Living kidney donor follow-up in a statewide health information exchange: health services utilization, health outcomes and policy implications

Indiana University-Purdue University Indianapolis (IUPUI) / Living donors have contributed about 6,000 kidneys per year in the past 10 years, but more than 100,000 individuals are still waiting for a kidney transplant. Living kidney donors undergo a major surgical procedure without direct medical benefit to themselves, but comprehensive follow-up information on living donors’ health is unfortunately limited. Expert recommendations suggest capturing clinical information beyond traditional sources to improve surveillance of co-morbid conditions from living kidney donors. Currently the United Network for Organ Sharing is responsible for collecting and reporting follow-up data for all living donors from U.S. transplant centers. Under policy implemented in February of 2013, transplant centers must submit follow-up date for two years after donation, but current processes often yield to incomplete and untimely reporting. This dissertation uses a statewide Health Information Exchange as a new clinical data source to 1) retrospectively identify a cohort of living kidney donors, 2) understand their follow-up care patterns, and 3) observe selected clinical outcomes including hypertension, diabetes and post-donation renal function.

College students' alcohol use, sexual aggression, and beliefs about sexual consent

Alcohol consumption among college students is a well-established phenomenon, as is the observation that they often do not abide by safe consumption practices. Of particular concern is the association between college students’ heavy drinking practices, reduced understanding and practice of sexual consent, and sexual aggression (e.g., coercion, unwanted physical contact, or rape). The aim of this study was to expand the current literature by assessing the degree to which college students’ self report of no, low/moderate, and heavy alcohol consumption was associated with (1) self reports of experiencing or committing sexually aggressive behavior, and (2) their understanding of what constitutes sexual consent between individuals. Understanding these issues may assist with the development of future prevention programs aimed at reducing the link between college students’ consumption of alcohol and risky sexual behaviors.

Information and education of senior patients in the frame of health care. This thesis focuses on informations providing by health care professionals to the seniors age patients/clients. I describe the contemporary situation in this field in the Czech republic, and define main attributes and conditions according to the historical, social, cultural and moral context in the theoretical part of thesis. I introduce the transmission and the acceptation of the information during the dialogue between a layperson and an expert in two basic levels: level of rules, especially an Inform consent, and level of an ethical discourse. I compare an atmosphere created by law and rules with human reactions, either as medical professionals, or as a patient. For the empirical section of the thesis, I describe and evaluate experiences of patients with information-giving by health care professionals. I have chosen qualitative method for the main body of my empirical research, which consists of semi-structured interviews with 20 patients. The collected data were cathegorised, analysed and then quantified and prepared for interpretation. Navigating accountability in humanitarian photography at sea : a snapshot of embedded photographer practices in obtaining informed consent during I/NGO search and rescue operations in the Mediterranean

This thesis aims to contribute to ethical discussions on the production of photography in different humanitarian contexts, and in circumstances where it is facilitated by non-governmental and international non-governmental organisations (I/NGOs). Humanitarian photography is often reproduced and circulated in various forms by different actors, highlighting the need for research on the actions, decisions, and interactions that influence how these images are produced. An extensive literature review captures the many ethical challenges surrounding humanitarian photography and provides an overview of related standards. A conceptual framework is then built around informed consent as an accountability mechanism, with consideration for certain relational and situational factors that influence the quality and effectiveness of the process of obtaining consent. Emphasising photographer and organisational accountability, an analysis of how photographers apply the concept of informed consent and its potential as an accountability mechanism is explored through the case study of embedded photographers in search and rescue (SAR) I/NGO operations in the Mediterranean Sea.     Four semi-structured in-depth interviews were completed with photographers involved in SAR I/NGO missions in the Mediterranean from 2015 to 2021. The interviews suggest that a continuous and deliberate process of individual, organisational, and collaborative self-regulation unfolds throughout a mission, largely through verbal communication and body language, in an effort to obtain consent to take or use images of people who have been rescued. Use of formal means such as written consent forms are only rarely used. Photographs during the rescues were generally taken without prior consent, and photographers’ ability to obtain meaningful subsequent informed consent was easily compromised due to the unpredictable conditions during SAR operations and the variation across I/NGO-photographer partnerships. These findings support the need for further dialogue in this context to ensure that practices and processes related to the production of humanitarian photography, such as obtaining informed consent, are compatible with humanitarian principles, respect the rights and dignity of people affected by crisis, and foster greater accountability.

In the age of the General Data Protection Regulation (GDPR) and the California Consumer Privacy Act (CCPA), privacy and consent control have become even more apparent for every-day users of the internet. Privacy banners in all shapes and sizes asks for your permission through more or less challenging designs and makes privacy control more of a struggle than actually helping the users’ privacy. This thesis presents a novel solution expanding on the Advanced Data Protection Control (ADPC) mechanism in order to bridge current gaps in user data and privacy control. It moves the consent control to the browser interface to give a seamless and hassle-free experience for users, while at the same time offering content providers a way to be legally compliant with legislation including the GDPR. Motivated by an extensive academic review to evaluate previous work and identify current gaps in user data control, the aim of this thesis is to present a blueprint for future implementation of suggested features to support privacy control online for users globally.

В связи с постоянным ростом мошенничества в отношении клиентов банков – держателей карт, в диссертации были рассмотрены виды мошенничества и произведена их систематизация по каналу взаимодействия с клиентом. При этом, были рассмотрены инструменты, используемые коммерческими банками и Банком России для снижения риска несанкционированных (мошеннических) операций по банковским картам и системам дистанционного банковского обслуживания, выявлен наиболее эффективный инструмент – мониторинг операций. Также были разработаны рекомендации по управлению рисками при осуществлении операций по банковским картам и системам дистанционного банковского обслуживания. / Due to the constant growth of fraud in relation to customers of card – holding banks, the dissertation examined the types of fraud and systematized them according to the channel of interaction with the client. At the same time, the tools used by commercial banks and the Bank of Russia to reduce the risk of unauthorized (fraudulent) transactions on bank cards and remote banking systems were considered, and the most effective tool was identified – monitoring of transactions. Recommendations were also developed for risk management in the implementation of bank card transactions and remote banking systems.

Sexuell och reproduktiv hälsa : En enkätstudie om undervisningen i sexualitet, samtycke och relationer samt ett förslag på en hälsofrämjande kursplan / Sexual and reproductive health : A survey study on the teaching of sexuality, consent and relationships and a proposal for a health-promoting curriculum

Introduktion: Den sexuella hälsan kan förslagsvis främjas genom insatser som en god sexualundervisning. Skolan har ingen kursplan i sexualitet, samtycke och relationer vilket medför att skillnaden på sexualundervisningen i Sveriges skolor är stor vilket kan leda till riskfyllda sexualvanor, fler partners och låg kondomanvändning.  Syfte: Det primära syftet var att undersöka hur människor upplever den nuvarande sexualundervisningen i skolan. Det sekundära syftet var att ta fram ett konkret material som skolväsendet kan använda för att kunna bedriva en hälsofrämjande sexualundervisning för barn och unga.  Metod: En empirisk kvantitativ enkätstudie har tillämpats. En webbenkät lades ut på Facebook och resultatet sammanställdes i Google Forms. Sen användes enkätresultaten från denna studie tillsammans med resultat från tidigare studier som har granskats i samband med detta arbete för att skriva en kursplan.  Resultat: 51 stycken respondenter svarade på enkäten i åldrarna 20-41 år. Majoriteten av respondenterna upplevde att de inte var nöjda med den sexualundervisning de har fått. Många av respondenterna uppgav att samtliga ämnen inte har tagits upp i tillräckligt stor utsträckning och många upplevde att de inte hade tillräckligt med information i dessa ämnen. 1 av 3 tyckte att undervisningen saknade en övergripande kvalitet och flertalet lyfte att de saknade en behörig lärare.  Diskussion: En möjlighet är att den sexuella hälsan blir lidande av den bristfälliga undervisningen i form av att eleverna missar tillfällen att utveckla empowerment och säkerhet kring sin sexualitet och identitet. En kursplan i sexualitet, samtycke och relationer kan skapa en ökad trygghet hos lärarna vilket också ökar kvaliteten på undervisningen. Ett förslag är att universitetsstudenter som har kompetens inom detta område, exempelvis studenter på sjuksköterskeprogrammet, hälsovägledarprogrammet eller andra studenter inom andra relevanta utbildningar, undervisar studenter på grundskolenivå som en del av båda parters utbildning. / Introduction: Sexual health can be promoted through initiatives such as good sex education. The school has no curriculum in sexuality, consent and relationships, which means that the difference in sexuality education in Sweden's schools is large, which can lead to risky sexual habits, more partners and low condom use. Purpose: The primary purpose was to investigate how people experience the current sex education in school. The secondary aim was to produce concrete material that the school system can use to be able to conduct health-promoting sex education for children and young people. Method: An empirical quantitative survey study has been applied. A web survey was posted on Facebook and the results were compiled in Google Forms. The survey results from this study were then used together with results from previous studies that have been reviewed in connection with this work to write a course plan. Results: 51 respondents answered the survey aged 20-41. The majority of respondents felt that they were not satisfied with the sex education they have received. Many of the respondents stated that all topics have not been covered to a sufficient extent and many felt that they did not have enough information on these topics. 1 in 3 thought that the teaching lacked an overall quality and the majority pointed out that they lacked a qualified teacher. Discussion: One possibility is that sexual health suffers from the inadequate teaching in the form of students missing opportunities to develop empowerment and security around their sexuality and identity. A curriculum in sexuality, consent and relationships can create increased security among teachers, which also increases the quality of teaching. One suggestion is that university students who have competence in this area, for example students in the nursing program, the health guidance program or other students in other relevant educations, teach students at primary school level as part of both parties' education.

Vers une phénoménologie féministe et critique de la sexualité

This thesis offers an analysis of Maurice Merleau-Ponty’s phenomenology of the body in the light of feminist philosophy. It reconstructs this philosopher’s account of corporeity, before showing its main weaknesses: that of obscuring the gendered dimension of the body, in particular in its sexual aspect. Thus, I point out the blind spots of a phenomenology of the body widely criticized for its phallocentrism. However, by focusing largely on the Phenomenology of perception, feminist phenomenology obscures an ontology of the flesh which is nevertheless rich for thinking the body in its gendered and sexual dimensions. By then focusing on a critical and feminist reformulation of a phenomenology of the erotic, this dissertation offers to analyze the later reformulations of the concept of corporeality in that of flesh in order to respond to certain issues localized in the Phenomenology of Perception, notably that of the very possibility of a thought of sexual difference. Finally, I apply the revised concepts of body and intentionality in a contemporary debate in feminist philosophy of sexuality: I propose a redefinition of the concept of sexual consent from a phenomenological perspective drawing from Merleau-Ponty, in order to show the emancipatory potential of thinking sexuality and the erotic in phenomenological, critical, and feminist terms. / Ce mémoire propose une analyse de la phénoménologie du corps de Maurice Merleau-Ponty à l’aune de la philosophie féministe. Il reconstitue la pensée de ce philosophe sur le concept de corporéité, avant de montrer ses faiblesses notoires : celles d’occulter la dimension genrée des corps, en particulier dans la sexualité. Ainsi, ce mémoire relève les angles morts d’une phénoménologie du corps largement critiquée pour son phallocentrisme. Or, en se concentrant sur la Phénoménologie de la perception, la phénoménologie féministe oublie l'ontologie de la chair pourtant riche pour penser le corps dans sa dimension genrée et sexuelle. En s’intéressant à une reformulation critique et féministe d’une phénoménologie de l’érotique, ce mémoire propose en somme d’analyser les reformulations tardives du concept de corporéité dans celui de chair pour répondre à certains problèmes inhérents à la Phénoménologie de la perception, notamment celui de la possibilité même d’une pensée de la différence sexuelle. Finalement, ces concepts de corps et d’intentionnalité sont appliqués à un débat actuel en philosophie féministe et en philosophie de la sexualité sur les limites du concept de consentement sexuel. Dans une perspective phénoménologique de tradition merleau-pontienne, ce mémoire a pour but de montrer le potentiel émancipateur issu d'une pensée de la sexualité et de l’érotique en des termes critiques et féministes.

Syftet med studien var att undersöka hur ungdomar upplever att deras föreställningar av sex och relationer påverkas utav pornografiskt material samt hur vuxna kan vara ett positivt stöd för ungdomar vid utforskandet av sin sexualitet och relationer. 217 ungdomar deltog varav 85 tjejer, 128 killar och 4 identifierade sig som icke-binär. Medelåldern på deltagarna var 17 år. Studien genomfördes via en kvantitativ ansats och webbenkät via Microsoft Forms bestående av 42 frågor. Analys av empiri gjordes genom programmet IMB SPSS statistics v. 28 där deskriptiva samt bivariata parametriska t-tester utfördes.Vidare användes Pearsons korrelationsanalys vid parametriska tester och vid fåtal tillfällen användes Spearmans korrelationsanalys vid icke normalfördelade variabler. Teori om sexuella skript brukades för att förstå, tolka samt analyseraresultat. Resultaten visade att ungdomar påverkades utav pornografiskt material i deras föreställningar om sex och relationer både i förväntningar på sex samt vad det ska innehålla samt att vi fann skillnader för de ungdomar som var i relation jämfört med de som var singlar. Fynd visade även att ungdomar hade behov av stöd av samhället och vuxna i utforskandet utav relationer och sex då många ungdomar saknade en vuxen att tala med. Det framkom även att ungdomarna upplevde brister i skolans sexualundervisning. Avslutningsvis redogörs diskussion kring problematiken att killar och tjejer uppvisade skillnader mellan våldsamt sex och samtycke samt förslag på framtida forskning.
