HBTQ-patienters vårdupplevelser : En litteraturöversikt / Care experiences among LGBTQ-patients : A literature review

Bakgrund: Sjuksköterskan har till uppgift att vårda och ta hand om patienter på lika villkor oavsett bakgrund och sjukdomstillstånd. HBTQ-patienter har exempelvis sämre psykisk hälsa jämfört med hetero- och cispersoner. Det beskrivs också att HBTQ-personer är en utsatt grupp i samhället och att detta orsakar HBTQ-personerna lidande. Den vårdande relationen som är avgörande för omvårdnaden kan därmed bli en trygghet för dessa personer. Syfte: Studiens syfte var att beskriva hur HBTQ-patienter upplever vården. Metod: För att besvara syftet användes en litteraturöversikt innehållande åtta analyserade artiklar. Resultat: Genom de meningsenheter som plockades ut ur artiklarna identifierades tre teman; att komma ut till vårdpersonalen, upplevelsen av heteronormativitet och homofobi i vården samt HBTQ-patientens upplevelse av vårdpersonalen. I temana beskrivs varför HBTQ-patienterna kommer ut till vårdpersonalen, att heteronormativiteten upplevdes vara hinder för god vård samt att vårdpersonalen behövde mer kunskap och ett annat förhållningssätt till HBTQ. Konklusion: HBTQ-patienterna upplevde att de var tvungna att komma ut till vårdpersonalen för att erhålla den vård de behövde, vården är baserad på heteronormer och att de blev utsatta för homofobi samt att vårdpersonalen inte hade tillräcklig kunskap för att kunna bemöta vårdbehovet och önskemålen hos HBTQpatienterna. / Background: The Nurse is responsible to care for patients on equal terms irrespective of background and state of illness. For example, LGBTQ patients have poorer mental health compared to heterosexual and cis persons. It is also described that LGBTQ people are a vulnerable group in society and this causes the suffering of LGBTQ people. The caring relationship that is crucial to nursing can thus be a security for these people. Purpose: The purpose of the study was to describe how LGBTQ patients experience the care. Method: To answer the purpose, a literature review containing eight analyzed articles was used. Result: Through the sentences picked out of the articles, three themes were identified; to get out to the healthcare staff, the experience of heteronormativity and homophobia in the care and the LGBTQ patient's experience of the healthcare staff. The themes describe why the LGBTQ patients come to healthcare professionals, that heteronormativity was perceived to be an obstacle to good care, and that healthcare professionals needed more knowledge and another approach to LGBTQ. Conclusion: The LGBTQ patients felt they had to come out to healthcare professionals to get the care they needed, care were based on heteronorms and they were exposed to homophobia and that the healthcare staff had insufficient knowledge to respond to the need for care and the wishes of LGBTQ patients.

Lesbians and Space: An Interpretive Phenomenological Analysis

In a moment when visibility and representations of LGBTTQAI+ people are proliferating in North American society, it is important to think critically about how visibility and representations function and to interrogate their meanings and a/effects. This thesis uses data produced from five semi-structured interviews conducted with lesbian identified participants living in non-urban spaces in Ontario to demonstrate the importance of a continued lesbian specificity, to draw attention to heteronormativity and heterosexism in Ontarian society, to challenge femme invisibility and complicate the notion of femme privilege, and to move beyond the urban/rural binary as a way of making sense of sexuality. The methodological framework guiding this thesis draws on interpretive phenomenological analysis as well as feminist and queer methodologies, which facilitated a responsive and reflexive research process. This thesis is grounded in ongoing debates around identity politics and representation, drawing on literature from lesbian theories, lesbian-feminist histories, queer theories, heterosexism, heteronormativity and homonormativity, lesbian-feminist histories, white privilege studies, queer and feminist geography, and LGBTTQAI+ rural studies.

Wed-Locked: Television and the Acceptance of Same-sex Relationships

2011 was the first year the majority of the American public were in favor of same-sex marriage—a nine point (and largest year-to-year) increase from 2010. That year gave the LGBTQ community a crucial win in the hard-fought cultural war over government validation of same-sex relationships. Not so coincidentally, 2010 saw mass media, specifically network television, depict same-sex relationships like never before. New shows like Modern Family, Glee and The Good Wife hit their ratings zenith alongside stalwarts like Grey’s Anatomy, Desperate Housewives and House. But were the relationships depicted diverse in terms of roles, race, class and gender? Or did they resemble the heteronormative ideal of the white, upper middle-class relationship and family? Through a discourse analysis of popular, scripted network television shows from the 2010-2011 season, I found the depictions to powerfully create a “normal” same-sex relationship towards a heteronormative ideal. Both the same-sex women and men’s relationships were heteronormative in that their statuses and roles within the relationship adhered to the classic masculine/feminine binary. However, the same-sex women’s relationships were queerer, exhibiting sexual fluidity and labels beyond gay and straight. Still, the women maintained Western, feminine appearances supporting Laura Mulvey’s male gaze. The same-sex men’s relationships fully supported Jasbir Puar’s notion of the “exceptional homosexual.” Beyond their roles, the men’s relationships were heteronormative by being same-raced/white, upper-class, and, in two out of the three couples, having children. Ultimately, all the depictions exemplified Monique Wittig’s frustration that historical “discourses of heterosexuality oppress us in the sense that they prevent us from speaking unless we speak on their terms.”

A partir dos estudos sobre os efeitos da matriz bioparental e heteronormativa na adoção, buscamos realizar a (re)leitura da história da adoção no Brasil, compreendendo os significados – religiosos, morais, sociais – que a prática ocupou ao longo do tempo. Paralelamente, desenvolvemos uma série de problematizações sobre identidades, homossexualidade e heterossexualidade e sobre o questionamento em Butler (2003a) se o parentesco está adstrito aos termos de uma matriz heterossexual legitimadora. Consideramos, segundo Teixeira Filho (2010) que a sobreposição dos laços de sangue hierarquiza os vínculos de filiação ao sustentar uma “essência” humana em procriar e continuar existindo através dos filhos, pondo em evidência os valores heteroreferentes dos papéis sociais de gênero que essa naturalização implica. Tal referência, reiterada/recitada na adoção, operaria uma série de efeitos, desde a expectativa de (hetero)normalização de casais do mesmo sexo à sua tentativa de docilização, por meio do uso reiterado do termo homoafetividade. Assim, o trabalho pretende realizar uma leitura, a partir de uma perspectiva interdisciplinar em direitos humanos, dos termos do debate no processo judicial em análise, com atenção à articulação entre as vozes dos pretendentes à adoção, das crianças, da equipe interprofissional e multidisciplinar, do Ministério Público e do órgão julgador. Por conseguinte, quer-se questionar em que medida, especialmente no âmbito da adoção conjunta por dois homens em análise, o parentesco é tido como heterossexual ou, no mínimo, adstrito aos termos da heterossexualidade normativa. / This dissertation aims to investigate, under Judith Butler’s queer lenses, the meanings underlying the adoption by a same sex couple in the Brazilian reality through the study of the first case prosecuted and judged in the Court of the State of Pernambuco (2007-2008). Inspired by studies on the effects of bioparental and heteronormative matrix in adoption practices, we seek to achieve a (re) reading of the history of adoption in Brazil, comprehending the meanings – religious, moral, social – revolved around this practice over time. At the same time, we problematized topics such as identities, homosexuality and heterosexuality, as well as Butler’s questioning of whether kinship is always bounded to the terms of a legitimizing heterosexual matrix. We believe that, as Teixeira Filho (2010) has clearly explained, the coincidence of blood ties ranks the bonds of belonging by sustaining a human "essence" to procreate and a desire to remain alive through the lives of their children. This naturalization implies the heteronormativity of social gender roles values. Such reference, repeated / recited in the adoption process, probably operates a number of effects from the expectation of (hetero) standardization of same-sex couples to the attempt of highlighting the docilization of these couples through the exhaustive use of the term “homoafetividade”. From an interdisciplinary perspective on human rights, this work pursues a different reading about the terms of the debate in the judicial process under review, with attention to the relation between the voices of the couple who applied for the adoption of the children, the judicial interprofessional and multidisciplinary team, the prosecutor and the judge. Therefore, we hope to discuss to what extent, especially in the context of the same sex joint adoption, is kinship always considered heterosexual or at least attached to the terms of normative heterosexuality.

The objective of this research was to analyze the perception of non-heterosexualy outh about their sexualities, as well as to verify their experiences during the schooling process in the contextof formal education. The adopted epistemological framework is based on post-structuralism and the qualitative approach wasadopted as a methodological design. The research was conducted with Young people who do not identify with the heteronormative patterns in terms of the expression. of their sexualities. In addition, it was considered that these participants stand out for their social actions related to debates about youth protagonism, sexual diversity and drug prevention in their communities, located in theperipheryof a medium-sizedcity in the interior of the state of São Paulo. The instruments used were semi-structured interviews and reflective interviews. The issuesofthe script wereformulatedfromdiscussionsbroughtbythearea-specificliterature. The focus group was also performed as a technique for data collection, conducted through the use of images that portrayed central analytical categories in the adopted the oretical framework. It was also understood that the images would allow a greater approximation of the life stories of the participants, allowing the evocation of subjects not explored just with the use of interviews. For the analysis of the material was used the technique of contente analysis. Fromtheanalyzes, three analytical categories were identified in the Study: a) Structural homophobia; b) Injury in the school environment; c) A network of affective and social support. The results of the work demonstrate that, before a heteronormative society, any expression. Of desireconsidered deviant can be constantly rejected and invisible with in a meticulous system engendered by social practices and discourses, including the formal environments of education at its diferente levels. The research is made relevant by the importance of the theme for thearea of education, especially at the moment when conservative and fundamentalista discourses gain diferente contours and spaces. In addition, the investigative process made it possible to reflect on the extentto which these "abnormal" subjects can transit in formal education spaces, despite the fact that these spaces are configured as homophobic, violent, oppressive and that produce psychic suffering in subjectsthat are notheteronormativepatterns. / A presente dissertação situa-se na linha de pesquisa intitulada “Políticas Públicas em Educação, Práticas Educativas e Diversidade”, do Programa de Pós-Graduação em Educação da Universidade do Oeste Paulista (UNOESTE). O objetivo desta pesquisa foi analisar a percepção de jovens não-heterossexuais sobre suas sexualidades, bem como verificar as suas experiências durante o processo de escolarização no contexto de educação formal. O referencial epistemológico adotado tem como referência o pós-estruturalismo e foi adotada a abordagem qualitativa como desenho metodológico. A pesquisa foi realizada com jovens que não se identificam com os padrões heteronormativos em termos da expressão de suas sexualidades. Além disso, foi considerado o fato destes participantes se destacarem por suas ações sociais relacionadas a debates sobre protagonismo jovem, diversidade sexual e prevenção às drogas em suas comunidades, situadas na periferia de uma cidade de médio porte do interior do estado de São Paulo. Os instrumentos utilizados foram entrevistas semiestruturadas e entrevistas reflexivas. As questões do roteiro foram formuladas a partir de discussões trazidas pela literatura específica da área. Também foi realizado o grupo focal como técnica para a coleta de dados, conduzido por intermédio do uso de imagens que retratavam categorias analíticas centrais no referencial teórico adotado. Também se entendia que as imagens possibilitariam uma maior aproximação das histórias de vida dos participantes, possibilitando a evocação de temas pouco explorados apenas com o uso de entrevistas. Para a análise do material foi utilizada a técnica de análise de conteúdo. A partir das análises, foram identificadas três categorias analíticas no estudo: a) Homofobia estrutural; b) A injúria no ambiente escolar; c) Rede de apoio afetivo e social. Os resultados do trabalho demonstram que, diante de uma sociedade heteronormativa, qualquer expressão de desejo considerada desviante pode ser rechaçada e invisibilizada constantemente dentro de um sistema meticuloso engendrado por práticas e discursos sociais, incluindo os ambientes formais de educação em seus diferentes níveis. A pesquisa se faz relevante pela importância da temática para a área da educação, especialmente no momento em que discursos conservadores e fundamentalistas ganham diferentes contornos e espaços. Além disso, o processo investigativo possibilitou a reflexão sobre até que ponto esses sujeitos considerados “anormais” podem transitar em espaços de educação formal, apesar da constatação destes espaços se configurarem como homofóbicos, violentos, opressivos e que produzem sofrimento psíquico em sujeitos que não se enquadram em padrões heteronormativos.

Att inte trampa i heteronormklaveret : En kvalitativ studie av hur familjebehandlare förhåller sig till heteronormen i mötet med samkönade föräldrar

I och med att regnbågsfamiljer blir allt vanligare ökar kraven på ett likvärdigt och professionellt bemötande, vilket således ställer krav på kunskap och kompetens hos professionella. Samtidigt råder fortfarande heteronormativa föreställningar om familjen i samhället. Denna kvalitativa intervjustudie undersöker, utifrån en socialkonstruktivistisk ansats, hur familjebehandlare förhåller sig till heteronormen i mötet med samkönade föräldrar. Genom tematisk analys och med utgångspunkt i queerteorin och dess heteronormativitetsbegrepp, i kombination med de teoretiska begreppen stigmatisering, under- och överfokusering, exotifiering samt den heterosexuella matrisen, har fem teman av familjebehandlares föreställningar kring bemötande av samkönade föräldrar urskilts. De teman som framkommit i materialet är att ha samma bemötande oavsett sexuell läggning, att göra anpassningar för inkludering i bemötandet, att förhålla sig nyfiket till samkönade par, att se kunskap som centralt för att säkerställa ett gott bemötande, samt föreställningar kring kön och sexualitet. I familjebehandlarnas resonemang kan heteronormativa föreställningar urskiljas men även försök till motstånd och förändring. Detta motstånd mot heteronormativt bemötande av samkönade föräldrar, riskerar dock att oavsiktligt reproducera heteronormativa föreställningar.

Att synliggöra det osynliga En kvalitativ studie om kvinnors erfarenheter av sexuellt våld i lesbiska relationer

Syftet med den här studien är att synliggöra sexuellt våld som sker i lesbiska relationer. Med en kvalitativ ansats ligger ett fokus på lesbiska kvinnors upplevelse och erfarenhet av sexuellt våld i en tidigare samkönad relation. Datainsamlingen har skett i form av fem semistrukturerade intervjuer med lesbiska kvinnor som utsatts för sexuellt våld i en tidigare lesbisk relation. Empirin analyseras utifrån teoretiska ramverk och begrepp.Resultaten i studien indikerar att det sexuella våldet är internaliserat bland lesbiska offer och utgör en del av andra former av våld. Det förekom psykisk ohälsa hos förövaren. Uppbrottsperioden för de lesbiska offren var långvarig och samtliga deltagare sökte professionellt stöd. Samtliga offer hade psykisk ohälsa som ett resultat av att ha utsatts för kronisk misshandel. Dessa resultat är viktiga för fortsatt forskning inom samkönat våld för att kunna synliggöra det osynliga. / The aim of this study is to visualise sexual violence in lesbian relationships. With a qualitative approach there is a focus on lesbian women’s experience of violence from a previous same-sex relationship. Data collection has been undertaken by five semi-structured interviews with lesbian women who have been exposed to sexual violence in a previous relationship. Theoretical frame works and terms will be utilized to analyse the empirical material. The results in this study indicate how the sexual violence is internalized by lesbian victims, thus hard to vocalize. Further to this, sexual violence is only one by many different forms of violence. The perpetrator show signs of mental health problems. The break up period was a long struggle for the victims and all of the victims had mental health problems as a result of long term abuse which they sought professional help for. These findings are pivotal for further research regarding same-sex violence in order to visualize the invisible.

Regnbågsfamiljer i förskolan : En kvalitativ studie om hur regnbågsfamiljer konstrueras i förskolan

Syftet med denna studie var att undersöka hur förskollärare konstruerar regnbågsfamiljer i förskolans verksamhet och vilka normer om familj som träder fram ur förskollärares berättelser. Studien vilar på det socialkonstruktionistiska perspektivet där kvalitativa intervjuer genomförts för att besvara studiens syfte och forskningsfrågor. Studiens resultat visar på att regnbågsbarn exkluderas i förskolan om regnbågsfamiljer inte inkluderas och konstrueras i verksamheten. I ett vidare perspektiv kan det innebära att regnbågsbarn i förskolan lider av ett utanförskap då de inte får sin familjebild bekräftad i verksamheten. Trots detta visar resultatet på en tydlig kärnfamiljsnorm i förskollärares berättelser / The aim of this study was to find out how preschool teachers construct LGBTQ families in preschool and what norms of family emerge from preschool teachers´ stories. The study rests on the social constructionist perspective where qualitative interviews were conducted to answer the study´s purpose and questions. The result show that LGBTQ children are excluded in preschool if LGBTQ families are not included and made visible. In a wider perspective this may mean that LBGTQ children in preschool suffer from socially alienation if they do not get their family construction confirmed in preschool. Despite this, the results show a clear norm of heteronormativity in preschool teachers´ stories

Bakgrund HBTQI-personer står för samma hälsoproblematik som övriga befolkningen. Däremot löper de större risk för depression, suicidalitet och ohälsa, såsom bröstcancer, analcancer och övervikt. Inom denna grupp används även beroendeframkallande substanser i högre grad. HBTQI-personer utsätts för sociala påfrestningar vilket påverkar det dagliga livet. Den osynlighet HBTQI-personer har i vården påverkar utkomsten av den medan vårdpersonal samtidigt uttrycker att okunskap finns för HBTQI-specificerad vård. Syfte Syftet var att belysa HBTQI-personers upplevelser av mötet med vården inom somatisk vård. Metod Denna icke-systematisk litteraturöversikt är utförd med en kvalitativ design där 15 vetenskapliga artiklar inkluderats. Sökordskombinationer i databaserna CINAHL och Pubmed användes för att inhämta artiklarna. Artiklarna kvalitetsgranskades utifrån ett bedömningsinstrument för att sedan genomgå en analys. I analysen skapades teman och subteman baserat på de olika artiklarnas resultat. Resulta tHBTQI-personer upplevde främst negativa upplevelser av vårdmötet. Heteronormativitet, bristande kunskaper i HBTQI-specifik vård och vårdkvalitet samt en oförmåga att bilda en tillitsfull relation till vårdgivaren var orsakerna till detta. Framgångsfaktorer i vårdmöte tvar när HBTQI-personer upplevde en personcentrerad vård där kommunikation, respekt, öppenhet, intresse och medkänsla upplevdes. Slutsats Samtliga artiklar påvisade negativa upplevelser av vårdmötet som övervägande. En utökad kunskap hos vårdpersonalen i HBTQI-specifik vård skulle kunna utmana de negativa upplevelserna HBTQI-personer upplever samt bjuda in till en mer välkomnande vård där kvalitet och kunskap finns men också en god kommunikation med öppenhet och intresse.
