Fatty Acid Desaturase Activities in Metabolic Syndrome and Cardiovascular Disease : Special Reference to Stearoyl-CoA-Desaturase and Biomarkers of Dietary Fat

The development of the metabolic syndrome (MetS) and cardiovascular diseases have been suggested to be influenced more by the quality than the amount of dietary fat. The FA composition of serum lipids may be used as biomarkers of dietary fat quality. FAs can, however, also be endogenously synthesized by lipogenic enzymes such as elongases and desaturases. Three desaturases are important in humans: Stearoyl-CoA-desaturase (SCD), ∆6-desaturase (D6D) and ∆5-desaturase (D5D) and surrogate measures of desaturase activities can be estimated as product-to-precursor FA ratios. In this thesis, we demonstrated that high SCD, D6D and low D5D estimated activities predicted MetS 20 years later, as well as cardiovascular and total mortality during a maximum of 33.7 years. The relation between D5D and MetS was independent of lifestyle and BMI, while the relation between SCD, D6D and MetS was confounded by BMI. Serum proportions of palmitic (16:0), palmitoleic (16:1) and dihomo-γ-linoleic acids were higher and the serum proportion of linoleic acid (LA) lower at baseline in those individuals who developed MetS. Further, LA was inversely related to mortality, while palmitic, palmitoleic and dihomo-γ-linoleic acids were directly associated with mortality. We also demonstrated that a diet rich in saturated fat “induced” a similar serum FA pattern (including estimated desaturase activities) that was associated with MetS, cardiovascular disease and mortality. We also propose that the SCD ratio [16:1/16:0] might be a novel and useful marker of dietary saturated fat, at least in Western high-fat diets. Finally, genetic variations in the human SCD1 gene were linked to obesity and insulin sensitivity, results that agree with data in SCD1 deficient mice. This thesis suggests that dietary fat quality and endogenous desaturation may play a role in the development of metabolic and cardiovascular diseases and the results support current dietary guidelines.

The purpose of this study is to examine what pupils in the ninth grade eats for lunch and also whether there is any connection between their lunch habits and perceived ability to concentrate and fatigue. A further aim was to study pupils' experience of dining environment in schools.The study is based on a quantitative method with questionnaires. The study was implemented on four schools in elementary school in themunicipalityofKristianstadand Tanum. Our results demonstrate, in accordance with several previous studies conducted in the same age group, that it is about half of the pupils who eats school lunch every day and that the majority are eating school lunch 3-5 days a week. Furthermore, our study shows that the pupils who rarely eat vegetables in school feel tired and distracted to a greater extent than the group of pupils who eat vegetables almost daily. Our result also shows that the group of pupils who eat the main course 3-5 days a week feel both tired and distracted to a greater extent than the group of students who eat the main course more rarely. In accordance with previous studies our study shows that differences exist between boys and girls regarding food that is eaten instead of school lunch. Food containing empty calories is the most popular kind, it causes glucose levels to get out of balance which makes students tired and distracted.The result of our study shows that most pupils claim that they often feel tired and distracted during the school day. Hunger affects the individual's emotions, motivation and cognition (Lennernäs, 2011), which can be linked to the fact that only 6 % of pupils feel that they are getting satisfied every day after eating the school lunch. There are also many pupils who choose not to eat anything else those days they choose to skip the school lunch. Nevertheless, the majority of pupils are still happy with the environment in the school dining room and it is barely half the pupils who want bigger influence on what is served for school lunch.

Reducing meat consumption : a mixed methods study investigating attitudes of young adult omnivores

Background Meat is one of the most nourishing and highly consumed foods, but the controversy around theconsumption of meat products, and the negative effects around the consumption of meatproducts led to increased scholarly attention on the user willingness to alter their meatconsumption. In Sweden, a guideline by the Livsmedelsverket (The Swedish National Food Agency) in 2015 encouraged people to consume less red and processed meat. This was because 72 per cent of men and 42 per cent of women in Sweden have individual consumption levels that exceed the 500 grams per week of red and processed meats as recommended by the World Cancer Research Fund. Objective This planned mixed-methods study inquires about the attitudes of young adult omnivorestowards the reduction of meat intake, their expected liking of changing dietary habits, andmotivation towards the reduction of meat consumption in Sweden. Changing dietary habits inthe current context refers to the participant’s willingness to favour meat alternatives. Methods The current study is a convergent mixed method design using both a quantitative survey andqualitative interviews to collect primary data. Results and conclusion The participants from both the qualitative and quantitative study are broadly worried about thehealth problems associated with high meat consumption and motivation for reducing meatconsumption was based on health as well as ethical issues. Being opposed to reducing meatconsumption is a common position for young adult omnivores. / Bakgrund Kött är ett av de mest näringsrika och mest konsumerade livsmedlena. Med anledning av attman funnit negativa effekter av en hög konsumtion av kött har man vetenskapligt alltmer börjatintressera sig för konsumentens vilja att ändra sin köttkonsumtion.I Sverige uppmanade Livsmedelsverket Sveriges befolkning att konsumera mindre rött kött ochbearbetade köttprodukter. Bakgrunden till detta var att 72 % av männen och 42 % av kvinnornai Sverige har individuella konsumtionsnivåer som överstiger 500 gram av rött kött ochbearbetade köttprodukter per vecka, vilket är en maxrekommendation från Världscancerforskningsfonden. Resultat och slutsats Deltagarna från både den kvalitativa och den kvantitativa undersökningen är i stort sett överensstämmande och visar att försökspersonerna är oroliga för de hälsoproblem som är förknippade med hög köttkonsumtion och deras motivation för att minska köttkonsumtionen baserades på frågor kring hälsa och etik. Det var vanligt förekommande bland försökspersonerna att vara bunden till köttkonsumtion.

Kan en lågfetthaltig växtbaserad diet få kranskärlsjukdomar att stagnera eller reversera? / Can a Low-Fat Plant-Based Diet Make Coronary Artery Diseases Stagnate or Reverse?

Enligt Socialstyrelsen år 2019 var hjärt-och kärlsjukdomar den vanligaste dödsanledningen i Sverige och svarade för 31 % av alla dödsfall i landet. Akut kranskärlssjukdom tillhör hjärt- och kärlsjukdomar och är en folksjukdom som drabbar både kvinnor och män i hela västvärlden med högst mortalitet till följd. Kliniska studier har visat att den västerländska kosten med för högt animaliskt proteinintag, för högt intag av raffinerat socker och fett är den primära bakomliggande orsaken till dödsfall i hjärt-kärlsjukdom. Största riskfaktorn för kranskärl-och andra hjärtsjukdomar är arterioskleros (åderförkalkning). En växtbaserad 10 % lågfetthaltig Whole Food Plant Based-diet (WFPB) har visat sig ha en positiv effekt på arteriosklerosprocessen och vidare på hjärt-kärlsjukdomars utveckling. Syftet med detta projekt var att genom en systematisk litteraturstudie undersöka om hjärtsjukdomar, framförallt kranskärlsjukdomar (CAD) kan stagneras och/eller reverseras med hjälp av en lågfetthaltig WFPB-diet. Studien är baserad på 10 vetenskapliga artiklar framtagna ur databaserna CINAHL, PubMed, Google Scholar samt från referenslistan på två av de redan utvalda artiklarna. Samtliga tio artiklar som inkluderats i litteraturstudien valdes genom datainsamling, relevansbedömning och kvalitetsgranskning. För att säkerställa artiklarnas kvalité kvalitetsgranskades de relevanta artiklarna utifrån frågor skapade från en mall från Statens beredning för medicinsk och social utvärdering, SBU. De analyserade mätparametrarna i artiklarna var bl. a. angiografiparametrar, lipidbiomarkörer, anginasymtom, Flödesmedierat vasodilatationstest (FMD) samt Positronemissions tomografi (PET). De flesta granskade studierna visade reversering av CAD, två artiklar visade både reversering och stagnering och en artikel kunde inte bedömas. Stagnering eller reversering av kranskärlsjukdomar kan åstadkommas antingen genom en kombination av dietintervention och andra livstilförändringar som komplement till lipidsänkande medicinsk behandling eller genom endast diet-och andra livstilförändringar. / According to the National Board of Health and Welfare in 2019, cardiovascular disease was the most common cause of death in Sweden and accounted for 31% of all deaths in the country. Acute coronary heart disease belongs to cardiovascular disease and is a common disease that affects both women and men throughout the Western world with the highest mortality as a result. Clinical studies have shown that the Western diet with too high animal protein intake, too high intake of refined sugar and fat is the primary underlying cause of death in cardiovascular disease. The biggest risk factor for coronary heart disease and other heart diseases is arteriosclerosis (atherosclerosis). A plant-based 10% low-fat Whole Food Plant Based Diet (WFPB) has been shown to have a positive effect on the arteriosclerosis process and further on the development of cardiovascular disease. The purpose of this project was to investigate through a systematic literature study whether heart disease, especially coronary heart disease (CAD) can be stagnated and / or reversed with the help of a low-fat WFPB diet. The study is based on 10 scientific articles produced from the databases CINAHL, PubMed, Google Scholar and from the reference list of two of the already selected articles. All ten articles included in the literature study were selected through data collection, relevance assessment and quality review. To ensure the quality of the articles, the relevant articles were quality examined on the basis of questions created from a template from the Swedish Agency for Medical and Social Evaluation, SBU. The analyzed measurement parameters in the articles were for example angiography parameters, lipid biomarkers, angina symptoms, Flow-mediated vasodilation test (FMD) and Positron emission tomography (PET). Most of the studies examined showed reversal of CAD, two articles showed both reversal and stagnation and one article could not be assessed. Stagnation or reversal of coronary heart disease can be achieved either through a combination of dietary intervention and other lifestyle changes in addition to lipid-lowering medical treatment or through dietary and other lifestyle changes only.

Bakgrund: Tidigare studier visar på att en hälsosam kost kan kopplas till kognitiv funktion, såsom minne och andra mentala funktioner i hjärnan. Människan behöver regelbundet intag av näringsämnen och vid ett balanserat intag av kolhydrater, fett och protein kan hjärnan och kroppen uppnå optimal funktion. Syfte: Syftet är att undersöka kostens samt olika livsmedels effekt på hjärnhälsa hos friska vuxna individer i åldern 50 år och uppåt. Metod: Litteraturöversikten baserad på åtta vetenskapliga artiklar som hittades i databaserna Medline och PubMed. Analysen utfördes enligt systematisk översikt med hjälp av ramverket PICO. Resultat: Sammanställningen av tidigare forskning lyfter fram kunskapen om sambandet mellan kost och hjärnhälsa samt betonar speciella framgångsfaktorer inom ämnet.  Samtliga artiklar uppnådde statistisk signifikans och syftet kunde besvaras. Slutsats: Fortsatt forskning bör studeras med bredare perspektiv för att tillgodose alla kriterier för främjande av hjärnhälsa. Aktuella studien har ringat in viktiga faktorer för att utveckla en god hjärnhälsa via kosten. Att studera kostens påverkan på hjärnhälsa kan vara nyckeln till att förbättra hälsan globalt. / Background: Previous studies show that a healthy diet can be linked to cognitive function, such as memory and other mental functions of the brain. Humans need a regular intake of nutrients, and with a balanced intake of carbohydrates, fat and proteins, can the brain and body achieve optimal function. Objective: The aim is to research the effect of nutrition and different food on brain health on healthy adults aged 50 and over. Method: The literature review was based on eight scientific articles found in the databases Medline and PubMed. The analysis was performed according to a systematic review using the PICO framework. Results: The compilation of previous research highlights the knowledge of the connection between diet and brain health and emphasizes special success factors within the subject. All articles achieved statistical significance and the purpose could be answered. Conclusion: Continued research should be studied with broader perspectives to satisfy all criteria for promoting brain health. The current study has identified important factors for developing good brain health through nutrition. Studying the impact of diet on brain health may be the key to improving health globally.

Changes in Running Technique At Shod and Barefoot Running Condition : - An analysis of Muay Thai Fighters

Background: everal studies mean that individuals accustomed to running barefoot and habitually barefoot individuals choose a fore foot or mid foot strike rather than a rear foot strike when running. This is partly to do with the proprioceptive feedback given from the soles of the feet. The common denominator of the barefoot runner and the Muay Thai fighter would be this proprioceptive feedback due to general bare foot training. A contributing factor to a non heal strike pattern could perhaps also be found in the sport specific movement pattern of Muay Thai where the athlete is trained to execute several actions from the fore foot. Aim: The first aim was to analyse differences in angles of the knee, ankle and foot at initial contact while running shod compared to barefoot in Muay Thai fighters accustomed to training barefoot. A second aim was to investigate if there was a difference in running technique regarding foot strike pattern when running shod compared to bare foot. The foot strike pattern is defined as heal strike, mid foot strike or forefoot strike. Method: Seventeen Muay Thai fighters (13 male 4 female) volunteered to participate in the test of the cross- sectional experimental study. The study took place at PT-Studion Halmstad. The participants performed 2x3minutes of running shod and barefoot respectively at a subjective speed equal to a 12 on the Borg scale. Both conditions were video recorded from a sagittal plane using an iPhone 6. The dominant leg defined as the non-weight bearing leg at the preferred boxing stance was analysed. Data was collected two-dimensionally of the angels of the knee, ankle and foot position to horizontal. Foot strike pattern was also determined. Landmarks were marked to ease the analysis. T-tests of paired samples were used to examine whether there were any differences between the angle of the knee, ankle and foot at barefoot and shod conditions. The significance level for this study was set to be statistically significant if p ≤ 0.05. Foot strike frequency distributions were compared between shod and barefoot runners using chi-square (X2) analysis. Result: A significant difference at the angle of the foot (p=0,034) at initial contact with the ground while running between shod (12 ± 9) and barefoot (17 ± 9) was found. No significant difference in the angle of the knee (p=0,076) or ankle (p=0,081) was found. Changes in foot strike pattern were observed. At shod condition 88% used a rear foot strike, 6% used a mid foot strike and 6% used a forefoot strike. At barefoot condition 41% used a rear foot strike, 0% used a mid foot strike and 59% a fore foot strike. Conclusion: This study shows that the there is a significant difference in foot strike pattern when running shod and barefoot which confirms previous studies. The test group have in common that they train Muay Thai although; their sport specific training doesn’t appear to in this case have any impact on the foot strike pattern. / Bakgrund: Flera studier menar att individer vana att springa barfota samt individer som lever barfota väljer en framfota insättning vid löpning snarare än en hälisättning. Den gemensamma nämnaren mellan barfotalöparen och Muay Thai atleten skulle kunna vara denna feedback beroende av barfotaträning. En bidragande faktor till en icke hälisättning skulle kunna förklaras av det idrottsspecifika rörelsemönstret i Muay Thai där flera moment utförs stående på den främre delen av foten. Syfte: Ett syfte var att analysera skillnader i knä, ankel och fotvinklar vid initial kontakt i löpsteget vid skodd och barfota löpning av i Muay Thai-fighters vana vid träning barfota. Ett andra syfte var att undersöka eventuella skillnader i fotisättning vid barfotalöpning samt löpning med skor. Fotisättning definieras som framfot, mellan fot eller bakre/häl-isättning. Metod: Sjutton (17) Muay Thai atleter (13manliga 4kvinnliga) ställde frivilligt upp att delta i tvärsnittsstudien. Studien ägde rum på PT-Studion Halmstad. Testpersoner utförde 2x3minuter av löpning med och utan skor vid en subjektiv hastighet lika med en 12 på Borg skalan. Båda förhållandena filmades i sagitalplan med iPhone 6. Det dominanta benet definierat som det icke viktbärande benet vid den föredragna boxningspositionen analyserades. Tvådimensionell analys av förändringar i fotisättning, vinklar i knäled, ankelled och fotens position till det horisontella samt fotisättning dokumenterades. Landmärken markerades för att underlätta analysen. T-test användes för att undersöka om det fanns några skillnader i vinkeln av knäled, fotled och ankel vid löpning med skor och barfota. Signifikansnivån för denna studie bestämdes vara statistiskt signifikant om p <0,05. Frekvensen av fotisättningen fördelning jämfördes via chi-square (X2) analys. Resultat: En signifikant skillnad (p=0,034) i fotvinkeln vid initial kontakt med marken under löpning med skor (12 ± 9) och barfota (17 ± 9) hittades, men inga signifikanta skillnader i knäets vinkel ((p=0,076) eller ankel (p=0,081) hittades. Med skor vald 88 % en hälisättning, 6 % en mittfotsisättning och 6 % en främre fotisättning. Vid löpning barfota valde 41 % en hälisättning, 0 % en mittfotsisättning och 59 % en främre fotisättning. Slutsats: Studien visar att det finns en signifikant skillnad i fotisättningen mellan löpning barfota och med skor vilket bekräftar tidigare studier. Testgruppen har gemensamt att de tränar thaiboxning, dock kan inte den sportspecifika träningen förklara det valda fotisättningsmönstret.

Dietary Fatty Acids and Inflammation : Observational and Interventional Studies

Dietary fat quality influences the risk of type 2 diabetes and cardiovascular disease. A low-grade inflammation is suggested to contribute to the disease development, often accompanied by obesity. Whereas n-3 polyunsaturated fatty acids (PUFA) have been considered anti-inflammatory, n-6 PUFA have been proposed to act pro-inflammatory. Saturated fatty acids (SFA) act pro-inflammatory in vitro. This thesis aimed to investigate effects of different fatty acids on low-grade inflammation in observational and interventional studies. In Paper I and II, fatty acid composition in serum cholesterol esters was used as objective marker of dietary fat quality and related to serum C-reactive protein (CRP) and other circulating inflammatory markers in two population-based cohorts, conducted in middle-aged men and elderly men and women, respectively. In Paper III and IV, the impact of diets differing in fat quality on inflammation and oxidative stress was investigated in randomised controlled studies, in subjects with metabolic syndrome and abdominal obesity. In Paper I and II, a low proportion of linoleic acid (18:2 n-6) in serum was associated with higher CRP concentrations, indicating that a low intake of vegetable fats may be related to low-grade inflammation. High CRP concentrations were also associated with high proportions of palmitoleic (16:1) and oleic (18:1) acids and high stearoyl coenzymeA desaturase index, possibly reflecting altered fat metabolism and/or high SFA intake in this population. When comparing two high-fat diets rich in either saturated or monounsaturated fat, and two low-fat diets with or without long-chain n-3 PUFA supplementation during 12 weeks (Paper III), no differences in inflammation or oxidative stress markers were observed. Moreover, a 10-week intervention (Paper IV) with high linoleic acid intake showed no adverse effects on inflammation or oxidative stress. Instead, interleukin-1 receptor antagonist and tumor necrosis factor receptor-2 decreased after linoleic acid intake compared with a diet high in SFA. The results in this thesis indicate that dietary n-6 PUFA found in vegetable fats is associated with lower inflammation marker levels, and to some extent reduces systemic inflammation when compared with SFA. Supplementation of n-3 PUFA did not exert any systemic anti-inflammatory effects, maybe due to a relatively low dose.

Preschool Staff perception of the pedagogical meal : A mixed methods study of the pedagogical work with meals in Swedish preschools from 2005 to 2020

Meals in Swedish preschools has undergone change between 2005 and 2020 regarding meals as important pedagogical tools. Due to new guidelines for conducting preschool meals presented by the Swedish Food Agency an increased focus has been placed on pedagogy during the meals to help develop the minds of young children. Previous research has shown that preschool staff had little to no education on food and meal or how to conduct pedagogical meals. To understand how the preschool staff, perceive the pedagogical meal and how it has changed over time, a convergent mixed method with a survey and interviews was used. The results showed that the staff does not have formal education on food, meal and how to conduct pedagogical meals, but instead rely on peer learning. They perceive themselves as role models in forming a positive relationship with the social and nutritional aspects of food and meal. While their job has many positive aspects, they face challenges with stress and increasing child groups. As the role of preschool staff has changed, from making sure children eat, to socially participating in the meal, the guidelines have changed alongside the role.

Sammanfattning  Bakgrund Vitamin- och mineralbrister hos gravida kvinnor kan leda till missfall och allvarliga störningar i barnets utveckling. Moderns tarmflora överförs med stor sannolikhet till barnet under förlossningen och kan därför innebära ett viktigt steg i utvecklingen av barnets tarmflora. En tänkbar lösning för att säkra ett adekvat intag kan vara konsumtion av kosttillskott och probiotika. I dagsläget finns osäkra uppgifter om hur många gravida kvinnor som intar tillskott.  Syfte Att undersöka hur många gravida kvinnor i Västerbottens län som valde att inta kosttillskott, främst järn och probiotika, samt om det fanns en skillnad mellan olika faktorer och intag.  Metod En kvantitativ tvärsnittsstudie där gravida kvinnor (n=1473) från Northpop-studien i Västerbottens län svarade på ett frågeformulär gällande intag av kosttillskott och faktorer som ålder, utbildning, kostregim etc. De statistiska tester som användes var Chi-2-test, oberoende t-test och Mann Whitney U-test. Materialet analyserades i SPSS. Signifikansnivån sattes till p<0,05.  Resultat Majoriteten av deltagarna svarade att de intog kosttillskott. Faktorer som ökade intaget av kosttillskott hos gravida kvinnor var högre ålder (p=0,030) jämfört med lägre ålder, högre utbildningsnivå (p=0,006) jämfört med lägre utbildningsnivå och vegetarisk/vegansk kost (p=0,021) jämfört med blandkost. Femtiofem procent uppgav att de intog järntillskott. De faktorer som ökade intaget av järntillskott hos gravida kvinnor var vegetarisk/vegansk kost (p=0,001) jämfört med blandkost. Probiotika intogs av 2 procent. Ett högre intag av probiotika sågs hos personer boende i stadsområde (p=0,024) jämfört med övriga boenderegioner samt de som åt vegetarisk/vegansk kost (p=0,001) jämfört med blandkost.  Slutsats Majoriteten av deltagarna intog någon typ av kosttillskott, hälften intog järntillskott och en liten andel intog probiotika. Lågutbildade, yngre, de som äter blandkost och bor utanför stadsområde verkar vara i riskgruppen för att inte inta kosttillskott. / Abstract  Background Vitamin and mineral deficiencies in pregnant women can lead to miscarriage and serious disturbances in children’s development. The intestinal flora of the mother is most likely transmitted to the child during childbirth and may lay the foundation for the child's health. One possible solution to ensure an adequate intake may be the consumption of dietary supplements and probiotics. At present, there is insufficient data on supplement consumption among pregnant women.  Objective The purpose of the study was to examine how many pregnant women in Västerbotten County chose to consume dietary supplements, mainly iron and probiotics, and whether there was a difference between different factors and intake.  Method A quantitative cross-sectional study where pregnant women (n=1473) from the Northpop-study in Västerbotten County responded to a questionnaire regarding consumption of dietary supplements and factors such as age, education, diet etc. The material was analyzed in SPSS with Chi-2-test, independent T-Test and Mann-Whitney U-Test. Using significance level <0.05.  Results The majority of participants, 90 percent, responded that they consumed dietary supplements. The factors that increased the intake of dietary supplements in pregnant women were higher age (p=0.030), higher education (p=0.006) and vegetarian/vegan diet (p=0.021). Iron was reported to be consumed by 804 people, 55 percent. The factors that increased the intake of iron supplement in pregnant women were vegetarian/vegan diet (p=0.001). Probiotics were consumed by 25 people, 2 percent. Living in urban areas (p=0.024) and eating vegetarian/vegan diet (p=0.002) increased consumption of probiotics.  Conclusion The majority of participants chose to consume some type of dietary supplement, half of the participants consumed iron supplements and a small part consumed probiotics. It appears that pregnant women who are low educated, younger, eating an omnivorous diet and living outside urban areas are in the risk zone for not consuming dietary supplements. / Northpop
