Évolution des discours publics des autorités de santé au Québec en matière de gestion du poids

L’augmentation observée de la prévalence du surpoids et de l’obésité au Québec comme ailleurs en Occident inquiète tant les gouvernements que les autorités médicales. Afin de contenir ce phénomène qui est désormais décrit comme une pandémie d’obésité, ces organisations y sont allées de différentes initiatives et recommandations, dans un contexte d’inefficacité avérée des interventions de gestion de poids à caractère clinique et d’émergence de stratégies de prévention dont l’efficacité et la sécurité à long terme restent encore à démontrer. Méthode : L’objet de cette recherche a été de décrire l’évolution du discours des organismes officiels de santé au Québec en matière de gestion du poids par l’analyse de contenu. Cette analyse a eu recours à une grille de plus de 160 documents produits au cours des 60 dernières années par les gouvernements, les autorités professionnelles et les médias québécois. Résultats et discussion : L’analyse révèle que l’évolution du discours de ces organisations s’inscrit dans trois continuums : le pathologique (une évolution, une gradation, une inflation étymologique du sens qui est donné au poids problématique); la surveillance (avec l’établissement de critères rationnels, la surveillance d’abord individuelle est devenue collective et s’est institutionnalisée); la responsabilisation (la responsabilité du poids s’est déplacée de l’individu vers le collectif puis vers le social). Ces continuums illustrent un déplacement de la manière de conceptualiser le poids de la sphère privée vers la sphère publique. Cette analyse révèle aussi qu’il y a à l’œuvre un exercice disciplinaire propre à une moralisation qui s’appuie sur la prémisse que l’augmentation de la prévalence touche toute la population de manière égale. Or, il n’en est rien. / The increase in overweight and obesity prevalence observed in Quebec as elsewhere worries governments and medical authorities. In order to contain what is described as an “obesity pandemic”, Quebec public health organisations have proposed a number of recommendations depsite the fact that long-term safety of clinical interventions have proven inefficient, and prevention strategies to manage weight undetermined. Objective: To examine the evolution of public discourses about weight management by Quebec’s public health organisations in order to identify if there is a moral standard being constructed and discuss what this reveals about modern societies. Method: Through content analysis of over 160 official documents produced by government, public health organisations and the media over the last 60 years, this thesis will describe and analyze weight management discourses of Quebec’s official health organisations. Results and Discussion: The evolution of public weight management discourses by official public health organisations can best be described using three distinct continuums which all illustrate a shift from the private domain to the public one in the way weight problems are conceptualised. These continuums are: the pathological, or the etymological evolution of meaning given to the problematic weight; surveillance, in that rational criteria has been established and surveillance is no longer in the realm of the personal but rather has become a problem of the collective and in so doing, has become institutionalised; and finally, responsibility, where weight management has migrated from the individual through to the collective and then firmly into the social domain. This analysis illustrates that the disciplinary exercise of weight management, which functions as a moralizing process, considers the increase in overweight and obesity prevalence is across the population. However, such is not the case.

Problemos aktualumas: Jaunesniame mokykliniame amžiuje (I-IV kl.) formuojasi nauji, svarbūs vaiko psichikos bruožai, kurių pagrindu ugdosi labai svarbios savybės ¬­ savistaba, s¬ugebėjimas analizuoti savo poelgius ir kt. sudaro sąlygas formuotis valingam dėmesiui. Tačiau pastaruoju metu aktualia problema tampa auganti karta, kurioje yra nemažai vaikų, turinčių emocinių ir elgesio sutrikimų, kurie turi neigiamą reikšmę vaiko tolesnei psichikos raidai. Apžvelgiant kokybinių mokslinių tyrimų publikacijas Lietuvoje pasigendama mokslinių darbų, kuriuose emocijų ir elgesio problemos būtų tirtos taikant objektyvias standartizuotas metodikas. Dažniausiai duomenys renkami taikant anketavimo metodą, kurio autoriai būna patys tyrėjai. Šiuo metu užsienio šalyse emocijų ir elgesio sutrikimams vertinti plačiai naudojama BASC-2 metodika (Behavior Assessment System for Children, Reynolds ir Kamphaus, 2004, 2005), kuria galima nustatyti vaikų asmenų emocijų, jausmų ir elgesio modelių (pozityvaus ir klinikinio) ypatumus. They are basic for developing such important qualities as self – observation, ability to analyze ones actions and others that help to maintain volitional attention. However a new generation has recently become an urgent problem because there are a lot of children with behavioral and emotional disorders and that has negative influence on the further development of mentality. After surveying the publications of scientific studies in Lithuania it can be stated that there are no studies in which emotional and behavioral problems would be analyzed using objective standardized methods. The data is usually collected using questionnaire method the investigators themselves. Acceptability of alternative treatments for problematic gambling.

Background & objective Cognitive Behavioural Therapy (CBT) and Motivational Interviewing (MI) have been the treatment of choice for problem or pathological gambling in the field in Western countries, and their efficacy has been supported by a considerable empirical research. Alternative treatments are little known; and such treatments for minority ethnic populations have been scarce. This study adopted Kazdin‘s procedures for assessing the acceptability of treatments (Kazdin, 1980a, 1980b, 1981) to test alternative treatments of problem or pathological gambling as a part of the broadening of treatment choices. This thesis presented 2009 survey results from counselling service providers in New Zealand on the acceptability of alternative treatments to problem or pathological gambling. The thesis, therefore, reports the responses of counsellors to counselling vignette case examples, not the views of actual clients viewing counselling. Methods The survey pack was distributed to counselling service providers in New Zealand. The survey included descriptions of sixteen vignettes of case examples of counselling treatments. Categories of clients in the vignette case examples included two genders (male, female) and three ethnicities (Pakeha, Maori, and Asian). Four counselling treatment conditions were selected from Solution-Focused Brief Counselling (SFBC), SFBC+Multicultural Counselling (SFBC+MC), Cognitive-Behavioural Therapy (CBT), and Motivational Interviewing (MI). CBT and MI were only administered to Pakeha clients for the purpose of comparison of the alternatives. Two measurements were used. The first a modified Problem Gambling Treatment Evaluation Inventory (TEI) was used to measure the acceptability levels of the alternative treatments for problem or pathological gambling. The Cross-Cultural Counselling Inventory-Revised (CCCI-R) was used for measuring the perceived cross-cultural competency of counsellors depicted in the vignette case examples. Findings Counsellors‘ ratings of the vignette case examples revealed the following findings: Measurement 1: Problem Gambling Treatment Evaluation Inventory (TEI). Overall, 1) The survey results of TEI questionnaires showed significant main effects across the four treatment conditions and the three client ethnicities, and there was no difference according to client genders. 1.1) SFBC+MC and SFBC were slightly more acceptable than CBT, and much more acceptable than MI. 1.2) The TEI scores for Pakeha clients were much higher than for Maori clients, and the scores for the Asian clients were in between. 2) There was a significant interaction effect between the four treatments and the three client ethnicities. 2.1) For Maori clients: SFBC+MC was much more acceptable than SFBC; for Pakeha clients: SFBC+MC was the most acceptable, closely followed by SFBC, CBT, then MI; and for Asian clients: SFBC was more acceptable than SFBC+MC. 2.2) SFBC+MC was most acceptable to Maori clients across all treatments and ethnic groups. 2.3) The variation in acceptability ratings for SFBC was larger than for SFBC+MC in Maori and Asian clients, and less variable in Pakeha. 2.4) Maori clients had the largest mean variation between SFBC and SFBC+MC, and Pakeha clients had the smallest mean variation. Measurement 2: Cross-Cultural Counselling Inventory-Revised (CCCI-R). The survey results of the CCCI-R showed significant main effects across the four treatment conditions and the three ethnicities. 1) The counsellors depicted in the vignette case examples under the SFBC+MC treatment condition were rated with the highest mean competence score and least variability across all the treatments and the ethnicities, the MI treatment condition were rated with the lowest mean score, CBT and SFBC were in between. 2) The counsellors described in the vignette case examples were rated more culturally competent with Pakeha clients and Maori clients than with Asian clients in the vignette case examples, the rating levels for both Pakeha and Maori were similar. 3) The Maori client in the vignette case examples had the largest mean gap between SFBC and SFBC+MC, and Pakeha client in the vignette case examples had the smallest mean gap. Clinical implications The tests of the acceptability of alternative treatment for problem or pathological gambling could provide useful information about 1) whether the above alternatives would be recommended or selected by the counselling service providers in their clinical practice, 2) which treatment would be more/less preferred by which ethnic group, 3) whether it would work or be worth the efforts to introduce or promote the above alternatives to the counselling service providers, 4) what needs to be explored for increasing levels of the acceptability of alternative treatment to problem or pathological gambling, 5) adding training in the techniques to counsellors training programme and curricula. The limitation of this study was discussed and future research was suggested.

Etude exploratoire, en population adolescente, des variables psychologiques impliquées dans l’utilisation des jeux vidéo et autres technologies numériques / Exploratory study in adolescent population, psychological variables involved in the use of video games and other digital technologies

Cette recherche avait pour objectif d’explorer les variables psychologiques impliquées dans l’Usage Problématique des Jeux Vidéo (UPJV) chez les adolescents, puis de façon secondaire les Réseaux Sociaux (RS) et le Téléphone Portable (TP). Ainsi, 1423 collégiens et lycéens d’Ile de France ont été inclus, dont 92,1% ont joué aux jeux vidéo au cours de l’année écoulée avec un sex ratio proche de 1. Les joueurs sont plus nombreux à avoir déjà consommé de l’alcool, présentent un score de symptomatologie dépressive plus faible, une « recherche de sensation » et une « émotionnalité » plus élevées que les non joueurs. Ceux avec un UPJV (17,7%) sont plus jeunes, ont joué plus précocement, jouent plus de temps, davantage sur ordinateur et dans une guilde. Ils possèdent de plus mauvais résultats scolaires, un usage excessif des Jeux d’Argent et Hasard (JAH) plus fréquent et commencent à boire plus jeunes. Leurs scores d’« urgence positive », de « manque de persévérance » sont plus élevés et celui d’« activité » plus faible. Le score à l’usage excessif des JAH est corrélé positivement au temps passé sur les jeux vidéo et au score d’UPJV. Ce dernier est corrélé positivement aux scores de symptomatologie anxio-dépressive, de « timidité » et d’impulsivité, puis négativement à ceux de satisfaction de vie, d’« activité » et de « sociabilité ». Les jeunes possédant un RS (85,9%) ou un TP (93,2%) sont plus souvent des filles et ont plus de risques liés à la consommation de substances. Malgré certaines limites de notre étude, nos résultats apportent des données nouvelles sur l’UPJV, les RS et le TP. / The aim of this research is to explore the psychological variables primarily involved in the Problematic Use of Video Games (PUVG) in adolescents and secondarily in Social Networks (SN) and Mobile Phones (MP). Thus, 1423 high school and college children from Ile de France region were recruited of which 92.1% had played video games in the past year with a sex ratio of 1. Players are more likely to have consumed alcohol, have lower scores on depressive symptoms, and higher "sensation seeking" and "emotionality" than non-players. Those with PUVG (17.7%) are younger, started playing earlier in life and play for more time, often on computers and in a guild. They have worse academic performances, gamble excessively and frequently and start drinking younger. Their scores on “positive urgency”, and “lack of perseverance” are higher while those on “activity” are lower. Scores on excessive gambling are positively correlated to time spent playing video games and PUVG. The latter, is positively correlated with scores on anxious-depressive symptomatology, “timidity” and impulsivity while it is negatively correlated with life satisfaction, “activity” and “sociability.” Youngsters on SN (85.9%) or MP (93.2%) are more often girls and are associated with substance use. Despite some limitations of our study, our results provide new data on PUVG, SN and MP.

Le comportement envers une activité physique (AP) peut être compris sur un continuum où les individus inactifs et les individus qui pratiquent de façon excessive représentent les deux extrémités. Au milieu de ce continuum se trouvent les individus qui pratiquent de façon modérée et régulière. Ce sont ces derniers qui semblent bénéficier le plus de la pratique d’une AP (Canning et al., 2014; Kern et al., 2013; Warburton et al., 2006; Spirduso, 1995) (Spirduso, 1995). Cette thèse a pour but de comprendre les comportements dites ‘à risque pour la santé’ par rapport à la pratique d’AP, notamment l’inactivité physique et la pratique problématique d’exercice physique (PPEP). Cette thèse s’articule donc en deux parties. La première partie ‘‘Perspectives théoriques’’ a pour but la présentation d’une recherche adaptée afin de fournir une compréhension complète des comportements vis à vis la pratique d’AP et plus particulièrement l’inactivité physique et la PPEP. Cette partie vise également à faire un état de lieu de leurs méthodes d’évaluation, leurs causes et conséquences, ainsi que leurs préventions et traitements. La deuxième partie ‘‘Perspectives empiriques’’ qui sera elle même divisée en deux sous sections, sera consacrée aux différents articles (publiés ou en cours) englobant l’inactivité physique chez les étudiants universitaires et la PPEP: le but de cette dissertation est donc double et s’articule autour de sept articles. 1/Pour comprendre le problème d’inactivité physique à travers: (a) Une revue systématique de la prévalence globale de l’inactivité physique au sein des étudiants universitaires; (b) Une étude croisée effectuée au sein des communautés estudiantines françaises et indiennes; (c) Une étude exploratoire afin de tracer le profil des étudiants inactifs en Inde. 2/ Afin de mieux cerner le phénomène de la PPEP grâce à:(a) La validation d’un modèle hiérarchique définissant la PPEP; (b) L’étude du cas clinique d’un homme de cinquante ans mettant en lumière le développement et les motivations vis à vis la PPEP;(c) Une etude exploratoire sur les liens entre la PPEP et les traits d’impulsivité; (d) Cette étude vise à reproduire le modèle de Cook & Hausenblas (2008) qui postule que la PPEP joue un rôle de médiateur entre l’exercice physique et les troubles du comportements alimentaires (TCA). / Behavior towards physical activity (PA) can be investigated on a continuum with physical inactivity on one end and excessive exercise leading to the dangers of doping and/or problematic practice of physical exercise (PPPE), on the other. The aim of the current dissertation is to further clarify the concepts of physical inactivity as well as PPPE, in order to promote the adoption of regular and moderate PA while taking into account the negative consequences of excessive exercising. This dissertation is divided into two parts. The purpose of Part I – ‘Theoretical Perspectives’ is to present pertinent research and provide a general understanding of physical inactivity and PPPE. Part II – ‘Empirical Perspectives’, consists of the various research papers encompassing the two phenomena. Thus, the aim of this dissertation is twofold and is achieved through seven papers: 1/To understand the epidemic of physical inactivity through (a) a systematic review of prevalence of physical inactivity in college students globally on studies done between 2002 and 2015, (b) a cross sectional study done on a cohort of French and Indian university students and (c) an analysis of the psychological correlates of physical inactivity amongst Indian university students. 2/ To understand the phenomenon of PPPE through (a) the validation of an hierarchical model explaining PPPE, (b) a clinical case study of a 50 year old man throwing light upon the development and motivations towards PPPE, and (c) the possible links between traits of impulsivity and PPPE. This section also presents a fourth working article which (d) aims to replicate the mediation model of exercise, PPPE and eating pathology validated by Cook & Hausenblas (2008) along with the motivation to control weight.

Another goal is to determine the real readiness of beginning teachers to solve problematic behavior of pupils, and in terms of real and outlined cases.

O estudo tem por temática a rede para a atenção às pessoas adoecidas pelo uso problemático de álcool e outras drogas, em particular as percepções de profissionais que participam do processo de construção da rede sobre o cuidado a álcool e outras drogas no município de Volta Redonda por meio dos objetivos específicos: Descrever o processo da construção da rede de Saúde Mental em Volta Redonda; Compreender como ocorrem as decisões na prática do cuidado aos usuários de álcool e drogas; Descrever as dificuldades das equipes/gerentes de unidades de Saúde Mental, no processo de cuidado integrado aos usuários de álcool e drogas e ao final, propor outras formas de organização para a rede de saúde mental no município de Volta Redonda. Em suma pensar a rede de Saúde Mental e fazer um retrato com objetivo de apontar as fragilidades e fortalezas no atendimento às pessoas com o uso problemático de álcool e outras drogas para ofertar proposta de construção de outra realidade. Demarcamos como percurso teórico desta pesquisa, as questões voltadas à atenção em saúde mental e os serviços de saúde e, às questões relativas ao uso problemático de álcool e outras drogas. Escolhemos a metodologia qualitativa como trajetória. Utilizamos como forma de obtenção dos dados a entrevista semi estruturada. Nossos depoentes foram pessoas que participaram do processo de construção da rede de atenção em saúde mental. Os relatos dos entrevistados revelam que a política municipal de saúde mental é uma realidade, porém existem interferências tanto na área gerencial quanto das práticas em saúde, contribuindo para o distanciamento dessa mesma política. Assim foi possível concluir que a rede de serviço é complexa, porém não existe de fato como rede. Isto implica diretamente na perda da dimensão cuidadora que é o princípio básico da integralidade. / The study it has for thematic the net for the attention the sickness people for the problematic alcohol use and other drugs, in particular the perceptions of professionals who participate of the process of construction of the net on the care to these people. We search to analyze the model of attention to the health of alcohol users and other drugs in the Volta Redonda city in return by means of the specific objectives: To describe the process of the construction of the net of Mental Health in Volta Redonda; To understand as the decisions in practical of the care to the users of alcohol and the drugs occur; To describe the difficulties of the controlling teams/of units of mental health, in the process of care integrated to the users of alcohol and drugs and to the end to consider other forms of organization for the net of mental health in the Volta Redonda city in return, in short to think the net of mental health and to make a picture with objective to point the fragilities and strong in the attendance to the people with the problematic alcohol use and other drugs to offer proposal of construction of another reality. We demarcate as theoretical passage of this research, the questions directed to the attention in mental health and the services of health and, to the relative questions to the problematic alcohol use and other drugs. We choose the qualitative methodology as trajectory, we use as form of attainment of the data the structuralized half interview. Our deponents had been the people who had participated of the process of construction of the net of attention in mental health. The stories of the interviewed ones disclose that the municipal politics of mental health is a reality, however in such a way exist interferences in the managemental area how much of practical in health, contributing for the distanciamento of this the same politics. Thus it was possible to conclude that the net of services is complex, however does not exist in fact as net. This implies directly in the loss of the care dimension that is the basic principle of the completeness.

The final thesis is focused on problematic question of social services quality inspection. First part of the thesis aids on conception of quality in context of social services. This part also describes development of social services and their practical use in law system of Czech Republic. We can also find description of inspection methods and their system. Third charter is mainly focused on dimension of inspector's ethic codex. It is aiming on systems of values, professional ethics, ethic codex and education of inspectors. Last and main chapter is trying to explain problematic areas in actual and real conditions of inspection process. Chapter contains suggestions with several solutions. Thesis uses and combines all collected data from verified sources, specialists external inspectors.

The aim for the study is to identify high school students’ perceptions of absenteeism. More specific the aim is to investigate their perceptions of the situations before, during and after the time they were absent. The chosen method for the study is phenomenography which includes qualitative semi-structured interviews. Six students currently in secondary school with a background of absenteeism have been interviewed. The interviews have been analyzed with an eight-step approach. The result of the study presents young students perceptions of circumstances that lead to and sustain their school absence and their return to school attendance. The findings in the analysis show that the informants perceive mental illness as a circumstance with major impact on leading to and sustaining school absence among the students. The mental illness is related to the students’ perceptions of social circumstances in and/or outside school and unmet needs in relation to school success. The perceptions of the circumstances that lead to and sustain attendance were related to a better mental health. Mental health was affected positively mainly by the circumstances related to social relationships and by the adaptations in the learning environment. However, the informants did not perceive better mental health as a key circumstance as explicitly as mental illness.
