Criminalisation for sexual transmission of HIV : emerging issues and the impact upon clinical psychology practice in the UK

Objective: Criminal liability for the sexual-transmission of HIV raises complex questions for both clinicians and service-users regarding their responsibilities and legal obligations to disclose information to others. This is the first research study to address the impact of these issues upon everyday clinical and professional management in the UK. The prevalence and incidence of clinical and HIV-legal issues reported by the 107 psychologists sampled are reported. Design: A cross-sectional approach comprising two components was utilised: Firstly, questionnaire survey (Response rate 22%) scoping the experiences of practice issues among psychologists from sexual-health and generic settings. Attitudes towards HIV-prosecutions and various measures of professional self-efficacy were also collected. Secondly, three focus groups (N=15) exploring the impact of practice issues upon clinicians’ likely confidentiality breaking behaviours. Methods: Clinical and legal issues are presented. Further statistical analyses explored the interaction of various demographic, clinical and attitudinal variables upon clinician’s perceived self-efficacy. Focus Group transcripts analysed using Thematic Analysis (Data-driven approach) with eight emergent themes. Results: Although no direct involvements in police investigations reported, two instances of psychology notes being subpoenaed plus multiple ‘near miss’ clinical experiences described. High proportions of sexual-health psychologists experienced HIV-clients disclosing problematic behaviours, including intentional transmission (9%; N=5) and/or ‘reckless’ behaviour (72%). Focus groups expressed high levels of anxiety regarding these scenarios associated to multiple influences (interpersonal, clinician, professional and service factors). Quantitative and qualitative results were triangulated to provide a detailed analysis of how psychologists manage the clinical impact of the issues. Conclusions: Psychologists broadly supported HIV-prosecutions for intentional transmission (81%) but only limited support around ‘reckless’ cases (44%), particularly among those sexual-health experienced. Those ‘critical’ attempted to mitigate the impact of legal issues by proactively raising awareness among HIV-clients and resisting overly-defensive service changes; whereas those ‘less-critical’ were more accepting. Clinical, training and therapeutic implications are briefly considered.

S’insérant dans les domaines de la Lecture et de l’Analyse de Textes Assistées par Ordinateur (LATAO), de la Gestion Électronique des Documents (GÉD), de la visualisation de l’information et, en partie, de l’anthropologie, cette recherche exploratoire propose l’expérimentation d’une méthodologie descriptive en fouille de textes afin de cartographier thématiquement un corpus de textes anthropologiques. Plus précisément, nous souhaitons éprouver la méthode de classification hiérarchique ascendante (CHA) pour extraire et analyser les thèmes issus de résumés de mémoires et de thèses octroyés de 1985 à 2009 (1240 résumés), par les départements d’anthropologie de l’Université de Montréal et de l’Université Laval, ainsi que le département d’histoire de l’Université Laval (pour les résumés archéologiques et ethnologiques). En première partie de mémoire, nous présentons notre cadre théorique, c'est-à-dire que nous expliquons ce qu’est la fouille de textes, ses origines, ses applications, les étapes méthodologiques puis, nous complétons avec une revue des principales publications. La deuxième partie est consacrée au cadre méthodologique et ainsi, nous abordons les différentes étapes par lesquelles ce projet fut conduit; la collecte des données, le filtrage linguistique, la classification automatique, pour en nommer que quelques-unes. Finalement, en dernière partie, nous présentons les résultats de notre recherche, en nous attardant plus particulièrement sur deux expérimentations. Nous abordons également la navigation thématique et les approches conceptuelles en thématisation, par exemple, en anthropologie, la dichotomie culture ̸ biologie. Nous terminons avec les limites de ce projet et les pistes d’intérêts pour de futures recherches. / Taking advantage of the recent development of automated analysis of textual data, digital records of documents, data graphics and anthropology, this study was set forth using data mining techniques to create a thematic map of anthropological documents. In this exploratory research, we propose to evaluate the usefulness of thematic analysis by using automated classification of textual data, as well as information visualizations (based on network analysis). More precisely, we want to examine the method of hierarchical clustering (HCA, agglomerative) for thematic analysis and information extraction. We built our study from a database consisting of 1 240 thesis abstracts, granted from 1985 to 2009, by anthropological departments at the University of Montreal and University Laval, as well as historical department at University Laval (for archaeological and ethnological abstracts). In the first section, we present our theoretical framework; we expose definitions of text mining, its origins, the practical applications and the methodology, and in the end, we present a literature review. The second part is devoted to the methodological framework and we discuss the various stages through which the project was conducted; construction of database, linguistic and statistical filtering, automated classification, etc. Finally, in the last section, we display results of two specific experiments and we present our interpretations. We also discuss about thematic navigation and conceptual approaches. We conclude with the limitations we faced through this project and paths of interest for future research.

The resilience of caregivers at a Gauteng-based hospice with patients living with HIV/AIDS

In English / This study investigates the resilience of caregivers at a Gauteng-based hospice with patients living with HIV/AIDS. The primary focus of this study was to describe the impact on caregivers working with terminally ill patients predominantly with HIV/AIDS. Six participants were selected for this study utilising purposive sampling. Their ages ranged between 30 and 70 years. For the purpose of data collection, face-to-face semi-structured interviews were undertaken. In analysing data, the themes and sub-themes were extracted and presented by employing thematic analysis. The findings of this study revealed that in times of adversity and setbacks most caregivers utilise various skills to bounce back, and they still showed contentment in serving community members. However, caregivers have also voiced the concern that the support offered at work was not sufficient in meeting their needs. Recommendations for further research, on the resilience of caregivers when working with terminally ill patients is highlighted. / Psychology / M. A. (Clinical Psychology)

Förlossningsrädsla har tidigare främst undersökts hos kvinnor som lever i heterosexuella relationer, och på senare tid även kommit att inkludera deras manliga partners upplevelser av rädsla. Förlossningsrädsla kan påverka negativt under familjebildningen, vilket gör den viktig att uppmärksamma i alla familjekonstellationer. Syftet med denna studie var att undersöka hur förlossningsrädsla erfars och tar sig uttryck hos lesbiska/bisexuella kvinnor och transpersoner, samt vad olika typer av bemötande i vården får för betydelse för dem och deras rädsla. Data samlades in genom semistrukturerade intervjuer med tio deltagare. De transkriberade intervjuerna bearbetades genom tematisk analys med en induktiv ansats.  Analysen resulterade i fem huvudteman som handlar om förlossningsrädslan, omkringliggande aspekter och hur bemötande i vården påverkar rädslan; Mina förlossningsrädslor, Naturlighet, cisnormer och femininitet, Vägen till barn, Att vara sårbar och utsatt under en förlossning och Bemötande i vården. Resultatet visar på att deltagarnas förlossningsrädsla är lik den som rapporterats i forskning i stort, men att det delvis tillkommer vissa nya aspekter för denna grupp. Förlossningsrädslan influeras av deras erfarenheter av bemötande i vården och den större omgivande sociala kontexten som påverkar genom processer av normer, minoritetsstress, hetero- och cisnormativitet och diskriminering. Every year, tens of thousands of patients in North America die from preventable errors. Incident learning can decrease this number, but is not currently reaching its full potential in health care. The goal of this research was to propose strategies to effectively report and learn from incidents to drive quality improvement. A detailed literature review, focus groups, in-depth interviews, and thematic analysis were used to identify potential process improvement strategies from aviation and nuclear power and assess these strategies for feasibility and desirability in health care. This work was guided by World Health Organization recommendations and Rasmussen’s risk management framework. An extensive list of strategies to address existing shortcomings was ultimately proposed for consideration in health care. These strategies can serve as guidelines to proactively improve incident learning processes. This should help create more effective systems and in turn, improve patient safety.

Syftet var att undersöka hur barn i olika familjeformer ser på familjer, sin egen och andras. Intervjuer har genomförts med sammanlagt 22 barn varav elva kom från familjer med olikkönade och sammanboende föräldrar som fått barn på egen hand, nedan kallade relationsbarn, och övriga elva kom från familjer med en frivilligt ensamstående mamma som skaffat barn med hjälp av assisterad befruktning, nedan kallade femmisbarn. Barnens ålder varierade från tre år och tio månader till nio år och nio månader. Studien kan ses som en del av barndomsforskningen, där barns ses som kompetenta aktörer vars röster förtjänar att lyftas fram. En semistrukturerad intervjuguide låg till grund för intervjuerna där barnen ombads att måla sin egen och en annan familj. Barnens berättelser har analyserats med hjälp av tematisk analys. Resultatet visar att femmis- och relationsbarnens berättelser huvudsakligen liknar varandra samtidigt som vissa skillnader framkommer mellan grupperna. Det förkommer också stora individuella skillnader inom grupperna. En kärnfamiljsnorm är tydligt framträdande i både barnens prat och teckningar, samtidigt som barnen visar på en öppenhet kring olika familjeformer. I relationsbarnens berättelser omnämns far- och morföräldrar, föräldrars syskon och kusiner som släktingar medan femmisbarnen i större utsträckning inkluderar dessa samt husdjur som medlemmar i sina familjer. Gemensamt för alla barnen är att deras berättelser om familjelivet utgår från barnet och har barnet i fokus. I såväl femmis- som relationsbarnens prat om mammor och pappor framträder en bild av traditionella könsroller. Såväl relations- som femmisbarnens berättelser om familj är mestadels i linje med tidigare forskning kring barns syn på familj. Femmisbarnens inklusion av såväl husdjur som släktingar i familjen, kan vara ett resultat av att nätverket får en större betydelse hos de ensamstående föräldrarna vilket även setts i tidigare forskning. Den kärnfamiljsnorm som blev tydlig i barnens prat kan ses som en avspegling av de rådande normerna i samhället. Trots en ökad variation gällande familjeformer är kärnfamiljen fortfarande ett rådande ideal.

Façons de faire l’évaluation formative d’enseignants de français sénégalais : une analyse de leurs savoirs pratiques en contexte d’effectifs pléthoriques au Lycée.

Cette recherche documente les façons de faire l’évaluation formative d’enseignants de français du Lycée exerçant en contexte de classes pléthoriques au Sénégal. Le choix récent dans ce pays de l’approche par compétences invite à privilégier cette fonction de l’évaluation, au regard de son potentiel pour la progression des apprentissages des élèves (Allal & Mottier Lopez, 2005; Black & Wiliam, 2009; Morrissette, 2010). Cependant, les orientations ministérielles concernant sa mise en œuvre sont très générales, et jusqu’ici, la recherche a laissé dans l’ombre son application en contexte de classes pléthoriques. Puisant au domaine des savoirs pratiques (Schön, 1983) et à une vision interactive et située de l’évaluation formative (Mottier Lopez, 2007; Morrissette, 2010), j’ai conduit une démarche de recherche collaborative auprès de 14 enseignants de français exerçant dans le même Lycée, ponctuée par 6 entretiens de groupe. Un premier registre d’analyse a décrit des façons de faire rattachées à trois dimensions de la pratique de l’évaluation formative: l’analyse du contexte de la pratique, la construction négociée du savoir et la gestion de l’effectif. Un second registre d’analyse de leurs façons de faire en contexte d’«étrangeté culturelle» (Douville, 2002) a permis de conceptualiser leur savoir-évaluer en relation avec leur façon d’interpréter les problèmes qui se posent aux élèves, leur conception de l’erreur et leurs manières de réinventer les modes d’accomplissement traditionnels de l’évaluation ancrés dans la culture scolaire. / This research documents the ways formative assessment is operationalized by French teachers working in the context of overcrowded classrooms in a Senegalese High School. The recent choice for implementing the competency-based approach in this country invites teachers to promote the use of formative assessment which is geared towards the provision of high quality learning (Allal & Mottier Lopez, 2005, Black & Wiliam, 2009 Morrissette, 2010). However, Ministerial guidelines for its implementation are very general, and so far, research has not been carried out as regards its application in overcrowded classes. Building on the field of reflective practice (Schön, 1983) and an interactive situated vision of formative assessment (Mottier Lopez, 2007; Morrissette, 2010), I led a collaborative research with 14 French teachers working in the same High School. Six focus groups discussions were carried out which shed light on three dimensions related to formative assessment practices: contextual analysis of teacher practices, the negotiated construction of knowledge and the management of the overcrowded classrooms. Analysis of formative assessment practices in the context of « cultural strangeness » (free translation) (Douville, 2002) helped to conceptualize their know-how on evaluation in light of how they interpret the problems faced by students, their conception of what error constitutes and the ways they reinvent the traditional performance evaluation methods rooted in the school culture.

A narrative inquiry of older adults' stories of choosing to not share information with health care professionals

This study used narrative inquiry to answer the question “What are the stories of older adults who choose not to share information with health care professionals?” The study explored the experiences of older adults who did not share information with health care professionals (HCPs), who they defined as doctors. A thematic analysis of five participants’ stories revealed three themes: Illusions of Care, describing the participants’ experiences with doctors before they chose not to share information; The Last Straw, revealing the final incident that caused participants to choose not to share information; and The Mask of the Non-sharing Older Adult, describing how participants interacted with their doctors after they decided not to share information. Relationship development between older adults and their doctors, sensitive topics, issues and perceptions of ageing, and structure of the health care system contribute to the complex issue of older adults not sharing information. The decision not to share information with health care professionals may adversely affect the health and health care of older adults.
