resposta obesidade centralizada em duas categorias (sim, não) e o stress psicológico em três fases (isento, resistência e exaustão), controlando-se as variáveis demográficas: idade e escolaridade. Resultados. As prevalências de obesidade centralizada foram semelhantes nos dois modelos, respectivamente 40,6 % e 42% para CC e RCQ. As prevalências de stress psicológico foram 61,7% e 8,4% para as fases resistência e exaustão. As associações entre a categoria sim foram positivas e significantes, respectivamente para CC e RCQ (RP 1,51, P 0,028 e RP 1,52, P 0,022) com o stress na fase de exaustão, com o aumento da idade (RP 1,02, P 0,001 e RP 1,01, P 0,002) e com baixa escolaridade (RP 0,67, P 0,030 e RP 0,59, P 0,005). O teste de tendência foi positivo (P 0,029) para a categoria sim do RCQ e aumento das categorias de stress. Conclusões. A fase de exaustão do stress mostrou associação positiva e significante com a obesidade centralizada nos dois modelos estudados, CC e RCQ. O teste significante de tendência com a RCQ sugere efeito gradativo das fases do stress sobre a obesidade centralizada. São necessários, no entanto, outros estudos que comprovem a associação da obesidade centralizada com o stress subdividido em categorias. / Objective. When observing modern life nowadays we find out that, at the same time that obesity increases all around the world and becomes a real concern to public health authorities, we also see stress proliferating in peoples everyday life, specially in the fight for survival. The purpose of this work is to verify the association between prevalence of abdominal obesity and stress indicators. Methods. This work uses given data of a transversal study, containing information of 298 women aged between 20 and 59, inhabitants of the Great São Paulo, who had been submitted to questionnaires especially formulated to evaluate psychological stress. The diagnosis of abdominal obesity was made using two models: measuring Waist Circumference (WC) and Waist - Hip ratio (WHR). Psychological stress was measured in scores attributed to answers of the questionnaires and classified in 3 categories: Exempt, Resistance and Exhaustion. The statistics analysis were carried through two models of multiple generalized linear regression between the variable which is the answer- abdominal obesity focused in two categories (Yes, No) and psychological stress focused in three categories (Exempt, Resistance, Exhaustion) maintaining under control the demographic variables such as age and scholarship. Results. The results referring to the prevalence of abdominal obesity were similar in the two models showing respectively 40.6% and 42% for WC and WHR. The results on the prevalence of psychological stress were 61.7% and 8.4% respectively for the phase of Resistance and the phase of Exhaustion. The associations in the Yes category were classified as being positive and significant, for WC and WHR respectively, Prevalence Ratio PR 1,51, significancy P 0,028 and PR 1,52, P 0,022 for the stress in the phase of Exhaustion, when considered also an increase in age (PR 1,02, P 0,001 and PR 1,01, P 0,002) and a decrease in the level of education (PR 0,67, P 0,030 and PR 0,59, P 0,005) The trend analysis was positive (P 0,029) for the increase of the WHR and the categories of stress. Conclusions. The phase of Exhaustion of Stress showed positive and significant association with the Abdominal Obesity in the two models, WC and WHR. The positive results in the trend tests with the WHR suggest that abdominal obesity may be gradually affected by the phases of stress. Nevertheless, there is the need of further investigation to confirm the association between abdominal obesity and the various categories of stress.

Sedentary lifestyles and energy-rich diets are driving an increasing prevalence of abdominal obesity, which is associated with cardiovascular risk. Reliable estimates of the worldwide prevalence of abdominal obesity are needed to quantify the associated health risk. The International Day for the Evaluation of Abdominal obesity (IDEA) study is a large, international epidemiological cross-sectional study designed to provide reliable data on the distribution of waist circumference according to region, gender, age, and socio-economic level in 177 345 primary care patients from 63 countries across five continents. Any non-pregnant patient aged 18–80 consulting one of the randomly selected primary care physicians on two pre-defined half days was eligible to participate in the study. The primary objective was to estimate the prevalence of abdominal obesity in primary care, in each participating country. Secondary objectives were to estimate the prevalence of hypertension, type 2 diabetes, dyslipidaemia, and smoking, and to evaluate their associations with abdominal obesity, according to age, gender, and socio-economic level and region. The IDEA study will provide the first global map of the prevalence of abdominal obesity and associated comorbidities in primary care practice.

The aim study was to evaluate effect of 24 weeks circuit-based exercise with low-cost equipment over anthropometric and biochemical risk factors for cardiovascular disease in body mass excess women assisted for basic health program attention. Methods: Thirty three women (42,7 ± 8,9 old) body mass excess (body mass index 34,6 ± 7,2 kg/m²) underwent at 24 weeks circuit-based free weight training combined with aerobic exercise. Was evaluated participation frequency, metabolic equivalent, body mass, body mass index, sum skin fold, waist circumference, waist-rip ratio, total cholesterol, LDL-cholesterol, HDL-cholesterol, triglycerides, glycemia, insulin and HOMA-IR. Was not considered minimum participation percentage from statistical analysis, however sample group was splited into tertiles according exercise program aderence. ANOVA from repetead measures with Bonferroni post hoc evaluated intragroup comparisions. ANOVA one-way with Bonferroni post hoc evaluated comparisions among groups at baseline, three and six months. Risk factors that indicated no statistical alterations, but clinical alterations were expressed as variation. Was regarded p<0,05 from statistical significance. Results: Sum skin fold decreased from all group (p< 0,001). Regular participation group showed waist circumference (p=0,023) and waist hip circumference (p=0,046) decrease and LDL-c and total cholesterol showed clinical reduction after six months. Low participation group indicated HDL-c reduction after three (p=0,024) and six months (p=0,035). Intergrop analysis showed HDL-c reduction from lower group when compared with higher tertil after three (p=0,008) and six months (p=0,001). Conclusion: Training program frequency participation is determinant from anthropometric and biochemical factor risk improvement in excess body mass women. Higher active group showed waist circumfereccen, waist-hip circumference, LDL-c and total cholesterol improvement, besides prevent HDL-c lowering. Trial registration: RBR-8rw7dz. / Introdução: O exercício físico é uma alternativa não medicamentosa para o tratamento de morbidades associadas ao excesso de massa corporal. O objetivo do estudo foi avaliar o efeito de 24 semanas de um programa de exercício em circuito com utilização de equipamentos de baixo custo sobre indicadores antropométricos e bioquímicos de risco para doenças cardiometabólicas em mulheres com excesso de massa corporal assistidas por um programa de atenção básica em saúde. Métodos: Participaram do estudo trinta e duas mulheres (42,7 ± 8,9 anos) com excesso de massa corporal (índice de massa corporal 34,6 ± 7,2 kg/m²). Foram submetidas a 24 semanas de treinamento em circuito com pesos livres combinado com exercício aeróbio. Foi avaliada frequência de participação, equivalente metabólico, massa corporal, índice de massa corporal, somatória de dobras cutâneas, circunferência de cintura, relação cintura/quadril, colesterol total, LDL-colesterol, HDL-colesterol, triglicerídeos, glicemia, insulina e HOMA-IR. Não foi considerado percentual mínimo de participação para inclusão das análises estatísticas, porém o grupo amostral foi dividido em três grupos (atividade física regular, atividade física irregular e grupo controle) de acordo com a distribuição, em tercis, da adesão ao programa de exercícios. As alterações intragrupo foram avaliadas por teste ANOVA para medidas repetidas com post hoc de Bonferroni. As comparações entre grupos foram realizadas pelo teste ANOVA de um fator com post hoc de Bonferroni nos momentos incial, após três e seis meses. Os fatores de risco que não indicaram redução estatística, mas que indicaram redução clínica foram expressos em forma de variação. Foi considerado p< 0,05 para significância estatística. Resultados: A somatória de dobras cutâneas reduziu em todos os tercis (p< 0,001). O grupo que teve participaão regular apresentou redução da circunferência de cintura (p = 0,023) e da relação cintura/quadril (p = 0,046), e houve redução clínica dos níveis de LDL-c e colesterol total após seis meses. O grupo que teve baixa participação indicou redução do nível de HDL-c após três meses (p=0,024), e seis meses (p=0,035) Na comparação intergrupos, as mulheres do grupo controleapresentaram redução do nível de HDL-c quanto comparado com o tercil superior (p=0,001). Conclusão: A frequência de participação no programa de treinamento é determinante para melhoras de fatores de risco antropométricos e bioquímicos em mulheres com excesso de massa corporal. O grupo mais ativo apresentou redução da circunferência de cintura, relação cintura/quadril, LDL-c e colesterol total, além de evitar a piora nos níveis de HDL-c. Registro da pesquisa: RBR-8rw7dz

METHODS: Cross-sectional study, developed with 270 PLHA, added 19 years or older recruited from October 2009 to January 2011 in a hospital school in Goiania. To survey the demographic, clinical and lifestyle standardized questionnaires were used. To assess food consumption it was used risk and protection for cardiovascular disease. It was collected measures of weight, height and waist circumference (WC). Body Mass Index (BMI) ≥ 25 kg/m2 was considered to classify overweight and WC ≥ 102 for men and ≥ 88 for woman for abdominal obesity. It was used chi-square test or Fisher exact test to assess the associations. We also calculated as a measure of effect the prevalence ratio (PR), using Poisson regression simple, with a confidence interval of 95% (CI95%). We used Poisson regression with multiple hierarchical analyses for both outcomes. The project was approved by the Ethics Committee of the Hospital of the Federal University of Goiás. RESULTS: The prevalence of overweight was of 33.7% (CI95% 28.2-39.7) and abdominal obesity of 12.6% (95% CI 9.0-17.3). It was observed after multivariate analysis, for the outcome overweight, association with age of 40-49 years (PR:1,16 – CI95%1,05-1,28) and ≥ 50 years (PR: 1.20 – CI95%1,06-1,36), non-use of Highly Active Antiretroviral Therapy (HAART) (PR: 1.15 – CI95%1,05-1,25) and first tertile of risk consuption (PR: 1.15– CI95%1,04-1,28). For the outcome abdominal obesity it was observed association with: female gender (PR: 4.91– CI95%2,49-9,70), age 40-49 years (PR: 4.88 – CI95%1,99-11,98) and ≥ 50 years (PR: 4.99 – CI95%1,87-13,29), personal income greater than four minimum wages (PR: 2.87 – CI95%1,35-6,11) and count CD4 +> 350 cells / mm ³ (PR: 2.45 – CI95% 1,13-5,35). CONCLUSIONS: It was high prevalence of overweight and abdominal obesity, especially amond females, persons older than 40 years without antiretroviral therapy in the lowest tertile of risk foof consumptio, with higher income and T lymphocyte count CD4+. The data in this study point to the need for further monitoring of the nutritional status of this population and development strategies and practices that produce desirable results to the prevention and control of these conditions. / OBJETIVO: Investigar a prevalência de excesso de peso, obesidade abdominal e fatores associados a essas condições em pessoas vivendo com HIV/aids (PVHA). MÉTODOS: Estudo transversal desenvolvido com 270 PVHA, com idade igual ou superior a 19 anos, recrutadas no período de outubro de 2009 a janeiro de 2011 em ambulatório de um Hospítal Escola em Goiânia. Para levantamento das características demográficas, clínicas e estilo de vida foram utilizados questionários padronizados. Para avaliação do consumo alimentar utilizou-se escore de risco e de proteção para doenças cardiovasculares. Realizou-se coleta de dados d peso, altura e circunferência da cintura (CC). Considerou-se como excesso de peso o Índice de Massa Corporal (IMC) ≥25 kg/m2 e como obesidade abdominal CC ≥102 para homens e ≥88 para mulheres. Utizou-se os testes qui-quadrado de Pearson ou teste exato de Fisher para verificar associações. Calculou-se como medida de efeito a razão de prevalência (RP), utilizando-se a regressão de Poisson simples, com intervalo de confiança de 95% (IC95%). Realizou-se regressão de Poisson múltipla com análise hierarquizada para os dois desfechos. O projeto foi aprovado pelo Comitê de Ética do Hospital das Clínicas da Universidade Federal de Goiás. RESULTADOS: A prevalência de excesso de peso foi de 33,7% (IC95% 28,2-39,7) e de obesidade abdominal 12,6% (IC95% 9,0-17,3), Observou-se após análise multivariada, para o desfecho excesso de peso, associação com: idade de 40-49 anos (RP:1,16 - IC95% 1,05-1,28) e ≥50 anos (RP:1,20 – IC95% 1,06-1,36), não uso de terapia antirretroviral de alta potência (TARV) (RP: 1,15 – IC95%1,05-1,25) e menor tercil de consumo de alimentos de risco (RP:1,15 – IC95% 1,04-1,28). Para o desfecho obesidade abdominal observou-se associação com: sexo feminino (RP:4,91 – IC95%2,49-9,70), idade de 40-49 anos (RP:4,88 – IC95%1,99-11,98) e ≥50 anos (RP:4,99 – IC95%1,87-13,29), renda própria maior que quatros salários mínimos (RP:2,87 – IC95%1,35-6,11) e contagem de linfócitos T CD4+ > 350 células/ mm³ (RP:2,45 – IC95% 1,13-5,35). CONCLUSÕES: Foram observadas elevadas prevalências de excesso de peso e obesidade abdominal, especialmente entre o sexo feminino, pessoas de idade maior que 40 anos, sem uso de TARV, em menor tercil de consumo de alimentos de risco, com maior renda e maior contagem de linfócitos T CD4+. Os dados desse estudo sinalizam para a necessidade de ações de monitoramento do estado nutricional dessa população e elaboração de estratégias e condutas que produzam resultados desejáveis à prevenção e controle dessas condições.

Asociación entre Obesidad Abdominal e Hiperuricemia en Pacientes con Diabetes Mellitus Tipo 2 en Lima, Perú / Association between Abdominal Obesity and Hyperuricemia in Patients with Type 2 Diabetes Mellitus in Lima, Peru

Objetivo: Evaluar la asociación entre Obesidad Abdominal (OA) e Hiperuricemia (HU) en adultos diagnosticados con diabetes mellitus tipo 2 (DM2). Métodos: Estudio analítico de tipo transversal, en adultos con DM2 que acudieron a una Clínica Privada de Lima - Perú durante el 2018. Se consideró HU a valores séricos de ácido úrico mayores o iguales a 6 mg/dL y para definir OA se utilizó el perímetro de cintura mayor o igual a 94 cm y 88 cm en hombres y mujeres, respectivamente. Se utilizaron razones de prevalencia (RP) crudas y ajustadas con sus intervalos de confianza al Resultados: Se evaluó a 737 adultos con DM2. El 61,7 % fueron varones y la edad promedio fue 57 años. El 83,2 % presentó OA y el 21,7% HU. La prevalencia de HU en los adultos con OA fue de 23,1% mientras que la prevalencia fue de 14,5 % para los que no eran obesos. La OA se asoció con la HU en el análisis crudo (RP: 1,59 IC95% 1,01-2,50) y se mantuvo estadísticamente significativa en el análisis ajustado (RP: 1,59 IC95% 1,04-2,41) Conclusión: Los participantes diabéticos con OA tienen casi el doble de probabilidad de presentar HU en comparación con los diabéticos sin OA. / Objective: To evaluate the association between Abdominal Obesity (AO) and Hyperuricemia (HU) in adults diagnosed with type 2 diabetes mellitus (TDM2). Methods: Cross-sectional observational study in adults with TDM2 in a private clinic in Lima - Peru. We define HU as serum uric acid values greater than or equal to 6 mg/dL and AO as a waist circumference greater than or equal to 94 cm and 88 cm in men and women, respectively. Crude and adjusted prevalence ratios (PR) were used by 95% confidence intervals. Results: 737 adults with TDM2 were evaluated. 61.7% were male, and the average age was 57 years. AO was present in 83.2 % and HU in 21.7%. The prevalence of HU in adults with AO was 23.1%, while the prevalence was 14.5 % in those not obese. AO was associated with HU in the crude analysis (PR: 1.59 95% CI 1.01-2.50) and remained statistically significant in the adjusted analysis (PR: 1.59 95% CI 1.04-2.41) Conclusion: Diabetic participants with AO are almost twice likely to have HU compared to patients with TDM2 without AO. / Tesis

Obésité abdominale et autres déterminants du risque cardiométabolique dans deux populations urbaines noires apparentées : Port-au-Prince (Haïti) et Cotonou (Bénin)

L’obésité est un problème de santé publique reconnu. Dans la dernière décennie l’obésité abdominale (OA) a été considérée comme une maladie métabolique qui contribue davantage au risque de diabète et de maladies cardiovasculaires que l’obésité générale définie par l’indice de masse corporelle. Toutefois, dans les populations d’origine africaine, la relation entre l’OA et les autres biomarqueurs de risque cardiométabolique (RCM) demeure obscure à cause du manque d’études chez ces populations et de l’absence de valeurs-seuils spécifiques pour juger d’une OA. Cette étude visait à comparer la prévalence des biomarqueurs de RCM (OA, hypertension artérielle, hyperglycémie, dyslipidémie, résistance à l'insuline et inflammation pré-clinique) chez les Béninois de Cotonou et les Haïtiens de Port-au-Prince (PAP), à étudier l’association de l’OA avec les autres biomarqueurs de RCM, à documenter le rôle du niveau socio-économique (NSE) et du mode de vie dans cette association et à ’identifier les indicateurs anthropométriques de l’OA -tour de taille (TT) et le ratio TT/hauteur (TT/H)- et les seuils qui prédisent le mieux le RCM à Cotonou et à PAP. Il s’est agi d’une analyse de données transversales chez 452 adultes (52 % hommes) apparemment en bonne santé, âgés de 25 à 60 ans, avec 200 sujets vivant à Cotonou (Bénin) et 252 sujets à PAP (Haïti). Les biomarqueurs de RCM considérés étaient : le syndrome métabolique (SMet) d’après les critères harmonisés de 2009 et ses composantes individuelles - une OA à partir d’un TT ≥ 94cm chez les hommes et ≥ 80cm chez les femmes, une hypertension, une dyslipidémie et une hyperglycémie; la résistance à l’insuline définie chez l’ensemble des sujets de l’étude à partir du 75e centile de l’Homeostasis Model Assessment (HOMA-IR); un ratio d’athérogénicité élevé (Cholestérol sérique total/HDL-Cholestérol); et l’inflammation pré-clinique mesurée à partir d’un niveau de protéine C-réactive ultrasensible (PCRus) entre 3 et 10 mg/l. Le ratio TT/H était aussi considéré pour définir l’OA à partir d’un seuil de 0,5. Les données sur les habitudes alimentaires, la consommation d’alcool, le tabagisme, les caractéristiques sociodémographiques et les conditions socio-économiques incluant le niveau d’éducation et un proxy du revenu (basé sur l’analyse par composante principale des biens et des possessions) ont été recueillies au moyen d’un questionnaire. Sur la base de données de fréquence de consommation d’aliments occidentaux, urbains et traditionnels, des schémas alimentaires des sujets de chaque ville ont été identifiés par analyse typologique. La validité et les valeurs-seuils de TT et du ratio TT/H prédictives du RCM ont été définies à partir des courbes ROC (Receiver Operating Characteristics). Le SMet était présent chez 21,5 % et 16,1 % des participants, respectivement à Cotonou et à PAP. La prévalence d’OA était élevée à Cotonou (52,5 %) qu’à PAP (36%), avec une prévalence plus élevée chez les femmes que chez les hommes. Le profil lipidique sérique était plus athérogène à PAP avec 89,3 % d’HDL-c bas à PAP contre 79,7 % à Cotonou et un ratio CT/HDL-c élevé de 73,4 % à PAP contre 42 % à Cotonou. Les valeurs-seuils spécifiques de TT et du TT/H étaient respectivement 94 cm et 0,59 chez les femmes et 80 cm et 0,50 chez les hommes. Les analyses multivariées de l’OA avec les biomarqueurs de RCM les plus fortement prévalents dans ces deux populations montraient que l’OA était associée à un risque accru de résistance à l’insuline, d’athérogénicité et de tension artérielle élevée et ceci, indépendamment des facteurs socio-économiques et du mode de vie. Deux schémas alimentaires ont émergé, transitionnel et traditionnel, dans chaque ville, mais ceux-ci ne se révélaient pas associés aux biomarqueurs de RCM bien qu’ils soient en lien avec les variables socio-économiques. La présente étude confirme la présence de plusieurs biomarqueurs de RCM chez des sujets apparemment sains. En outre, l’OA est un élément clé du RCM dans ces deux populations. Les seuils actuels de TT devraient être reconsidérés éventuellement à la lumière d’études de plus grande envergure, afin de mieux définir l’OA chez les Noirs africains ou d’origine africaine, ce qui permettra une surveillance épidémiologique plus adéquate des biomarqueurs de RCM. / Obesity is recognized as a major public health problem. In the last decade abdominal obesity (AO) was considered as a metabolic disorder which further contributes to the risk of diabetes and cardiovascular disease than the general obesity defined by body mass index. However, in African origin population groups, the relationship between AO and other cardiometabolic risk (CMR) biomarkers remained unclear because of the lack of studies in these population groups and the lack of specific cut-off values to define AO. This study aimed to compare the prevalence of CMR biomarkers (OA, hypertension, hyperglycemia, dyslipidemia, insulin resistance and subclinical inflammation) in Beninese from Cotonou and Haitians from Port-au-Prince (PAP), to assess the association between AO and other CMR biomarkers in relation to Cotonou and PAP participants’ socioeconomic and lifestyle parameters and to define anthropometric indicators of AO -waist circumference (WC) and waist-to-height ratio (WHtR)- and specific thresholds that best predict the CMR in both Cotonou and PAP. The cross-sectional study included 452 apparently healthy adults (52 % men), aged 25-60 years, 200 subjects from Cotonou (Benin) and 252 subjects from PAP (Haiti). The CMR biomarkers were: the metabolic syndrome (MetS) according to the harmonized criteria of 2009, its individual components - AO defined by a WC ≥ 94cm in men and ≥ 80 cm in women, hypertension, dyslipidemia and hyperglycemia - Insulin resistance was set at the 75th centile of Homeostasis Model Assessment (HOMA-IR) for the whole sample of subjects. High atherogenicity index (total serum cholesterol/HDL-Cholesterol), subclinical inflammation according to high-sensitivity C-reactive protein (hsCRP) concentration between 3 to 10 mg/L was also assessed. WHtR from a cut-off point of 0.5 was also considered to assess AO. Data on dietary habits, alcohol consumption, tobacco, sociodemographic and socioeconomic status, including education level and a proxy of income (based on principal component analysis of property and possessions) were documented by a questionnaire. Dietary patterns were generated from Western, urban and traditional food frequency in each city by cluster analysis. Receiver Operating Characteristic (ROC) curves were used to assess the performance of specific WC and WHtR cut-offs to predict the CMR. MetS was present in 21.5 % and 16.1 % of participants in Cotonou and PAP respectively. AO was higher in Cotonou (52.5 %) than in PAP (36 %) with higher prevalence in women than in men. The serum lipid profile was more atherogenic in PAP with 89.3 % of low HDL-C in PAP comparing to 79.7 % in Cotonou and high seum TC / HDL-C ratio of 73.4 % in PAP versus 42 % in Cotonou. The specific WC and WHtR cut-off values were respectively 94 cm and 0.59 in women and 80cm and 0.50 in men. Multivariate analysis of AO with the most prevalent CMR biomarkers in these two population groups showed that AO was associated with increased risk of insulin resistance, high atherogenic index and high blood pressure, irrespective of socioeconomic and lifestyle factors. Two dietary patterns were defined: transitional and traditional in each city, but these were not associated with the CMR biomarkers although they were related to socioeconomic variables. This study confirms the presence of several CMR biomarkers in apparently healthy subjects. Additionally, AO was a key element of the CMR in both population groups. However, current WC thresholds should be reconsidered in light of larger studies to better define AO among Black African groups, which will improve the epidemiological surveillance of the CMR biomarkers.

Efeito do consumo de frutas, legumes e verduras na saúde cardiovascular em adolescentes: uma revisão sistemática / Effect of fruit and vegetable consumption on cardiovascular health in adolescents: a systematic review

Introdução: O consumo de frutas, legumes e verduras é pouco frequente entre os adolescentes, e o possível efeito deste consumo na saúde cardiovascular durante esta faixa etária é indefinido. Objetivo: Verificar se existe associação entre o consumo de frutas, legumes e verduras e indicadores de risco cardiovascular em adolescentes (obesidade abdominal, hiperglicemia, hipertrigliceridemia, dislipidemia, hipertensão arterial sistêmica, e síndrome metabólica). Métodos: Registrou-se esta revisão sistemática no PROSPERO (CRD42013004818) para realizar uma revisão sistemática em seis bases de dados eletrônicas (Biomed Central, CINAHL, MEDLINE, PsycINFO, Scopus, Web of Science). Considerou-se o período desde a criação destas bases de dados até sete de Dezembro de 2015 como data mais recente para a atualização das buscas. A estratégia de busca utilizou os seguintes grupos de descritores: faixa etária; frutas, legumes e verduras; indicadores de risco cardiovascular; estudos transversais ou coorte. Os critérios de elegibilidade foram: Artigos em Inglês, Espanhol e Português? estudos originais? amostra composta de adolescentes (dez a 19 anos de idade segundo a organização mundial de saúde); descritores de acordo com os indicadores de risco cardiovascular estabelecidos para adolescentes. Artigos potencialmente elegíveis foram selecionados por dois revisores separadamente. Resultados: Foram identificados 5632 artigos. Após a leitura dos títulos e resumos, 102 artigos potencialmente relevantes permaneceram para a leitura na íntegra. Após seleção, 11 artigos preencheram os critérios de elegibilidade e foram incluídos (dez transversais, uma coorte). As principais razões para a exclusão dos estudos foram classificação da adolescência diferente da preconizada pela Organização Mundial de Saúde, o consumo de frutas, legumes e verduras analisado como parte de um padrão alimentar (por exemplo, juntamente com peixes, laticínios ou cereais), e os indicadores de risco cardiovascular que não foram especificados ou que diferiram das definições estabelecidas. Os artigos avaliaram a ingestão de frutas, legumes e verduras em diversas unidades de medida, utilizando-se questionários de frequência de consumo alimentar (54.5%), recordatório alimentar de 24 horas (27.3%) e registro alimentar (18.2%). Além disso, o consumo de frutas, legumes e verduras foi avaliado separadamente (54.5%), em conjunto (36.4%), apenas legumes e verduras (9.1%), e um estudo incluiu suco de frutas (9.1%). Um terço dos estudos mostraram associações significativas entre o consumo de frutas, legumes e verduras e a pressão arterial sistólica, obesidade abdominal, triglicérides, HDL colesterol e síndrome metabólica. Conclusão: As associações entre o consumo de frutas, legumes e verduras e indicadores de risco cardiovascular em adolescentes são inconsistentes. Isto se deve provavelmente à heterogeneidade nos métodos utilizados para avaliar/classificar o consumo e/ou definir o risco cardiovascular neste grupo etário. Uma vez que os benefícios deste consumo já são bem estabelecidos na saúde cardiovascular de adultos, ainda são necessários estudos adicionais que abordem alta qualidade metodológica para melhor compreender esse fenômeno nos adolescentes / Background: Fruit and vegetable consumption is infrequent among adolescents, and the possible effect of this consumption on cardiovascular health during this age group is undefined. Aim: To investigate the association between fruit and vegetable consumption on cardiovascular risk indicators in adolescents (abdominal obesity, hyperglycemia, hypertriglyceridemia, dyslipidemia, hypertension and metabolic syndrome). Methods: This systematic review was registered in PROSPERO (CRD42013004818), and a systematic review searching electronic databases (Biomed Central, CINAHL, MEDLINE, PsycINFO, Scopus, Web of Science) from inception to December 7, 2015 was conducted. The search strategy used the following sets of descriptors related to: age group; fruits and vegetables; cardiovascular risk indicators; cross-sectional and cohort studies. Eligibility criteria were: Articles in English, Spanish and Portuguese; original studies; sample of adolescents (10-19 year-old according to World Health Organization); descriptors according to the cardiovascular risk indicators. Potentially eligible articles were selected by two reviewers separately. Results: A total of 5632 articles were identified. After reading the titles and abstracts, 102 potentially relevant articles remained for full reviewed. After selection, 11 articles meeting the inclusion criteria were included (10 cross-sectional; 1 cohort). The main reasons for study exclusion were misclassifying adolescence, assessing fruits and vegetables as part of a food pattern (for example, together with fish, dairy, or cereal), and cardiovascular risk indicators that were not specified or that differed from the definitions established. Articles evaluated fruit and vegetable intake in diverse units, using food frequency questionnaires (54.5%), 24-hour-dietary-recalls (27.3%), and food records (18.2%). In addition, fruit and vegetable consumption were assessed separately (54.5%), together (36.4%), or only vegetables (9.1%); and 1 article included fruit juice (9.1%). A third of the studies showed significant inverse associations of fruit and vegetable intake with systolic blood pressure, abdominal obesity, triglycerides, HDL cholesterol, and metabolic syndrome. Conclusion: The associations between fruit and vegetable consumption and adolescent cardiovascular risk indicators are inconsistent. Since the benefits of this consumption are well established in adult cardiovascular health, further studies are necessary, addressing high methodological quality to better understand this phenomenon in adolescents

O Diâmetro Abdominal Sagital (DAS) é uma medida antropométrica relacionada com a gordura visceral e empregada para avaliar a obesidade abdominal, uma variável associada à síndrome metabólica. Sua utilização é indicada na prática clínica para avaliação de risco cardiometabólico em adolescentes obesos. OBJETIVO: Verificar a concordância entre o DAS e a circunferência abdominal (CA) na avaliação da obesidade central e sua associação com os critérios da Síndrome Metabólica e HOMA-IR em adolescentes obesos. CASUÍSTICA E MÉTODOS: Estudo de corte transversal constituído por 83 adolescentes obesos entre 14 e 18 anos, (46 do sexo feminino e 37 do sexo masculino) matriculados no Instituto da Criança do Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo, nos ambulatórios das Unidades de Endocrinologia Pediátrica e de Adolescentes. Foram submetidos a avaliações antropométricas (IMC, Escore Z do IMC, percentual de gordura corporal, circunferência abdominal, diâmetro abdominal sagital), laboratoriais (HDL-c, triglicérides (TG), glicemia (GLIS) e insulina para o cálculo do HOMA-IR) e de pressão arterial sistólica (PAS) e diastólica (PAD), utilizadas para classificação dos critérios da SM. RESULTADOS: Todos os adolescentes apresentaram valores elevados de IMC (36,9±6,7 kg/m2), Z-IMC (+3,27±0,94) e 91,6% da casuística tiveram valores alterados no percentual de gordura corporal (41,4% para o grupo feminino e 36% para o grupo masculino), confirmando a obesidade grave do grupo. Considerando o percentil >=90 do Center for Disease Control and Prevention (CDC), no National Health and Nutrition Examination Survey (NHANES 2011-2014), 85,5% dos adolescentes apresentaram valores elevados de CA (117,7 ± 14,7) e 89,2% valores alterados no DAS (26,9±3,7). Quanto às variáveis laboratoriais, 32,5% dos pacientes apresentaram diminuição de HDL-c e níveis aumentados de: TG (10,8%); GLIS (3,6%); PAS (32,5%,); PAD (21,7%) e HOMA-IR (79,5%), considerando toda a amostra. De acordo com os critérios utilizados pelo International Diabetes Federation, 27,7% da casuística apresentou SM. O DAS demonstrou estar significantemente correlacionado com as variáveis PAS (r=0,489 p < 0,001), PAD(r=0,277 p 0,011) e HOMA-IR (r=0,462 p < 0,001) nos grupos geral, feminino, masculino, com e sem SM. A correlação encontrada entre as medidas do DAS e CA no grupo geral e feminino foi de r = 0,91 (p 0,000) e, no grupo masculino, de r = 0,93 (p 0,000). A concordância entre a CA e o DAS é significante (Kappa k = 0.511; p < 0,001). Nos grupos geral, feminino e masculino com SM, a concordância é mais expressiva (Kappa k = 1,00; p < 0,001.). Esses resultados mostram que os adolescentes apresentavam risco cardiometabólico aumentado e expressiva obesidade central, identificada pelo DAS e CA, apesar de 73,5% deles estarem medicados. O DAS oferece vantagem metodológica na sua mensuração. With regard to laboratory variables, 32.5% of the patients displayed decreased HDL-c and increased values of: triglycerides (10.8%); glycemia (3.6%); systolic blood pressure (32.5%); diastolic blood pressure (21.7%) and HOMA-IR (79.5%). According to the criteria of the International Diabetes Federation (IDF), 27.7% of patients presented metabolic syndrome. SAD was significantly correlated with systolic (r=0.489 p < 0.001) and diastolic (r=0.277 p 0.011) blood pressures and HOMA-IR (r=0.462 p < 0.001) in the general, female and male groups, with and without metabolic syndrome. The correlation between SAD and abdominal circumference in the general and female groups was r = 0.91(p 0.000) and in the male group was r = 0.93 (p 0.000). In the general, male and female groups with metabolic syndrome, the concordance was more expressive (Kappa coefficient; k = 1.00 and p < 0.001). These results show that the adolescents presented increased cardiometabolic risk and significant central obesity identified by SAD and abdominal circumference although 73.5% of the studied patients were maintained under medication for clinical metabolic syndrome symptoms. SAD displayed methodological advantages concerning its measurement. CONCLUSIONS: Under the conditions of this study, it is concluded that: the anthropometric measurements of SAD and abdominal circumference are equivalent for metabolic syndrome classification of the studied adolescents; that SAD is a predictor of systolic and diastolic blood pressures and HOMA-IR and is a strong indicator of cardiometabolic risk in obese adolescents

In the general, male and female groups with metabolic syndrome, the concordance was more expressive (Kappa coefficient; k = 1.00 and p < 0.001). These results show that the adolescents presented increased cardiometabolic risk and significant central obesity identified by SAD and abdominal circumference although 73.5% of the studied patients were maintained under medication for clinical metabolic syndrome symptoms. SAD displayed methodological advantages concerning its measurement. CONCLUSIONS: Under the conditions of this study, it is concluded that: the anthropometric measurements of SAD and abdominal circumference are equivalent for metabolic syndrome classification of the studied adolescents; that SAD is a predictor of systolic and diastolic blood pressures and HOMA-IR and is a strong indicator of cardiometabolic risk in obese adolescents
