Den som slutar att bli bättre, slutar snart att vara bra : -en kvalitativ studie om ständiga förbättringar inom hälso- och sjukvård

Dagens hälso- och sjukvård är komplex och i konstant förändring, detta kräver att medarbetare och ledare ständigt arbetar med förbättringar, både för att kvalitetssäkra vården men också för att hushålla med begränsade resurser av såväl personal som ekonomi. För att lyckas med förbättringsarbete krävs medvetenhet och ett engagemang av hela organisationen, för att tillsammans skapa förutsättningar för ständiga förbättringar. Syftet med denna studie var att undersöka hur ständiga förbättringar bedrivs, samt vilka förutsättningar som finns för att driva förbättringsarbete enligt medarbetare och ledare inom hälso- och sjukvård. För att uppfylla studien syfte gjordes en kvalitativ fallstudie och datainsamlingen bestod av en enkätundersökning till samtliga medarbetare samt en intervjustudie där semistrukturerade intervjuer genomfördes med samtliga enhetschefer samt ett antal medarbetare inom den studerade organisationen. Resultatet analyserades genom en tematisk analys, baserade på 74 insamlade enkätsvar och totalt 10 intervjuer och presenterades utifrån sex teman med tillhörande subteman, totalt 27 stycken. Studiens teman utgjordes av att driva förbättringsarbete, att vara en engagerad ledare, motivation till förändring, hinder för förbättringsarbete, framgångsfaktorer för förbättringsarbete och att motverka förändringsmotstånd. Studiens resultat visar att motivationen och viljan till att utveckla, förändra och förbättra är påtaglig hos både medarbetare och enhetschefer. Men förbättringsarbetet förhindras av brist av ekonomiska resurser, avsaknad av helhetssyn, brist på kompetens inom förbättringsområdet samt otillräckliga personalresurser. Detta bidrar till att förbättringsarbetet inom den studerade organisationen inte drivs i holistiskt samförstånd. / Today’s healthcare is complex and under constant change. It requires employees and leaders to continuously works with improvement to ensure the quality of care and make the limited resources last, regarding both finance and staff. To succeed with this continuous drive for quality improvement, the same awareness and level of engagement must be shared across the whole organization. This study aimed to explore how employees and leaders in healthcare are executing these continuous improvements alongside the available prerequisites. To fulfill the studys aim, a qualitative casestudy was conducted and the data was collected with a survey circulated to all members of staff and an semi-structured interview study with all first- line managers and a few employees of the studied organization. The results were analyzed through a thematic evaluation of the 74 survey replies and the 10 interviews. It was presented as 6 themes with 27 associated subthemes. The results formed the themes; Driving Quality Improvement, Being an Engaged Leader, Motivation for Change, Quality Improvement Obstacles, Improvement Success Criteria and Change Resistance Avoidance. The result of the study shows that employees and first-line managers alike share a willingness and motivation for development, change and improvement. But this is hampered by a lack of economic resources, holistic views, improvement competence and staff. The detection of high-qualified indels in exomes and their effect on cognition

Plusieurs insertions/délétions (indels) génétiques ont été identifiées en lien avec des troubles du neurodéveloppement, notamment le trouble du spectre de l’autisme (TSA) et la déficience intellectuelle (DI). Bien que ce soit le deuxième type de variant le plus courant, la détection et l’identification des indels demeure difficile à ce jour, et on y retrouve un grand nombre de faux positifs. Ce projet vise à trouver une méthode pour détecter des indels de haute qualité ayant une forte probabilité d’être des vrais positifs. Un « ensemble de vérité » a été construit à partir d’indels provenant de deux cohortes familiales basé sur un diagnostic d’autisme. Ces indels ont été filtrés selon un ensemble de paramètres prédéterminés et ils ont été appelés par plusieurs outils d’appel de variants. Cet ensemble a été utilisé pour entraîner trois modèles d’apprentissage automatique pour identifier des indels de haute qualité. Par la suite, nous avons utilisé ces modèles pour prédire des indels de haute qualité dans une cohorte de population générale, ayant été appelé par une technologie d’appel de variant. Les modèles ont pu identifier des indels de meilleure qualité qui ont une association avec le QI, malgré que cet effet soit petit. De plus, les indels prédits par les modèles affectent un plus petit nombre de gènes par individu que ceux ayant été filtrés par un seuil de rejet fixe. Les modèles ont tendance à améliorer la qualité des indels, mais nécessiteront davantage de travail pour déterminer si ce serait possible de prédire les indels qui ont un effet non-négligeable sur le QI. / Genetic insertions/deletions (indels) have been linked to many neurodevelopmental disorders (NDDs) such as autism spectrum disorder (ASD) and intellectual disability (ID). However, although they are the second most common type of genetic variant, they remain to this day difficult to identify and verify, presenting a high number of false positives. We sought to find a method that would appropriately identify high-quality indels that are likely to be true positives. We built an indel “truth set” using indels from two diagnosis-based family cohorts that were filtered according to a set of threshold values and called by several variant calling tools in order to train three machine learning models to identify the highest quality indels. The two best performing models were then used to identify high quality indels in a general population cohort that was called using only one variant calling technology. The machine learning models were able to identify higher quality indels that showed a association with IQ, although the effect size was small. The indels predicted by the models also affected a much smaller number of genes per individual than those predicted through using minimum thresholds alone. The models tend to show an overall improvement in the quality of the indels but would require further work to see if it could a noticeable and significant effect on IQ.

Feng shui: implications of selected principles for holistic nursing care of the open heart patient

This qualitative study sought to explore the Hong Kong Chinese patient's perceptions of an Intensive Care experience and their views on the introduction of feng shui principles focussing on sleep orientations, dietary management and exercise regimes. The study explored the background of feng shui as an authentic traditional Chinese belief. It also explores if incorporating these feng shui principles into the health care setting would provide a positive effect for open-heart patients in an Intensive Care Unit at the Hong Kong Adventist Hospital in Hong Kong. The major inference drawn from this study is that Chinese patients seek culturally related experiences from the health care setting. The Chinese informants showed strong belief patterns in traditional practices of feng shui, however, practiced these within the confines of their homes as these experiences were denied to them in the hospital setting. / Health Studies / M.A. (Nursing Science)

Diese Dissertation umfasst sowohl einen theoretisches als auch einen empirischen Beitrag zur Analyse der Messung der gesamten Faktorproduktivität (TFP). Das erste Kapitel inspiziert die bestehende Literatur über die häufigsten Techniken der TFP Messung und gibt einen Überblick über deren Limitierung. Das zweite Kapitel betrachtet Daten, die durch ein Real Business Cycle Modell generiert wurden und untersucht das quantifizierbare Ausmaß von Messfehlern des Solow Residuums als ein Maß für TFP Wachstum, wenn der Kapitalstock fehlerhaft gemessen wird und wenn Kapazitätsauslastung und Abschreibungen endogen sind. Das dritte Kapitel schlägt eine neue Methodologie in einem bayesianischen Zusammenhang vor, die auf Zustands- Raum-Modellen basiert. Das vierte Kapitel führt einen neuen Ansatz zur Bestimmung möglicher Spill-over Effekte auf Grund neuer Technologien auf die Produktivität ein und kombiniert eine kontrafaktische Zerlegung, die von den Hauptannahmen des Malquist Indexes abgeleitet wird mit ökonometrischen Methoden, die auf Machado and Mata (2005) zurückgehen. / This dissertation consists of theoretical and empirical contributions to the study on Total Factor Productivity (TFP) measurement. The first chapter surveys the literature on the most used techniques in measuring TFP and surveys the limits of these frameworks. The second chapter considers data generated from a Real Business Cycle model and studies the quantitative extent of measurement error for the Solow residual as a measure of TFP growth when the capital stock is measured with error and when capacity utilization and depreciation are endogenous. Furthermore, it proposes two alternative measurements of TFP growth which do not require capital stocks. The third chapter proposes a new methodology based on State-space models in a Bayesian framework. Applying the Kalman Filter to artificial data, it proposes a computation of the initial condition for productivity growth based on the properties of the Malmquist index. The fourth chapter introduces a new approach for identifying possible spillovers emanating from new technologies on productivity combining a counterfactual decomposition derived from the main properties of the Malmquist index and the econometric technique introduced by Machado and Mata (2005).

I propose an integrative approach to address the above questions. My findings suggest that certain kinds of the organizational form were always interwoven with the social situation at the time and could not be simplistically explained in terms of efficiency alone. It was the state and the development of the market that determined the dynamics of the organizational form. In my opinion, efficiency theory, specifically the concept of asset specificity, could explain why the internal market system failed. On the other hand, power theory, specifically the phenomenon of isomorphism, could shed light on why the enterprise adopted the internal market system in 1994. The study suggests that the perspective of power has more strength to explain the social process of the organizational form framing. Furthermore, it explores the source of the internal transaction costs, which was underdeveloped in neo-institutional economic theory. It is my argument that there is a missing link between asset specificity and the internal transaction costs. / In the institutional approach, the studies of organizational forms have long been influenced by two theories: efficiency theory and power theory. The general theoretical concern of this study is: Is efficiency theory adequate to explain the internal organizational form? Which of these two theories is more appropriate to explain the internal organizational form of the state-owned enterprises in China? / The twenty-year economic reform in China has witnessed frequent changes in the titles and structures of China's economic organizations. This case study investigates the internal organizational form of a state-owned enterprise in Guangzhou, China. It depicts the history of its organizational form from 1949 to 2000. Under the planned economy, the said enterprise, The Southern Heavy Machinery Corporation, had an over-integrated organizational structure. From 1994 to 2000, the enterprise has set up a system consisting of an enterprise group corporation and dozens of subsidiary companies, moving toward a market system for its internal transactions among the production units. The subsidiary companies were set up on the basis of the earlier production plants established before 1984. As profits decreased year by year, the new general manager who started his tenure in September 2000 made the decision to bring some of its crucial subsidiary companies back into the form of production plants to render stronger continuities in production. / Why then, after 16 years of internal market practice that has begun in 1984, did the enterprise restructure itself to adopt the unitary form (U form) in some crucial production units? This qualitative study uses narrative inquiry (Connelly & Clandinin, 1988, 1990, 2001) and self-study to investigate ways to further understand and facilitate the integration of holistic philosophies of education with media literacy pedagogies. As founder and director of the Youth Media Literacy Project and a self-titled Imagitator (one who agitates imagination), I have spent over 10 years teaching media literacy in various high schools, universities, and community centres across North America. This study will focus on my own personal practical knowledge (Connelly & Clandinin, 1982) as a culture jammer, educator and cancer survivor to illustrate my original vision of a ‘holistic media literacy pedagogy’. This research reflects on the emergence and impact of holistic media literacy in my personal and professional life and also draws from relevant interdisciplinary literature to challenge and synthesize current insights and theories of media literacy, holistic education and culture jamming.
