Den invasiva Blomsterlupinens (Lupinus polyphyllus) förekomst inom och utanför artrika vägkanter / Occurrence of the invasive Garden lupine (Lupinus polyphyllus) within and outside species rich road verges

Invasiva arter är ett stort hot mot den biologiska mångfalden världen över och den invasiva blomsterlupinen bidrar till minskad biodiversitet i de miljöer som den invaderar. Den är ett stort hot mot organismer i artrika infrastrukturmiljöer såsom artrika vägkanterna, vilka innehar en hög biologisk mångfald med hotade och sällsynta arter. I denna studie används GIS (Geografiskt informationssystem) för att undersöka blomsterlupinernas utbredning längsmed vägnätet i Jämtlands län. Resultatet visar att icke artrika vägkanter har en större andel vägkant med förekomst av blomsterlupiner än artrika vägkanter. Det finns ingen skillnad mellan artrika vägkanter med en bedömd högre värdeklass än artrika vägkanter med en bedömd lägre värdeklass vad gällande andelen vägkant med förekomst av blomsterlupiner. Att urskilja bakomliggande orsaker till det framkomna resultatet är problematiskt eftersom vägkanterna är heterogena miljöer som dessutom påverkas av många abiotiska och biotiska faktorer. Förekomsten av störningar snarare än biotisk resistens framhålls som tänkbar förklaring och utpekas som en inriktning för vidare forskning kring spridningen av blomsterlupiner inom vägnätet. / Invasive species are a major threat for biodiversity worldwide and the invasive Garden Lupine has shown to decrease biodiversity in the environments it invades. The invasion of Garden Lupines threatens infrastructure habitats, such as species rich road verges, which possess a high biodiversity with endangered and rare species. In this study, I used GIS (Geographic Information System) to investigate the spread of the Garden Lupine along the road network in the Swedish county of Jämtland. The result shows that non species-rich road verges have a larger proportion of road sections with occurrence of Garden Lupine than species rich road verges. A comparison between higher status species-rich road verges and lower status species rich road verges showed no difference in the proportion of the road section that contain Garden Lupine. Distinguishing the underlying causes to the result is problematic since road verges are heterogenic habitats under influence of a large variety of abiotic, biotic and anthropogenic factors. The presence of disturbances is a more likely explanation to the occurrence patterns than biotic resistance. This would also be a potential direction for further research concerning the spread of Garden Lupines along road verges.

Hur Biofouling Påverkar Östersjön : En allmän litteraturstudie / How Biofouling Affects the Baltic Sea : A general literature study

The aim of this literature study was to investigate the effects of biofouling in the Baltic Sea. This was done to increase environmental awareness and understanding of the importance of a sustainable marine environment. A systematic search in selected databases resulted in the study being based on tio research articles. The overall results of the study show that biofouling demonstrate negative effects, but the effects are not fully understood to date and the focus is still on economic aspects. Furthermore, it was observed that the studied organizations are currently focused on greenhouse gas issues, but they have also been paying attention to biofouling problems in recent time. The literature study provides important insights into the effects of biofouling.

Hunden som resurs inom svensk naturvård

Användningen av sökhundar som hjälpmedel och verktyg inom naturvården är i Sverige hittills ett litet område med några få verksamma aktörer. Men precis som hos tull, polis och försvarsmakt kan hundarna implementeras i naturvårdsverksamheten som ett standardverktyg. Hunden har en unik förmåga att selektera dofter och vilja att samarbeta med människan. Dessa egenskaper kan utnyttjas för att effektivisera arbetet inom naturvården. Vi har gjort en sammanställning av aktörer i Sverige samt en litteraturstudie över forskningsområdet och jämfört dessa för att ge förslag till vidare användning av naturvårdssökhundar i Sverige. För flera av användningsområdena finns forskning som stödjer hundarnas effektivitet. Inom några användningsområden finns däremot ingen forskning, därför hade dessa behövt studeras närmare för att bekräfta hudarnas effektivitet. Flera potentiella nationella användningsområden för naturvårdssökhundar har identifierats, som spillningsinventering av fladdermöss, skadegörare som almsjuka, hotade arter, invasiva växter och däggdjur, läckor på avloppsledningar, nedlagda deponier och laboratorieanalyser. Vårt förslag för att underlätta framtida arbete både för kunder och aktörer, är en samlad databas där alla operativa aktörer inom naturvårdssökhund är inkluderade. / In Sweden the use of detection dogs as a tool in nature conservation has until now been a small field, with few practitioners. In the daily operations of customs, police and the armed forces, detection dogs are a natural part of the organization and the same could be possible in conservation work. The dog’s ability to select between scents and their willingness to work with humans make them unique, utilizing these traits can make conservation work more efficient. The Swedish practitioners are compiled, and the available research is reviewed, the practitioners are then compared to the international research, to suggest further implication in Sweden. There are some practitioners working with conservation detection dogs today, research supports the dog’s efficiency in some areas, but in some areas research is absent. These areas would need studies to confirm the dog’s efficiency. We suggest potential areas for implantation in Sweden, such as scat detection of bats, pests like elm decease, threatened species, invasive species, sewage leaks, disused landfills, and laboratory analyses. Our suggestion is a national database for all conservation detection dogs, to aid future work for both practitioners and customers.

Tesis por compendio / [ES] Esta tesis se enfoca en el desarrollo de dispositivos inteligentes para detectar metabolitos de sustancias tóxicas y la sobreexpresión enzimática relacionada con enfermedades. Se aborda la limitación de las técnicas de diagnóstico convencionales y se destaca la utilidad de la orina como muestra biológica. Se discuten conceptos de materiales mesoporosos de sílice, reconocimiento molecular y sondas moleculares fluorogénicas. Los objetivos se detallan, y luego se presenta un nanodispositivo para detectar ácido trans,trans-mucónico (t,t-MA) en orina (S4). Se describe una sonda fluorogénica en el infrarrojo cercano para la detección de elevados niveles de alanina aminopeptidasa en orina (NB-ALA). También se presentan nuevas sondas para detectar biomarcadores de cáncer (NB-SO3-Leu y NB-SO3-Ala), resaltando la importancia de su solubilidad y eliminación renal. Finalmente, se propone una sonda molecular no invasiva (Cy7-MAO) para detectar la enzima monoamina oxidasa a través de medidas de fluorescencia en la orina. It addresses the limitations of conventional diagnostic techniques and highlights the usefulness of urine as a biological sample. Concepts of mesoporous silica materials, molecular recognition, and fluorogenic molecular probes are discussed. The objectives are outlined, and then a nanodevice for detecting trans, trans-muconic acid (t,t-MA) in urine (S4) is presented. A near-infrared fluorogenic probe for the detection of high levels of alanine aminopeptidase in urine (NB-ALA) is described. New probes for detecting cancer biomarkers (NB-SO3-Leu and NB-SO3-Ala) are also introduced, emphasizing the importance of their solubility and renal elimination. Finally, a non-invasive molecular probe (Cy7-MAO) is proposed for detecting monoamine oxidase enzyme through fluorescence measurements in urine. Mekanisk uppgrävning: ett effektivt verktyg mot den invasiva främmande arten gul skunkkalla (Lysichiton americanus) : En utredning av behandlingsmetoder

Den biologiska mångfalden minskar globalt och ett av de största hoten utgörs av invasiva främmande arter. Natura-2000 området Fylleån, Halland, huserar en av de största svenska populationerna av gul skunkkalla (Lysichiton americanus). Arten utgör ett hot mot de naturvärden som finns i Fylleån och är subjekt för extensiva behandlingsarbeten i området för att motarbeta växtens effekt på ekosystemet. Det finns i dagsläget ingen evidens för att behandlingsmetoderna som presenteras av Naturvårdsverket och Havs- och vattenmyndighetens metodkatalog har effektivitet i svenska vattendrag. Målet med min studie är att undersöka effektiviteten av mekanisk uppgrävning samt ytlig hetvattensbehandling som behandlingsmetoder för gul skunkkalla. Jag undersöker även lämpligheten för användandet av övriga metoder presenterade av metodkatalogen och metoderna hetvattensbehandling och fröställningsborttagning som inte listats i metodkatalogen men som testas av myndigheter i viss utsträckning. Den första delen av studien genomförs som jämförelsestudie mellan områden som behandlats i olika grad med mekanisk grävning och hetvattenbehandling för att bestämma metodernas effektivitet. Den andra delen av min studie var att genom en litteraturgenomgång med metodkatalogen som utgångspunkt avgöra lämpligheten på och reflektera över metoderna samt reflektera över metodernas lämplighet att användas civilt. Min slutsats är att mekanisk grävning ger minskande populationer och signifikanta skillnader i individantal, förutsatt att behandlingen utförs rätt och konsekvent. Jag resonerar att andra behandlingsmetoder kan påverka mer organismer än vad man avsett och att de därför är olämpliga att bruka i Fylleån. Jag diskuterar vad jag anser är rimligt idéellt arbete i nuläget samt framtida potential och behovet av information i rätt kanaler. / Biodiversity is decreasing globally and one of the biggest threats is invasive alien species. The Natura-2000 area Fylleån, Halland, houses one of the largest Swedish populations of the American skunk cabbage (Lysichiton americanus). The species poses a threat to the natural values ​​found in Fylleån and is subjected to extensive treatment to counteract the plant's effect on the ecosystem. There is currently no evidence for the effectiveness of the treatment methods presented by Naturvårdsverket and Havs- och vattenmyndighetens method catalog in Swedish watercourses. The goal of my study is to investigate the effectiveness of mechanical excavation and superficial hot water treatment as treatment methods for American skunk cabbage, along with hot water treatment and seed stand removal which are not listed in the method catalog but are tested by authorities to a certain extent. Through a comparative study between areas treated to varying degrees with mechanical digging and hot water treatment, I determine the effectiveness of the methods. The second part of my study was to try to determine the suitability of and reflect on the methods, as well as reflect on the suitability of the methods for civilian use through a literature review using the method catalog as a starting point. My conclusion is that mechanical digging produces declining populations and significant differences in found individuals, provided the treatment is carried out correctly and consistently. I reason that other treatment methods can affect more organisms than intended, and that they are therefore unsuitable for use in Fylleån.

Aspergilose invasiva em pacientes imunodeprimidos: comparação entre as provas de galactomanana, 1,3 betaD-glucana, dados tomográficos e desfecho clínico / Performance of galactomannan and 1,3 beta-glucan enzyme assays in the serum and bronchoalveolar lavage and comparison with computer tomography scan for the diagnosis of invasive aspergillosis in immunocompromised hosts

A aspergilose invasiva (AI) é a infecção por fungos filamentosos mais comum em pacientes imunodeprimidos, especialmente em transplantes de células tronco hematopoiético e neoplasias hematológicas. Objetivo: Geral: Estabelecer a comparação entre a dosagem de Galactomanana (GM), 1,3betaD-glucana (BDG) e dados tomográficos no diagnóstico da AI bem como seu papel no desfecho clínico. Específicos: 1. Verificar a sensibilidade e especificidade dos ensaios de Galactomanana e de 1,3betaD-glucana no soro e lavado broncoalveolar. 2. Comparar os resultados da galatomanana e 1,3betaD-glucana com os dados de imagem em pacientes com suspeita de AI. 3. Verificar a relação entre a evolução dos níveis de GM e desfecho clínico (óbito e sobrevida). Casuística, Materiais e Métodos: Realizou-se um estudo tipo coorte prospectiva, incluindo 398 sujeitos das diversas enfermarias de pacientes imunodeprimidos do HCFMUSP, sendo incluídos dois grupos de pacientes: 202(51%) AI e 198(49%) controles. Resultados: Dos casos, 18 (8,8%) tinham aspergilose provada, 28 (13,7%) provável e 158 possível (77,5%), de acordo a classificação de 2002 EORTC/MSG (European Organization for Research and Treatment of Cancer / Mycoses Study Group). Os sujeitos submetidos ao TCTH eram 42,7%, com neoplasias hematológicas 37%, TOS 9% e outras doenças 11,3%. Os fatores de risco associados ao desenvolvimento da AI foram neutropenia, monocitopenia, uso de corticóide, presença de doença pelo citomegalovírus e rejeição ou doença do enxerto contra o hospedeiro. O fator de risco associado à evolução para o óbito foi a presença de AI. Foram observados bons desempenhos para a GM tanto no soro como no LBA com LR menores que os registrados na literatura. O melhor desempenho da GM no soro para aspergilose+provável ocorreu com LR de 0,35 com sensibilidade-S, especificidade-E, valor preditivo positivo- VPP), valor preditivo negativo-VPN) e área sob a curva-ASC de 54,4%, 73,4%, 50,8%, 76,2% e 0,64, sendo os valores superiores para aspergilose provada tanto na S, como E, VPN. No LBA os valores de S-E-VPP-VPN-ASC para GM para LR de 0,65 para aspergilose provável + provada foram 58,3%, 92,6%, 87,5%,71,4% e 0,75, sendo na aspergilose provada os valores de S, e VPN superiores. Nesta casuística, o melhor desempenho para BDG no soro apontou para uma LR de 100 pg/mL na aspergilose provável+provada, com 54,5%, 73,4%, 50,8% e 76,2%, 0,64 respectivamente para S-E-VPP-VPN-ASC. Para BDG no LBA, a LR na aspergilose provável + provada foi de 140 pg/mL, com os mesmos valores de 46,7%, 76,7%, 70%, 55,6% e 0.62, respectivamente. Conclusão: A GM no LBA e no soro foram úteis no diagnóstico da aspergilose mediante emprego de LR menores, sendo mais sensível na LBA, principalmente em estágios iniciais da forma angioinvasiva. A persistência de GM sérica foi relacionada ao óbito em relação à negativação da mesma. A proporção de concordância entre a TC e os biomarcadores no soro e no LBA variou de 0,5 a 0,6, com pequena concordância na estatística kappa. Excelente concordância foi observada entre dois radiologistas independentes, que analisaram de maneira cega as TC de sujeitos com aspergilose provada. Nesta casuística com inclusão de doenças sistêmicas e endêmicas, a BDG teve baixo desempenho diagnóstico / Invasive aspergillosis (IA) has become the leading infectious cause of death in immunocompromised hosts, particularly in subjects under SCTH and hematologic neoplasias. Objectives: General: To compare the performance of GM and BG tests in serum and bronchoalveolar lavage fluid (BAL) and computer tomography (CT) scans in the diagnosis of IA in immunocompromised hosts as well as their role in the patient outcome. Specific: 1. To analyse the sensitivity and specificity of Galactomannan and 1,3 betaD-glucan assays in the serum and bronchoalveolar lavage. 2. To compare the results of Galactomannan and 1,3betaD-glucan assays with CT scans in patients with invasive aspergilosis. 3. To analyse the relationship between the evolution of galactomannan levels and clinical outcome (death or survival). Patients, Materials and Methods: From December 2008 to March 2013, a prospective cohort of 398 patients from several wards of immunocompromised patients of Hospital das Clínicas, Faculdade de Medicina, University of São Paulo was included classified in two groups of patients: 202 (51%) with invasive aspergillosis (IA) and 198 (49%) control patients. Results: Considering 202 cases, 18(8.8%) were subjects with proven, 28(13.7%) with probable aspergillosis and 156(77.5%), with possible aspergillosis, according to 2002 EORTC/MSG (European Organization for Research and Treatment of Cancer/Mycoses Study Group) criteria. The most common underlying disease were: HSCT (42.7%), hematologic malignancy (37%), SOT (9%), or other diseases (11.3%). The main risk factors associated with IA were neutropenia, monocytopenia, patients under corticosterois, presence of CMV disease, and rejection or graft versus host disease. The risk factor associated with death was the presence of invasive aspergillosis. Good performances for serum and BAL GM were registered with lower cutoffs in the present workin relationship to those found in the literature. The best cutoff for proven + probable aspergillosis for serum GM was observed at 0.35 vallue with Sensitivity-S, Specificity-Sp, Positive Predictive value-PPV), Negative Predictive Value-NPV) and AUC of 54.4%, 73.4%, 50.8%, 76.2% and 0.64; the values for proven aspergillosis alone were higher for S, Sp and NPV. On BAL tests for GM (cutoff value of 0.65) in proven+probable aspergillosis we observed 58.3%, 92.6%, 87.5%,71.4%, 0.75, respectively as S-Sp-PPV-NPVAUC; the sensitivity and VPN were higher in proven aspergillosis alone. In this work, the best performance in proven+probable aspergillosis for serum BDG showed 100 pg/ML as cutoff value, with 54.5%, 73.4%, 50.8%,76.2%, 0.64 for S-Sp-PPVNPV- AUC, respectively. For BAL- BDG, the cut off for proven+probable aspergillosis was 140 pg/mL, and we observed 46.7%, 76.7%, 70.0%, 55.,6%, 0.62, respectively for for S-Sp-PPV-NPV-AUC. Conclusion: The serum and BAL GM are useful tests for diagnosis in early stages of angioinvasive form at lower cutoffs; BAL GM is more sensitive. Agreement proportion between CT scan and each biomarker in the serum or BAL ranged from 0.5-0.6, with low ? index. Perfect ? statistic was observed for analysis of CT scan of subjects in proven aspergillosis by two independent radiologists, blinded for diagnosis. Persistence of serum GM was associated to death in relationship with its negativation. BDG test showed low performance in this work, where systemic and endemic diseases were included

Estudos recentes têm demonstrado que a monitorização de pressão intracraniana (PIC), como forma de cálculo da pressão de perfusão cerebral (PPC) é uma ferramenta importante para avaliação do fluxo sanguíneo cerebral (FSC), provocando sensível redução nas taxas de mortalidade. Além do TCE, outras patologias ou situações neurocirúrgicas tem utilizado a técnica de monitorização de PIC. A monitorização desse parâmetro foi proposta já na década de 50, onde um tubo com fluido em contato com o líquido cefalorraquidiano (LCR) era introduzido no espaço intracraniano e conectado a um transdutor de pressão externo. Com a evolução da indústria microeletrônica e dos sistemas microeletromecânicos, foi possível colocar os transdutores na ponta do cateter, permitindo uma monitorização menos invasiva, com menos riscos de infecções. Os cateteres atuais com micro transdutor na ponta podem ser divididos em três grupo: straingauge, fibra óptica e pneumático. Cada grupo possui suas características, entretanto o primeiro tem se demonstrado como solução mais robusta e confiável, com boa relação custo benefício. No presente trabalho foi proposto o desenvolvimento de um cateter implantável de monitorização de pressão intracraniana do tipo micro transdutor strain-gauge. Foram construídos protótipos funcionais e submetidos a ensaios de desempenho, especificados em norma técnica para monitorização de pressão sanguínea, a influência da temperatura na medição de pressão, bem como a exatidão das medições. Os processos empregados no trabalho são utilizados comumente na indústria de encapsulamento de semicondutores, porém foram levadas em consideração as especificidades da aplicação, adequando as técnicas disponíveis às geometrias e materiais empregados, considerando a necessidade de utilização de materiais biocompatíveis. / The traumatic brain injury (TBI) is nowadays the third cause of death in the world. Recent studies have shown the intracranial pressure (ICP) monitoring as an important tool for cerebral perfusion pressure (CPP) calculation and cerebral blood flow (CBF) assestment, reducing significantly the mortality statistics. Besides TBI, several others pathologies and neurosurgery conditions have been using the ICP monitoring technique. The proposal of ICP monitoring first appeared on the 50’s, where a tube fulfilled with fluid in contact with cerebrospinal fluid (CSF) was introduced into the intracranial space and connected to an external pressure transducer. With the waves of the microelectronics and microelectromechanical systems (MEMS) industry evolution, it was possible to put the transducer and all the electronics inside the catheter tip, allowing a less invasive monitoring, decreasing the risk of infection. The state of art catheters with micro transducer on the tip can be divided into three groups: strain-gauge, optical fiber and pneumatic. Each group has it’s own characteristics, however the first has been demonstrated as the rugged solution, being reliable, cost effective and with good accuracy. In the present work, it was proposed the development of an strain-gauge micro transducer implantable catheter for intracranial pressure monitoring. Functional prototypes were built and submitted to performance tests, according to the technical standards in the medical equipment area, the temperature influence over the pressure measurements was evaluated, as well as the accuracy. The adopted processes are commonly used in the semiconductor packaging industry, however it was considered the application special requirements, adapting the processes to the geometry and materials used, considering the needs of biocompatible materials.

Específicos: 1. Verificar a sensibilidade e especificidade dos ensaios de Galactomanana e de 1,3betaD-glucana no soro e lavado broncoalveolar. 2. Comparar os resultados da galatomanana e 1,3betaD-glucana com os dados de imagem em pacientes com suspeita de AI. 3. Verificar a relação entre a evolução dos níveis de GM e desfecho clínico (óbito e sobrevida). Casuística, Materiais e Métodos: Realizou-se um estudo tipo coorte prospectiva, incluindo 398 sujeitos das diversas enfermarias de pacientes imunodeprimidos do HCFMUSP, sendo incluídos dois grupos de pacientes: 202(51%) AI e 198(49%) controles. Resultados: Dos casos, 18 (8,8%) tinham aspergilose provada, 28 (13,7%) provável e 158 possível (77,5%), de acordo a classificação de 2002 EORTC/MSG (European Organization for Research and Treatment of Cancer / Mycoses Study Group). Os sujeitos submetidos ao TCTH eram 42,7%, com neoplasias hematológicas 37%, TOS 9% e outras doenças 11,3%. Os fatores de risco associados ao desenvolvimento da AI foram neutropenia, monocitopenia, uso de corticóide, presença de doença pelo citomegalovírus e rejeição ou doença do enxerto contra o hospedeiro. O fator de risco associado à evolução para o óbito foi a presença de AI. Foram observados bons desempenhos para a GM tanto no soro como no LBA com LR menores que os registrados na literatura. O melhor desempenho da GM no soro para aspergilose+provável ocorreu com LR de 0,35 com sensibilidade-S, especificidade-E, valor preditivo positivo- VPP), valor preditivo negativo-VPN) e área sob a curva-ASC de 54,4%, 73,4%, 50,8%, 76,2% e 0,64, sendo os valores superiores para aspergilose provada tanto na S, como E, VPN. No LBA os valores de S-E-VPP-VPN-ASC para GM para LR de 0,65 para aspergilose provável + provada foram 58,3%, 92,6%, 87,5%,71,4% e 0,75, sendo na aspergilose provada os valores de S, e VPN superiores. Nesta casuística, o melhor desempenho para BDG no soro apontou para uma LR de 100 pg/mL na aspergilose provável+provada, com 54,5%, 73,4%, 50,8% e 76,2%, 0,64 respectivamente para S-E-VPP-VPN-ASC. Para BDG no LBA, a LR na aspergilose provável + provada foi de 140 pg/mL, com os mesmos valores de 46,7%, 76,7%, 70%, 55,6% e 0.62, respectivamente. Conclusão: A GM no LBA e no soro foram úteis no diagnóstico da aspergilose mediante emprego de LR menores, sendo mais sensível na LBA, principalmente em estágios iniciais da forma angioinvasiva. A persistência de GM sérica foi relacionada ao óbito em relação à negativação da mesma. A proporção de concordância entre a TC e os biomarcadores no soro e no LBA variou de 0,5 a 0,6, com pequena concordância na estatística kappa. Excelente concordância foi observada entre dois radiologistas independentes, que analisaram de maneira cega as TC de sujeitos com aspergilose provada. Nesta casuística com inclusão de doenças sistêmicas e endêmicas, a BDG teve baixo desempenho diagnóstico / Invasive aspergillosis (IA) has become the leading infectious cause of death in immunocompromised hosts, particularly in subjects under SCTH and hematologic neoplasias. Objectives: General: To compare the performance of GM and BG tests in serum and bronchoalveolar lavage fluid (BAL) and computer tomography (CT) scans in the diagnosis of IA in immunocompromised hosts as well as their role in the patient outcome. Specific: 1. To analyse the sensitivity and specificity of Galactomannan and 1,3 betaD-glucan assays in the serum and bronchoalveolar lavage. 2. To compare the results of Galactomannan and 1,3betaD-glucan assays with CT scans in patients with invasive aspergilosis. 3. To analyse the relationship between the evolution of galactomannan levels and clinical outcome (death or survival). Patients, Materials and Methods: From December 2008 to March 2013, a prospective cohort of 398 patients from several wards of immunocompromised patients of Hospital das Clínicas, Faculdade de Medicina, University of São Paulo was included classified in two groups of patients: 202 (51%) with invasive aspergillosis (IA) and 198 (49%) control patients. Results: Considering 202 cases, 18(8.8%) were subjects with proven, 28(13.7%) with probable aspergillosis and 156(77.5%), with possible aspergillosis, according to 2002 EORTC/MSG (European Organization for Research and Treatment of Cancer/Mycoses Study Group) criteria. The most common underlying disease were: HSCT (42.7%), hematologic malignancy (37%), SOT (9%), or other diseases (11.3%). The main risk factors associated with IA were neutropenia, monocytopenia, patients under corticosterois, presence of CMV disease, and rejection or graft versus host disease. The risk factor associated with death was the presence of invasive aspergillosis. Good performances for serum and BAL GM were registered with lower cutoffs in the present workin relationship to those found in the literature. The best cutoff for proven + probable aspergillosis for serum GM was observed at 0.35 vallue with Sensitivity-S, Specificity-Sp, Positive Predictive value-PPV), Negative Predictive Value-NPV) and AUC of 54.4%, 73.4%, 50.8%, 76.2% and 0.64; the values for proven aspergillosis alone were higher for S, Sp and NPV. On BAL tests for GM (cutoff value of 0.65) in proven+probable aspergillosis we observed 58.3%, 92.6%, 87.5%,71.4%, 0.75, respectively as S-Sp-PPV-NPVAUC; the sensitivity and VPN were higher in proven aspergillosis alone. In this work, the best performance in proven+probable aspergillosis for serum BDG showed 100 pg/ML as cutoff value, with 54.5%, 73.4%, 50.8%,76.2%, 0.64 for S-Sp-PPVNPV- AUC, respectively. For BAL- BDG, the cut off for proven+probable aspergillosis was 140 pg/mL, and we observed 46.7%, 76.7%, 70.0%, 55.,6%, 0.62, respectively for for S-Sp-PPV-NPV-AUC. Conclusion: The serum and BAL GM are useful tests for diagnosis in early stages of angioinvasive form at lower cutoffs; BAL GM is more sensitive. Agreement proportion between CT scan and each biomarker in the serum or BAL ranged from 0.5-0.6, with low ? index. Perfect ? statistic was observed for analysis of CT scan of subjects in proven aspergillosis by two independent radiologists, blinded for diagnosis. Persistence of serum GM was associated to death in relationship with its negativation. BDG test showed low performance in this work, where systemic and endemic diseases were included

However, by March 2010, a vaccination campaign with MenC-V was performed in Salvador in individuals under five years-old, and from 10 to 24 yearsold. In São Paulo state, the outbreaks occurred in teenagers and young adults prompting one-time vaccination campaign from 2010 to 2014 targeting these age-groups. OBJECTIVE: To assess the direct and indirect impact (herd effect) of vaccination on invasive meningococcal disease (MD) for capsular group C (MenC) four years after the introduction of MenC-V in three scenarios: i) Brazil as a whole (routine vaccination in childhood only); ii) Brazil except for Salvador (vaccination campaign with teenagers during the year of MenC-V introduction); and iii) São Paulo state (vaccination campaign for adolescents and young adults during 2010-2014 to control outbreaks). METHODS: We performed an ecological quasi-experimental design from 2008 to 2014 using data from the National Reference Laboratory for Meningitis, and data from the National Information System for Notifiable Diseases. A deterministic linkage was performed between the two databases to improve the accuracy of the detection of MD, especially in capsular groups. An interrupted time-series analysis was conducted using the Holt-Winters technique to control for pre-existing trends and seasonal variations. The MenC vaccination impact was evaluated as the percentage of reduction in the incidence rates of MenC in the post-vaccination period (2012 to 2014), using the pre-vaccination period (2008 to 2010) to estimate what would be expected on the post-vaccination period, whether the vaccination had not been introduced. For Salvador, we analyzed the effect of the vaccination on the number of MenC cases. RESULTS: A total of 18,136 invasive MD cases were analyzed. For Brazil as a whole, the vaccination reduced 67.4% (lower 95%CI 42.5%) the rates for MenC for infants under 12 months, 92.3% (lower 95%CI 77.7%) for the age-group 12-23 months, and 65.7% (lower 95%CI 28%) for children aged 2-4 years. Indirect impact (20-24.7%) was observed in the age-group 5-19 years. When excluding Salvador from the analysis of Brazil, the indirect impact was observed only for children in the age-group 5-9 years. In the scenario of São Paulo state, similarly to Brazil, significant impact was observed in the target age-groups, in addition to indirect impact in the age group 5-9 years. In Salvador, in addition to the effect on the vaccinated population a sharp and sustainable decline of MenC cases was observed in all age-groups not target for vaccination. Overall, 1,170 cases of MenC were averted in Brazil after the introduced of Men-C vaccination. CONCLUSION: The strategy of catch-up for adolescents and young adults, especially during the year of MenC-V introduction may lead to rapid and sustainable herd effect. / A vacina meningocócica conjugada contra o grupo capsular C (MenC-V) foi introduzida no calendário de imunização infantil brasileiro em novembro de 2010, sendo administrada aos três, cinco e 12 meses de idade sem catch-up para os demais grupos etários. Entretanto, em março de 2010, uma campanha de vacinação com MenC-V foi realizada em Salvador para indivíduos menores de cinco anos de idade e de 10 a 24 anos. No estado de São Paulo os surtos ocorreram em adolescentes e adultos jovens, determinando campanhas de vacinações de bloqueio nessa faixa etária nos anos de 2010 a 2014. OBJETIVO: Avaliar o impacto direto e indireto (rebanho) da vacinação nas taxas de incidência de doença meningocócica (DM) invasiva pelo grupo capsular C (MenC) após quatro anos da introdução da MenC-V em três cenários: i) Brasil como um todo (imunização de rotina somente de crianças); ii) Brasil exceto Salvador (campanha de vacinação em adolescentes no ano de introdução da MenCV); e iii) estado de São Paulo (vacina de rotina na infância e vacinações de bloqueio em adolescentes e adultos jovens para controlar surtos). MÉTODOS: Foi realizado um estudo ecológico quasi-experimental para avaliar o impacto da vacinação em série histórica de 2008 a 2014 usando os bancos de dados do Laboratório Nacional de Referência para Meningites Bacterianas, Instituto Adolfo Lutz (IAL) e o Sistema de Informação de Agravos de Notificação (Sinan). Um processo de vinculação (linkage) determinístico entre as duas bases foi realizado para melhorar a acurácia da detecção de casos de DM, especialmente de grupo capsulares. Uma análise de série temporal interrompida foi conduzida utilizando a técnica de Holt-Winters para controlar por tendência pré-existente e variações sazonais. O desfecho foi taxa de MenC. O impacto da vacinação foi avaliado pelo percentual de redução da incidência de MenC no período pós-vacinal (2012 a 2014), utilizando o período pré-vacinal (2008 a 2010) para estimar o que seria esperado no período pós-vacinal, caso a vacinação não tivesse sido introduzida. Para Salvador foi analisado o efeito da MenC-V no número de casos de MenC. RESULTADOS: Um total de 18.136 casos de DM invasiva foram analisados. Para o Brasil como um todo, a vacinação reduziu significativamente a DM por MenC na faixa etária alvo, com redução de 67,4% (limite inferior do IC95% 42,5%) em menores de 12 meses, 92,3% (limite inferior do IC95% 77,7%) para faixa etária de 12-23 meses e 65,7% (limite inferior do IC95% 28%) em crianças de 2-4 anos, e efeito rebanho foi observado na faixa etária de 5 a 19 anos com 20-24,7%. Quando se exclui Salvador na análise do Brasil, impacto indireto significativo foi observado somente em crianças de 5-9 anos. No cenário São Paulo, semelhante ao Brasil, observou-se impacto estatisticamente significante nas faixas etárias alvo do PNI, além do efeito rebanho na faixa etária de 5-9 anos de idade. Para Salvador, o impacto da vacinação apresentou um declínio acentuado e sustentável em todas as faixas etárias fora do alvo da vacinação. Ao todo, 1.170 casos de MenC foram evitados no período estudado. CONCLUSÃO: A estratégia de vacinação de catch-up em adolescentes e adultos jovens, especialmente no ano de introdução da MenC-V, promoveu um rápido e sustentável rebanho.
