O presente trabalho tem como objetivo investigar a família no contexto da loucura, considerando-se as formas cotidianas da família compreender o que vive neste cenário e as formas de lidar com o sujeito da loucura em sua casa e nos serviços de saúde mental. Para tanto, buscou-se identificar: as representações da família em relação à loucura; as formas da família se relacionar com o sujeito da loucura; as relações estabelecidas entre a família e a rede de atenção em saúde mental; e o espaço social e político destinado às famílias no campo das políticas públicas de saúde mental. Realizou-se um estudo de caso, com base nas entrevistas realizadas com a mãe de um usuário de um CAPS da capital paulista, entendendose que conhecer aspectos de uma realidade familiar, nos fornece subsídios para refletirmos sobre os rumos tomados pela atenção em saúde mental no Brasil, no âmbito da reforma psiquiátrica. Oferece, ainda, a possibilidade de levantar questões pertinentes à sua constante reformulação. A questão da desconstrução da cultura manicomial apresentou-se como um processo que exige novas formas de ver e lidar com a loucura. A família e a rede de serviços de saúde mental expressaram a contradição entre as antigas formas de se relacionar com a loucura institucionalizada e as novas formas propostas pelos ideais da reforma psiquiátrica, que busca a desinstitucionalização da loucura. A discussão tem, por referencial teóricometodológico, a Psicologia Social proposta por Pichon-Rivière. / The aim of this research is to investigate family in insanity context considering the daily ways used to understand what they live and the ways used to deal with the insane at their home and at the mental health services. In order to do that, we intended to identify the following: familys representations towards insanity; the ways the family and the insane interact; the relations set up between family and the mental health services; and the social and political space families have in the mental health policies field. We studied a case based upon interviews with an attendant of CAPS mother, in the capital of the State of São Paulo. We understand that knowing some aspects of the family reality gives us conditions to think about how the mental health attention evolved in Brazil after the psychiatric reform. It also offers us the possibility to make important questions about its constant reformulation. The asylum culture dismantling issue can be faced as a process that demands new ways of seeing and dealing with insanity. In this research, family and mental health services expressed the contradiction between former ways to deal with the institutionalized insanity and new ways proposed by the ideals of psychiatric reform, which pursues the end of institutionalized insanity. The discussion is supported, in theory and methods, by the Social Psychology proposed by Pichon-Rivière.

O trabalho tem por objetivo explicitar as possíveis relações entre loucura e feminilidade, tomando como ponto de partida observações efetuadas no exercício clínico, tanto em consultório particular como em instituições de saúde mental, a partir das contribuições teóricas de Freud e Lacan. Denominamos de atravessamentos do feminino as expressões de sofrimento psíquico, em particular nos pacientes que passavam momentos de crise ou desorganização psíquica grave e que, de formas variadas, apresentavam signos referidos ao universo feminino. Inicialmente, realizamos uma retomada histórica da construção do conceito de histeria e, neste percurso, abordamos o quadro da loucura histérica, a partir do qual salientamos como a inclusão da histeria no campo médico acabou por desarticular os três termos que o quadro de loucura histérica carregava: loucura - histeria - feminino. Em seguida, foram analisados os relatos clínicos freudianos de Dora e de Schreber, desde que ambos são ilustrativos de como certos aspectos da feminilidade que irrompem na vida desses sujeitos podem ser entendidos como manifestações clínicas, produzidas como respostas possíveis à hiância aberta pelo encontro com o enigma da feminilidade. / The aim of this paper is to establish the possible relationships between insanity and femininity, using observations from clinical practice, (private practice as well as mental health institutions), as a jumping off point, beginning with theorical contributions from Freud and Lacan. The title feminine cross-over is given to expressions of psychological suffering, particularly in patients who have been through moments of crisis or grave psychological disorientation and which, in their varied manifestations, show signs which refer to the feminine universe. First, a background study of the construction of the concept of hysteria was carried out, in the course of which the clinical profile of hysterical insanity became apparent. Using this, the inclusion of hysteria in the medical field ended up being articulated by three terms loaded into the profile of hysterical insanity: insanity - hysteria - feminine. Next, Dora and Schreber\'s Freudian clinical reports were analyzed, since both illustrate how aspects of femininity which appear in the subjects\' lives can be understood as clinical manifestations, produced as possible answers to the beance (gap) opened by the shock with the enigma of femininity.

I here present the subject's response to anguish, especially in passage to the act and acting-out, recurring in the clinical case analyzed. I approach the discursive structure of hysteria and hysterical madness, both of them associated to a complaint of father's inadequacy and an insistent demand for the Other to supply to the overflowing inward. I also mention the differences in paternal function in psychosis and in hysteria, the constitution of the subject – based on The Time of Oedipus –, and Lacan’s Mirror Stage. Finally, from the theoretical-clinical articulation, I develop the treatment conduction, from transference management to possible subjective constructions / Tomando como base o caso clínico, estre trabalho articula o conceito de loucura histérica como uma condição que participa da neurose histérica e tem manifestações radicais e severas, muitas vezes semelhantes a um surto psicótico devido à proporção de suas manifestações dissociativas e de suas excessivas crises que parecem uma ruptura, como na psicose. Apresento as respostas do sujeito frente à angústia, principalmente na passagem ao ato e acting-out, recorrentes no caso clínico analisado. Abordo a estrutura discursiva da histeria e da loucura histérica, ambas associadas a uma queixa de insuficiência do pai e a uma insistente demanda de que o Outro dê conta do interno que transborda. Trata-se de uma investigação teórica, documental e qualitativa que triangulou métodos e analisou as diferentes epistemologias historicamente presentes neste constructo, incluindo visões rivais, paradoxais, contraditórias e complementares. O estudo foi efetivado através de diferentes óticas, partindo do resgate histórico e conceitual de cidadania pelos ângulos da clínica asilar e da clínica de cuidado psicossocial; pela perspectiva da cidadania nos documentos oficiais nacionais e internacionais, com destaque para os documentos do movimento da sociedade civil organizada e dos relatórios das Conferências Nacionais de Saúde Mental; pelo cotidiano de cuidados de um Centro de Atenção Psicossocial e de uma Associação de Usuários e Familiares. O trabalho no conjunto, em seus componentes históricos e documentais e os colhidos no campo, foi submetido a uma análise Hermenêutico-Dialética. Não é possível conceber cidadania sem participação e inclusão, o que remete à constatação de que a cidadania é relacional, visto que ela não existe fora de um território e de um grupo social. Considerando os dados analisados, foi possível identificar que existem dificuldades de diferentes ordens na inclusão da práticas cidadãs no cotidiano de cuidados de serviço CAPS: presença de modelo verticalizado com predomínio de uma disciplina em detrimento de outras, foco nos sinais e sintomas e tratamento medicamentoso, atitudes protetivas que, de forma implícita, remetem à desqualificação das pessoas em tratamento, supremacia de ações com caráter humanitário para além de um cuidado que preze por ações inclusivas e de resgate à cidadania. Conclui-se que as categorias fundantes da psiquiatria e dos direitos do homem, tais como o princípio da racionalidade e o princípio da igualdade, são nucleares na definição do lugar social ocupado pelas pessoas com transtorno mental e estão umbilicalmente associadas com as dificuldades para concretizar os pressupostos de sua existência cidadã. / This thesis has as goal of identifying how the sense and meaning of being a person with mental health is configured on the Brazilian Psychiatric Reform, on the routine operations in one Center of Psychosocial Care (Centro de Atenção Psicossocial-CAPS) and on a local community association. It’s a theoretical, documentary and qualitative investigation that triangulates methods and analyzes different epistemologies, which are present in the concept of wellbeing and mental health problems, including many rival, paradoxical, contradictory and complementary visions. The work was made from different perspectives, starting with one theoretical and conceptual study of the significance of being a mental health Patient, using the perspectives of the asylum clinic system and the psychosocial care clinic; it considers the perspective of human rights in national and international official documents, highlighting those that come from the wide range of civil society organizations and from the National Conferences on Mental Health reports and on the praxis of one Center of Psychosocial Care and one local Family and Users Association for Mental Health. The work as a whole was submitted to a hermeneutic-dialectic analysis. Is not possible to think of mental health without inclusion. This refers to the finding that a patient with mental health disorder is relational since it does not exist out side of the territory of a social group. Considering the analyzed data it was possible to identify that there are difficulties of different orders in the inclusion of practices with citizenship in the assistance: the presence of a vertical model with the supremacy of one discipline to the detriment of others, focus on the signals and symptoms and drug treatment, protective attitudes that implicitly refer to disqualify people under treatment, domination on humanitarian character actions besides the appreciation of inclusive actions of rescue of citizenship. It is concluded that the categories that founded Psychiatry and human rights, such as, the principle of rationality and the principle of equality are nuclear of the definition of the social place occupied by people with mental disorder; also associated with difficulties to achieve the assumptions of the human being existence.

The main objective of this research is investigate the relationship between space and subjectivity production in the practical work of caring for the insane, or more specifically investigate how the space and the production of subjectivity in the creation of existencials territories. For this we use concepts mainly made by Felix Guattari. We also use some texts the author's partnership with Gilles Deleuze, especially those that discusses the concept of existential territory. The research draws on empirical contributions recorded in a field journal. We went through a trial to produce cartographic narratives that deal with the assemblages produced between space and subjectivity. The diary was not transcribed in their original content, was read and rewritten in a format we call scene. This dissertation was written in four chapters. In the first we seek evidence from the book "History of Madness in the Classical Age" of Michel Foucault, successive displacement experienced by madness in the West and in the second we discussed the notion of the production of subjectivity and existential territories, the third chapter deals with the movements Psychiatric Reform and processes of dis-institutionalization, and the fourth chapter with concluding remarks. At the end of the study, the possibility of building new practices from an ethic of hospitality. Having hospitality with one another in their uniqueness, staying in a foreign country of madmen. This hosting as a finding of the research. During the course of writing, a finding that may indicate possibilities of working in clinical settings. An indication of the importance for careful management practices driven by PR, interventions and actions produce relief. The services must be built daily in a temporary plan. Decreasing the risks of reps buoyed by the strength and logic, whether disciplinary or control, and possible ways of acting not bureaucratized that could harbor the natural productions of subjectivity / O contato com a loucura foi um dos disparadores de perguntas que culminaram nesta pesquisa. Destes contatos decorre esta dissertação que tem como o objetivo principal investigar a articulação entre espaço e produção de subjetividade no trabalho prático do cuidado com a loucura, ou mais especificamente, investigar como se agenciam o espaço e a produção de subjetividade na construção de territórios existências. Para tanto serão utilizadas algumas conceituações formuladas principalmente por Félix Guattari. Também nos valeremos de alguns textos da parceria do autor com Gilles Deleuze, principalmente aqueles que versam sobre a conceituação do território existencial. A pesquisa realizada se vale de aportes empíricos registrados num diário de campo. Percorremos uma experimentação cartográfica ao produzir narrativas que versam sobre os agenciamentos produzidos entre o espaço e a subjetividade. O diário não foi transcrito em seu conteúdo original, foi lido e reescrito em um formato que chamamos de cena. Esta dissertação foi escrita em quatro capítulos. No primeiro buscamos evidenciar, a partir da obra História da loucura na idade clássica de Michel Foucault, os sucessivos deslocamentos espaciais experimentados pelo louco no Ocidente; no segundo discutimos a noção de produção de subjetividade e territórios existenciais; o terceiro capítulo trata dos movimentos de Reforma Psiquiátrica e processos de desinstitucionalização; e um quarto capitulo com as considerações finais. Ao final da pesquisa, a possibilidade de construir novas práticas a partir de uma ética da hospitalidade. Ter hospitalidade com o outro em sua singularidade, hospedar-se no território estrangeiro dos loucos. Essa hospedagem como um achado da pesquisa. Uma indicação de manejo atenta para a importância das práticas impulsionadas pela RP, produzirem intervenções e ações provisórias. Os serviços devem ser inventados diariamente em um plano temporário. Assim, diminuem-se os riscos de repetições balizadas pelas forças e lógicas, sejam disciplinares ou de controle, e possibilita maneiras de atuação não burocratizadas que possam abrigar as produções de subjetividade singulares

The internal and external rules and communications of the companies define attitudes and the labor activity in a way that orchestrate the relations among employees. Because the demands of the job are imposed through ritual habits, which are based in the maintenance of the employee s behavior, this relationship between people and companies generates goods, but also can drive to madness. In a context based in profits, the companies expect their employees to work like actors in mechanical rules, performing regular and repetitive actions. The company becomes a stage of coercion, keeping workers because it appropriates their possibilities of survival. Two distinctive symbolic universes act on this stage: the corporative culture and the employee culture, which results in new corporate bonds and rearranged social bonds. Such bonds, where the people s beliefs and myths are, live on what Ivan Bystrina calls first reality , that contains their basic survival necessities, and on the second reality , that means their symbolic world in the author s conception, where are the human beliefs, fantasies, dreams and hope of professional and personal realization. Submitted to the pressure of rules, people get into conflicts and get so upset that they can become mad, being madness one of the roots of culture. People can loose, in this process, their conscience, or the ability to distinguish reality from imagination. This kind of symptom is observed in the study case Herbalife. / A investigação tem por objetivo analisar a relação entre os homens, o trabalho e a loucura. A relação vivenciada no espaço organizacional é orquestrada pela comunicação interna da empresa que, através das normas internas e externas, define atitudes e rege a atividade laboral. Pelo fato do comportamento desejado pela empresa ser imposto aos funcionários através de hábitos ritualizados, pautados pela manutenção das atitudes dos que querem manter os vínculos profissionais, a relação entre os homens e a empresa tanto gera trabalho como também produz loucura. Neste contexto, determinado por fins lucrativos, a corporação treina insistentemente seus homens e faz com que trabalhem como atores que representam mecanicamente determinados papéis. Como esta representação é pautada por gestos e ritmos repetidos com regularidade em vista da consolidação de metas, a empresa se transforma em palco de coerção e mantêm os funcionários porque captura deles as necessidades de sobrevivência reais e possíveis. Neste palco contracenam dinamicamente dois universos simbólicos distintos: a cultura corporativa e a cultura do funcionário. Nessa arena de enfrentamento cultural e produção de bens ou serviços são construídos e reconstruídos novos vínculos corporativos que funcionam como rearranjos dos vínculos sociais. Estes vínculos, com raízes onto e filogenéticas, são balizados por mitos e crenças de que no universo do trabalho os homens alimentam seu mundo real, a primeira realidade na concepção de Ivan Bystrina, composto de necessidades básicas de sobrevivência, e o seu mundo simbólico, a segunda realidade, composto de crenças, fantasias, sonhos e esperanças de realização pessoal e profissional. Submetidos às pressões das normas, os homens entram em conflito e enredados por uma das raízes da cultura, os estados alterados de consciência, se perdem de si mesmos resvalando o universo da loucura real ou imaginária. Indícios deste processo são observados no estudo de caso da empresa Herbalife.

La création artistique est souvent associée injustement à la folie, à ce trou noir où la souffrance règne. Ce travail de recherche se construit à partir de l’expression « la folie, l’absence d’œuvre » de Michel Foucault et se penche sur la notion de folie en tant qu’entrave à la création. Loin de tout éloge immérité qu’on a attribué à cette notion vaste et équivoque tout au long de l’histoire, d’Erasme au surréalisme, cette thèse entreprend de réfléchir sur la fonctionnalité du travail créatif en cas de démence. Un regard historique sur la représentation de la folie dans l’art occidental éclaircira la prise de position de l’artiste à l’égard de l’aliénation mentale ou collective. Ainsi, à travers une étude approfondie de différents parcours artistiques ébranlés par des troubles mentaux, cette thèse s’engage à esquisser le trajet de l’artiste depuis le trouble psychotique jusqu’à sa tentative de guérison par la création. Considérant des liens entre l’état psychique et le dessin au sein de mon propre travail plastique, je m’interroge également sur la responsabilité sociale d’une œuvre troublante dans le monde contemporain. / Artistic creation is often associated unjustly with madness, this black whole where suffering reigns. This research is based on the expression of “The madness, the absence of artwork" by Michel Foucault and studies the notion of madness as an obstacle to creation. Far from any undeserved compliments which have been attributed to this vastand equivocal notion throughout the history, from Erasmus to Surrealism, this thesis is undertaken to reflect on the features and functions of creative artwork in case of insanity. A historical look at the representation of madness in Western art will shed light on the artist's position regarding his mental or collective alienation. Thus, through an in-depth study of different artistic paths influenced by mental disorders, this thesis sketches the artist’s path from the psychotic disorder to his attempt at recovery through creation. Considering the links between psychic state and drawing in my own artwork, I raise question of the social responsibility of a troubling work in the contemporary world.

[pt] O objetivo do trabalho é, a partir da análise de algumas obras, escritos pessoais, artigos e crônicas de Lima Barreto, estabelecer uma relação entre aqueles personagens criados por Lima Barreto que têm, de alguma forma, uma ligação com a loucura, e a visão questionadora do escritor a respeito do novo cenário urbano que se desenvolvia, levando em conta dois aspectos fundamentais: a concepção de literatura defendida pelo escritor, que era de uma literatura utilitária, capaz de contribuir para o combate às distorções do regime republicano, e o momento de consolidação da psiquiatria, ressaltando as críticas do escritor ao caráter discriminatório da ciência no início do século XX. / [en] This work objective is to establish a relation, starting from the analysis of some of Lima Barreto s works, personal writings, newspaper articles and chronicles, between those characters - created by him - which are, in some way, bound to madness, and the author s questioning view on the new urban scene, then on development, considering two fundamental points: (1) the author s utilitarian conception of literature, by him defended, able to contribute to the fighting against republican regime distortions, and (2) the moment of consolidation of psychiatry, with prominence to the author`s critics towards this science s discriminatory character in early XXth Century.

At the same time a paralell movement is observed marked by the maintenance of the hospital centered or psychiatric ward ideology present not only in the psychiatric institutions but also in the substitute services in the cities and , above all, in the ways of contemporary subjectivation. We affirm that the idea of deinstitutionalization is intertwined with capitalistic agent both from the epistomological, assistance, and legal aspect as from the cultural one. This work aims mainly to discuss madness and subjectivity from the ethic esthetic-political perspective, specially the wishes of the psychiatric hospital, which we inhabit and who inhibits our interactions and our desired and creative productivity. Esta tese conta uma história de Arthur Bispo do Rosário (1909-1989), procurando problematizar sua trajetória através de uma análise enunciativa, mostrando a maneira como o personagem se delineia de diferentes formas, conforme o olhar de quem o apreende, de quem o toma e o institui como objeto. Loucura e arte se entrelaçam para compor a capacidade artística e a genialidade de um sujeito tido como único, que engendraria, em suas criações, todas as referências da arte contemporânea. Bispo não é o produtor central dos acontecimentos que perpassam estas páginas, mas sim o resultado da batalha discursiva aqui problematizada. Objetivou-se mostrar que não existe um único Bispo anterior às tramas discursivas que buscam representá-lo, um sujeito fundante, um ponto de partida que inauguraria seus gestos e palavras. O que existe são diferentes Bispos, produzidos pelos discursos que o apreenderam. Para isso, estudos acadêmicos, documentos institucionais, entrevistas, poemas, sambas enredo, fotografias, reportagens, inventários, bem como as peças por ele produzidas, hoje tidas como obras de arte, foram tomadas como monumentos que procuram dizer quem foi Arthur Bispo do Rosário. O olhar que norteou o presente estudo foi direcionado pelas noções foucautianas de “práticas discursivas e não discursivas”, “sujeito”, “enunciado” e “invenção de si”, além de outros conceitos como “enquadramento da memória” e “monumentalização”, e seus resultados apresentam-se em cinco partes, as quais tentam dar conta da intriga proposta: o período anterior à internação, sua vivência da Colônia Juliano Moreira (RJ), sua versão de si, sua incursão pelo mundo das artes plásticas e sua monumentalização. / This thesis tells the story of Arthur Bispo do Rosário (1909-1989), looking to investigate and problematize his history through an analysis of enunciation, showing how the character outlines in different ways, depending on the look of someone who understands him, who takes him and who instituting him as an object. Madness and art intertwine to compose the artistic capacity and geniality of a unique man that would engender in his creations, all references in contemporary art. Bispo is not the producer of the central events that run through these pages, but the result of the discursive struggle herein analyzed. This work aimed to show that there isn‟t just one Bispo before the discursive intrigues that looking to represent him, a founding subject, a starting point to inaugurate his gestures and words. What exists are different Bispos, produced by the discourses that had apprehended him. To this end, academic studies, institutional documents, interviews, poems, sambas storyline, photographs, reports, inventories, and documents produced by him, now regarded as works of art were taken as monuments that tell who was looking for Arthur Bispo do Rosário. The look that guided the present study was directed by Foucault‟s notions of "discursive and non-discursive practices," "subject", "utterance" and "self-invention", and other concepts such as "classification of memory" and "monumentalization". Their results are presented in five parts, which try to account for the intriguing proposal: the period prior to his internment, his life experience in Juliano Moreira colony (RJ), his version of himself, his foray into the world of plastic arts and his monumentalization.
