Kan selentillskott behandla autoimmun tyreoidit? : En litteraturstudie / Could selenium supplementation treat autoimmune thyroiditis? : A litterature study

Introduktion. Autoimmun tyreoidit (AITD) är en kronisk autoimmun sjukdom där immunförsvarets antikroppar (ab) attackerar tyreoideaproteinerna tyreoideaperoxidas (TPO) och/eller tyreoglobulin (TG). Studier visar att selentillskott hos patienter med AITD kan minska tyreoideaantikroppar, storleken och antalet noduler hos en förstorad tyreoidea. Syftet med detta arbete var att undersöka hur selentillskott påverkar serumnivåer av TPOab, samt tyreoideahormonnivåer vid AITD.   Metod. Arbetet är en litteraturstudie och därför har metoden varit att samla relevant litteratur genom PubMed med sökningar som ”selenium autoimmune thyroiditis”, ”selenium thyroid” och ”autoimmune thyroiditis”. Bland sökresultaten valdes nio artiklar ut baserat på studiekvalitet, publikationsår och relevans. Bland artiklarna granskades och sammanställdes uppmätta nivåer av TPOab samt tyreoideahormonnivåer, vilka valdes som indikation på effekt utav selentillskott. Resultat. Resultaten var inkonsekventa. Majoriteten av studierna (7 av 9) tydde på att oral administrering av selentillskott effektivt minskade serumkoncentrationerna av TPOab hos patienter med AITD i alla åldersgrupper. De studier som resulterade i störst minskning av TPOab pågick i 3-12 månader. Utav de 9 studerade artiklarna var det endast en studie som inte rapporterade någon som helst positiv klinisk effekt hos patienterna. Två av studierna visade att selen förhindrar vidare försämring av tyreoideans ekogenitet, vilket tyder på att selen kan hejda inflammationsprocessen men ej reversera tyreoideaskadorna den orsakat. Majoriteten av studierna (7 av 9) visade att selentillskott ej ger några signifikanta förändringar i tyreoideahormonerna: TSH, fT4 och fT3. Diskussion. Varför AITD-patienter svarar olika på selenadministrering är ännu okänt, men kan misstänkas bero på selenbehandlingens varaktighet, patienternas intratyroidnivåer av selen vid studiens början, förekomst av jodbrist, samt patienternas ålder och sjukdomsprogression. Slutsats. Att ha adekvata fysiologiska nivåer av selen är av stor vikt för att bevara tyreoideans hälsa och förebygga tyreoidearelaterade sjukdomar. Majoriteten utav de granskade studierna visar att tillskott av selen kan minska antalet TPOab. Selentillskott kan även ha immunrelaterade fördelar men verkar inte påverka nivån tyreoideahormonnivåer. Inga negativa effekter påvisades vid intag av selentillskott vilket gör dess administrering säker. Fler studier behöver dock göras för att fastställa effektiviteten av selentillskott vid AITD. / Introduction. Autoimmune thyroiditis (AITD) is a chronic autoimmune disease in which the immune system's antibodies (ab) attack the thyroid proteins thyroid peroxidase (TPO) and/or thyroglobulin (TG). Studies show that selenium supplementation in patients with AITD can reduce thyroid antibodies and the size and number of nodules in an enlarged thyroid. The purpose of this study was to investigate how selenium supplementation affects the serum levels of thyroid peroxidase antibodies (TPOab) and thyroid hormone levels in autoimmune thyroiditis. Method. This is a literature study and therefore the method has been to gather relevant literature through searches on PubMed such as "selenium autoimmune thyroiditis", "selenium thyroid" and "autoimmune thyroiditis". Among the search results, nine articles were selected based on quality, publication year and relevance. Among the articles, measured levels of TPOab and thyroid hormone levels were examined and compiled, and were chosen as an indication of the effect of selenium supplementation. Results. The results were inconsistent. The majority of the studies (7 of 9) suggest that oral administration of selenium supplements effectively reduced serum concentrations of TPOab in patients with AITD in all age groups. The studies that resulted in the largest decrease in TPOab lasted for 3-12 months. Out of the 9 examined studies, only one study did not report any positive clinical effect in patients. Two of the studies showed that the selenium prevents further impairment of thyroid echogenicity, suggesting that selenium can inhibit the inflammatory process but not reverse the pre-existing thyroid damage it’s caused. The majority of studies (7 out of 9) show that selenium supplementation does not produce significant changes in the thyroid hormones: TSH, fT4 and fT3. Discussion. Why AITD-patients respond differently to selenium administration is still unknown, but it may be due to the duration of selenium treatment, the patients' intrathyroid levels of selenium at the onset of the study, the presence of iodine deficiency, as well as the age and disease progression of the patients. Conclusion. Having adequate physiological levels of selenium is of great importance in preserving thyroid health and preventing thyroid-related diseases. The majority of the studies show that selenium supplementation can reduce the number of TPOab. Selenium supplementation may also have immune related benefits but does not appear to affect the thyroid hormone levels. No adverse effects were observed during selenium supplementation, which makes its administration safe. However, more studies are needed to determine the effectiveness of selenium supplementation for AITD.

Mit physikalischen Messverfahren werden die in der Literatur nur unzureichend untersuchten Eigenschaften von Selen in Silizium erfasst und als Basis für Bauelementsimulationen verwendet. Für die Untersuchung der Störstelleneigenschaften kommt die klassische aufwändige DLTS zum Einsatz. Des Weiteren werden für diese Untersuchungen die Vorteile der einfacheren frequenzabhängigen Admittanzspektroskopie ausführlich dargelegt. Anhand der Bauelementsimulationen erfolgt ein Vergleich mit Messungen und führt zur Vorstellung und Erläuterung einer verbesserten soften und robusten Diodenstruktur mit tiefen Donatoren aus Selen. / The focus of the following work is the correlation between the field-stop design and the behaviour of high-voltage power diodes. The objective is to present a further improvement of the diode performance using a special field-stop, which optimizes the diode in relation to a soft switching behaviour and an increased robustness. The function of such a field-stop is investigated. Benefits are shown of materials for field-stops with deep impurities in the semiconductor material and of multiple stepped deep field-stop structures. Therefore a central role have silicon diodes with selenium in the field-stop layer. Measurements and simulations with the power device simulator Sentaurus TCAD are done and explain the named correlations. The deep level transient spectroscopy is used as method to analyse the required impurity parameters. Beside this method the evaluation is done by the introduced frequency resolved admittance spectroscopy.

Speciační analýza selenu v kvasinkách kultivovaných v médiu s přídavkem selenu / Speciation analysis of selenium in selenized yeast

The aim of the theses was determination of selenium species in yeast Saccharomyces cerevisiae cultivated in medium with added inorganic form of selenium (Sodium Selenite). Concentrations of Sodium Selenite in cultivation medium were 0,1; 1; 10 and 100 mg.l-1. Cultivation was undertaken in fermenting tub for period of 72 hours. Cultivated yeasts were extracted by use of enzymes and subsequently the species of selenium in particular parts of yeasts were determined. In order to determine selenium species, the method of high-performance liquid chromatography in combination with atomic fluorescent spectrometer and technique of hydride generation was used. Having analysed different fractions of the yeasts Saccharomyces cerevisiae it was ascertained that during cultivation the sorption of selenium occurred in form of Se4+ in cell membranes while in cytoplasm no inorganic forms of selenium were found. Furthermore, it was stated that yeasts Saccharomyces cerevisiae are able to metabolically change inorganic forms of selenium to organic forms (selenomethionine), while these forms are present in cytoplasm and they are likely to be bound to proteinic structures of cell membranes. An increase of concentration of Se4+ in cell membranes could be observed as a result of increasing concentration of Sodium Selenite in cultivation medium. In proteinic structures the concentration of organic selenium forms increased only to concentration 10 mg.l-1 of Sodium Selenite in cultivation medium.

Studium miniaturních zařízení pro kolekci hydridotvorných prvků v atomové spektroskopii / Investigation of Miniature Devices for Collection of Hydride Forming Elements in Atomic Spectrometry Methods

Capability of a prototype of miniature collection device based on a strip of the molybdenum foil for collecting hydride forming elements (As, Se, Sb and Bi) was studied. The device was combined with a miniature hydrogen diffusion flame for detection by atomic absorption spectrometry. The conditions for trapping and subsequent vaporization of analytes of interest were optimized. A twin-channel hydride generation system was used for study of mutual interference effects of co-generated hydride forming elements. The influence of modification of the molybdenum surface with noble metals - Rh, Pt and Ir on trapping and vaporization processes was also studied and changes of microstructure of the foil surface after modification were investigated using scanning electron microscope equipped with energy dispersive x-ray analyzer and electron backscattered diffraction system. Complementary radiotracer and radiography experiments were performed in order to determine trapping efficiency and to assess the spatial distribution of collected analytes within the device. Practical application of the method was demonstrated on determination of antimony in water samples at trace level. Possibility of multi-element analysis was demonstrated by combining the collection device with atomization and excitation of the analyte in microwave induced plasma and with detection by atomic emission spectrometry method. The results of the experiments proved that tested miniature collection device is capable of trapping analytes that form volatile hydrides. This device can be coupled to various types of atomizers, typically used in spectrometry methods. Thus, very sensitive and specific detection of hydride forming elements can be performed.

The presented thesis deals with UV-photochemical generation of volatile compounds of Se and Te from various species. The aim of the project was to expand the current state of knowledge by the application of photocatalytic reduction of higher oxidation states of Se and Te for the speciation analysis based on UV-photochemical generation of volatile compounds. The first step of the study was the assembly of the apparatus for the photocatalysed UV-photochemical generation of volatile compounds. The material of reactor and the whole experimental set-up were based on literature survey and previous research done in our research group. Experiments were directed towards finding the optimum conditions for generation of volatile compounds of selected model elements Se and Te. Se was studied as the element most commonly determined by the UV-photochemical generation of volatile compounds. Conversely, Te was selected as a model analyte representing elements forming less stable volatile compounds. The second part was the application of the optimised method of photocatalysed UV- photochemical generation of volatile Se compound to the determination of Se in water matrices, liquid certified reference materials and also samples of dietary supplements. Methodische und klinische Evaluation eines modernen Flachbettdetektors und des Dual Energy Verfahrens

In einer initialen Studie verglichen wir das XQi Revolution, welches auf indirektem CsI (Cäsium Iodit) /a: Si (amorphes Silizium) basiert mit einem direkten Digitalröntgengerät a: SE (amorphes Selen) an einem CDRAD-Phantom bei vier unterschiedlichen Eintrittsdosen und an einem TRG-Phantom bei zwei unterschiedlichen Eintrittsdosen. Mittels des berechneten Bildqualitätsfaktors des CDRAD-Phantoms konnten wir zeigen, daß das indirekte im Vergleich zum direkten System bei niedrigeren Dosen eine bessere Detailerkennungsrate aufweist. Ein positiver Trend läßt sich auch beim TRG-Phantom darstellen. In einer weiteren Studie untersuchten wir anhand von Patientenbildern die Bildqualität des Dual Energy Systems bei zwei unterschiedlichen Dosisniveaus, der Standarddosis sowie einer doppelten Dosis, was einem Speed-Äquivalent von 400/1000 bzw. 200/500 entspricht. Bei hoher Dosis konnten wir eine signifikante Reduktion des Rauschens im Knochen- und Weichteilbild feststellen, gleichzeitig nahmen die Störungen durch Bewegungsartefakte signifikant zu. Im Anschluß verglichen wir die Erkennbarkeit verkalkter Lungenpathologien im Standard P/A Bild mit zusätzlichem Einsatz von Dual Energy. Als Goldstandard erfolgte der sichere Nachweis der Pathologien im CT. Bei zusätzlichem Einsatz von Dual Energy konnten wir eine signifikante Steigerung der Sensitivität erkennen. Dieses Ergebnis wurde durch den Qualitätsfaktor, der die Bildeigenschaften kumulativ beschreibt, bestätigt. Weiterhin untersuchten wir analog die Erkennbarkeit von nichtverkalkten Lungenrundherden. Auch bei diesen Pathologien ließ sich ein positiver Trend der Sensitivität und Spezifität bei zusätzlichem Einsatz von Dual Energy erkennen. Zusätzlich stieg die durchschnittliche Entscheidungssicherheit der Gutachter signifikant an. Damit bietet die Dual Energy Subtraktionstechnik eine wertvolle Ergänzung in der Diagnostik verkalkter und nichtverkalkter Lungenpathologien eine wertvolle Ergänzung zum Standardröntgen. / First study assess and quantify the image quality at two dose levels for an amorphous Silicon (a:Si) Cesium Iodide (CsI) flat panel system compared with a direct amorphous Selenium (a:Se) digital radiography system. Image quality of a:Si flat panel digital radiography proved to be superior to a:Se drum digital radiography using low-dose settings. Second study assess the image quality of subtracted soft tissue and bone images of a CsIdetector-based dual-energy system for chest radiography at varying dose levels. Radiation dose did not significantly influence the perception of dual-energy image quality. Next study assess the value of dual-energy chest radiography obtained using a cesium iodide flat-panel detector in addition to standard posteroanterior chest radiography for the detection of calcified chest abnormalities. When dual-energy images were added, sensitivity increased significantly. Brunner and Langer’s test revealed a highly significant difference between posteroanterior chest radiography and dual-energy imaging in the detection of calcified chest abnormalities. Dual-energy images added to standard posteroanterior chest radiographs significantly improve the detection of calcified chest lesions. Last study compare the sensitivity and specificity of digital chest radiography alone with digital chest radiography combined with dual-energy chest radiography in the detection of small non-calcified pulmonary nodules. Standard and dual-energy radiographs were obtained with a flat-panel digital chest system. The increase of nodule detection overall as well as for different size categories was significant. The increase of the confidence level rating was also significant. Dual energy added to standard posteroanterior chest radiography significantly improves the sensitivity, specificity, and confidence in detection of small non-calcified pulmonary nodules. Dual-energy subtraction has the potential to become a future routine application in chest radiography.

UV-fotochemické generování těkavých sloučenin selenu pro potřeby ultrastopové analýzy metodou AAS / UV-photochemical generation of volatile selenium compounds for ultratrace analysis by AAS

This thesis deals with the study of UV-photochemical generation of volatile compounds (UV-PVG) in connection with atomic absorption spectrometry (AAS). Selenium (Se(IV)) was selected as a model analyte and many experiments were carried out to expand the current sum of knowledge of the topic. The study was commenced by assembling the UV-PVG apparatus in the continuous flow mode. The detection method used was AAS with externally heated quartz furnace atomizer. The focus of the first step of the study was on the construction of the volatile compounds generator (UV-photoreactor) with emphasis on the material used; tubes made of teflon or quartz of different diameters were tested. The construction of the apparatus was followed by optimization of the reaction conditions (the type and concentration of the photochemical agent and other agents, which increase the analytical signal; the carrier gas and the auxiliary hydrogen gas flow rate; the sample flow rate). Eventually, the analytical figures of merit of the selenium determination using the proposed method were found. The results showed that teflon reaction tubes are a good competitor to those made of quartz. Sammanfattning  Bakgrund Vitamin- och mineralbrister hos gravida kvinnor kan leda till missfall och allvarliga störningar i barnets utveckling. Moderns tarmflora överförs med stor sannolikhet till barnet under förlossningen och kan därför innebära ett viktigt steg i utvecklingen av barnets tarmflora. En tänkbar lösning för att säkra ett adekvat intag kan vara konsumtion av kosttillskott och probiotika. I dagsläget finns osäkra uppgifter om hur många gravida kvinnor som intar tillskott.  Syfte Att undersöka hur många gravida kvinnor i Västerbottens län som valde att inta kosttillskott, främst järn och probiotika, samt om det fanns en skillnad mellan olika faktorer och intag.  Metod En kvantitativ tvärsnittsstudie där gravida kvinnor (n=1473) från Northpop-studien i Västerbottens län svarade på ett frågeformulär gällande intag av kosttillskott och faktorer som ålder, utbildning, kostregim etc. De statistiska tester som användes var Chi-2-test, oberoende t-test och Mann Whitney U-test. Materialet analyserades i SPSS. Signifikansnivån sattes till p<0,05.  Resultat Majoriteten av deltagarna svarade att de intog kosttillskott. Faktorer som ökade intaget av kosttillskott hos gravida kvinnor var högre ålder (p=0,030) jämfört med lägre ålder, högre utbildningsnivå (p=0,006) jämfört med lägre utbildningsnivå och vegetarisk/vegansk kost (p=0,021) jämfört med blandkost. Femtiofem procent uppgav att de intog järntillskott. De faktorer som ökade intaget av järntillskott hos gravida kvinnor var vegetarisk/vegansk kost (p=0,001) jämfört med blandkost. Probiotika intogs av 2 procent. Ett högre intag av probiotika sågs hos personer boende i stadsområde (p=0,024) jämfört med övriga boenderegioner samt de som åt vegetarisk/vegansk kost (p=0,001) jämfört med blandkost.  Slutsats Majoriteten av deltagarna intog någon typ av kosttillskott, hälften intog järntillskott och en liten andel intog probiotika. Lågutbildade, yngre, de som äter blandkost och bor utanför stadsområde verkar vara i riskgruppen för att inte inta kosttillskott. / Abstract  Background Vitamin and mineral deficiencies in pregnant women can lead to miscarriage and serious disturbances in children’s development. The intestinal flora of the mother is most likely transmitted to the child during childbirth and may lay the foundation for the child's health. One possible solution to ensure an adequate intake may be the consumption of dietary supplements and probiotics. At present, there is insufficient data on supplement consumption among pregnant women.  Objective The purpose of the study was to examine how many pregnant women in Västerbotten County chose to consume dietary supplements, mainly iron and probiotics, and whether there was a difference between different factors and intake.  Method A quantitative cross-sectional study where pregnant women (n=1473) from the Northpop-study in Västerbotten County responded to a questionnaire regarding consumption of dietary supplements and factors such as age, education, diet etc. The material was analyzed in SPSS with Chi-2-test, independent T-Test and Mann-Whitney U-Test. Using significance level <0.05.  Results The majority of participants, 90 percent, responded that they consumed dietary supplements. The factors that increased the intake of dietary supplements in pregnant women were higher age (p=0.030), higher education (p=0.006) and vegetarian/vegan diet (p=0.021). Iron was reported to be consumed by 804 people, 55 percent. The factors that increased the intake of iron supplement in pregnant women were vegetarian/vegan diet (p=0.001). Probiotics were consumed by 25 people, 2 percent. Living in urban areas (p=0.024) and eating vegetarian/vegan diet (p=0.002) increased consumption of probiotics.  Conclusion The majority of participants chose to consume some type of dietary supplement, half of the participants consumed iron supplements and a small part consumed probiotics. It appears that pregnant women who are low educated, younger, eating an omnivorous diet and living outside urban areas are in the risk zone for not consuming dietary supplements. / Northpop
