Avaliação e mensuração da dor no envelhecimento: instituições de longa permanência / Assessment and measurement of pain in aging: long-term institutions

O envelhecimento é um fenômeno novo e uma tendência mundial. Com ele, há um aumento de doenças e de queixas de dor. Este estudo permitiu que os profissionais de saúde tivessem uma melhor percepção na avaliação da dor no envelhecimento. O objetivo geral foi avaliar os diferentes tipos de dor crônica no envelhecimento. Os objetivos específicos foram mensurar a dor percebida, avaliar os descritores de dor crônica de maior atribuição, identificar diferentes temáticas de dor crônica percebida e avaliar sinais e sintomas de depressão. Foi realizada identificação sócio-demográfica, com perguntas relacionadas ao sexo, à idade, à escolaridade, à religião, ao estado civil, às atividades desenvolvidas na instituição, à situação econômica, ao tempo de institucionalização, ao recebimento de visitas e às doenças diagnosticadas e, posteriormente, foram feitos quatro Experimentos (amostras dependentes). Experimento 1 - Mensurar a dor percebida - a intensidade da dor crônica foi avaliada pelo método psicofísico de estimação de categorias e se percebiam dor no momento da entrevista, para identificar o local, o tempo e o horário. Experimento 2 - Avaliar os descritores de dor crônica de maior atribuição - a intensidade da dor foi avaliada por dois métodos psicofísicos independentes: estimação de categorias e estimação de postos. Experimento 3 - Identificar diferentes temáticas de dor crônica percebida - utilizou-se uma entrevista semiestruturada com sete perguntas relacionadas à dor crônica. Experimento 4 - Avaliar sinais e sintomas de depressão - utilizou-se a Escala de Depressão Geriátrica de 15 pontos. Participaram 46 idosos residentes em duas Instituições de Longa Permanência em Ribeirão Preto, São Paulo; dentre estes, 25 eram de instituição filantrópica e 21 de instituição particular. Os resultados foram a média de idade de 78,26±8,33 anos, 26 do sexo feminino, 23 viúvos, 28 católicos, 23 cursaram o ensino fundamental incompleto, 46 aposentados e 37 recebem até 1 salário mínimo, 16 tem hipertensão arterial, 37 residem nas instituições referidas no tempo de 0 a 5 anos, 34 declararam não realizar atividade nas instituições pesquisadas e 24 recebem visitas de familiares. No Experimento 1, 14 idosos atribuíram o valor 10 para a intensidade da dor percebida na última semana, sendo a média aritmética de 7,02±2,74 pontos; 28 relataram sentir dor no momento da entrevista e 32 não haver horário específico, sendo as regiões mais afetadas os membros inferiores e a região dorsal. No Experimento 2, no método de estimação de categorias, o descritor de dor de maior atribuição foi \"dolorosa\" e o de menor foi \"desastrosa\". No método de estimação de postos, o descritor de dor de maior atribuição foi \"desastrosa\" e o de menor atribuição foi \"dolorosa\". No Experimento 3, foram identificadas unidades temáticas como \"percepção quanto ao tempo\", \"dimensão da dor\", \"estratégias de enfrentamento\", \"causas relacionadas à dor\", \"percepção da situação atual\" e \"outras percepções\". No Experimento 4, observou-se que 33 idosos responderam à Escala de Depressão Geriátrica (EDG) - 15 pontos - e obtiveram um escore de 5 ou mais pontos podendo sugerir episódio de depressão, ao passo que 13 obtiveram um escore abaixo de 5 pontos, não sugerindo episódio de depressão. / Aging is a new phenomenon and a global trend. With it, there is an increase of diseases and complaints of pain. This study has allowed health professionals have a better assessment of pain perception in aging. The overall objective was to evaluate the different types of chronic pain in aging. The specific objectives were to measure the perceived pain, to evaluate the descriptors of chronic pain with higher scores, identify different issues of chronic pain perceived and assess for signs and symptoms of depression. Identification was carried out socio-demographic, with questions related to sex, age, education, religion, marital status, the activities of the institution, the economic situation at the time of institutionalization, to receive visits and disease diagnosed and later, four experiments were conducted (dependent samples). Experiment 1 - Measuring the perceived pain - chronic pain intensity was assessed by the psychophysical method of category estimation and perceived pain during the interview, to identify the location, time and time. Experiment 2 - Assess the descriptors of chronic pain with higher scores - the intensity of pain was evaluated by two independent psychophysical methods: estimation and estimation of categories of posts. Task 3 - Identify the different themes of chronic pain perceived - we used a semi-structured interviews with seven questions related to chronic pain. Experiment 4 - Assess signs and symptoms of depression - used the Geriatric Depression Scale of 15 points. Attended by 46 elderly residents of two long-term institutions in Ribeirao Preto, São Paulo, among these, 25 were from philanthropic institutions and 21 private. The results were the mean age of 78.26 ± 8.33 years, 26 females, 23 were widowed, 28 Catholics, 23 attended the elementary school, 46 retirees and 37 to receive a minimum wage, 16 have high blood pressure, 37 reside in the institutions mentioned in the time from 0 to 5 years, 34 said they did not carry out activities in the institutions surveyed and 24 receive family visits. In Experiment 1, 14 elderly people attributed the value 10 for the intensity of perceived pain last week, and the arithmetic mean of 7.02 ± 2.74 points, 28 reported pain at the time of interview and 32 there is no specific time, and regions most affected lower limbs and the dorsal region. In Experiment 2, the method of category estimation, the greatest pain descriptor assignment was \"painful\" and the least was \"disastrous.\" In the method of estimation of posts, the more pain descriptor assignment was \"disastrous\" and the assignment was less \"painful\". In Experiment 3, thematic units were identified as: \"the perception of time,\" \"dimension of pain,\" \"coping strategies\", \"pain-related causes,\" \"perception of the current situation\" and \"other perceptions.\" In Experiment 4, it was observed that 33 seniors responded to the Geriatric Depression Scale (GDS) -15 points - and obtained a score of 5 or more points may suggest a depressive episode, while 13 had a score below 5 points, not suggesting a depressive episode.

International portfolio choice and trading behavior

This thesis consists of four essays on topics relating to the fields of international portfolio choice, trading behavior, and asset pricing. "Direct Foreign Ownership, Institutional Investors, and Firm Characteristics" analyzes portfolios of Swedish stocks held by foreign investors. The analysis reveals that foreigners tilt their portfolios to firms with certain attributes. It is also shown that the seemingly specific preferences of foreign investors are driven by the fact that they are large institutional investors, and are not linked to their national origin. "Foreigners' Trades in Risky Assets: An assessment of  Investment Behavior and Performance" analyzes foreigners' trading activities. It is shown that foreigners trade more than domestic investors. Further, they trade as non-informed trend followers in that they buy stocks that have recently done well. Nonetheless, after the liberalization of Sweden's stock market, foreigners' purchases have led to a permanent price increase and to a reduction in the cost of equity capital. "Exchange Rate Exposure, Risk Premia, and Firm Characteristics" shows that about fifty percent of Swedish listed firms are affected by exchange rate fluctuations. The sign and magnitude of exchange rate exposure are characterized across industries as well as firm attributes. The empirical analysis suggests that exposure can be eliminated through diversification, and that exchange rate risk is not priced. "Conditioning Information in Tactical Asset Allocation" examines whether investors can exploit the predictability in time-varying expected returns on Swedish stocks and bonds. It is shown that dynamic allocation strategies, based on conditioning information, significantly outperform several benchmark portfolios. The aim of this thesis is to examine and elucidate the introduction of new first names and the patterns of name giving from a social perspective based on material from baptismal registers. The region I have chosen to examine is Skellefteå and its surroundings 1791-1890. The point of departure is that the name giving may be considered a symbolic indication of the division between "us" and "them", based on constantly changing taste. During the period under examination, 1791-1890, 71 149 first names occur and 582 of which has been defined as new, i.e. first names that, as far as I know, did not exist in the area before the first example. Seen as the percentage of names occurring within different social groups the largest portion of new first names is introduced by the bourgeoisie and the smallest by the agrarian group. The new first names have been categorised as either a name choice or a name formation, with the former category containing more names than the latter. As regards categories of new names the bourgeoisie predominantly utilize name choice while for the agrarian group name formation predominates. The new pattern of name giving I have examined is the polyname system, which expands from the 18th Century onwards. The custom first grows in the name giving of girls, especially if born into the bourgeoisie. This social group also introduces the custom for the boys. When the custom of giving two names becomes too common in the region the bourgeoisie returns increasingly to one first name for the children, or to three or four names for each child. As regards the order of the new or old names, for boys the old name is generally placed first, especially among the agrarian group, if the two names are even in other respects e.g. have the same number of syllables. Once a new first name has been introduced it has to spread to survive. Of the 582 new names 422 spread and 160 do not. To examine the initial diffusion process from both a social and a geographical perspective 23 names have been chosen as representatives of different patterns of diffusion. The diffusion is described in terms of influence spreading from district to district or influence in the immediate surroundings. Categorising a historic building stock - an interdisciplinary approach

The EU Directive for building energy performance requires all member states to reduce energy consumption and CO2 emissions in the existing building stock. A key instrument in achieving this is using building stock modelling as a tool for planning and development of policies. But since the building stock as a whole is a complex element to study, new interdisciplinary methods are required to facilitate a sustainable management of the built heritage. Moreover, as the goal of energy conservation is brought into a supposed conflict with the built heritage, the field of integrated conservation has a responsibility to be a part of the development of such methods. This thesis accordingly investigates state-of-the-art building stock models from several disciplines with the aim of developing a new method for categorising historic building stocks. The historic buildings in the case study of World Heritage Site Visby, Sweden, were surveyed and triangulated using e.g. on-site inspections, digital cadastre maps, the national EPC database and existing inventories, ultimately leading to 1048 buildings from before 1945 being included in a new inventory. This inventory, along with tools acquired from previous buildings stock models, enabled an iterative process to develop and validate the new categorisation method. The proposed method itself is based on the principal idea of categorisation where the building stock is represented by a limited number of categories which allow for further typology investigations, e.g. energy modelling, and extrapolation back to district level. The results show that the building stock can be represented by nine physical categories covering 86 % of the total number of buildings, and 70 % of the entire building volume. To encompass aspects regarding cultural heritage significance, the respective historic character of the buildings are assessed and described by combining statistical information and the Conservation plan of Visby. In all, the method shows to provide a supportive platform for investigations of a trade-off between energy conservation on one hand and building conservation on the other. / EFFESUS / Spara och Bevara

Η συμπεριληπτική εκπαίδευση αποτελεί μια σύγχρονη πολιτική τακτική, η οποία κερδίζει έδαφος σε πολλές χώρες σε όλο τον κόσμο. Οι έρευνες και οι εκπαιδευτικές προσπάθειες προσανατολίζονται προς την εφαρμογή της συμπεριληπτικής εκπαίδευσης και στον χώρο της Τριτοβάθμιας Εκπαίδευσης. Η Ελλάδα ακολουθεί με αργούς ρυθμούς τις διεθνείς αυτές εκπαιδευτικές τάσεις, στην προσπάθειά της να παράσχει εκπαίδευση και σε κατηγορίες ατόμων, που παραδοσιακά αποκλείονταν από το θεσμό και να κατοχυρώσει την ισότητα της πρόσβασης και τη δικαιοσύνη της παροχής για όλα ανεξαιρέτως τα άτομα. Η παρούσα έρευνα στοχεύει να δείξει κατά πόσον η Ελληνική εκπαιδευτική πολιτική προωθεί τη συμπεριληπτική εκπαίδευση στα Ανώτατα και Ανώτερα Ιδρύματα. Η μεθοδολογία που ακολουθείται για τη διεξαγωγή της έρευνας είναι η μελέτη τεκμηρίων, αφού στηρίζεται στην αναλυτική και διαχρονική παρουσίαση των νομοθετικών ρυθμίσεων που αφορούν το συγκεκριμένο ζήτημα. Καταδεικνύεται ότι η Ελληνική εκπαιδευτική πολιτική, ακολουθώντας τα διεθνή ρεύματα, ενδιαφέρεται σταδιακά για την ίση πρόσβαση στην Ανώτατη Εκπαίδευση ορισμένων ατόμων Ειδικών Κατηγοριών. Αρκετές φορές όμως, μέσα από τον ορισμό των Ειδικών Κατηγοριών και τα νομοθετικά κείμενα που τις αφορούν ικανοποιούνται και άλλες πολιτικές από την εκάστοτε κυβέρνηση ώστε να απομακρύνεται από τους αρχικούς στόχους της που είναι η ισότητα των εκπαιδευτικών ευκαιριών για όλους τους σπουδαστές της. / Inclusive Education is developing in many countries all over the world. At present, several studies and educational attempts are focusing on Inclusive Education in the field of Higher Education. Greece tries gradually to follow these international tendencies, in an attempt to offer educational opportunities to some categories of students, who traditionally excluded from the educational system. This study is trying to point out weather Greek educational policy promotes Inclusive Education in Tertiary Education. The method of the study was Study Documents, in an attempt to study the relevant legislation of the issue. The results revealed that the Greek educational policy, based on the international tendencies, is concerned with the equal opportunities given to some students of Special Categories. Nevertheless, each government appears also to develop policies in favour of some groups instead of issuing legislations for all special needs students.

當行動裝置逐漸取代傳統電腦成為連結到網路世界的主要途徑後,反映出的就是app市場的急劇成長。如同以往建構網頁開啟企業網路溝通的通道,現在的行銷人員則是利用app來創造一個新的品牌溝通管道(brand communication channel),即所謂的「品牌app(Branded app)」。縱覽過去學術界對品牌app的相關研究,本研究發現過去品牌app的內容分析偏重於介面部分,忽略了內容面;研究主題也聚焦於單一產業的品牌app,缺乏綜觀性的角度。 因此,本研究以96個在台灣上市的品牌app(包含消耗品/耐久品/服務三種產品類別)為樣本進行內容分析,同時以介面功能(事先允許、導航、客製化、回饋、平台串聯與線上線下串聯)與推廣內容(廣告、公共關係、促銷與直效行銷)兩個角度切入研究。結果發現不同產品類別的品牌在介面功能的設計上並無差異存在;推廣內容上則發現服務類的品牌app相較於消耗品類呈現較多的「產品資訊」與「線上購買」這兩類內容。此外,本研究也發現消耗品與耐久品類的app傾向推出娛樂型的app,而服務類型的則多推出實用型的app。

Cortical spatiotemporal plasticity in visual category learning

Central to human intelligence, visual categorization is a skill that is both remarkably fast and accurate. Although there have been numerous studies in primates regarding how information flows in inferiortemporal (ITC) and prefrontal (PFC) cortices during online discrimination of visual categories, there has been little comparable research on the human cortex. To bridge this gap, this thesis explores how visual categories emerge in prefrontal cortex and the ventral stream, which is the human homologue of ITC. In particular, cortical spatiotemporal plasticity in visual category learning was investigated using behavioral experiments, magnetoencephalographic (MEG) imaging, and statistical machine learning methods. From a theoretical perspective, scientists from work on non-human primates have posited that PFC plays a primary role in the encoding of visual categories. Much of the extant research in the cognitive neuroscience literature, however, emphasizes the role of the ventral stream. Despite their apparent incompatibility, no study has evaluated these theories in the human cortex by examining the roles of the ventral stream and PFC in online discrimination and acquisition of visual categories. To address this question, I conducted two learning experiments using visually-similar categories as stimuli and recorded cortical response using MEG—a neuroimaging technique that offers a millisecond temporal resolution. Across both experiments, categorical information was found to be available during the period of cortical activity. Moreover, late in the learning process, this information is supplied increasingly in the ventral stream but less so in prefrontal cortex. These findings extend previous theories by suggesting that the ventral stream is crucial to long-term encoding of visual categories when categorical perception is proficient, but that PFC jointly encodes visual categories early on during learning. From a methodological perspective, MEG is limited as a technique because it can lead to false discoveries in a large number of spatiotemporal regions of interest (ROIs) and, typically, can only coarsely reconstruct the spatial locations of cortical responses. To address the first problem, I developed an excursion algorithm that identified ROIs contiguous in time and space. I then used a permutation test to measure the global statistical significance of the ROIs. To address the second problem, I developed a method that incorporates domainspecific and experimental knowledge in the modeling process. Utilizing faces as a model category, I used a predefined “face” network to constrain the estimation of cortical activities by applying differential shrinkages to regions within and outside this network. I proposed and implemented a trial-partitioning approach which uses trials in the midst of learning for model estimation. Importantly, this renders localizing trials more precise in both the initial and final phases of learning. In summary, this thesis makes two significant contributions. First, it methodologically improves the way we can characterize the spatiotemporal properties of the human cortex using MEG. Second, it provides a combined theory of visual category learning by incorporating the large time scales that encompass the course of the learning.

Students’ argumentative writing is substandard in the sense that the necessary relations, amongst other things, are not indicated in their texts. These texts also often lack an author’s voice. In a module such as academic literacy, it is important to pay attention to the means in which these particular problems can be solved. Part of the aims of a course in academic literacy is to equip students with the necessary academic literacy abilities (which include reading and writing ability) and in doing so, teach them to function properly in a tertiary discourse community. In this study, only the written component of academic literacy will be considered. Following the above mentioned problems, the focus will be specifically on items of metadiscourse, which may form part of a possible solution to improve students’ writing. Hyland (2004) distinguishes between two main categories of metadiscourse, namely the interactive and the interactional categories (which each consists of five subcategories). The aim of these categories is to guide the reader through the text in a specific way, and also to actively involve the reader with the textual content and the reading process. If these aspects of metadiscourse are applied effectively, the text may be more cohesive and coherent and a stronger reader-writer-relationship may be established. A corpus-linguistic approach has been followed in the investigation of the frequency of the occurrence of the subcategories of metadiscourse, as well as the functional suitability thereof. The data analysis is based on Hyland’s (2004) analytical framework of metadiscourse categories, which has been adapted according to the data that has been processed with WordSmith Tools (version 6.0). In this study, the focus group is Afrikaans L1 first-year students at the North-West University’s Vaal Triangle Campus in the year 2010. All 109 participants in the study were registered for AGLA111 (Introduction to Academic Literacy) and AGLA121 (Academic Literacy). The texts that were gathered from AGLA111 are represented in corpus 1 whereas the texts gathered from AGLA121 are represented in corpus 2. The data that was provided by these two corpora was measured against an honours corpus (consisting of 39 texts), which served as the norm for this study. The data interpretation can be divided into four categories, namely phenomena that show a statistically significant change in the correct direction, phenomena that were correct from the start and did not show any change between corpus 1 and corpus 2, phenomena that did not show any change between corpus 1 and corpus 2 but that differed from the honours corpus, as well as phenomena that show incorrect development. Recommendations, which have been based on the literature review and text analysis, are made with regard to specific aspects relating to metadiscourse and the teaching of academic literacy modules (on which this study is founded).
