Différence dans la qualité de l’alimentation en relation au risque d’excès de poids chez des populations autochtones canadiennesMorou, Karimou 12 1900 (has links)
L‘obésité constitue un problème de santé publique au Canada, particulièrement chez
les populations autochtones où les prévalences les plus élevées ont été rapportées.
D’après les écrits recensés, plusieurs méthodes ont été essayées pour étudier la relation entre l’alimentation et l’obésité, mais les résultats sont inconstants.
Le but de cette thèse est d’identifier, en termes quantitatif et qualitatif, les différences dans l’alimentation des obèses et non-obèses. Pour y parvenir, nous avons développé une nouvelle méthode à l’aide d’une banque de données portant sur les enfants Mohawk de Kahnawake afin d’identifier les différences dans les choix alimentaires.
Cette même méthode a été ensuite appliquée à deux autres banques de données (celle
des adultes cris de la Baie James et celle des autochtones de l’enquête ESCC 2.2).
Globalement, les résultats n’ont pas montré de différences significatives dans
l’alimentation des participants selon les catégories d’IMC en considérant les
indicateurs reliés à la quantité et à la qualité de l’alimentation comme l’apport
énergétique total, l’apport énergétique en provenance des lipides, les fibres
alimentaires, la densité énergétique et la diversité alimentaire.
Par contre, les résultats de la nouvelle méthode fondée sur la sélection des items
alimentaires fréquemment consommés par au moins 10 % des participants ont révélé que les enfants de Kahnawake à risque d’excès de poids consommaient plus fréquemment de croustilles (p=0.001) et moins fréquemment de craquelins que les enfants avec excès de poids ou ceux ayant un poids normal (p=0.015). Ensuite, en prenant la catégorie de poids normal comme référence, le rapport de côte (Odds ratio : OR) d’être à risque d’excès de poids était de 2.16 (95 % IC : 1.14 - 4.09) fois plus élevé chez les enfants de Kahnawake qui consommaient plus fréquemment de croustilles comparativement aux non-consommateurs de croustilles, et ce, après ajustement pour l’âge. Par contre, le rapport de côte d’être à risque d’excès de poids diminuait de 79 % (OR = 0.21; 95 % IC : 0.06 – 0.72) chez les enfants consommateurs de craquelins comparativement à leurs homologues non-consommateurs. Après avoir corrigé les quantités pour l’âge, on note que les enfants avec excès de poids
consommaient plus de frites que les enfants à risque d’excès de poids ou ceux ayant un
poids normal (p = 0.027).
Chez les femmes cries, les résultats de la nouvelle méthode ont montré que le colorant
à café était associé à un risque élevé d’obésité (OR = 4.64, 95 % IC : 1.04 - 0.54);
alors que le lait faible en matières grasses était associé à un moindre risque
d’embonpoint (OR = 0.38, 95 % IC : 0.17 - 0.82), après ajustement pour l’âge. Quant
aux hommes cris, le lait entier était associé à un moindre risque d’avoir de
l’embonpoint (OR ajusté pour l’âge = 0.38, 95 % IC : 0.20 - 0.71) et, en termes de
quantité corrigée pour l’âge, les hommes obèses buvaient plus de boissons sucrées aux
fruits comparativement aux hommes de poids normal ou ceux ayant de l’embonpoint
(p=0.015).
Selon les résultats de cette méthode appliquée aux données de l’enquête ESCC 2.2, les garçons à risque d’excès de poids ou avec excès de poids consommaient moins
fréquemment de pain blanc que ceux de poids normal (p=0.048). En termes de
quantité toutefois, ils consommaient plus de pain blanc (p=0.040), utilisaient plus de
farine de blé (p=0.006) et de levure (p=0.002). Après avoir ajusté les quantités
consommées pour l’âge et l’indice d’activité physique, les femmes avec embonpoint
ou obèses utilisaient plus de farine de blé (p< 0.001) que leurs homologues de poids
normal. Chez les hommes, il n'y avait pas de différences ni dans les fréquences de
consommation ni dans les quantités consommées. Concernant les filles, leurs apports alimentaires n'étaient pas valides (facteur d'activité de Goldberg < 1.2 dans la
catégorie embonpoint / obèse).
Les résultats de cette méthode innovatrice pourraient d’une part, permettre d’axer la
sensibilisation sur des aliments particuliers en plus des recommandations générales du Guide Alimentaire Canadien. D’autre part, ils nous renvoient aux données biologiques de laboratoire afin d’identifier les composantes des items susceptibles de contribuer au développement de l’obésité. / Obesity is a public health problem in Canada, particularly among Aboriginal
populations where the highest prevalences have been reported. In the literature, several methods have been tried to study the relationship between diet and obesity, but results are inconsistent.
The objective of this thesis is to identify differences in selected dimensions of diet
quality and quantity across body mass index (BMI) categories for Aboriginal children
and adults in Canada. To achieve this goal, we developed a new method by using data
from Mohawk children in Kahnawake. The same method was then applied to two other datasets (James Bay Cree adults and off-reserve adults and children from CCHS
2.2).
Overall, in each dataset, the results showed no differences in the diets of participants
considering indicators such as energy intake, percent fat, fiber intake, energy density and dietary diversity.
On the contrary, using the new method based on the most-frequently consumed food
items by at least 10% of participants, we found that Mohawk children “at risk of
overweight” consume potato chips more frequently (p=0.001) and crackers less
frequently (p = 0.015), compared to normal-weight or overweight children. Compared
to normal-weight, and after adjusting for age, Mohawk children who consumed more
frequently chips increased their probability of being at risk of overweight (Odds ratio :
OR = 2.16, 95 % CI : 1.14 - 4.09), while those who consumed crackers more
frequently decreased their risk (OR = 0.21; 95 % CI : 0.06 – 0.72). In terms of
quantity, and after adjusting for age, overweight children consumed larger portions of french fries (p = 0.027).Among the Cree women (compared to normal-weight, and after adjusting for age), we found out that nondairy creamer was associated to increase risk of being obese (OR = 4.64, 95 % CI : 1.04 - 20.54) while low-fat milk was associated to lower risk of overweight (OR = 0.38, 95 % CI : 0.17 - 0.82). Among Cree men, (after adjusting for age), whole milk was associated to a high risk of the men being overweight (OR = 0.38, 95 % CI : 0.20 - 0.71). Finally, in terms of quantity (after adjusting for age), obese men drank more sweetened fruit drinks compared to men of normal weight or those with overweight (p = 0.015).
In the third dataset CCHS2.2, results of the new method show that boys at “risk of
overweight” or overweight consumed white bread less frequently (p = 0,048) but in
greater quantity when they do so (p = 0,040). They also used more flour (p = 0.006)
and yeast (p = 0.002). After adjusting the quantities consumed for age and physical
activity, women with overweight or obese used more flour (p < 0.001) than those of
normal weight. No differences were found in terms of frequency and quantity for men.
The data were not analysed for girls since their Goldberg activity factor was less than
1.2 with greater body weight.
The results of this new method could, on one hand, allow us to focus awareness on
foods in addition to general recommendations of the Canadian Food Guide. On the
other hand, they refer us back to laboratory data to identify components of items that
may contribute to the development of obesity.

Les déterminants psychosociaux du poids corporel dans la population québécoise adultePlourde, Hugues 09 1900 (has links)
Titre : Étude des déterminants psychosociaux du poids corporel dans la population
québécoise adulte. Objectif : L’objectif principal des travaux effectués était l'étude des
déterminants psychosociaux du poids corporel dans quatre groupes d’adultes qui ont
participé à l’Enquête sociale et de santé 1998 (ESS 98). Méthodologie : Les microdonnées
de l'ESS 98 ont été accédées en utilisant les services de l’Institut de la statistique du
Québec. Les groupes étudiés étaient les hommes et les femmes âgés entre 25 et 44 ans ou
45 et 64 ans. Résultats : La pratique d’activités physiques reliées au transport et un niveau
de scolarité plus élevé ont été associés à moins de probabilités de rapporter un excès de
poids chez les hommes âgés entre 25 et 44 ans. Une meilleure perception des habitudes
alimentaires a aussi été associée à moins de probabilités d’avoir un excès de poids dans la
plupart des groupes à l’exception des femmes âgées entre 25 et 44 ans. Le niveau d’AP a
été associé négativement à l’excès de poids uniquement chez les femmes plus âgées. Une
meilleure perception de l’état de santé a été associée à moins de probabilités de rapporter
un excès de poids chez les femmes âgées entre 25 et 44 ans et à plus de probabilités chez
les hommes âgés entre 45 et 64 ans. Chez les hommes des deux groupes, le tabagisme a été
associé à moins de probabilités de rapporter avoir un excès de poids. Chez les femmes, la
consommation d’alcool a été associée à moins de probabilités d’avoir un excès de poids.
Dans tous les groupes, tenter présentement de perdre du poids a été associé à plus de
probabilités de rapporter un excès de poids. Les travaux effectués démontrent que ces
déterminants du poids corporel ne sont pas nécessairement associés aux habitudes alimentaires et au niveau d’AP. Conclusion : Les déterminants psychosociaux associés à
l’obésité divergent selon le sexe et l’âge. L’identification de ces associations illustre le
besoin d’intégrer les spécificités de chacun de ces groupes dans les interventions
populationnelles qui visent la problématique du poids corporel. / Title: Psychosocial correlates of body weight in the Quebec Adults Population.
Objective: Within the variables available in the 1998 Social and Health survey, identify
psycho-social correlates of body mass index (BMI) in the Quebec population. Method:
Access to the Social Lifestyles and Health 1998 survey was done at the «Institut de la
Statistique du Québec». Groups studied included the 25- to 44-years and the 45- to 64-
years old men and women. Results: Higher number of physical activity (PA) related to
transport and levels of education were associated with less odds of reporting an excess body
weight only in the 25- to 44-years old men. Cigarette smoking was also associated with less
odds of reporting an excess weight in both men groups. Regular practice of leisure time PA
was associated with less odds of reporting an excess weight only in 45- to 64-years old
women. In both women groups, more frequent consumption of alcoholic beverages
decreased the odds of reporting an excess weight. Perceived eating habits were also
associated with less odds of an excess weight in most groups except in the 25- to 44-yearsold
women where the trend was not significant. Opposite associations were observed
between perceived health and BMI. In the 45- to 64-year old men, better perceived health
increased the odds of reporting an excess weight. On the opposite, the odds of reporting an
excess weight decreased with better health in 25- to 44-years-old women. In all groups,
currently trying to loose weight increased the odds of reporting an excess weight. Others
analyses also indicate that those correlates are not always associated with eating habits and
PA level as it would be expected. Conclusion: Many correlates differ between age-group and sex. The identification of these factors illustrates the need to adapt obesity related
program toward specific sub-group within the general population.

Associations between area-level unemployment, body mass index, and risk factors for cardiovascular disease in an urban areaNaimi, Ashley I. 03 1900 (has links)
INTRODUCTION: Il existe peu d’évidences sur l’association entre le taux de chômage dans le milieu résidentiel (CR) et le risque de maladies cardiovasculaires parmi les résidents de milieux urbains. De plus, on ne sait pas si ce lien diffère entre les deux sexes. Cette thèse a pour objectif de déterminer la direction et la taille de l’association entre le CR et le risque de maladies cardiovasculaires, et d’examiner si cette association varie en fonction du sexe. MÉTHODES: Un sous-échantillon de 342 participants de l’Étude sur les habitudes de vie et la santé dans les quartiers montréalais a rapporté ses habitudes de vie et sa situation socio-économique. Des mesures biologiques et anthropométriques ont été recueillies par une infirmière. Le CR a été opérationnalisé en fonction d’une zone-tampon d’un rayon de 250 m centrée sur la résidence de chacun des participants à l’aide d’un Système d’Information Géographique (SIG). Des équations d’estimation généralisées ont été utilisées afin d’estimer l’association entre le CR et l’Indice de Masse Corporelle (IMC) et un score cumulatif de Risque Cardio-métabolique (RC) représentant la présence de valeurs élevées de cholestérol total, de triglycérides, de lipoprotéines de haute densité et d’hémoglobine glyquée. RÉSULTATS: Après ajustement pour l’âge, le sexe, le tabagisme, les comportements de santé et le statut socio-économique, le fait de vivre dans un endroit classé dans le 3e ou 4e quartile de CR était associé avec un IMC plus élevé (beta pour Q4 = 2.1 kg/m2, IC 95%: 1.02-3.20; beta pour Q3 = 1.5 kg/m2, IC 95%: 0.55-2.47) et un taux plus élevé de risque cardiovasculaires Risque Relatif [RR pour Q4 = 1.82 (IC 95 %: 1.35-2.44); RR pour Q3 = 1.66 (IC 95%: 1.33-2.06)] par rapport au 1er quartile. L'interaction entre le sexe et le CR révèle une différence absolue d’IMC de 1.99 kg/m2 (IC 95%: 0.00-4.01) et un risque supérieur (RR=1.39; IC 95%: 1.06-1.81) chez les femmes par rapport aux hommes. CONCLUSIONS: Le taux de chômage dans le milieux résidentiel est associé à un plus grand risque de maladies cardiovasculaires, mais cette association est plus prononcée chez les femmes. / INTRODUCTION: Little is known about whether area-level unemployment is independently associated with individual-level Cardiovascular Disease (CVD) in an urban setting. Furthermore, it is unclear whether this relationship differs by sex. This thesis examined the direction and magnitude of the association between area-level unemployment (ALU) and Body Mass Index (BMI) and a marker for CVD risk, and whether this association differs by sex. METHODS: A sample of 342 individuals from the Montreal Neighbourhood Survey of Lifestyle and Health (MNSLH) self-reported behavioural and socioeconomic information. A registered nurse collected biochemical and anthropometric data. ALU was operationalised within a 250 m radius buffer centered on individual residence using a Geographic Information System (GIS). Generalized Estimating Equations were used to determine if body mass index (BMI), and a cumulative score for total cardiometabolic risk (TCR) representing elevated values for total cholesterol, triglycerides, high-density lipoprotein cholesterol, and glycosylated hemoglobin, were associated with ALU. RESULTS: After adjustment for age, gender, smoking status, behavioural, and socioeconomic covariates, living in an area in the upper ALU quartiles was associated with an elevated BMI [Q4 beta = 2.1 kg/m2 (95% CI: 1.02-3.20)] and greater TCR [Q4 RR = 1.82 (95 % CI: 1.35-2.44); Q3 RR = 1.66 (95% CI: 1.33-2.06)] relative to the 1st quartile. Sex-by-ALU interaction revealed a 1.99 kg/m2 (95% CI: 0.00-4.01) difference in BMI and 1.39-fold (95% CI: 1.06-1.81) greater TCR Score for women compared to men. CONCLUSIONS: Area-level unemployment is associated with greater CVD risk in men and women but associations are stronger among women.

Diets which emphasize intakes of plant-based foods are recommended to reduce disease risk and for promoting healthy weight. The aim of this study was to examine the association between fruit, vegetables, pulses and nut intake and body mass index (BMI) across countries in adolescents (13-14 years) and children (6-7 years). Data from the International Study of Asthma and Allergies in Childhood; 77,243 children's parents and 201,871 adolescents was used to examine the association between dietary intake (Food Frequency Questionnaire) and BMI using general linear models, adjusting for country gross national index. Adolescents who consumed fruit, vegetables, pulses and nuts three or more times a week had a lower BMI than the never or occasional group; eating nuts three or more times a week, was associated with a BMI value of 0.274 kg/m² lower than the never group (p < 0.001). Compared to children who never or occasionally reported eating vegetables, those reporting that they ate vegetables three or more times per week had a lower BMI of -0.079 kg/m². In this large global study, an inverse association was observed between BMI and the reported increasing intake of vegetables in 6-7 years old and fruit, vegetables, pulses and nuts in adolescents. This study supports current dietary recommendations which emphasize the consumption of vegetables, nut and pulses, although the effect sizes were small. / Acknowledgments: This work was supported by Cure Kids New Zealand through a grant to E.A. Aim: To examine the relationship between reported vigorous physical activity (VPA) and body mass index (BMI) in children (6–7 years) and adolescents (13–14 years). Methods: In the International Study of Asthma and Allergies in Childhood Phase Three, 75 895 children's parents and 199 502 adolescents answered questions relating to VPA, height and weight. The association between VPA and BMI was analysed using general linear models, adjusting for country gross national index. Results: Compared to children who undertook no VPA, those in the infrequent group (once or twice per week) and those in the frequent group (three or more times per week) had mean (95% CI) BMI values 0.07 kg/m 2 (0.03–0.11) and 0.09 kg/m 2 (0.03–0.15) greater, respectively (p = 0.001). Compared to adolescents reporting no VPA, those in the infrequent group had a BMI 0.19 kg/m 2 (0.15–0.23) greater while those in the frequent group had a BMI 0.01 kg/m 2 (−0.03–0.05) greater (p < 0.0001). Conclusion: Reported VPA is not associated with lower BMI among children and adolescents. Investigation of VPA and BMI may be best undertaken in conjunction with other variables in the energy expenditure equation. A focus on VPA alone may be an inefficient way to manage BMI. / Revisión por pares

Crescimento de recém-nascidos pré-termos tardios nos primeiros seis meses de idade corrigida em Cuiabá-MT / Growth of late preterm newborn in the first six months of age adjusted in Cuiabá-MTLopes, Margareth Corrêa Lima 05 August 2014 (has links)
Introdução: O nascimento de recém-nascidos pré-termo tardios (RNPT-T), geralmente tratados como \"quase termo\", vem aumentando nas últimas décadas. Vários fatores são atríbuídos à sua etiologia e contribuem para aumentar as taxas de morbimortalidade perinatal e infantil. Atualmente se dispõe de poucos estudos sobre esses recém-nascidos, especialmente sobre o seu crescimento. Objetivos: Analisar o crescimento de RNPT-T nos seis primeiros meses de idade corrigida. Métodos: Estudo observacional, analítico de uma coorte de 64 RNPT-T, coletados nos quatro maiores hospitais de Cuiabá, entre janeiro e setembro de 2013 e acompanhados até 6 meses de idade corrigida. Os dados foram coletados ao nascer, às 40 semanas, 3 e 6 meses de idade corrigida. Foram realizadas análises de correlação entre os parâmetros antropométricos perímetro braquial (PB) e prega cutânea tricipital (PCT) em função dos índices antropométricos PB/PC (perímetro cefálico), peso/comprimento (P/C) e Índice de Massa Corpórea (IMC) e realizada regressão linear múltipla. Resultados: Ao nascimento, 81,3% foram classificados como adequados para a idade gestacional, com a média de peso de 2343,80g ± 430,50. Todos os recém-nascidos apresentaram crescimento contínuo nos seis meses de idade corrigida para os valores de peso, comprimento e perímetro cefálico (PC). Já os valores médios de PCT, de PB/PC e de IMC não mostraram diferença aos 6 meses de idade corrigida. Todos os coeficientes de correlação (r) dos parâmetros PCT e PB em relação aos índices antropométricos apresentaram significância estatística, sendo os valores mais altos para PB versus PB/PC e P/C e menores para PB versus IMC. No modelo final da regressão linear múltipla da PCT, o PB/PC contribuiu significantemente para predizer o depósito de gordura ao nascimento, às 40 semanas e aos 3 meses de idade corrigida (33,8%%, 46,07% e 18,08% respectivamente); aos 6 meses o melhor preditor foi a razão P/C (10,45%). O melhor preditor para o PB foi o PB/PC, que permaneceu no modelo com valores de 73,71%% em todos os tempos avaliados. Conclusão: Os parâmetros peso, comprimento, perímetro cefálico e perímetro braquial e a relação P/C aumentaram nos quatro tempos de aferição, enquanto o PCT e as relações PB/PC e IMC não se modificaram do primeiro para o segundo trimestre e o PB/PC foi o melhor preditor do depósito de gordura subcutânea tricipital (PCT) e de massa gorda e muscular (PB) / Introduction: The birth of newborns late preterm (RNPT -T), usually treated as \"near term\", has been increasing in recent decades. Several factors are attributed to its etiology and contribute to increased rates of perinatal and infant morbidity and mortality. Currently there are few studies of these newborns, especially about their growth. Objectives: To analyze the growth pattern of late preterm infants from birth to six months of corrected age. Methods: This cohort study included 64 RNPT-T, in four major hospitals in Cuiabá, between January and September 2013 and followed until 6 months corrected age. Data were collected at birth, at 40 weeks, 3 and 6 months corrected age. Correlation analyzes between anthropometric parameters mid- arm circumference (MAC) and triceps skinfold (TS) on the basis of anthropometric indices MAC / HC (head circumference), weight / length (W / L) and body mass index (BMI) were performed multiple linear regression. Results: At birth, 81.3 % were classified as appropriate for gestational age, with the average weight of 2343.80 ± 430.50 g. All newborns had a continuous growth in the six months corrected age for the values of weight, length and head circumference (HC). The average value of TS values of MAC / HC and BMI showed no difference at 6 months of corrected age. All correlation coefficients (r) of the TS and MAC parameters in relation to anthropometric indices showed statistically significant, with the highest values for MAC versus MAC / HC and W / L and lower for MAC versus BMI. At the end of the TS model of multiple linear regression, the MAC / HC contributed significantly to predict the deposition of fat at birth, at 40 weeks and at 3 months\' corrected age (33.8 %, 46.07 % and 18.08 % respectively); at 6 months was the best predictor of the ratio W/ L (10.45%). The best predictor for the MAC was the MAC / HC, which remained in the model with values of 73.71 % at all evaluated times. Conclusion: The parameters weight, length, head circumference and arm - circumference and the ratio W / L increased in the four times of measurement, while the TS and MUAC / HC relations and BMI did not change from the first to the second quarter and MAC / HC was the best predictor of subcutaneous fat deposit triceps (TS) and fat and muscle (MAC) mass

Objetivo - Avaliar o estado nutricional (EN) e sua associação com força muscular de membros inferiores (FMI) e de preensão manual (FPM), flexibilidade (FLX) e equilíbrio (EQ) de idosos do município de São Paulo. Delineamento - Estudo epidemiológico transversal, de base populacional e domiciliar, parte de estudo multicêntrico (Pesquisa SABE), coordenado pela Organização Pan-Americana de Saúde. Casuística - A Pesquisa SABE abrangeu 2143 idosos (≥ 60 anos), de ambos os sexos, no período de janeiro/2000 a março/2001, selecionados por amostragem probabilística; sendo que 1894 (88,4%), participaram deste estudo. Métodos - Os idosos foram estratificados segundo sexo e grupo etário (60-69, 70-79 e ≥ 80 anos) e o EN foi avaliado de acordo com Índice de Massa Corporal (IMC): baixo peso (IMC ≤ 23kg/m2) e obesidade (IMC ≥ 28kg/m2). A FMI e a FLX foram avaliados por tempo, a FPM por dinamômetro e o EQ por 4 medidas estáticas. As associações foram verificadas por análise de sobrevida, segundo Cox (FMI e FLX) e por quiquadrado (FPM e EQ). Resultados - Verificou-se maior prevalência de obesidade em mulheres e de baixo peso em homens. Houve aumento na freqüência de baixo peso e redução da obesidade nos grupos etários mais avançados. Observaram-se associações estatisticamente significativas entre FLX e EQ e mulheres obesas; e entre FPM e baixo peso em homens e mulheres. Conclusão: A inadequação nutricional foi constatada em ambos os sexos e todos os grupos etários. As associações entre EN e capacidade funcional mostraram-se específicas para sexo, grupo etário e teste utilizado. / Objective - To evaluate the nutritional status (NS) and its association with handgrip strength (HS), lower limb muscle strength (LMS), flexibility (FLX) and balance (BL) of the elderly of the city of São Paulo. Design - Cross-sectional epidemiological study, in a population and household basis, is part of a multicenter study (the SABE Survey), which was coordinated by the Pan-American Health Organization. Setting - The SABE survey reached 2143 elderly (≥ 60 yrs old), from both genders, from january/2000 to march/2001, who were selected by probabilistic sampling; out of those, 1894 (88.4%) took part in this study. Methods - The elderly were separated by gender and age group (60-69, 70-79 e ≥ 80 yrs old) for the tests, and the NS was evaluated according to the Body Mass Index (BMI): underweight (BMI ≤ 23kg/m2) and obesity (BMI ≥ 28kg/m2). The LMS and the FLX were measured by timing, the HS by a dynamometer and the BL by four static measurements. The associations were verified through Cox Regression analysis (LMS e FLX) and by chi square (HS e BL). Results - A larger prevalence of obesity was verified in women and of underweight in men. There was an increase in the underweight frequency and a reduction of the obesity level in the oldest age group. There were associations (statistically significant) between FLX and BL and obese women; and HS and underweight (men and women). Conclusions: The nutritional inadequacy was verified in both genders and all age groups. The associations between nutritional status and functional capacity were proved to be specific to each gender, age group and test.

Abstract
Polycystic ovary syndrome (PCOS) is the most common endocrine disorder affecting reproductive aged women, with reproductive, metabolic and cardiovascular implications across the life span. The typical features of PCOS include irregular menstruation, androgen excess and polycystic ovaries in ultrasonography. The majority of women with PCOS are overweight or obese, and, at least partly, obesity-driven metabolic abnormalities often coexist with PCOS. Despite intensive research, it has remained unclear whether PCOS per se is a risk factor of metabolic abnormalities, and cardiovascular disease and events.
The main aim of the current work was to investigate whether PCOS is an independent risk factor of metabolic abnormalities and cardiovascular diseases. The study population consisted of the prospective population-based Northern Finland Birth Cohort 1966, and we used data collected at ages 14, 31 and 46. The definition of PCOS was based on self-reported PCOS symptoms at age 31 and/or PCOS diagnosis by age 46.
The results revealed that weight gain in early life was a risk factor for the development of PCOS. As for metabolic outcomes, at age 46, normal-weight women with PCOS did not display increased odds of abnormal glucose metabolism. However, weight gain during early adulthood was significantly associated with abnormal glucose metabolism in women with PCOS by age 46. Interestingly, PCOS per se was already associated with elevated blood pressure at age 31 and hypertension at age 46, independently of obesity. Women with PCOS also displayed reduced cardiac vagal activity, which was associated with metabolic abnormalities and hypertension. Furthermore, even though no major anatomical or functional impairments were observed in echocardiography, women with PCOS displayed a significantly greater prevalence of myocardial infarction and a two-fold higher prevalence of cardiovascular events than controls.
In conclusion, our findings indicate that even though PCOS is an independent risk factor of metabolic derangements, related obesity is a major metabolic risk factor in these women. The role of PCOS in cardiovascular events per se remains controversial and requires follow-up of this cohort. Given all this, maintaining normal weight and preventing weight gain, especially during early adulthood, should be the main priority in the prevention of adverse metabolic changes in women with PCOS. / Tiivistelmä
Munasarjojen monirakkulaoireyhtymä (polycystic ovary syndrome, PCOS) on lisääntymisikäisten naisten yleisin hormonaalinen häiriö aiheuttaen runsaasti sairastavuutta ja terveydenhuollon kustannuksia. PCOS:n diagnostisiin kriteereihin kuuluvat epäsäännöllinen kuukautiskierto, lisääntynyt miessukupuoli-hormonivaikutus sekä monirakkulaiset munasarjat. Merkittävä osa oireyhtymää sairastavista naisista on ylipainoisia tai lihavia ja oireyhtymän kanssa yhtä aikaa esiintyykin useita, ainakin osittain ylipainosta johtuvia, metabolisia häiriöitä. Lukuisista tutkimuksista huolimatta on kuitenkin epäselvää, altistaako PCOS itsessään metabolisille häiriöille sekä sydän- ja verisuonisairauksille.
Väitöskirjatutkimuksen tavoitteena oli selvittää, onko PCOS itsenäinen metabolisten ja sydän- ja verisuonisairauksien riskiä lisäävä tekijä. Tutkimus pohjautui Pohjois-Suomen syntymäkohortti 1966 tutkimuksen 14-, 31- ja 46-vuotisseurantoihin. PCOS luokittelu perustui 31- ja 46-vuotiskyselyissä itse ilmoitettuihin tyypillisiin PCOS oireisiin ja/tai diagnoosiin.
Tutkimuksessa havaittiin, että 14- ja 31-ikävuoden välillä tapahtuva painonnousu oli yhteydessä PCOS diagnoosiin myöhemmällä iällä. 46-vuotiaana normaalipainoisilla PCOS naisilla ei ollut suurentunut tyypin 2 diabetes riski, mutta painonnousu varhaisaikuisuudessa oli merkittävästi yhteydessä sokeriaineenvaihdunnan häiriöön PCOS naisilla. PCOS oli yhteydessä kohonneeseen verenpaineeseen 31-vuotiaana ja hypertensioon 46-vuotiaana ylipainosta riippumatta. Oireyhtymään liittyvät metaboliset häiriöt olivat tärkein sydämen autonomisen hermoston säätelyyn vaikuttava tekijää, kun taas PCOS itsessään ei vaikuttanut autonomisen hermoston toimintaan. PCOS:ään sairastavien naisten sydämen rakenne ja funktio eivät merkitsevästi poikenneet kontrolloiden vastaavista muuttujista. Kuitenkin suhteellisen nuoresta iästä huolimatta PCOS naisilla esiintyi enemmän sydäninfarkteja ja kaksi kertaa enemmän sydän- ja verisuonitapahtumia, kuin kontrolleilla.
Tutkimuksen tulokset osoittavat, että vaikkakin PCOS on itsenäinen riskitekijä metabolisille häiriöille, oireyhtymään liittyvä ylipaino vaikuttaa merkittävästi metabolisten häiriöiden esiintymiseen. PCOS:n ja sydän- ja verisuonitautitapahtumien yhteyden tarkempi tutkiminen vaatii kohortin jatkoseurantaa. Painonhallinnan tukemisen tulisi olla PCOS:ää sairastavien naisten hoidon kulmakivi.

Estudo de variantes da leptina do receptor de leptina: impacto sobre as características relacionadas com a obesidade / Study of the leptin and the leptin receptor gene variants: impact on characteristics related with obesityOliveira, Raquel de 17 June 2008 (has links)
Neste estudo, foi avaliada a relação entre polimorfismos dos genes da leptina (LEP) e receptores da leptina (LEPR) e parâmetros antropométricos, leptinemia glicemia e lipídeos séricos, em indivíduos da população brasileira. Foram incluídos 238 indivíduos com idade entre 30 e 80 anos. Foram medidos o índice de massa corporal (IMC), a cintura abdominal (CA) e a razão cintura quadril (RCQ). Amostras de sangue periférico foram obtidas para análise do perfil bioquímico e extração de DNA. Os polimorfismos de nuleotideo único (SNPs) LEP G-2548A e LEPR Lys109Arg, Gln223Arg e Lys656Asn foram detectados por PCR-RFLP. Os SNPs LEPR Lys109Arg e Gln223Arg foram associados com obesidade e com IMC e CA aumentados (p<0.05). Estes polimorfismos também foram associados com leptina e glicose elevada (p<0,05). O perfil lipídico sérico foi influenciado pelo polimorfismo LEPR Lys109Arg (p<0.05). A relação entre os SNPs LEPR Lys109Arg e Gln223Arg e o perfil lipídico foi modificada pelo gênero. Os haplótipos LEP G-2548/ LEPR Lys109Arg foram relacionados com diferenças no IMC de obesos. Os haplotipos LEPR Lys109Arg/Gln223Arg foram associados com diferenças na CA e glicemia e lipídeos séricos. Em conclusão, os polimorfismos LEPR Lys109Arg e Gln223Arg estão associados com obesidade e alterações de leptina, glicose e lipídeos circulantes de forma dependente do gênero. / We have assessed the relationship between polymorphisms of the leptin (LEP) and the leptin receptor (LEPR) genes and anthropometric parameters, plasma leptin and glucose and serum lipids in individuals of the Brazilian population. We included 238 individuais with 30 to 80 years. Body mass index (BMI), abdominal circumference (AC) and waist-to-hip ratio (WHR) were measured. Peripheral blood samples were collected for analysis of the biochemical profile and DNA extraction. The single nucleotide polymorphisms (SNP) LEP G-2548A and LEPR Lys109Arg, Gln223Arg and Lys656Asn were detected by PCR-RFLP. The SNPs LEPR Lys109Arg and Gln223Arg were associated with obesity and with increased BMI and AC (p <0.05). These polymorphisms were also associated with increase leptin and glucose (p<0,05). The serum lipid profile was influenced by the LEPR Lys 1 09Arg (p<0.05). The relationship between the SNPs LEPR Lys 1 09Arg and Gln223Arg and the lipid profile was modified by gender. The haplotypes LEP G-2548A1 LEPR Lys109Arg were related with differences on BMI in obese group. The haplotypes LEPR Lys109Arg/Gln223Arg were associated with differences on AC, glucose and serum lipids. In conclusion, the LEPR Lys109Arg and Gln223Arg polymorphisms are associated with obesity and alterations in blood leptin, glucose and lipids in a gender-dependent manner.

Análise do ganho de peso gestacional em mulheres da região Sudeste do Brasil e desfechos perinatais / Analysis of gestational weight gain at Brazilian sotheastern women´s and perinatal outcomes,Nunes, Caroline Teixeira Graf 17 December 2015 (has links)
Introdução: A obesidade é um dos grandes problemas de Saúde Pública e atinge níveis epidêmicos em grande parte do mundo. A maioria dos indivíduos com excesso de peso são mulheres, no Brasil o tamanho desta população também é expressivo, as em idade fértil são as que apresentam maior risco para o desenvolvimento da obesidade, o que está associado ao ganho de peso excessivo durante a gestação e a retenção de peso após o nascimento. O excesso de peso materno está relacionado a desfechos negativos para saúde materno-infantil. Objetivo: Analisar o peso gestacional e desfechos perinatais em mulheres da região sudeste do Brasil. Método: estudo transversal, com a utilização de dados provenientes de uma coorte nacional, com base hospitalar denominada: Nascer no Brasil: Inquérito Nacional sobre Parto e Nascimento, inquérito realizado no período de 2011 e 2012.Partindo da amostra inicial total do Sudeste composta por 10.154 mulheres entrevistadas e considerando os fatores de inclusão e exclusão para esta pesquisa, chegou-se a uma amostra de 3.405 binômios (mãe /recém-nascido).As variáveis estudadas foram ganho de peso, idade materna, peso pré-gestacional, Índice de Massa Corporal inicial e final, idade gestacional, tipo de parto e peso ao nascer. Análise foi realizada através das medidas de tendência central. Foi utilizado teste de Mann-Whitney para dados de distribuição normal e coeficiente de Pearson para variáveis contínuas. Foram considerados como significante os resultados com um p a 0,05. Resultados: A maioria das participantes apresentou faixa etária entre 21 e 30 anos, os nascimentos ocorreram entre a 38ª e 39ª semana gestacional, e seus recém-nascidos tiveram peso mediano de 3.219 g. Grande parte das pesquisadas (61,04 por cento ) iniciaram a gestação com um estado nutricional considerado adequado e 31,51 por cento apresentavam excesso de peso anterior à gestação. O ganho de peso excessivo ocorreu em todas as categorias de IMC pré-gestacional representando 49,6 por cento da população total estudada. O peso anterior à gestação apresentou elevada correlação com ganho de peso total ao final da gestação. Também foi observada influência do ganho de peso na gestação com a via de parto, idade gestacional e peso do bebê ao nascer. Conclusão: A maioria da população iniciou a gestação com estado nutricional adequado, porém, houve ganho de peso excessivo considerável em todas as categorias de IMC, este influenciou na via de parto onde a maioria aconteceu por operação cesariana e no peso ao nascer. O estado nutricional inicial influencia fortemente o estado nutricional ao final da gestação. Por isto, é importante que os programas de intervenção atuem em todas as etapas deste período, inclusive na conscientização da importância de um peso adequado anterior a concepção. Além de promover ações que auxiliem nos cuidados quanto ao ganho de peso na gestação. / Introduction: Obesity is one of the biggest public health problem and reaches epidemic levels in many parts around the World. Most of the people who are overweight are women, in Brazil the size of this population is also expressive, the child-bearing age are at greatest risk to the obesity development, which is associated with excessive weight gain during pregnancy and weight retention after the birth. The maternal overweight is related to negative outcomes for maternal and child health. Objetive: To analyse gestational weight and perinatal outcomes in women of southeastern Brazil. Method: cross-sectional study using data from a national cohort with known hospital database: \"Nascer no Brasil: Inquérito Nacional sobre parto e nascimento survey conducted in 2011 and 2012. Start with initial sample of 10,154 southeastern women interviewed and considering the inclusion and exclusion factors for this research, come up with a sample of 3,405 binomials (mother /newborn). The studied variables were weight gain, maternal age, before pregnancy weight, initial and final body mass index, gestational age, mode of delivery and birth weight. Analysis was carried out through measures of central tendency. It used the Mann-Whitney test for normal distributed data and Pearson coefficient for continuous variables. They were considered as significant results for p 0.05. Results: Most participants had aged between 21 and 30 years old, the births occurred between the 38th and 39th week of gestation, and their newborns had average weight of 3,219 g. Most of the surveyed (61.04 per cent ) started pregnancy with adequate nutritional status and 31.51 per cent had excess weight prior to pregnancy. Excessive weight gain occurred in all prepregnancy BMI categories representing 49.6 per cent of the total studied population. The weight before pregnancy showed high correlation with total weight gain at the end of pregnancy. It was also observed influence of weight gain during pregnancy related to the mode of delivery, gestational age and weight of the baby at birth. Conclusion: Most of the population started pregnancy with adequate nutritional status, however, there was considerable excessive weight gain in all BMI categories, that influenced the type of delivery where most happened by cesarean and the birth weight. The initial nutritional status strongly influences the end of nutritional status in the pregnancy. Therefore, it is important that intervention programs operate in every stage of this period, including the awareness of the importance of a healthy weight before conception. In addition to promoting actions that help us care for weight gain during pregnancy.
